THE PERFECT scenario FOR INNOVATION Throughout 25 years, implantodontics in Brazil went from zero to extraordinary accomplishments, turning it into the second largest market in the world for that segment.

There are no doubts that this position has been supported, mainly, by the clinical excellence of Brazilian implantodontics professionals, a specialization that has grown more than 260% in the last four years. That expansion has brought along opportunities for growth and has motivated us to dedicate six years to study and development, presenting to the market a unique way to do implants: the Arcsys System.

The news spread around the Brazilian implantodontics, which promptly recognized it was something innovative. Arcsys has broken paradigms, set professors and students out to think about new possibilities, proposed and proved that it is possible to do more and better for planning, aesthetics, function and health. Two years after its commercial introduction in Brazil, Arcsys has made professionals able to minimize exhaustive steps, plan with more resources and reduce the volume of components in stock, rediscovering simplicity.

Simultaneously to the creation of Arcsys, we deepened our knowledge on the fascinating world of bone regeneration. Understanding what the Frictional Morse Taper is and how it works

Frictional Morse Taper is a fixation system normally used in mechanics when one needs to unite, in a stable way, two bodies without the need of screws.

It works through the mechanical principle of conic connection, which juxtaposes the cone inside an also conic cavity with a small angle. Therefore, the interface of the metal suffer a micromechanical bracing that is called cold melting. That bracing offers high connecting endurance and can be reverted with a specific technique.

El Cono Morse Friccional es un sistema de fijación comúnmente utilizado en la mecánica cuando se necesita unir, de forma estable, dos cuerpos sin necesidad de tornillos. Él funciona a través del principio mecánico de la conexión cónica, que yuxtaposiciona el cono dentro de una cavidad también cónica de pequeño ángulo. Así, la interfaz del metal sufre un imbricamiento micromecánico llamado soldadura fría. Este imbricamiento ofrece alta resistencia de unión y puede ser revertido con técnica específica.

In the Pure Morse Taper systems, the pressure exerted for the locking of the component into the implant generates a frictional resistance that is enough for a safety connection. The non-use of the passing screw eliminates the problem of an eventual loosening of the screw and favors the performance of the system in the long term, something that is different from what happens with implants with a EH or IH platform. Another important issue for the longevity of the frictional system is the bacterial infiltration and colonization in the component/implant interface. The microbial accumulation around dental implants can lead to a peri-implantitis condition, which, if not adequately treated, may compromise the implant culminating in its loss. Considering the information, according to several studies published, the sealing promoted by the Morse Taper coupling demonstrates to be more favorable as far as bacterial infiltration is concerned.

FGM foldable components are manufactured according to ASTM F138-13a and ABNT NBR ISO 58321:2008 standards, which define the characteristics of their chemical composition, mechanical properties, microstructure and requirement for the application of that material in implantable products. The components were approved in a series of tests for fatigue, statics (compression test) and dynamic fatigue. The angulation of the components can only be carried out with the Arcsys Folding Abutment Device and the Arcsys Angle Referrer.

But, then, can’t it break?

Based on the tests carried out, the limit resistance force reaches an average value of 3067N (folded at 17°).

According to a study by Ferrario et al (2004), the average maximum force applied over a single tooth (first molar) of a heathy male adult individual, is approximately 306N. Therefore, the force needed for there to be any possibility of rupture is 10 times greater than the force applied by the patient.

Is there a risk of loosening up?

When activated according to the protocol (3 complete hits with the Arcsys Abutment Placement Tool) the possibility of its loosening up is null. The Morse Taper guarantees functional and aesthetic stability through time, because the components placed over the Morse Taper implants tend to get tighter with time, differently from the other screw-retained interfaces, which, during mastication, tend to loosen up.

Is the Morse Taper connection safe?

The Arcsys System was idealized to be a pure frictional coupling, solid and without a screw, which, most times, is responsible for the loosening up of prosthetic components, in function of the fatigue process due to the masticatory forces. That frictional concept brings more safety for the patient, because the retention of the prosthetic component on the implant is assured by the continuous action of the masticatory forces, which maintain the component/implant connection in place.

What surface treatment does the Arcsys Frictional Implant receives and how long does osteointegration take?

The Arcsys Frictional Implant receives a finishing through double acid attack (DuoAttack), developed at the University of New York by professor Paulo Guilherme Coelho. That finishing allows for the acceleration of the integration of bone tissues with the surface of the implant when compared to other finishings (acid etching blast and no finishing).

It is important to point out that there is no standardized methodology that defines the osteointegration times for a dental implant, therefore, it is recommended to use the time standards already described in the literature.

Can you explain the interaction of stainless steel materials with grade V titanium?

The ABNT NBR 16044:2012 standard defines the general requirements for dental implants. It specifies several issues, among which, the contact with materials with different chemical compositions. That standard quotes another one (ABNT ISO 21534), which defines that the combination of ASTM F138-13a stainless steel, to be used as the material for the manufacture of the component with the Ti6Al4V which is used in the manufacture of the implant, is considered adequate for the application, allowing for its use in this case without problem.

What allows for single step drilling, without the need of scaling?

The main factors that allow for the perforation in one single step are: the geometry of FGM drills, containing side grooves that are deep enough to reduce the thickness of the material, allowing for fast heat exchange with the cooling liquid; the guiding tip that stabilizes the direction of the instrument penetration and the coating in TiN (Titanium Nitrate) which reduces the friction coefficient between the FGM Drill and the bone tissue.

Los componentes Angulables de FGM son fabricados conforme la norma ASTM F138-13a y ABNT NBR ISO 5832-1:2008, que definen las características de la composición química, propiedades mecánicas, microestructura y requisitos para aplicación de este material en productos implantables. Los componentes fueron aprobados en una serie de pruebas, tanto de fatiga estática (ensayo de compresión), como de fatiga dinámica. La angulación de los componentes solo puede ser realizada con el uso del Angulador Arcsys y el Referenciador Arcsys.

Pero entonces, ¿no existe la posibilidad de rotura?

Con base en las pruebas realizadas, la fuerza límite de resistencia logra el valor promedio de 3067N (angulados a 17º). Según estudio realizado por Ferrario et al (2004), el promedio de la fuerza máxima aplicada sobre un diente unitario (primer molar) de un individuo adulto, saludable y del sexo masculino es de aproximadamente 306N. Siendo así, la fuerza necesaria para que exista la posibilidad de ruptura es 10 veces mayor que la fuerza aplicada por el paciente. ¿Y existe la posibilidad de soltura?

Cuando accionado conforme protocolo (3 golpes completos con el uso del Instrumento para Inserción de Pilares Arcsys) la posibilidad de soltura es nula. La interfaz Cono Morse garante estabilidad funcional y estética a lo largo del tiempo, pues componentes puestos sobre implantes Cono Morse tienden a apretar a lo largo de los años, diferente de las demás interfaces atornillables, que durante la masticación tienden a soltar.

¿La conexión Cono Morse es segura?

El Sistema Arcsys fue idealizado para ser un encaje friccional puro, macizo y sin la presencia del tornillo, el cual es muchas veces el responsable por la soltura de componentes protésicos, en función del proceso de fatiga ocasionada por las fuerzas masticatorias. Este concepto friccional trae mayor seguridad para el paciente, pues la retención del componente protésico en el implante es asegurada por la continua acción de las fuerzas masticatorias, lo que mantiene la conexión componente/ implante segura. ¿Cuál el tratamiento de superficie y el tiempo de oseointegración del Implante Friccional Arcsys?

El Implante Friccional Arcsys posee un tratamiento de doble ataque ácido (DuoAttack), desarrollado en la Universidad de Nueva York, por el Profesor Paulo Guilherme Coelho. Este tratamiento proporciona aceleración de la integración de los tejidos óseos con la superficie del implante cuando comparado a otros tratamientos (choreo con ataque ácido y sin tratamiento). Importante destacar que no hay una metodología estandarizada que define el tiempo de oseointegración de un implante dentario, por lo tanto se mantienen los estándares de espera tradicionales ya descriptos en la literatura. ¿Cómo es la interacción de los materiales acero inoxidable con el titanio grado v?

La norma ABNT NBR 16044:2012 define los requisitos generales para implantes dentarios. En ella son especificadas diversas cuestiones, entre ellas, el contacto con materiales de diferentes composiciones químicas. Planning Planning

The respect for the best positioning for the implant(s) should EN be approached very seriously by the professional. That importance is demonstrated through the extensive clinical and scientific evidence shown in the last decades. Besides, lots of problems present in the daily routines of dentists are related exactly to the incorrect placement of implants.

According to that philosophy, the professional should plan the implantation according to the ideal positioning, respecting the anatomic conditions, the adjacent teeth and structures, aesthetic and mechanical needs, etc. However, the great difference of the Arcsys System is that the positioning does not need to coincide with the long axis of the intermediary abutment, since it can be easily folded to a customized angulation of up to 20°. That way, the surgical planning becomes way simpler and efficient, because the placement of the implant will depend practically exclusively of the anatomic availability of the patient without any aesthetic or mechanic losses with manufacturer pre-folded components.

Define the prosthetic component that will be used, according EN to the planning (screw-retained or cement-retained prosthesis) and the transmucosal height with the Transmucosal Height Measurer Tool. In the case of cement-retained prosthesis, the diameter and height of the component must also be defined.

In both cases, if the transmucosal height is greater than 2.5mm (which will occur when the implant is positioned correctly – infraosseous), it can have its angulation customized. If not, with shorter transmucosal heights (0.5mm/1.0mm/1.5mm), however, there is no possibility for angulation.

To define the angulation of the component, adapt the Angle Referrer to the implant with finger pressure and direct it according to the prosthetic need, to either side. After that definition, remove the Angle Referrer from the implant and attach it to the Abutment Folding Device in its specific place (on the side of the disk with angle markings). Turn the disk until the angulation of the Angle Referrer coincides with the angulation on the disk.

Adapt the chosen prosthetic component on the other side of the disk (without angle markings) and protect it with its Protection Cap (Ø3 or Ø4.2 abutment for cement-retained restoration or abutment for screw-retained restoration).

Close the lever coinciding with the internal niche on the Protection Cap and press down until it completes its full course.

Remove the Prosthetic Component + Protection Cap set from the Abutment Folding Device. If necessary, use the Extractor Screw (positioned on the side of the Abutment Folding Device).

Remove the Protection Cap. If necessary, use again the Extractor Screw.

Position the Prosthetic Component onto the implant and activate it with the Abutment Placement Tool.

Caso contrario, transmucosos más cortos están disponibles (0.5mm/1.0mm/1.5mm), pero sin posibilidad de angulación.

Para definir la angulación del componente, adaptar el Pilar Referencia en el implante con presión del dedo y direccionarlo de acuerdo con la necesidad protésica, para cualesquier lado. Tras esta definición, retirar el Pilar Referencia del implante y adaptarlo en el Dispositivo Angulador, en su local específico (en el lado del disco con grabaciones de ángulo). Girar el disco hasta que la angulación del Referenciador coincida con la angulación del disco.

Adaptar el componente protésico elegido en el otro lado del disco (sin grabaciones de ángulo) y protegerlo con su Capa Protectora (muñón Ø3, muñón Ø4.2 o pilar atornillable). Cerrar la palanca coincidiendo el nicho interno en la capa protectora y ejercer fuerza para abajo, hasta su curso total.

Retirar el conjunto componente protésico + capa protectora del Dispositivo Angulador. Caso sea necesario, utilizar el Tornillo Extractor (posicionado en la lateral del Dispositivo Angulador).

Retirar la Capa Protectora. Caso necesario, utilizar también el Tornillo Extractor.

Posicionar el componente protésico en el implante y activarlo con el Instrumento para Inserción de Pilares.

2 ND OPTION: ACTIVATION OF THE ABUTMENT + PROSTHETIC CROWN SET

2 A OPCIÓN: ACTIVACIÓN DEL CONJUNTO MUÑÓN + CORONA PROTÉSICA.

In case the professional chooses, the selection of the component may be done in the model, with the simple molding of the impression cap of the implant with the Multifunctional Healing Abutment. The selection and angulation will be done in the prosthesis laboratory.

Also, cementing (if that is the option) will be done extra-orally and the activation of the component+prosthetic crown set should be done with the Abutment Placement Tool placed on an acrylic support, made over the prosthetic crown.

Caso el profesional prefiera, la selección del componente podrá ser hecha en el modelo, bastando realizar el moldeo de transferencia del implante con el Cicatrizador Multifuncional. La selección y angulación serán entonces realizadas en el laboratorio de prótesis.

También, la cementación (caso sea la opción del profesional) será extra-oral y la activación del conjunto componente + corona protésica deberá ser hecha con el Instrumento para Inserción de Pilares apoyado en anteparo de acrílico, confeccionado sobre la corona protésica.

Place the Angle Referrer and the Prosthetic Component (for cement- or screw-retained restoration) in the Abutment Folding Device.

The choice of the system is due to the possibility of customizing the angulation of the selected components, which allows for a better positioning for the implants together with a better positioning for the components without the addition of structural, mechanical, aesthetical or functional losses that could negatively affect the final results.

Exodoncia de los elementos remanecientes y rehabilitación bimaxilar. La elección del sistema se debe a la posibilidad de personalizar la angulación de los componentes seleccionados, que permite un mejor posicionamiento de los implantes concomitante a un mejor posicionamiento de los componentes, sin introducir perjuicios estructurales, mecánicos, estéticos o funcionales al resultado final.

In spite of all the technical and philosophical advancements in the prevention of dental loss, there is still a high number of patients with some sort of edentulism. Data from the Brazilian Institute of Geography and Statitics (IBGE) reveal that 16 million Brazilians are totally toothless – numbers that represent 11% of the whole population of the country. Still, according to the same institute, 41.5% of Brazilians older than 60 have already lost their teeth1.

A pesar de los avances técnico-filosóficos objetivando mayor prevención a la pérdida dentaria, el número de pacientes portadores de algún tipo de edentulismo permanece alto. Datos del Instituto Brasileño de Geografía y Estadística (IBGE) desvelan que 16 millones de brasileños son totalmente desdentados – números que representan el 11% de toda población nacional. Aún según el Instituto, el 41,5% de los brasileños con más de 60 años ya perdieron todos los dientes1.

Considering the increase in life expectancy in the country – which spiked from 54 years old in 1960 to 72 in 2012, the number of toothless people may be even greater, since the periodontal disease, which is responsible for great part of edentulism, affects more commonly patients in advanced ages 2-4. It is worth pointing out that even patients with all teeth may be considered inapt for the masticatory function (pre-toothless) when they show advanced dental mobility. Those cases, usually, require intervention by an implantodontics professional “anticipating” tooth loss by means of exodontic maneuvers to preserve the bone tissue in a volume that will allow for the installation of implants 5-6.

Considerando el aumento en la expectativa de vida en el país que saltó de 54 años en 1960, para 72 años en 2012 - el número de desdentados podrá ser aún mayor, una vez que la enfermedad periodontal, mal que responde por gran parte del edentulismo, acomete más comúnmente pacientes en edad avanzada2-4. Vale destacar que aun pacientes con todos los dientes pueden ser considerados inaptos a la función masticatoria (pre-desdentados) cuando presentan mobilidad dentaria avanzada. Tales casos, vía de regla, requieren intervención por parte del implantologista “anticipando” la pérdida de los dientes a través de maniobras de exodoncias, para preservar tejido óseo en volumen que viabilice la instalación de implantes5,6.

TREATMENT

TRATAMIENTO

A non-smoker 53-year-old male patient came to the office with a complaint about dental mobility in all arches. Anamnesis and laboratory exams evidenced a good general health and lack of other illnesses.

The clinical and image exams showed generalized periodontal disease in an advanced stage, affecting all teeth in both arches – except for elements 44 and 45 which did not show mobility. The radiographic analysis allowed to identify a major bone loss at the basis of the maxillary sinuses, a region occupied by molars. That type of bone loss is common to periodontopathic patients and, usually, makes it impossible for the installation of implants in the posterior areas of the maxilla, obliging the professional to incline them to the anterior areas (premaxillary sinus) in case they do not want to carry out “sinus lift” maneuvers. When implants are inclined to the anterior area, there is inevitably, difficulty to obtain an insertion plan for the future prosthesis, requiring the use of angled abutments for the adequate obtainment of function and aesthetics.

Paciente con 53 años, género masculino, no tabaquista, se presentó a la clínica con queja de mobilidad dentaria generalizada en ambos arcos. Anamnesis y exámenes de laboratorio evidenciaron buen estado general de salud y ausencia de otras enfermedades en curso.

Al examen clínico y de imagen se observó enfermedad periodontal generalizada en estado avanzado, con comprometimiento de todos los dientes de ambas arcadas – excepción hecha a los elementos 44 y 45 que no presentaban mobilidad. El análisis radiográfico permitió aun constatar exhuberante pérdida ósea en la base de los senos maxilares, región ocupada por los molares. Este estándar de pérdida ósea es común en pacientes periodontopatas y, rutinariamente, imposibilita la instalación de implantes en áreas posteriores de la maxila, obligando el dentista a inclinarlos para anterior (pre-seno maxilar) caso no se desee realizar maniobras de “sinus lift”. Al inclinarse implantes para la región anterior se genera, inevitablemente, una dificultad en la obtención del plano de inserción para la futura prótesis, exigiendo la utilización de pilares angulados para la obtención adecuada de la función y de la estética.

ANGULATION OF THE ABUTMENTS FOR SCREW-RETAINED RESTORATION

ANGULACIÓN DE LOS PILARES

Although necessary many times, screw-retained folded abutments show clinical problems7. Among the many problems, we can mention: difficulty in installation and screw driving; besides the precocious loss of the initial torque applied to the retention screw resulting in a questionable biomechanical performance, exactly at a region with a high functional demand. Conventional screw-retained folded abutments come with a standard angulation supplied by their manufacturer(s) not offering to the professional the possibility to customize that angulation according to each particular case – another disadvantage.

In this clinical approach, we used the Arcsys System, whose great difference lies on the possibility for customizing the angulation of the abutment which, for being impacted (frictional) does not require screws, avoiding problems with loss of torque, and denoting a important aspect to guarantee better mechanical behavior.

Aunque muchas veces necesarios, pilares angulados atornillables presentan problemas clínicos7. Entre los principales podemos citar: dificultad de instalación y atornillado; además de la pérdida precoz del torque inicial aplicado al tornillo retentor resultando en desempeño biomecánico dudoso justamente en una región de alta demanda funcional. Pilares angulados convencionales presentan aun angulación estándar dada por su(s) fabricante(s) no ofreciéndole al clínico la posibilidad de personalizar la angulación de éstos para cada caso en particular – otro aspecto desfavorable.

En este abordaje clínico, nos utilizamos del Sistema Arcsys cuyo gran diferencial reside en la posibilidad de personalización de la angulación del pilar que, por ser impactado (friccional) dispensa la necesidad de tornillos, evitando problemas de pérdida de torque y denotando importante aspecto garantidor de mejor comportamiento mecánico.

SURGERY

CIRUGÍA

In a single session, the extraction of the upper teeth was performed and four implants were installed – two inclined in the area of second premolars and two straight ones in the area of the lateral incisive.

Installation of the customized abutments and immediate prosthesis five hours later.

Twenty-four hours after the rehabilitation of the superior arch, the professional took care of the antagonist arch extracting the teeth, installing customized implants and abutments, followed by molding and installation of an immediate temporary prosthesis.

En sesión única se procedió a exodoncia de los dientes superiores e instalación de cuatro implantes – siendo dos inclinados en la región de segundos premolares y dos rectos en la región de incisivos laterales. Instalación de los pilares personalizados y prótesis inmediata cinco horas tras la instalación de éstos.

Veinticuatro horas tras rehabilitación de la arcada superior se procedió a actuación en la arcada antagonista con extracción de los elementos dentarios, instalación de implantes y pilares personalizados seguido de moldeo y asentamiento de una prótesis provisoria inmediata.

The immediate prosthesis remained in function for sixty days when, after the confirmation of the osteointegration of the implants, permanent prostheses were installed over the abutments fixed onto the implants.

Las prótesis inmediatas permanecieron sesenta días en función cuando, tras confirmación de la oseointegración de los implantes, prótesis permanentes fueron instaladas sobre los pilares impactados a los implantes.

For the surgical step, the plan was the installation of an implant with immediate load on element 21 and subepthelial connective tissue graft for tissue volume gain in the vestibular area of elements 12 and 22. After the healing period, the aesthetic rehabilitation would consist of 4 crowns and 6 metal-ceramic veneers.

Para la etapa quirúrgica, se planeó implante con carga inmediata en el elemento 21 e injerto de tejido conjuntivo subepitelial para ganancia de volumen tejidual en la región vestibular del 12 y 22. Tras el período de cicatrización, la rehabilitación estética consistiría en 4 coronas y 6 carillas metalo-cerámicas.

Implants have been of great importance for many people who have lost their teeth to trauma, cavities, internal/external radicular reabsorption or root fracture (Quirynen et al., 2005). The biological benefits of not preparing the adjacent teeth for making a fixed partial prosthesis have to be emphasized. The conservation of the dental tissues, the preservation of the vitality of the pulp, the respect to the periodontal area and the maintenance of the residual crest are the most important factors to be considered (Quaranta et al., 2010).

Los implantes han ofrecido un gran servicio para muchas personas que perdieron los dientes debido a trauma, carie, reabsorción radicular interna/externa o rotura de raíz (Quirynen et al., 2005). Los beneficios biológicos de no preparar los dientes adyacentes para una fabricación de prótesis parcial fija deben ser enfatizados. La conservación de los tejidos dentarios, la preservación de la vitalidad de la pulpa, el respeto al periodonto y el mantenimiento de la crista residual son los factores más importantes a ser considerados (Quaranta et al., 2010).

Concerning the area to be rehabilitated through implants, it is known that after the exodontia of a dental element, there is a remodeling of the vestibular bone wall that has to be taken into account during the prosthetic and surgical planning (Araújo and Lindhe, 2005).

The installation of the implant into a fresh exodontic alveolus and the immediate provisioning were proposed as alternatives to maintain the volume and contour of the soft tissue, decreasing the cost and duration of the treatment (Park, 2010). However, the professional must always fill the gap between the surface of the implant and the vestibular wall, searching to prevent or decrease the loss of volume that is inherent to the bone remodeling of the vestibular wall. Another factor that has to be considered is the surgical technique, that, in elastic areas, should, as much as possible, respect the palatal approach and the non lifting of the graft.

Besides the carefull pink aesthetics, the white aesthetics has a fundamental role in the success of the anterior sector rehabilitation treatments. The restorative treatment begins with the appropriate use of temporary restorations that allow for the preparation of the gingival architecture and an appropriate emergency profile (Bulser et al., 2011).

For final restorations, ceramic materials are recommended in aesthetic dentistry, allowing for a biometric approach when they allow for the maintenance of the dental tissue and the use of adhesive systems, achieving longevity and superior aesthetic results (Tirlet et al., 2013).

Ceramic materials have important characteristics that include physical-chemical stability, biological compatibility, compression resistance, reproduction of visual properties and color stability (Pini et al., 2012). However, in face of clinical cases in which there are different substrates, the selection of the restorative material and its thickness are critical (Volpato et al., 2009). Therefore, the multidisciplinary planning of the treatment and the communication with the patient play a crucial role for the aesthetic result of complex cases.

se sabe que tras la exodoncia de un elemento dental hay un remodelado de la pared ósea vestibular que debe ser considerado durante el planeamiento protésico y quirúrgico (Araújo y Lindhe, 2005). La instalación del implante en un alvéolo fresco de exodoncia y la provisionalización inmediata fueron propuestas como alternativas para mantener el volumen y el contorno del tejido blando, disminuyendo los costos y el tiempo de tratamiento (Park, 2010). Sin embargo, se debe siempre rellenar el gap entre la superficie del implante y la pared vestibular, buscando prevenir y/o disminuir la pérdida de volumen inherente al remodelado óseo de la pared vestibular. Otro factor que debe ser considerado es la técnica quirúrgica, que en áreas estéticas, siempre que posible debe respetar la realización de un approach palatino y la no elevaciónde retazo.

Además de los cuidados de la estética rosa, la estética blanca tiene un rol fundamental en el éxito de tratamientos rehabilitadores del sector anterior. El tratamiento restaurador comienza con el uso apropiado de restauraciones provisorias que permitan la preparación de la arquitectura gingival y un apropiado perfil de emergencia (Bulser et al., 2011). Para las restauraciones finales, los materiales cerámicos son indicados en la odontología estética, permitiendo un abordaje biomimético al posibilitar la preservación del tejido dental y el uso de sistemas adhesivos, obteniendo longevidad y resultados estéticos superiores (Tirlet et al., 2013). Los materiales cerámicos poseen importantes características que incluyen estabilidad físico-química, compatibilidad biológica, resistencia a la compresión, reproducción de propiedades visuales y estabilidad del color (Pini et al., 2012). Pero, ante casos clínicos en donde se presentan diferentes sustratos, la selección del material restaurador y su espesor son fundamentales (Volpato et al., 2009). Por lo tanto, el planeamiento multidisciplinar del tratamiento y la comunicación con el paciente desempeñan un papel crucial para el resultado estético de casos complejos.

CASE REPORT

RELATO DEL CASO

42-year old female patient, good systemic condition, came to the Implantodontics Specialization Course at Unique Ensino Odontológico, complaining specially about internal and external reabsorption in element 21 (identified by her private dentist), and consequent need for a dental implant. During the anamnesis, the patient also complained about the darkening of the gums around elements 12 and 22.

During the clinical examination (Figures 1A, 1B and 1C), the professional noticed that the patient had 4 zirconia crowns on the 4 upper incisors, made approximately 6 years ago, and also temporary composite veneers on elements 23 and 24. The cone beam CT scan revealed that there was enough bone height and thickness for the immediate installation of the implant. After diagnostic waxing, the aesthetic rehabilitation plan consisted of 4 metal-ceramic crowns on the incisors (being the one on 21 over an implant) and 6 feldspathic veneers from the canines to the pre-molars.

The first surgical step taken was the minimally traumatic exodontia of element 21 through intrasulcular incision with a 15C blade, periotome and forceps (Figures 2A, 2B and 2C). The exodontia was followed by the curettage of the alveolus and irrigation with 0.12% chlorhexidine. The surgery proceeded according to the manufacturer’s standards (FGM) for the installation of 3.3 x 13mm Arcsys Morse Taper Implant. Sub-instrumentation was carried out through a single drilling with a 2.4mm drill (Figs. 3A and 3B) for 15mm.

In search for the primary stability for the application of immediate load, the drilling was done using the palatal approach technique in order to search for anchorage on the palatal wall of the extraction alveolus. The implant was installed 2mm below the vestibular wall and 5mm from the gingival margin (Fig. 4A, 4B and 4C). As a the prosthetic component, a 3.3 x 6 cement-retained foldable abutment was installed (Figs. 5A and 5B), it was activated by means of three hits with the Arcsys Abutment Placement Tool. However, thanks to the correct tridimensional placement of the implant, the angulation of the component was not necessary. The surgical part of the process was followed by the subepithelial conjunctive grafts in the areas of element 12 and 22, to mask the color of the roots that were showing from below the gingiva (Figs. 6A and 6B) and the filling of the gap left between the surface of the implant and the vestibular wall with xenogeneic biomaterial (Fig. 7).

The temporary prosthesis was made according to an old crown (Fig. 8) and captured in a burn-out coping of the abutment to cementretained restoration with the help of a composite guide manufactured previous to the surgery (Fig. 9). The crown was united to the coping with flowable composite, resulting in an adequate emergency profile (Figs. 10A and 10CB).

Paciente con 42 años de edad, sexo femenino, buena condición sistémica, se presentó en el curso de especialización en Implantología Unique Ensino Odontológico, con queja principal de reabsorción interna y externa del elemento 21 (identificada por el dentista particular), y consecuente, necesidad de realización de implante dentario. Durante la anamnesis la paciente también se quejó de oscurecimiento de la encía en la región de los elementos 12 y 22.

Al examen clínico (Figs. 1A, 1B y 1C), se constató que la paciente poseía 4 coronas en circonia en los 4 incisivos superiores, realizadas hace aproximadamente 6 años, y también carillas provisorias en resina compuesta en los elementos 23 y 24. Tras la realización de la tomografia computadorizada cone beam, se constató que existían altura y espesor óseo suficientes para instalación inmediata de implante.

Tras enceramiento diagnóstico, el planeamiento rehabilitador estético consistió en 4 coronas metalo-cerámicas en los incisivos (siendo la corona de la región del 21 sobre implante) y 6 carillas feldespáticas de caninos a pre-molares.

El primer paso quirúrgico realizado fue la exodoncia minimamente traumtática del elemento 21 a través de incisión intrasulcular con lámina 15C, periótomo y fórceps (Figs. 2A, 2B y 2C). Tras la exodoncia, fue hecho el legrado del alvéolo e irrigación con clorexidina 0,12%. La cirugía procedió de acuerdo con lo preconizado por la fabricante (FGM) para instalación de Implante Cono Morse Arcsys 3.3x13mm. Fue realizada la subinstrumentación a través de fresado único con fresa 2.4mm (Figs. 3A y 3B) hasta la profundidad de 15mm. Buscando la estabilidad primaria para realización de la carga inmediata, el fresado fue realizado a través de la técnica del approach palatino, buscando anclaje en la pared palatina del alvéolo de extracción. El implante fue instalado 2mm debajo de la pared vestibular y 5 mm del margen gingival (Figs. 4A, 4B y 4C).

Como componente protésico fue instalado un muñón angulable para prótesis cementada de 3.3x6 con cinta de 3,5mm (Figs. 5A y 5B), que fue activado a través de 3 golpes como martelete activador Arcsys. Sin embargo, por el correcto posicionamiento tridimensional del implante, no fue necesaria la angulación del componente. La parte quirúrgica siguió con injertos conjuntivos subepiteliales en la región del elemento 12 y 22, para mascarar la coloración de las raíces que estaban transpareciendo por debajo de la encía (Figs. 6A y 6B) y rellenado del gap dejado entre la superficie del implante y la pared vestibular con biomaterial xenógeno (Fig. 7).

El provisorio fue confeccionado a partir de la corona antigua (Fig. 8) de la paciente y capturado en un cilindro calcinable del muñón angulable con auxilio de una guía de resina compuesta confeccionado previamente a la cirugía (Fig. 9). La corona fue unida al coping con resina compuesta y complementado con resina fluida, dando un adecuado perfil de emergencia (Figs. 10A y 10B).

60 days after the surgery (Fig. 11), a CT scan was requested to evaluate the peri-implantar bone maintenance/neo-formation (Fig. 12) and begin the permanent rehabilitation procedures, which consisted in the change of the old crowns over elements 11, 12 and 22 and manufacture of veneers on elements 13, 14, 15, 23, 24 and 25. In that case, it was not necessary to carry out the diagnostic waxing, because the patient wanted to keep the size and format of the teeth and old prosthesis. Teeth 11, 12, 21 and 22 were prepared on a plaster model and the veneers in stock were adapted over those elements, maintaining the same length of the old crowns. Then, a composite guide was manufactured on the incisal edge of teeth 13 to 23 for the veneers to be adapted in the mouth over the preparations (Fig. 13).

The gingival conditioning of 21 was carried out in order to copy the format of the gingival zenith of 11. Besides, the impression cap of the molding was customized with flowable composite in order to copy the gingival architecture during the tissue conditioning process.

After that, the molding was accomplished in 2 steps by means of the double cord technique (Fig. 14) The choice for metal-ceramic crowns is related to the presence of a cast metallic nucleus in teeth 12 and 22 (Figures 15A and 15B). In case the choice were dissilicate, it would be necessary to use different layers in the crowns and veneers to mask the darkened substrates, which could generate a color mismatch between the pieces, therefore, the option chosen was to do all the work in feldspathic ceramic.

The following section comprised the metallic coping transference molding and the molding of the preparations of the veneers (Fig. 16) and color selection that, with the agreement of the patient was set as BL4. 10 days later, the pieces (Fig. 17A, 17B and 17C) that were made by Laboratório Vieira (Curitiba/PR, Brazil) were checked, and the first dry proof for the verification of the fitting of the pieces and the wet proof with Allcem Veneer Try-in (FGM) were carried out.

Tras 60 días de pos-operatorio (Fig. 11) fue solicitada una tomografia para evaluar el mantenimiento/neoformación ósea peri-implantar (Fig. 12) e iniciar los procedimientos de la rehabilitación definitiva, que consistia en el cambio de las coronas antiguas de los elementos 11, 12 y 22 y confección de carillas en los elementos 13, 14, 15, 23, 24 y 25. En este caso no fue necesaria la realización de enceramiento diagnóstico, pues el deseo de la paciente era mantener el tamaño y formato de los dientes y prótesis antiguas. Los dientes 11, 12, 21 y 22 fueron preparados en modelo de yeso y carillas de stock fueron adaptadas sobre estos elementos, manteniendo el mismo largo de las coronas antiguas. En seguida un guía de resina compuesta fue confeccionado apoyado en el borde incisal del 13 hasta el 23 para que las carillas pudieran ser adaptadas en boca sobre los preparos (Fig. 13) Fue realizado el acondicionamiento gingival del 21 para copiar el formato del cenit gingival del 11. Además, el transferente de moldeo fue personalizado con resina fluida, para copiar la arquitectura gingival adquirida durante el proceso de acondicionamiento tejidual.

En seguida, moldeo fue hecho en 2 pasos con la técnica del doble hilo (Fig. 14). La opción de trabajar con coronas metalocerámicas está relacionada a la presencia de núcleo metálico fundido en los dientes 12 y 22 (Figs. 15 A y 15B). Caso la opción fuera trabajar con disilicato, sería necesario trabajar con pastillas diferentes en las coronas y carillas para mascarar los sustratos oscurecidos, lo que podería generar diferencia de color entre las piezas, por ello se optó por realizar todo el trabajo en cerámica feldespática.

En la sesión siguiente se realizó el moldeo de transferencia de los cilindros metálicos, moldeo de los preparos de las carillas (Fig. 16) y selección de color, que en acuerdo con la paciente fue establecida en BL4.

Tras 10 días, las piezas (Figs. 17A, 17B y 17C), que fueron confeccionadas por el Laboratorio Vieira (Curitiba/PR, Brasil) fueron probadas, siendo hecha primeramente la prueba seca para verificación de adaptación de las piezas y la prueba húmeda con Allcem Veneer Try-in (FGM).

For the preparation of the veneers, the professional used 10% hydrofluoric acid Condac Porcelana 10% (FGM) for 1 minute (Fig. 18A), 37% phosphoric acid Condac 37 (FGM) application for removing residue of the acid etching (Fig. 18B), and application of silane Prosil (FGM) (Fig. 18C) and Ambar (FGM) adhesive. For the treatment of the tooth, acid etching was carried out for 30 seconds on enamel and for 15 seconds on the dentine with Condac 37 (FGM), application of Ambar (FGM) adhesive and Allcem Veneer (FGM) cement in color A1. The cementation of the crowns was made with dual resinous cement Allcem (FGM) following the manufacturer’s instructions. In spite of the complexity of the case, the treatment was carried out in a relatively short time, needing only four months between the surgery for the installation of the implant untill the cementation of the permanent pieces (Figs. 19A, 19B and 19C).

Para el preparo de las carillas se utilizó el ácido fluorhídrico al 10% Condac Porcelana 10% (FGM) por 1 minuto (Fig. 18A), aplicación de ácido fosfórico al 37% Condac 37 (FGM) para remoción de los residuos del ataque ácido (Fig. 18B), aplicación del silano Prosil (FGM) (Fig. 18C) y adhesivo Ambar (FGM). Para el tratamiento del diente fue realizado grabado ácido por 30 segundos en el esmalte y 15 en la dentina con Condac 37 (FGM), aplicación del adhesivo Ambar (FGM) y cemento Allcem Veneer (FGM) en el color A1. Ya la cementación de las coronas fue hecha con cemento resinoso dual Allcem (FGM) siguiendo las instrucciones del fabricante. A pesar de la complejidad del caso, el tratamiento fue realizado en un período relativamente corto, necesitando solamente cuatro meses entre la cirugía de instalación del implante hasta la cementación del trabajo definitivo (Figs. 19A, 19B y 19C).

Aesthetic oral rehabilitation is extremely predictable, once the multidisciplinary reverse planning steps are respected.

The primary function of a dental implant is to act as a pillar for EN a prosthetic element, similarly to a root and a crown of natural teeth.

Any success criteria, therefore, must include not only the primordial support of a functional prosthesis, but also the implant in perfect osseointegration. It must not only match prosthesis clinical criteria that are beyond patient satisfaction with aesthetic appearance, which is necessary in clinical practice, but also respond to technical criteria of good functional and peri-implantar health. The restorative dentist designs and manufactures a prosthesis similar to one that is supported by a tooth, and as such, in many times, treats the dental implant in a similar way as a natural tooth. However, the fundamental differences in the support system between these entities must be acknowledged and the concept of what is considered to be a successful implant, must consider more than only the implant’s survival or the implant’s failure.1 The treatment possibilities with odontological implants have revolutionized the field of oral rehabilitation in the last years, bringing many advantages over traditional prosthesis. For many years, professionals followed a surgical protocol that advocated for the continuity of an implant with no load for a determined period, in order for osseointegration to occur. However, experimental studies have proven that immediate load in implants also promotes the formation of peri-implantar bone tissue during time, in a way comparable to that observed in conventional load implants3.

Still, one of the biggest complaints presented by professionals and patients was the duration of therapy and the wait between the surgical and the prosthetic stage, with a consequent increase in the patient's suffering during treatment. This limitation was one of the main reasons why researchers started looking for a viable alternative with a shorter treatment duration. From this need, arose concepts of placing immediate load implants and early load implants. Although the definition varies from author to author, immediate load would be the implant activation and exposition to the mouth simultaneously to the placement of the prosthesis right after the installation of the implants, and early load in an intermediate time between the placement of the implant and the time interval suggested by Branemark4.

Roccuzzo et cols (2009) assessed the evolution of immediate and early load activated implants obtaining a success rate compatible to that of conventional7 therapeutics, just like Callucci et cols (2004), separated the works of Branemark protocol rehabilitated patients, in conventional, early, and immediate load. The results showed a success rate ranging from 97.2 to 98.7% for cases of conventional load. They observed a success rate of 98 to 98.6% for early loads and of 95.4% for immediate load8.

Due to anatomical limitations, implants installed in the posterior regions of the arches are smaller than those installed in the anterior regions. Many recent studies have concluded that short implants can be safely used as prosthetic support in the rehabilitation of lost teeth, with success and longevity similar to long implants, following an activation protocol under primary stability9.

Another great challenge for researchers has been to minimize periimplantar bone loss, very common after the installation of the majority of implant systems. One of the main flaws studied in implantodontology is marginal bone loss around the implants, that had even come to be accepted as natural, called saucerization. This bone loss, however, is highly associated with bacterial colonization of the implant’s structures. The colonization can happen in the external surface as well as in the internal areas of the implant. Internally, bacterial colonization is related to an existing space between the prosthetic part of the system (prosthetic intermediary) and the endosteal part (implant). The internal colonization as well as the external one relate to a disease called peri-implantitis.10 Diniz Pimental et cols (2010) assessed bacterial micro infiltration in morse taper-type implants, with frictional intermediaries, according to the authors, the bacteria were not capable of colonizing the inside of the implants. The authors state that the space between the intermediary and the implant, in this particular system, is incapable of allowing bacteria penetration, be it from the external part to the internal part, or from the internal part to the external part.10

La función primaria de un implante dentario es actuar como un ES pilar para un elemento protésico, a la semejanza de un raíz y corona de dientes naturales. Cualquier criterio de éxito, por lo tanto, debe incluir además del soporte primordial de una prótesis funcional, el implante en perfecta oseointegración. Deberá corresponder a los criterios clínicos de la prótesis que estén además de la satisfacción del paciente con la apariencia estética que es necesaria en la práctica clínica, pero también respondiendo a los criterios técnicos de buena salud funcional y periimplantar. El dentista restaurador proyecta y fabrica una prótesis semejante a una apoyada por un diente y, como tal, muchas veces evalúa y trata el implante dentario de forma semejante a un diente natural. Sin embargo, las diferencias fundamentales en el sistema de soporte entre esas entidades deben ser reconocidas, y el concepto de lo que se considera ser el éxito del implante, debe ser considerado más que solamente la sobrevivencia del implante y la falla del implante.1 Las posibilidades de tratamiento con implantes odontológicos revolucionaron el área de rehabilitación bucal en los últimos años, trayendo una serie de ventajas sobre las prótesis convencionales.

Durante años, fue seguido un protocolo quirúrgico que preconizaba el mantenimiento del implante sin carga por un determinado período para que ocurriera la oseointegración. Pero, estudios experimentales han mostrado que la carga inmediata en implantes también promueve formación de tejido óseo peri-implantar a lo largo del tiempo, de manera comparable a aquel observado para implantes con carga convencional.3 Sin embargo, una de las mayores quejas presentadas por profesionales y pacientes era el tiempo de terapia y espera entre la fase quirúrgica y la protésica, con consecuente aumento del sufrimiento del individuo bajo tratamiento. Esta limitación fue uno de los principales motivos que llevaron investigadores a buscar una alternativa viable con menor tiempo de duración del tratamiento. De esta necesidad surgieron conceptos de colocación de implantes en carga inmediata y carga precoz. Aunque la definición varíe de autor para autor, carga inmediata sería la activación del implante y su exposición al medio bucal concomitante con la colocación de prótesis enseguida de la instalación de los implantes, y carga precoz en un período intermediario entre la colocación del implante y el intervalo de tiempo propuesto por Branemark.4 Roccuzzo et cols (2009) evaluaron la evolución de implantes activados en carga inmediata y precoz obteniéndose índice de éxito compatible a la terapéutica convencional,7 así como Gallucci et cols (2004), separaron trabajos de pacientes rehabilitados por protocolo de Branemark, en carga convencional, precoz e inmediata. Los resultados demostraron índices de éxito del 97,2 al 98,7% para casos de carga convencional. Observaron del 98 al 98.6% de éxito para carga precoz y el 95,4% para carga inmediata.8 Debido a las limitaciones anatómicas, implantes instalados en las regiones posteriores de los arcos son frecuentemente menores que aquellos instalados en las regiones anteriores. Muchos trabajos recientes han concluido que implantes cortos pueden ser utilizados con seguridad como soporte protésico en la rehabilitación de dientes perdidos, con éxito y longevidad semejante a los implantes largos, siguiendo protocolo de activación bajo estabilidad primaria.9 Otro gran reto de los investigadores han sido minimizar la pérdida ósea peri-implantar, bastante común tras la instalación de la mayoría de los sistemas de implantes. Una de las principales fallas estudiada en implantología es la pérdida de hueso marginal alrededor de los implantes, que llegó a ser aceptada como natural, llamada de saucerización. Esa pérdida ósea, sin embargo, está muy asociada a la colonización bacteriana de las estructuras del implante. La colonización puede ser tanto de la superficie externa, como de los espacios internos del implante. Internamente, la colonización bacteriana está relacionada con un espacio existente entre la porción protésica del sistema (intermediario protésico) y la porción endósea (implante). Tanto la colonización externa como la interna se relaciona con la enfermedad denominada periimplantitis .10 Diniz Pimentel et cols (2010) evaluaron microinfiltración bacteriana en implantes del tipo cone morse, con intermediarios friccionales, según los autores, las bacterias no fueron capaces de colonizar el interior de los implantes. Los autores afirman que el espacio entre el intermediario y el implante, en ese sistema estudiado, es incapaz de permitir la penetración de bacterias, sea del medio interno para el externo, sea del externo para el interno.10 FGM 2018 | EDITION | EDICIÓN 01 | 35

OBJETIVOS

This research has as its objective to assess the success rate of a new concept in frictional implants, through immediate load and early load rehabilitation, in patients of osseointegrated implants for unitary, partial or total cases, of daily clinical routine.

El presente trabajo tiene como objetivo evaluar el índice de éxito de un nuevo concepto de implantes friccionales, a través de rehabilitaciones de carga inmediata y precoz, de pacientes de implantes oseointegrables para casos unitarios, parciales y totales, de la rutina clínica diaria.

MATERIALS AND METHODS

MATERIALES Y MÉTODOS

This study was conducted in a private clinic taking into consideration the treatment protocol used in the Manari de Ilhabela Clinic.

The patients have been selected taking into consideration the need to use osseointegratable implants in cases of total, partial and unitary oral rehabilitation, showing systemic and local indication for implant surgery.

All the installed implants (Arcsys) correspond to a new design, having as macrogeometric characteristics the hybrid shape, parallel walls that are cone-like in their apical portion, that provide excellent primary stability levels. All used implants have been manufactured with level V titanium and double acid attack activated surface. The diameters and lengths of the installed implants have been determined by the individual characteristics of each case, using 5, 8, 9, 11 and 13 millimeters lengths and diameters of 3.3, 3.8, 4.3, 5.0 and 6.0 millimeters.

The surgical protocol for implant installation has initially used drilling sequences suggested by the manufacturer, however, as a high level of insertion torque was observed, we used the escalated drill sequence, in order to keep the surgical torque levels between 30 and 45N preferably, despite the safety that the system provides for high torque (60N). The system does not indicate Countersink, neither the use of the drill’s bur. The surgical preparation was performed with abundant saline solution irrigation.

Implant installation was performed with low rotation (20Ncm), the implants settlement was performed with calibrated surgical torque meter, and the final torque of each installation was marked for later result assessment. The implants were installed in the alveoli after extraction or healed alveoli, and associated or not, to reconstructive techniques, immediately activating the prosthesis after its installation (immediate load) or after the healing period of 45 days (early load).

There was a follow-up of the patients for a period of up to 12 months after installation of the prosthetic rehabilitation over the implants. The used success criteria were based on clinical and radiographic conditions, according to the Pisa Consensus Conference (MISCH et al., 2008),11 responding to the patterns: no pain or discomfort during its function, lack of mobility, radiographic bone loss lower than 2 millimeters from the initial surgery, excluding exudates.

Este estudio fue conducido en clínica particular, llevando en consideración el protocolo de atendimiento instituido en la Clínica Manari de Ilhabela.

Los pacientes fueron seleccionados llevando en consideración la necesidad de la utilización de implantes oseointegrables en la rehabilitación oral de casos unitarios, parciales y totales, debiendo presentar indicación local y sistémica para la cirugía de implantes.

Todos los implantes instalados (Arcsys) corresponden a un nuevo diseño, teniendo como características macro-geométricas el formato híbrido, paredes paralelas con conicidad en su porción apical, que proporcionan excelente grado de estabilidad primaria. Todos los implantes utilizados fueron confeccionados en titanio de grado V y superficie activada con doble ataque ácido. El diámetro y el largo de los implantes instalados fueron determinados por las características individuales de cada caso, siendo utilizados largos de 5, 8, 9, 11 y 13 milímetros y diámetros de 3.3, 3.8, 4.3, 5.0 y 6.0 milímetros.

El protocolo quirúrgico para instalación de los implantes utilizó inicialmente las secuencias de fresado recomendadas por el fabricante, pero como fue observado un elevado grado de torque de inserción, instituimos la utilización de secuencia escalonada de fresas, para mantener los niveles de torque quirúrgico entre 30 y 45N preferencialmente, a pesar de la seguridad que el sistema ofrece a altos torques (60N). No son indicadas en el sistema la preparación de courtersink, ni la utilización del macho de rosca. Toda la preparación quirúrgica fue realizada con irrigación abundante de solución salina.

La instalación de los implantes fue realizada en baja rotación (20 Ncm) y el asentamiento final de éste realizado con torquímetro quirúrgico calibrado, y el torque final de cada instalación marcado para posterior evaluación de resultados. Los implantes fueron instalados en alvéolos pos exodoncia o alvéolos cicatrizados y asociados o no a técnicas reconstructivas, siendo la activación protésica realizada inmediatamente después de su instalación (carga inmediata) o después del período cicatricial de 45 días (carga precoz).

Los pacientes fueron acompañados por un período de hasta 12 meses después de la instalación de la rehabilitación protésica sobre los implantes. Los criterios de éxito utilizados fueron basados en condiciones clínicas y radiográficas, conforme establecido en el Tratado de Consenso de Pisa (MISCH et al., 2008),11 respondiendo a los estándares: sin dolor o molestia durante la función, ausencia de movilidad, pérdida ósea radiográfica inferior a 2 milímetros a partir de la cirugía inicial, sin historia de exudados.

This study was based on the clinical analysis of 100 implants installed in 38 patients (16 men and 22 women) with a clinical follow-up from the moment of the implant’s installation, radiographic control on the 45th day, and 30 days after the installation of the prosthetic restoration. When the implants were installed, the age range was from 24 to 83 years old. Out of the 100 installed implants, 29 (29%) were: installed immediately after tooth extraction and 08 (08%) needed some sort of bone grafting. From the total, 19 (19%) immediate prosthetic load, compared to 81 (81%) with premature load. There was a loss of 1 implant, totaling a global success rate of 99%.

The lost element was lost in a case of an implant that was installed immediately after extraction and immediate load. The implant was replaced promptly after its removal with new immediate load and was successful. The definitive prosthesis has already been installed.

Este estudio fue basado en el análisis clínico de 100 implantes instalados en 38 pacientes (16 hombres y 22 mujeres) con acompañamiento clínico desde la instalación del implante, control radiográfico en el 45º día, y 30 días después de la instalación de la rehabilitación protésica. En el momento de la inserción de los implantes, el rango etario era de 24 a 83 años. De los 100 implantes instalados, 29 (29%) fueron realizados inmediatamente después de exodoncia y 08 (08%) necesitaron algún tipo de injerto óseo. Del total, 19 (19%) recibieron carga protésica inmediata, comparado con 81 (81%) en carga precoz. Hubo la pérdida de 1 implante, representando una tasa global de éxito del 99%.

El elemento perdido, ocurrió en un caso de implante inmediatamente después de exodoncia y carga inmediata. El implante fue repuesto inmediatamente después de su retirada con nueva carga inmediata y obtuvo éxito, ya habiendo sido realizada su prótesis definitiva.

Regarding success in implant therapeutics, many studies only take into consideration the implants’ osseointegration.

However, a successful procedure happens when the aesthetic and functional criteria that the patient and the professional desire are met. Martin, W. et al describes as factor of success not only the implants stability, but also hygiene capability and periimplantar tissue maintenance, good placement and aesthetic result. The prosthetic solution of the Arcsys System is highly effective in this sense, since the individualization of the foldable component allows for the manufacturing of more adequate prosthesis, making aesthetics, function and hygiene easier.

In these factors, inter-tooth space is also evaluated; the space of dental elements may vary making proper installation harder and implant placement can harm the prosthetic result. For this, a proper system must have a range of different diameters for better placement in the edentulous space. The used Arcsys implants in this study presents wide measurements for height and thickness; they are 3.3, 3.8, 4.3, 5.0, and 6.0 mm thick and 5.0, 8.0, 9.0, 11.0 and 13.0 mm long.

Local and systemic factors can influence dental implant success rate. Adequate patient selection, implant design, adequate implant material, good surgical technique, and restorative treatment are crucial for dental implant procedure success12.

Implants with less than 10 mm of length have been associated with high failure rates in the past (GOODACRE et al, 1999)13, however, a review of the literature on success rates in short implants of articles published between 1991 and 2003 has shown an average success rate of 85.3% against 90% for long implants (MISCH, 2006)14.

Considering the types of implants in the market, the ones of morse taper interface are clearly more advantageous. Several articles have shown higher maintenance of the alveolar bone crest when compared to the ones of hexagonal connections.

This is because, in hexagonal connections, the formation of biological distances happens from the platform of the implant in apical direction. Now, in Morse Taper implants, such formation of biological distances happens around the prosthetic component, which will, consequently, enable bone crests’ maintenance, since there will be no saucerization process that is inherent to the healing process in hexagonal implants. Therefore, today we can bring teeth and implants closer or bring implants closer to other implants without fear of bone or papilla loss15.

Relating bone loss to implants’ types may not be conclusive yet. Even though, it seems to suggest that morse taper implants cause lower bone loss. The reason for these results is not yet clarified. It is believed, on the other hand, in an association of favorable factors to this system, mechanic as well as biological.10 The evaluation of these modifications in in vitro and in vivo researches is extremely important before a commercial launch, a procedure that is often abandoned by some manufacturers.

Cuando se trata de éxito en terapéutica de implantes, muchos trabajos llevan en consideración solamente la oseointegración de los implantes. Sin embargo, el éxito del trabajo se da cuando logramos los criterios estéticos y funcionales deseados por el paciente y el profesional. Martin, W. et al describen como factores considerados de éxito, además de la estabilidad del implante, capacidad de higienización y mantenimiento de los tejidos periimplantares, buen posicionamiento y resultado estético. La solución protésica del Sistema Arcsys, se muestra extremamente eficiente en ese sentido, pues la individualización del componente angulable, permite la realización de prótesis más adecuadas, facilitando la estética, función e higienización.

En estos factores, también es evaluado el espacio interdental; el espacio entre los elementos dentales puede variar dificultando la instalación adecuada y disposición de implantes puede perjudicar el resultado protésico. Para eso un sistema adecuado debe contener un abanico de diámetros variados de implantes para mejor disposición del espacio edéntulo. Los implantes Arcsys utilizados en este trabajo, presentan medidas en altura y espesor bastante amplia; con 3.3, 3.8, 4.3, 5.0 y 6.0 mm de espesor y 5.0; 8.0; 9.0;11.0 y 13.0 mm de largo.

Factores locales y sistémicos pueden influir la tasa de éxito de los implantes dentarios. Selección adecuada de pacientes, planeamiento de tratamiento, diseño de los implantes, materiales de implantes adecuados, buena técnica quirúrgica y tratamiento restaurador son cruciales para el éxito del procedimiento de implantes dentarios.12 Los implantes con menos de 10 mm de largo fueron en el pasado asociados con altas tasas de no éxito (GOODACRE et al, 1999)13, sin embargo una revisión de literatura sobre la tasa de éxito de los implantes cortos realizada en artículos publicados entre 1991 y 2003 presentó un promedio del 85,3% de éxito en contra del 90% para implantes largos (MISCH,2006).14 Considerando los tipos de implantes presentes en el mercado, los de interfaz cono morse son claramente los más ventajosos.

Diversos artículos científicos muestran mayor mantenimiento de la crista ósea alveolar cuando comparado a los de conexiones hexagonales. Eso porque, en implantes hexagonales, la formación de las distancias biológicas ocurre desde la plataforma del implante en dirección apical. Ya en los implantes cono morse, tal formación de distancia biológica se dará alrededor del componente protésico, lo que, consecuentemente, posibilitará el mantenimiento de las cristas óseas, visto que no ocurrirá saucerización inherente al proceso cicatricial en implantes hexagonales. Con eso, hoy conseguimos aproximar más dientes e implantes o implantes a otros implantes sin miedo de que haya pérdida ósea y pérdida de papila.15 Relacionar la pérdida ósea con tipos de implantes aún no puede ser conclusivo. Aun así, parece ser sugestivo que los implantes del tipo cone morse causan una menor pérdida ósea. El por qué de esos resultados aún no está completamente aclarado. Se cree, sin embargo, en una asociación de factores favorables a ese sistema, tanto mecánicos, como biológicos.10 La evaluación del desempeño de estas modificaciones en investigaciones in vitro e in vivo se hace de suma importancia antes de un lanzamiento comercial, procedimiento que no raro es abandonado por algunos fabricantes. Sin embargo, lo que determina el gran potencial de éxito de un implante es su potencial innovador

Despite the limitations in this study, we can conclude that the clinical performance of the Arcsys implant, when evaluated in the standards suggested by Misch, has shown itself compatible with the success levels of world implant therapeutics, independently from the patient’s gender, age range, locations, and prosthetic load protocol.

A pesar de las limitaciones de este estudio, podemos concluir que el desempeño clínico del implante Arcsys, cuando evaluado en los estándares propuestos por Misch, se mostró compatible a los niveles de éxito de la terapéutica de implantes mundial, independiente del género de los pacientes, rango etario, localización y protocolo de carga protésica.

INTRODUCTION

INTRODUCCIÓN

It is a consensus that prosthetic planning is highly important facing a rehabilitation1,2 case. However, when identifying that the situation is not favorable to the stablished planning, the dentist must look for alternatives, in a way as to maintain the original programing and not compromise the final result1,3.

Such alternatives cover pre-operatory procedures that are consistent with preparing the region in which the rehabilitation will occur, as for example, enough space in the bone tissue to anchor the implant that will support the restoration, regarding its dimensions as much as inclination and location that are compatible with the planning’s outline. We can mention as resources, orthodontic movements, grafting surgeries, separation of nerves/vases or even extraction of the teeth. The cost benefit relation must guide the

Pero, al identificar que el escenario no es favorable con el planeamiento establecido, el dentista deberá buscar alternativas, de forma a mantener la programación original y no comprometer el éxito en el resultado final.1,3 Tales alternativas, comprenden procedimientos pre-operatorios condecentes en preparar la región donde la rehabilitación será conducida, como por ejemplo, espacio suficiente en el tejido óseo para ancorar el implante que soportará la restauración, tanto respecto a sus dimensiones como localización e inclinación compatibles con el delineado del tratamiento. Podemos citar como recursos movimientos ortodónticos, cirugías para injerto, separación de nervios/vasos o incluso exodoncias. La relación costo X beneficio deberá guiar al profesional durante esta decisión, teniendo siempre en mente que la rehabilitación deberá ser conducida con la premisa de que

professional during this decision, always having in mind that the rehabilitation must be conducted with the premise that it will cause the least trauma possible, shorter time of treatment and lower financial cost.

Arcsys Implant System has innovative characteristics so that the dentist can aim at carrying out his/her rehabilitation in a more agile way, but not necessarily less successful biologically, mechanically or aesthetically. Such resourcefulness is found in the prosthetic components folding capability, be it for screw- or cement-retained prosthesis, in accordance with the planning, and without hurting the component’s functional and aesthetics characteristics.

In the clinical case that will be presented now, the challenge is to rehabilitate the right posterior superior sextant replacing the missing teeth 14 and 16, without subjecting the patient to previous orthodontic treatment neither extract teeth that are considered to be biomechanically adequate.

El Sistema de Implantes Arcsys posee características innovadoras para que el profesional busque justamente una conducción ágil en sus rehabilitaciones, pero no por esto menos exitosas biológicamente, mecánicamente o estéticamente. Tal ingenio está en la especial capacidad de angulación de los componentes protésicos, sean para prótesis atornilladas como para cementadas, de acuerdo con lo exigido por el planeamiento y sin detrimento a las características funcionales y estéticas del componente.

En el caso clínico que será presentado enseguida, el reto sería rehabilitar el sextante superior posterior derecho con sustitución de los dientes ausentes 14 y 16, sin someter el paciente a un tratamiento ortodóntico previo ni tampoco extraer dientes considerados biomecánicamente adecuados. Tal terapia solamente fue posibilitada y ejecutada con precisión y excelencia, gracias a la poderosa herramienta que el clínico dispone actualmente: un componente protésico pasible de personalización de su angulación.

TREATMENT

TRATAMIENTO

The patient is a 57-year old female individual, with main complaint of absence of two superior teeth on the right side (Figs. 1 and 2).

The prosthetic spaces were consistent with the manufacturing of prosthetic crowns for elements 14 and 16. However, during the analysis of the radiographic images, it is learnt that the implants could not be installed with their long axis coinciding with the prosthetic crowns long axis (Fig. 3).

As previously related, during the case-study it was requested that no viable maintenance tooth were extracted and the patient did not demonstrate to be willing to consider orthodontic treatment.

Pero, durante análisis de las imágenes radiográficas, se percibe que los implantes no podrían ser instalados con su largo eje coincidente con el largo eje de las coronas protésicas (Fig. 3).

However, on the contrary of what several colleagues would consider in face of this scenario, the solution is simple and practical as long as a rehabilitation system that possesses the required demands is used. Facing this, the Arcsys System, was chosen.

The main reason for this choice was the possibility to compensate the angles in which the devices would necessarily be installed. On this matter Arcsys in unbeatable since no other system allows to fold the prosthetic component up to 20o without damaging the rehabilitation mechanical and aesthetical response4,5,6.

During the perforation, surgical radiographies were taken to ensure its path and consequently the integrity of the adjacent teeth roots (Figs. 4 and 5). Element 15 endodontic treatment was carried out immediately after the installations and its control suggest a regression of the infectious process.

el tratamiento ortodóntico. Pero, al contrario de lo que varios compañeros juzgarían ante este escenario, la solución es sencilla y práctica, desde que utilizado un sistema para rehabilitación que comprenda las demandas requeridas. Ante esto, el Sistema Arcsys, fue electo para conducción del caso.

El principal motivo de esta elección fue la posibilidad de compensación de la angulación que, obligatoriamente, los dispositivos serían implantados. En este requisito Arcsys es imbatible pues ningún otro permite angular el componente protésico hasta 20° sin perjudicar las respuestas mecánicas y estéticas de la rehabilitación4,5,6.

Durante las perforaciones, radiografías transoperatorias fueron hechas para garantizar su direccionamiento y consecuentemente la integridad de las raíces de los dientes adyacentes (Figs. 4 y 5). El tratamiento endodóntico del elemento 15 fue realizado inmediatamente tras las implantaciones y se control sugiere regresión del proceso infeccioso.

After waiting 90 days, the prosthetic components for unitary cement-retained prosthesis were selected (Arcsys 3x4x3.5mm abutment for cement-retained restoration for element 14 and 4.2x6x3.5mm abutment for cement-retained restoration for element 16), folded with the aid of Angle Referrers (Figs. 6 to 9) and placed (Figs. 10 and 11). Note that path compensation is relatively small (6o and 7o), however essential in the construction of the prosthetic restorations with anatomic and aesthetic excellence.

After this moment, tissue conditioning was started with temporary prosthesis to mimic the concave arch surrounding the implants (Figs.

12 to 14).

The porcelain crowns (lithium disilicate) were cemented with notable care in order to remove the excess with an analog of the prosthetic components out of the mouth and immediately before placing the intermediaries. Therefore, the restoration of element 15 was replaced (Figs. 15 and 16).

During the observation period (seven months), one can clearly see the stability of peri-implantar tissues, made possible by, among other factors, intended bacterial sealing in the conic frictional connection (“real” Morse taper), proper placing of the devices (implant/component/restoration) concerning adjacent teeth and adjusted occlusion according to treatment demands (Fig. 17).

Tras espera de 90 días, los componentes protésicos para prótesis unitarias cementadas fueron seleccionados, (muñón Arcsys 3 x 4 x 3,5mm para elemento 14 y muñón 4,2 x 6 x 3,5mm para elemento 16), angulados con auxilio de los referenciadores (Figs. 6 a 9) y activados (Figs. 10 y 11). Se percibe que la compensación de direccionamiento es relativamente pequeña (6° a 7°), pero fundamental para que las restauraciones protésicas fueran posibles de ser construidas con anatomía y estética de excelencia.

A partir de este momento, se inició con las provisorias el acondicionamiento tejidual para mimetizar el arco cóncavo regular alrededor de los implantes (Figs. 12 a 14).

Las coronas de porcelana (disilicato de litio) fueron entonces cementadas, con notable cuidado para extravasar el exceso con un análogo de los componentes protésicos extraoralmente e inmediatamente antes de acomodar en los intermediarios. Entonces, la restauración del elemento 15 fue sustituida (Figs. 15 y 16) Durante observación, (siete meses) se nota claramente la estabilidad de los tejidos peri-implantares, posible por, entre varios factores, sellado bacteriano pretendido en la conexión cónica friccional (cono Morse “verdadero”), adecuado posicionamiento de los dispositivos (implante/ componente/restauración) respecto a los dientes adyacentes y oclusión ajustada de acuerdo con la exigencia del tratamiento. (Fig. 17).

CONCLUSION

CONCLUSIÓN

With extraordinary resources, inconceivable not long ago, however available now to dentists and dental prosthesis technicians, it is perfectly possible to transform seemingly complex rehabilitation procedures, in perfectly simple and viable treatments.

This possibility also increases significantly the perspective of dental implants rehabilitation success and becomes an essential tool for every oral rehabilitator professional.

Con extraordinarios recursos hace poco impensables, pero actualmente disponibles para los dentistas y técnicos en prótesis dentaria, es perfectamente posible transformar terapias rehabilitadoras complejas a primera vista, en tratamientos perfectamente sencillos y viables.

Además, esta posibilidad aumenta significativamente la perspectiva de éxito de las rehabilitaciones sobre implantes dentarios y se convierte en una herramienta esencial para todo profesional rehabilitador oral.

The difficulty to evaluate the best positioning for the implants to the emergency tracks of the intermediate pieces lead to the use of Arcsys. The system allows for the angular refinement which allows for the desired parallelism for the better fitting of the prosthesis with multiple supports.

Sustitución de la prótesis total inferior por protocolo. La dificultad de aliar el mejor posicionamiento de los implantes a los deseados trayectos de emergencia de los intermediarios indujo a la utilización del Arcsys. Éste, permite el refinamiento angular que confiere el paralelismo deseado para el mejor asentamiento de las prótesis con múltiples soportes.

Since their introduction by Branemark, dental implants have been widely used to rehabilitate edentulous patients1;2, contributing to a significant improvement of the life quality of those individuals.

Presently, among all the proposals for rehabilitation of patients that are completely edentulous, the hybrid prosthesis of the “Branemark protocol” type, similar to that suggested in the 60’s is still one of the most used, in face of the evident advantages in relation to the cost/ benefit ratio provided by that technique.

Desde que fueron introducidos por Branemark, implantes dentarios han sido ampliamente utilizados para rehabilitar pacientes edéntulos1;2, contribuyendo para significativa mejora de la calidad de vida de estos individuos. Actualmente, entre las opciones propuestas para rehabilitación de pacientes totalmente edéntulos, la prótesis híbrida tipo “protocolo de Branemark”, semejante a aquella sugerida en la década de 60 aún es una de las más hechas, ante las evidentes ventajas respecto costo X beneficio proporcionada por tal técnica.

Such procedure consists in supporting a total prosthesis on implants anchored in bone tissue and demonstrates high success rates3;4;5, if determined requirements are respected and the patient is oriented adequately. Those requirements are: adequate initial stability (for immediate load), passivity, lever arm compatible to the anchorage, appropriate hygiene possibility.

For the aesthetics of the final piece, and the hygiene possibilities for the patient not to be compromised, it is essential that the accesses to the screws are located in an area between the occlusal/incisal of the teeth and the lingual/palatal edge of the prosthesis. However, if the implant, for any reason, is not at its ideal inclination, the use of an angled intermediate piece, so as to correct the angulation, is crucial for the professional to achieve the results mentioned above. Besides, using angled intermediate pieces in those situations, it is noticed that the distribution of masticatory forces is favored, in comparison to the use of a straight intermediate in the same scenario.6 The use of components angled up to 20 degrees seem to be within the limits of tolerance of the bone tissue7, suggesting that this will not be the cause of reabsorption around the implant/component.

Also, there seems to be no significant difference between straight and angled components in relation to rotation, deflection and the necessary torque to unscrew.8 However, the use of pre-angled components is not the first option of the professionals, which can be explained by the greater difficulty in handling, the risks of aesthetic loss and probable lower mechanical resistance.

For that, the Arcsys Implant System offers the possibility to customize the angulation of the prosthetic components, without, however, implying in inherent losses usually found in pre-angled components. The only thing needed is to customize the direction of the component immediately before its activation, according to the demonstration that follows.

Tal terapia consiste en sostener una prótesis total en implantes anclados en el tejido óseo, y demuestra altos índices de éxito3;4;5, desde que respetadas determinadas exigencias como estabilidad inicial adecuada (para carga inmediata), pasividad, brazo de palanca compatible con el anclaje, posibilidad de apropiada higiene, así como dar debidas orientaciones al paciente.

Para que la estética de la pieza final y la posibilidad de higiene del paciente no sean comprometidas, es esencial que los accesos a los tornillos estén localizados en una región entre la oclusal/incisal de los dientes y el borde lingual/palatino de la prótesis. Pero, caso el implante, por cualesquier motivos no esté en su inclinación ideal, la utilización de un intermediario angulado, de forma a corrigir su angulación es imprescindible para lograr los objetivos supracitados. Además, usando intermediarios angulados en estas situaciones, se percibe que es favorecida la distribución de las fuerzas masticatorias, en comparación a la utilización de un intermediario recto en el mismo escenario.6 El uso de componentes angulados hasta 20 grados parecen estar dentro de los límites de tolerancia del tejido óseo7, sugiriendo que ésta no será causa de reabsorción alrededor del implante/componente.

También, parece no haber diferencia significativa entre componentes rectos y angulados respecto a la rotación, deflexión y torque necesario para desatornillado.8 Sin embargo, la utilización de componentes pre-angulados no es la primera opción de los clínicos, lo que puede ser explicado por la mayor dificultad de manoseo, posibilidad de comprometimiento estético y probable menor resistencia mecánica.

Para tanto, el Sistema de Implantes Arcsys ofrece la posibilidad de personalizar la angulación de los intermediarios protésicos, sin, entretanto, implicar en los perjuicios inherentes a los componentes preangulados, bastando personalizar el direccionamiento del componente inmediatamente antes de su activación, conforme demostrado en el caso a seguir.

RELATO DE CASO

52-year-old female patient, user of total upper and lower dental prosthesis since age 18, searched for treatment because she was not satisfied with the lower prosthesis’ instability. The last change of pieces had been done 3 years ago and the soft tissues were healthy (Fig. Initial). That way, the proposal was for a rehabilitation with Branemark protocol prosthesis for the lower arch and, in a near future, the same treatment for the upper arch.

The clinical exam revealed good general health conditions.

However, for confirmation, complementary exams were requested: complete hemogram, fasting glycemia, coagulation, prothrombin and glycated hemoglobin.

The tomographic exam of the region in question revealed good bone quality and quantity for the installation of dental implants (Figures 01 to 03). As the prosthesis showed adequate aesthetic function and maintenance of OVD, a multifunctional guide was made using the duplication of the lower prosthesis (Figures 04 and 05). The surgery was carried out with the installation of 5 Arcsys implants measuring 3.8x11mm that had primary stability of 45N, oriented by the multifunctional guide (Figures 06 to 12).

In the sequence, the Angle Referrers were positioned for the evaluation of the need for angulation, when it was noted that only one of them needed folding (Figures 12 to 15).

According to the depth of the implants and thickness of the mucous tissue 2.5mm transmucous Arcsys abutments for screwretained restorations were selected, installed and activated (Figures 16 and 17).

Following that, Arcsys Multifunctional Impression Caps were positioned, screwed and united with acrylic resin (Fig. 19). The molding was made with polyether based molding material and the interocclusal record was complemented with acrylic resin over the multifunctional guide (Fig. 19).

The total lower prosthesis itself was used for making the temporary prosthesis, being united to other multifunctional impression caps (working as a temporary coping) with self-polymerizable acrylic resin (Fig. 20).

The analogs were positioned in the impression mold, screwed and sent to the laboratory for the manufacture of the protocol bar. In 48 hours, the bar was tested for fitting and passivity. The next day, the final prosthesis was fitted and installed (Figures 21 to 23).

Paciente del sexo femenino, 52 años, usuaria de prótesis dentaria total superior e inferior desde los 18 años, buscó atención en la clínica insatisfecha con la instabilidad de la prótesis inferior. El último cambio de las piezas fue hecho hace 3 años y los tejidos blandos presentaban características saludables (Fig. Inicial). De esta forma, fue propuesta una rehabilitación con prótesis tipo protocolo para la arcada inferior y en un futuro próximo el mismo tratamiento para arcada superior.

El examen clínico reveló buenas condiciones de salud general. Sin embargo, a criterio de confirmación fueron solicitados examenes complementares: hemograma completo, glicemia en ayuna, tiempo de coagulación, tiempo de protrobina y hemoglobina glicada.

El examen tomográfico de la región en cuestión reveló buena calidad y cantidad ósea para instalación de los implantes dentarios (Figs. 01 a 03). Como las prótesis presentaban adecuada función estética y manutenimiento de la DVO, fue hecho un guía multifuncional a partir de la duplicación de la prótesis inferior (Figs. 04 y 05). La cirugía fue realizada con la instalación de 5 Implantes Arcsys (FGM), con dimensiones 3.8x11mm que tuvieron estabilidad primaria de 45N, orientados por la guía multifuncional (Figs. 06 a 12).

En la secuencia fue hecho el posicionamiento de los Pilares Referencia para evaluación de la necesidad de angulación, donde se constató necesidad de angular solamente uno de ellos (Figs. 13 a 15).

De acuerdo con la profundidad de los implantes y espesor del tejido mucoso fueron seleccionados, instalados y activados los pilares atornillables Arcsys de 2.5mm de transmucoso (Figs. 16 y 17). Enseguida, Transferentes Multifuncionales Arcsys (FGM) fueron posicionados, atornillados y unidos con resina acrílica (Fig. 18). El moldeo fue realizado con material de moldeo a base de poliéter y el registro interoclusal fue complementado con resina acrílica sobre el guía multifuncional (Fig. 19).

La propia prótesis total inferior fue utilizada para confección de la provisoria, siendo unida a otros transferentes multifuncionales (funcionando como cilindro provisorio) con resina acrílica autocurable (Fig. 20).

Los análogos fueron posicionados en el molde de transferencia, atornillados y enviado al laboratorio para la confección de la barra protocolo. En 48 horas la barra fue probada respecto a la adaptación y pasividad. Al día siguiente, la prótesis final fue ajustada e instalada (Figs. 21 a 23).

The innovating angulation characteristic of the Arcsys EN prosthetic components allows for unique planning possibilities and execution of clinical cases, making it much easier for obtaining aesthetic and functional results.

The angulation became possible by means of the combination of a special design for the prosthetic components with a device that allows for converting the force of a manual procedure into enough force to curve the intermediate area of the components.

In spite of the perceptible mechanical hardness of the Arcsys prosthetic components and of the fact that it has been carefully planned, the fact that the components may be folded calls the attention of the public who intuitively asks: “If the components may be folded, are they hard enough?”

That question led us to carry out these specific tests to evaluate the mechanical deformation of conventional folded components made by two manufacturers in comparison to Arcsys components.

INTRODUCTION When planning a rehabilitation with dental implants, the dentist has as his/her premise the positioning of the implant with its long axis coinciding with the long axis of the future prosthetic component, so as to use the latter, preferably, straight.

Unfortunately, it is known that that parallelism is not always possible, especially in areas with very little bone volume (thickness or height) and/ or around vital anatomic areas1. In those situations, considering that the implant is in its best positioning as far as anchorage, if that compensation of the angulation does not occur in the intermediary component, the peri-implantar tissue or the prosthetic crown itself may have their aspect and/or stability compromised.

For that, professionals have used prefolded prosthetic components supplied by manufacturers for decades, so as to fulfill the attempt to improve the prosthetic solution of those rehabilitations.

However, those components are avoided in the majority of the cases due to various disadvantages in comparison to straight components2,3, leaving to the professional the hard decision about what option to choose (straight or pre-folded), according to the several advantages and disadvantages inherent to each of them.

Arcsys Implant System allows for, in an innovative way in implantodontics, the customization of the angulation of the prosthetic component (figure 02), with the purpose of allowing for the compensation for its angulation, without, however, implying in the inherent compromise of the pre-folded abutments.

Foldable prosthetic components may have their angle customized for each clinical case. With that feature came unprecedented possibilities for planning and executing rehabilitations, making it much easier to obtain aesthetic and functional results. That angulation possibility became viable by means of the advanced design combination of the prosthetic components allying the use of high mechanical resistance alloys with biocompatibility (stainless steel manufactured according to the U.S. ASTM F 138 standard10) 50 | FGM 2018 | EDITION | EDICIÓN 01

Al planear una rehabilitación con implante dentario, el dentista tiene como premisa posicionar el implante con su largo eje coincidiendo con el largo eje del futuro componente protésico, de forma a utilizar éste último preferencialmente recto. Infelizmente, es sabido que ni siempre este paralelismo será posible, principalmente en regiones de diminuto volumen óseo (espesor o altura) y/o proximidades con áreas anatómicas nobles1.

En estas situaciones, considerando que el implante esté en su mejor posicionamiento respecto al anclaje, caso esta compensación de la angulación no ocurra en el intermediario, el tejido peri-implantar o la propia corona protésica podrán tener su aspecto y/o estabilidad sacrificados.

Para tanto, se ha utilizado componentes protésicos pre-angulados, proveídos por los fabricantes hace décadas, de forma a proporcionar intento de mejora en la solución protésica de estas rehabilitaciones.

Pero, tales componentes son evitados en la mayoría de los casos debido a las varias desventajas en comparación a los componentes rectos2,3, dejando el profesional con la ingrata decisión de cual opción tomar (recto o pre-angulado), de acuerdo con las varias ventajas y desventajas inherentes a cada uno de ellos.

El Sistema de Implantes Arcsys posibilita, de manera inédita en la implantología, la personalización de la angulación del componente protésico (figura 02), con el intuito de proporcionar la compensación de angulación de éste, sin, entretanto, implicar en los perjuicios inherentes de los intermediarios pre-angulados. Los componentes protésicos angulables pueden tener el ángulo personalizado para cada caso clínico. Con esta característica surgieron posibilidades inéditas de planeamiento y ejecución de las rehabilitaciones, facilitando de sobremanera la obtención de resultados estéticos y funcionales. Tal posibilidad de angulación se hizo posible a través de la combinación de diseño avanzado de los componentes protésicos, aliando el uso de ligas especiales de alta resistencia mecánica y biocompatibilidad (acero inoxidable fabricado conforme norma americana ASTM F13810) con un dispositivo que permite convertir la fuerza

The ISO 14801 standard specifies one method for dynamic fatigue tests in a unitary assembly of dental implant and its respective prosthetic component12. That method is the most usual for the comparison of the mechanical behavior between implants and prosthetic components in different configurations. The configuration of the test simulates the use of a dental implant in its most critical condition. The same configuration is used also for assessing an implant and abutment assembly in an absolute manner, compressing the assembly until failure and not applying the cyclic load. For the use of pre-folded components, the standard defines that the test configuration that is shown in figure 03.

Then, a puncher, attached to a test machine, applies a compressing force according to what is shown in figure 04. The samples are compressed until they show failure, the maximum obtained force being recorded.

The results showed that Arcsys foldable components have a resistance that is 3 times as high of that of pre-folded machined component, if used in the same prosthetic dimensional conditions and transmucosal height. The results obtained are the consequence of three main factors: hardening of the neck of the foldable component, materials used in the manufacture both of the component and the implant and the mechanical design of the abutment.

Los resultados apuntaron que los componentes angulables Arcsys presentan resistencia hasta 3 veces superior a componentes mecanizados pre angulados, para una misma condición dimensional protésica y de altura de transmucoso. Los resultados obtenidos son consecuencia de tres principales factores: endurecimiento del cuello del componente angulable, materiales utilizados en la fabricación tanto del componente como del implante y proyecto mecánico del abutment.

INTRODUCCIÓN

The purpose of the oral rehabilitation is to develop the patient’s characteristics and functions previously existent and that have been lost after the loss of dental organs that go beyond mastication.

Thanks to the present development, the implantodontics allow exactly for the compensation of those losses and, consequently, the repair of those qualities in patients, not affected by other therapies1.

The criteria used to estimate the success rates shown in the past2,3,4 can not be considered isolated. Presently, we should, as a rule, analyze all criteria included in the main criterias involved in the main factors related to implantodontics such as health, aesthetics and function5,6.

And aesthetics is exactly one of the main reasons why patients search for professionals with the objective of rehabilitation.

Unfortunately, this is, several times, an extremely complex factor to be solved, be it for the consequences of dental losses, or the individual limitations of each patient. Add to that the need to install the implant in the ideal tridimensional position, a scenario that is even more complex in patients partially edentulous5, under the risk of not reaching successful total rehabilitation.

In face of those considerations, according to which, the positioning and direction of the prosthesis is of crucial importance for the professional and/or prosthetics technician to reconstruct the dental crown adequately. The Arcsys System allows these professionals to personalize this angulation without, however, affecting the geometry of the components neither the peri-implantar tissues.

El objetivo de la rehabilitación oral es devolverle al paciente características y funciones anteriormente existentes y que se perdieron luego de la pérdida de órganos dentales y que van más adelante de la función masticatoria. Gracias al actual desarrollo, la implantodontía permite justamente compensar estas pérdidas y consecuentemente reparar estas calidades en los pacientes, no logradas por otras terapias1.

Los criterios utilizados para calcular índices de éxitos apuntados en el pasado2,3,4 no pueden más ser considerados como únicos. Actualmente, debemos obligatoriamente, analizar todos los criterios envueltos en los factores primordiales que envuelven la implantodontía, como salud, estética y función5,6.

Es justamente la estética una de las principales razones por la cual los pacientes buscan a los profesionales con objetivo de rehabilitación.

Infelizmente, este es innúmeras veces un factor de extrema complejidad para resolución, sea por las secuelas inherentes a las pérdidas dentarias, sea por los límites individuales de cada paciente. Añadido a este factor está la necesidad de instalación del implante en la posición tridimensional ideal, escenario aún más complejo en pacientes parcialmente edéntulos5, bajo pena de no alcanzar el éxito de la rehabilitación en su plenitud.

Frente a tales consideraciones, en el cual el posicionamiento y direccionamiento del componente protético es de suma importancia para que el profesional y/o el técnico en prótesis reconstruyan la corona dental adecuadamente, el Sistema de Implante Arcsys propicia a que ambos personalicen esta angulación, sin entretanto, perjudicar la geometría de los componentes, ni comprometer los tejidos periimplantares.

CASE REPORT

RELATO DE CASO

51 year-old male patient came to the clinic complaining mainly about the rehabilitation of the missing teeth. In the anamnesis, he showed good health conditions, being, therefore, fit for the rehabilitation treatment. The planning consisted in beginning the treatment at the left inferior posterior area, for the substitution of the teeth lost around 3 years before, with 2 elements united and screwed over 2 implants. The remaining areas would be rehabilitated later.

For the prosthesis of the designated area, the choice was the Arcsys Implant System so that the angulation and positioning of the prosthesis would be the closest possible to the ideal and would facilitate the construction of the prosthesis with complete aesthetic harmony, function and health for the patient.

The professionals installed two implants measuring 4.3 X 11mm.

The perforations were carried out with just one FGM drill (Ø3.9) with a 13mm stop. It is important to point out that those drills, thanks to their innovative geometry, allow for the professional to opt for the nonscaling to prepare the base (single perforation) and to use “cursors” (stops) for incrementing the safety of the depth of the cutting.

The reopening was carried out 6 months later. To allow for the provisional character and the tissue conditioning, Multifunctional Healing Abutments (PEEK) were used and teeth stock were rebased on those with autocuring acrylic resin.

Paciente con 51 años, sexo masculino se presentó en en el consultorio con queja principal para rehabilitación de los dientes ausentes. En la anamnesis, mostró buenas condiciones de salud, estando por lo tanto apto al tratamiento rehabilitador. El planeamiento consistió en iniciar el tratamiento por la región inferior posterior izquierda, para sustitución de los dientes ausentes perdidos hace cerca de 03 años, con 02 elementos unidos y atornillados sobre 02 implantes. Las demás áreas serian rehabilitadas posteriormente.

Para resolución protética del área designada, se optó por utilizar el Sistema de Implantes Arcsys, de manera que la angulación y posicionamiento de los componentes protéticos sea la más próxima de lo ideal y facilite la confección de la prótesis con completa armonía estética, función y salud al paciente.

Fueron instalados 02 implantes con dimensiones 4.3x11mm. Las perforaciones fueron realizadas con solamente 01 fresa FGM (Ø3.9) con limitador de 13mm. Es importante sobresaltar que estas fresas, gracias a su geometría innovadora, permite que el profesional opte por el no escalonamiento al preparar el lecho (perforación única) y utilice “cursores” (stops) para incrementar la seguridad de la profundidad de corte.

La reapertura fue realizada 06 meses después de la instalación.

Para permitir la provisionalización y acondicionamiento tejidual, fueron utilizados los Cicatrizadores Multifuncionales (PEEK) y dientes de almacenado reenvasados sobre estos con resina acrílica autocurable.

After 30 days, during the activation of the final prosthetic components (abutments for screw-retained restoration), it was confirmed that those would be better positioned and would have better angulation to facilitate the final restoration and, consequently, have better anatomy. Therefore, in the same session and with the help of the Angle Referrers and the Arcsys Abutment Folding Device, the 2 components were adequately angled and later installed over the implants. A temporary prosthesis united over the Multifunctional Impression Caps (PEEK) was made.

After transfer molding, the final restoration in porcelain with metal infrastructure was made and installed.

Luego de 30 días, durante activación de los componentes protéticos finales (pilares para prótesis atornillables), se confirmó que estos podrían estar mejor ubicados, así como tener mejor angulación, de forma a facilitar la confección de la restauración final y consecuentemente tener mejor anatomía. Por lo tanto, en la misma sesión y con auxilio de los Pilares Referencia y del Dispositivo Angulador Arcsys, los 02 componentes fueron debidamente angulados y posteriormente accionados sobre los implantes. Una prótesis provisoria unida sobre los Transferentes Multifuncionales (PEEK) fue confeccionada.

Luego del moldeo de transferencia, la restauración final en porcelana con infraestructura en metal fue confeccionada e instalada.

The future of bone grafting el futuro del injerto óseo

EN The filling of bone defects with biomaterials is a practice that is becoming more and more common at dental clinics. By using bone substitutes, the dentist is able to achieve excellent results within the needed lead times. The grafting process depends on some factors that guarantee the conditions for the osteoblasts to proliferate and lead to the formation of new tissue, promoting the conversion of the bone graft into a process defined as osteointegration. In the bone grafting process, the main properties of a biomaterial are osteogenesis, osteoinduction and osteoconduction. Those concepts may be defined in a very simple way: 1 - Osteogenesis implies in the presence of osteoblasts in the biomaterial, which are capable of promoting the synthesis of the new bone tissue right after the surgical procedure; 2 - Osteoinduction is characterized as the capacity to recruit new cells that form bone tissue to the area of the graft; 3 - Osteoconduction refers to the capacity to promote adhesion, survival and proliferation of bone cells, coming from a interconnected structure for the sedimentation of new bone tissue as well as the formation of new blood vessels.

Considered the golden standard in grafting procedures, the autogenous bone is the only one to provide all three properties.

However, that procedure is associated to several inconveniences such as longer recovery of the patient, painful post-operatory phase, blood loss, infection, just to name a few that appear to be the consensus among professionals.

In this scenario, the use of synthetic biomaterials appears as an alternative to the autogenous bone. Those synthetic biomaterials are, in their majority, composed by hydroxyapatite (HA) based ceramics and ß-tricalcium phosphate (β-TCP). Both HA and β-TCP are highly biocompatible, HA having a longer degradation period, remaining in the place of implantation for a longer period than β-TCP. Like all synthetic ceramics, separately, those materials do not have osteogenesis and osteoinduction properties. Therefore, their efficacy depends a lot on how efficient their osteoconduction is.

El rellenado de defectos óseos con biomateriales es una práctica ES cada vez más común en los consultorios odontológicos. Utilizándose de sustitutos óseos, el dentista consigue, hoy, alcanzar excelentes resultados dentro del plazo deseado. El proceso de injerto es dependiente de algunos factores que garantan condiciones para que los osteoblastos puedan proliferarse y llevar a la formación de nuevo tejido, promoviendo la conversión del injerto en hueso, en un proceso definido como osteointegración. En el proceso de injerto óseo, las principales propiedades de un biomaterial son osteogenicidad, osteoinducción y osteoconducción. Estos conceptos pueden ser definidos de manera bien sencilla: 1 - Osteogenicidad implica en la presencia de osteoblastos en el biomaterial, el cual es capaz de promover la síntesis de nuevo tejido óseo luego después del procedimiento quirúrgico; 2 - Osteoinducción es caracterizado como la capacidad de reclutar nuevas células formadoras de tejido óseo para el local de injerto; 3 - Osteoconducción se refiere a la capacidad de promover adhesión, sobrevivencia y proliferación de células óseas, proviendo una estructura interconectada para la sedimentación de nuevo tejido óseo así como la formación de nuevos vasos sanguíneos.

Considerado estándar oro en procedimientos de injerto, el hueso autógeno es el único a reunir todas estas propiedades. Entretanto este procedimiento está asociado a diversos inconvenientes como mayor tiempo de recuperación del paciente, pos-operatorio doloroso, pérdida sanguínea, daños en nervios adyacentes al local de colecta del tejido óseo, infección, solamente por nombrar algunos puntos que son consenso entre los profesionales. En este escenario el uso de biomateriales de origen sintético surge como alternativa al hueso autógeno. Estos biomateriales sintéticos son en su mayor parte compuestos por cerámicas a base de hidróxiapatita (HA) y β-tricálcio fosfato (β-TCP). Tanto HA como β-TCP son altamente biocompatibles, siendo que la HA posee un mayor tiempo de degradación, permaneciendo en el local de implantación por un tiempo mayor que el β-TCP. Como toda cerámica sintética, solitos estos materiales no poseen propiedades de osteogénesis y osteoinducción. Así la eficacia de estos materiales depende en mucho de cuan eficiente es su osteoconducción.

Analyzing the organization of the bone tissue more deeply, we note that the bone is a nano-hybrid tissue composed by nanocrystals of HA and nanofibers of collagen, which assume a very porous structure, with several interconnected pores. Since the bone is naturally nanostructured, synthetic materials with nanometric structures appear as the best choice for grafting procedures. The advantage of that type of material relies on the similarity with the size of the HA particles, the high porosity and high surface area. HA crystals in nanometric scale increase the adhesion of osteoblasts, elevating the proliferation and the adhesion force between those cells and the biomaterial1. Like the cellular adhesion, the proliferation and deposition of calcium are substantially greater in HA ceramics with nanometric surface, when compared to the traditional micrometric ceramics available presently in the market1,2. That fact is possibly related to the elevated surface area of nanoceramics, which allows for a greater deposition of proteins related to the cell adhesion process.

Besides the size of the HA crystals, the size of the HA granules also interfere in the production of several proteins related to the process of cellular signalization, called cytokines3,4. Those cytokines are small proteins produced by several cells with capacity to modulate the formation of bone tissue. Cytonkines, like Interleukin 6 (IL-6) and the alpha tumoral necrosis factor (TNF-α) are related to the activation of Osteoclasts (bone cells related to the reabsorption of bone tissue). On the other side another cytokine operates, the IL-187. That cytokine, produced by cells of the immune system and by osteoblasts is able to indirectly inhibit the activation of osteoblasts by elevating the production of GM-CSF5, 6, 7. It was demonstrated in tests that spherical granules of HA with sizes between 150 and 300μm make cells of the immune system produce smaller quantities of IL-6 and TNF-α, when compared to the control group3. By decreasing the size of the HA granules, the levels of those cytokines tend to increase, which would lead to a greater amount of osteoclasts and, consequently, a reduction in the quantity of bone formed. Like what was observed for cytokines IL-6 and TNF-α, spherical particles of HA with sizes between 150 and 300μm can also stimulate the production of IL-18, which leads to a smaller quantity of osteoclasts4. That equilibrium between lower levels of IL-6 and TNF-α, conjugated to an elevation in the levels of IL-18 could lead to a greater among of bone tissue formed when HA particles with sizes equal or greater than 200μm are used.

Analizando más profundamente la organización del tejido óseo, notamos que el hueso es un tejido nanohíbrido compuesto por nanocristais de HA y nanofibras de colágeno, que asumen una estructura altamente porosa, con diversos poros interconectados.

Una vez que el hueso es naturalmente nanoestructurado, materiales sintéticos con estructuras nanométricas se presentan como la mejor elección para procedimientos de injerto. La ventaja de este tipo de material reside en la semejanza con el tamaño de las partículas de HA, alta porosidad y elevada área de superficie. Los cristales de HA en escala nanométrica aumentan la adhesión de osteoblastos, elevando la proliferación y la fuerza de adhesión entre estas células y el biomaterial1.

De la misma forma que la adhesión celular, la proliferación y deposición de calcio son sustancialmente mayores en cerámicas de HA con superficie nanométrica, cuando comparadas con las tradicionales cerámicas micrométricas disponibles en el mercado actualmente1,2.

Hecho posiblemente relacionado a la elevada área de superficie de las nanocerámicas, la cual posibilita una mayor deposición de proteínas relacionadas con el proceso de adhesión celular.

Además del tamaño de los cristales de HA, el tamaño de los gránulos de HA también interfiere con la producción de diversas proteínas relacionadas con el proceso de señalización celular, llamadas de citocinas3, 4. Estas citocinas son pequeñas proteínas producidas por diversas células con capacidad de modular la formación de tejido óseo.

Citocinas como Interleucina 6 (IL-6) y el factor de necrosis tumoral alfa (TNF-α) están relacionados a la activación de osteoclastos (células óseas relacionadas con la reabsorción del tejido óseo). De modo opuesto actúa otra citocina, la IL-187. Esta citocina producida por células del sistema inmune y porosteoblastos es capaz de inhibir indirectamente la activación de osteoclastos al elevar la producción de GM-CSF5, 6, 7. Fue demostrado en estudio, que gránulos esféricos de HA con tamaño entre 150 y 300µm llevan células del sistema inmune a producir menores cantidades de IL-6 y TNF-α, cuando comparados al grupo control3. Al disminuir el tamaño del granulo de HA, los niveles de estas citocinas tienden a aumentar, lo que podría llevar a una mayor cantidad de osteoclastos y consecuente reducción en la cantidad de hueso formado. De la misma forma como observado para las citocinas IL-6 y TNF-α, partículas esféricas de HA con tamaño entre 150 y 300µm también pueden estimular la producción de IL18, lo que lleva a una menor cantidad de osteoclastos4. Este balance entre menores niveles de IL-6 y TNF-α, conjugado a una elevación en los niveles de IL-18 podria llevar a una mayor cantidad de tejido óseo formado cuando partículas de HA con tamaño igual o superior a 200 µm son utilizados.

That knowledge about the high porosity, nanostructure and interconnected pores in biomaterials were applied to the new biomaterial

by FGM. Recently introduced in the market, Nanosynt has micropores and nanostructures aligned with HA and β-TCP crystals in nanometric scale. Images obtained on the surface of the Nanosynt material show that the nanocrystals of HA and β-TCP have average size of 200nm (Figures 2 and 3). The union of those nanocrystals forms an intricate network of nano and micro connected pores which elevates even more the surface area and the porosity of the FGM biomaterial. With that organization, Nanosynt presents around 80 to 90% porosity on its surface, which guarantees excellent wettability.

Those surface characteristics provide to Nanosynt a greater capacity to form bone tissue when compared to other biomaterials with microstructured surfaces. In a test conducted at Universidade Federal Fluminense (to be printed), carried out with patients in a variety of ages, Nanosynt has shown to be more efficient than a similar synthetic graft largely known in the market, showing a bone formation 20% greater when compared to the other biomaterial, and around 40% greater when compared to the control group (clog). That larger formation of bone tissue, can also be observed when Nanosynt is compared to biomaterials of bovine origin. A work using rabbits showed that, when compared to the most traditional bone substitutes in the market (synthetic and bovine), Nanosynt showed to be more efficient than both8. Four weeks after the application, the animal group that used Nanosnt showed around 23% of new bone, compared to 11% in the case of the other synthetic biomaterial and 17% for bovine biomaterial. Eight weeks after the application, the difference was maintained, when Nanosynt showed around 30% of newly formed bone, compared to 27% for the other synthetic biomaterial and 24% for the bovine biomaterial.

The choice of biomaterial by the professional has always been related to the easiness to use and cost. The introduction of more efficient materials with better results, brings to the professional a new horizon to be explored. The possibility to carry out the placement of an implant in a safer and shorter procedure, made dentists search for alternatives in face of the existing options that had not evolved much in the last decade. This technological advancement, is achieved with the application of new technology added to the knowledge of biochemical processes of cell signaling that regulate the formation of new tissue.

Este conocimiento sobre alta porosidad, nanoestructura y poros interconectados en biomateriales fueron aplicados en el nuevo biomaterial de FGM. Recién lanzado en el mercado, Nanosynt posee poros micro y nanoestructurados alineados con cristales de HA y β-TCP en escala nanométrica. Imágenes obtenidas de la superficie del Nanosynt muestran que los nanocristales de HA y β-TCP poseen tamaño promedio de 200nm (Figuras 2 y 3). La unión de estos nanocristales forma una intrincada red de poros nano y micro conectados, que eleva aún más el área de superficie y la porosidad del biomaterial de FGM. Con esta organización, Nanosynt presenta cerca del 80 al 90% de porosidad en su superficie, lo que le garante una excelente mojabilidad.

Estas características de superficie le confieren al Nanosynt una mayor capacidad de formación de tejido óseo cuando comparado a otros biomateriales con superficie micro estructurada. En trabajo conducido en la Universidad Federal Fluminense (manuscrito en prensa), realizado en pacientes de diversas edades, Nanosynt se mostró más eficaz que un injerto sintético similar largamente conocido en el mercado, presentando una formación ósea hasta el 20% mayor cuando comparado al otro biomaterial, y cerca del 40% mayor cuando comparado al grupo control (coágulo). Esta mayor formación de tejido óseo también puede ser observada cuando Nanosynt es comparado a biomateriales de origen bovino.

Trabajo utilizando conejos mostró que cuando comparado a los más tradicionales sustitutos óseos del mercado (sintético y bovino), Nanosynt se mostró más eficaz que ambos8. Tras 4 semanas de aplicación, el grupo de animales que hizo uso de Nanosynt presentó cerca del 23% de nuevo hueso, comparado al 11% del otro biomaterial sintético y el 17% del biomaterial de origen bovino. Tras 8 semanas esta diferencia se mantuvo, donde Nanosynt presentó cerca del 30% de nuevo hueso formado, comparado al 27% del otro biomaterial sintético y al 24% del biomaterial de origen bovino.

La elección del biomaterial por el dentista siempre fue pautada por aspectos relacionados a la facilidad de uso y precio de los biomateriales. La llegada de productos más eficientes y con mejores resultados le trae al profesional un nuevo horizonte a ser explorado.

La posibilidad de realizar la colocación del implante de manera más segura y en menor tiempo hace con que el dentista pase a buscar por nuevas alternativas en detrimento de opciones que poco evolucionaron en la última década. Este avance tecnológico es logrado con la aplicación de nuevas tecnologias, añadido al conocimiento sobre los procesos bioquímicos de señalización celular que regulan la formación de nuevos tejidos.

INTRODUCTION

INTRODUCCIÓN

During the planning for the rehabilitation of the posterior area of the maxilla, it is not uncommon for the surgeon to face insufficient height of the residual border to anchor the dental implant1, making it inviable for the implantation without the previous reconstruction of the area.

In face of that need, the described technique to raise the maxillary sinus2,3,4 is considered safe and with a predictable prognostics for the reconstruction for the purpose of rehabilitation1. Also, what is important is to consider that the nature of bone grafting (autogenous or substitute) for filling seems not to have an influence in the success rate of the reconstruction5,6, allowing for considerable decrease in post-operative morbidity when avoiding the collection of the patient’s own bone.

Durante el planeamiento para rehabilitar el área posterior de la maxila, no es fuera de lo común que el dentista se depare con insuficiente altura en el reborde residual para anclar el implante dentario1, inviabilizando la implantación sin que el área sea previamente reconstruida.

Ante esta necesidad, la técnica descripta para levantamiento del seno maxilar2,3,4 es considerada segura y con previsible prognóstico para reconstrucción vislumbrando la rehabilitación1. También, es importante considerar que la naturaleza del injerto óseo (autógeno o sustituto) para rellenado parece no influenciar en la tasa de éxito de la reconstrucción5,6, posibilitando reducción considerable de la morbilidad pos-operatoria al evitar la colecta de hueso del propio paciente.

Those reasons justify why this technique has been so widely recommended in the last decades as the chosen option for reconstruction in those areas, in case they show low height in the alveolar border. When the planning includes unitary dental crowns, it is even more important that the bone support is mechanically compatible to the masticatory function.

The manufacture of prosthetic crowns by means of CAD/CAM systems (Computer Aided Design/Computer Aided Manufacturing) shows a precise and fast option to the professional7, since it allows for the conclusion of the work not only optimized but also with adequate precision in the finish line8 and great adaptation between the internal surface of the crown and the intermediate piece. Although follow up studies still need to be carried out, such technology seems promising in the area of oral rehabilitation.

Estas razones justifican tal técnica haber sido ampliamente indicada en las últimas décadas como elección para reconstrucciones en estas áreas, caso presenten poca altura del reborde alveolar.

Cuando el planeamiento prevee coronas dentarias unitarias, es aun más importante que el soporte óseo sea mecánicamente compatible con la función masticatoria.

La confección de coronas protésicas a través de sistemas CAD/ CAM (Computer Aided Design/Computer Aided Manufacturing) le presenta una opción precisa y rápida al clínico7, ya que permite la conclusión del trabajo no solamente optimizada, pero con adecuada precisión en la línea de terminación8 y óptima adaptación entre la superficie interna de la corona y el intermediario. Aunque aún necesiten estudios de acompañamiento a largo plazo, tal tecnología parece ser promisora en la rehabilitación oral.

CASE REPORT

RELATO DEL CASO

64-year-old patient, smoker, searched for help at a private clinic complaining about dental absence in the posterior area of the left side.

During anamnesis, the patient showed compatible health for the procedures proposed for the rehabilitation with dental implants and image and blood tests (hemogram, fasting glycemia and coagulogram) were requested. At the clinical exam, the absence of teeth 24, 25 and 26 was noticed, however, with prosthetic space for only one premolar and one molar (25 and 26). (Figures Initial and 01).

The blood tests did not show alterations; however, the images showed that the height of the border of the residual bone would be incompatible for the installation of regular length implants, more specifically in the distal area (Fig. 02). The option for short implant (Short) was discarded due to the diminutive thickness of that area.

The planning consisted of reconstructing the border’s height by means of the technique for raising the maxillary sinus by lateral access, with filling of the sinus antrum with synthetic bone substitute, composed by Hydroxyapatite (60%) and ß phosphate tricalcium (Nanosynt). One membrane of bovine cortical bone was used previously to the insertion of the bone granulate due to a perforation in the Schneiderian membrane classified as class I9, occurred during its elevation (Figures 03 and 07). That maneuver is in accordance to what is suggested in the literature for that scenario, where the success rate is not decreased by such intercurrence10,11, if kept within the acceptable limits of control and remediation. No membrane in the lateral access to the antrum was used12.

Paciente del sexo femenino, 64 años, fumante, buscó atendimiento en la clínica privada con queja de ausencia dentaria en la región posterior de la maxila del lado izquierdo.

Durante la anamnesis a la paciente demostró tener salud compatible con los procedimientos propuestos para rehabilitación con implantes dentarios y exámenes de imágenes y sangre (hemograma, glicemia en ayuna y coagulograma) fueron solicitados. En el examen clínico, fueron identificadas ausencias de los dientes 24, 25 y 26, pero con espacio protésico para solamente un premolar y un molar (25 y 26) (Figs. Inicial y 01).

Los exámenes sanguíneos no presentaron alteraciones, pero las imágenes muestran que la altura del reborde óseo residual sería incompatibe con instalación de implantes con largo regular, más específicamente en el área distal (Fig. 02). La opción de implante corto (Short) fue desechada por el diminuto espesor de esta región.

El planeamiento consistió en reconstruir el reborde en altura a través de la técnica de levantamiento del seno maxilar por acceso lateral, con rellenado del antro sinusal con sustituto óseo sintético, compuesto por Hidroxiapatita (60%) y ß fosfato tricálcio (Nanosynt).

Una membrana de cortical ósea bovina fue utilizada previamente a la inserción del granulado óseo debido a una perforación en la membrana de Schneiderian clasificada como clase I9, ocurrida durante su elevación (Figs. 03 a 07). Tal maniobra condice con lo sugerido en la literatura en este escenario, donde la tasa de éxito no es perjudicada por tal intercurrencia10,11, desde que dentro de los límites aceptables de control y remediación. Ninguna membrana en el acceso lateral al antro fue utilizada12.

The post-operative phase happened without intercurrences and around 06 months were waited for the implantation planning (Fig. 08), being 02 Arcsys Implants, one in the area of tooth 25 (Ø3.8X11mm) and other in the area of tooth 26 (Ø4.3X11). Both obtained locking of 40N.cm and 30N.cm respectively, however, because the patient used partial removable prosthesis, the option was to bury them instead of installing healing abutments (Fig. 09).

The reopening occurred 06 months later (Figures 10 and 11), in which two temporary crowns were placed on multifunctional healing abutments (5mm – high profile), beginning the pre peri-implantar tissue conditioning and there they remained for 02 months (Figures 12 and 14). The quality of the mucous tissue conquered with the temporary crowns on the healing abutments was excellent and corresponds to their biocompatibility (PEEK) (Fig. 15).

During the activation of the final components (a 4.2x6x2.5mm abutment for cement-retained restoration for element 25 and a 4.2x6x3.5mm one for element 26), it was noticed that the prosthetic component over the distal implant (tooth 26) could have a better positioning with a slight alteration of the direction, allowed by the capacity for customization of the angulation of the intermediate pieces of the Arcsys System (Figures 16 and 17). It was a seemingly small change but enough for potentiate the future aesthetic capacity of the future prosthetic crown. The intermediate pieces were then positioned and activated (Figures 18 and 19).

After activation, two other temporary crowns were placed over the respective Multifunctional Impression Caps returning to the patient the aesthetic and the function, in order to continue the tissue conditioning (Figures 20 and 21).

El pos-operatorio ocurrio sin intercorrencias, y fue aguardado cerca de 06 meses para planeamiento de las implantaciones (Fig. 08), siendo 02 Implantes Arcsys, uno en la región del diente 25 (Ø3.8X11mm) y otro en la región del diente 26 (Ø4.3X11). Ambos obtuvieron trabado de 40N.cm y 30N.cm respectivamente, pero como la paciente hacia uso de prótesis parcial removible, se optó por sepultarlo al revés de instalar cicatrizadores (Fig. 09).

La reapertura ocurrió en 06 meses (Figs. 10 y 11), donde dos coronas provisorias fueron reembasadas sobre cicatrizadores multifuncionales (5mm – cinta alta), iniciando en este momento el acondicionamiento tejidual peri-implantar y permanecieron por 02 meses (Figs. 12 a 14). La calidad del tejido mucoso conquistado con las provisorias sobre los cicatrizadores fue excelente y condice con su biocompatibilidad (PEEK) (Fig. 15).

Durante la activación de los componentes finales (muñones 4,2x6x2,5mm para el elemento 25 y 4,2x6x3,5mm para el 26), se percibió que el componente protésico sobre el implante distal (diente 26) podría tener su posicionamiento mejorado con tenue alteración de dirección, posibilitada por la capacidad de personalización de la angulación de los intermediarios del Sistema Arcsys (Figs. 16 y 17). Ésta fue aparentemente pequeña, pero suficiente para potencializar la capacidad estética de la futura corona protésica. Los intermediarios fueron entonces posicionados y activados (Figs. 18 y 19).

Tras la activación, otras dos coronas provisorias fueron reembasadas bajo los respectivos Transferentes Multifuncionales, devolviendo estética y función y dando continuidad al acondicionamiento tejidual (Figs. 20 y 21).

the tissue form achieved with the conditioning, the Multifunctional Transfers were positioned and involved by a light-curing resinous material (originally used for gingival protection) Top dam (FGM) (Figures 22 and 23). The peri-implantar health was confirmed with probing the area, confirming tissue stability (Figures 24 and 25).

The crowns were manufactured with blocks of cured composite for CAD/CAM Brava Block (FGM) (Fig. 26) and the cementation with Allcem Core (FGM) resinous cement was carefully done so that there would be no overflowing to the peri-implantar tissues (extraoral overflow with the analog of the components). The occlusion was then adjusted and the final polishing was carried out with Diamond Pro sandpaper disks (FGM) and Diamond felt disks (FGM) with Diamond Excel polishing paste (FGM). There was an optimum combination of cost-benefit, mechanical resistance, aesthetics (Figures 27 and 28), excellent adaptation/fitting of the pieces and great response of the peri-implantar tissues (Fig. 29).

The conclusion was that the planning, the technique and the materials used are adequate for the excellent and predictable result for such recommendation.

los transferentes fueron posicionados y envueltos por material resinoso fotocurable (originalmente utilizado para protección gingival) Top dam (FGM) (Figs. 22 y 23). La salud peri-implantar fue confirmada con sondeo del área, confirmando estabilidad tejidual (Figs. 24 y 25).

Las coronas fueron confeccionadas con bloques de composite curado para CAD/CAM Brava Block (FGM) (Fig. 26) y las cementaciones con cemento resinoso Allcem Core (FGM) fueron realizadas con absoluto cuidado para que no haya desborde de éste en los tejidos peri-implantares (desborde extraoral con análogo de los componentes). La oclusión fue entonces ajustada y el pulido final hecho con discos de lija Diamond Pro (FGM) y discos de fieltro Diamond (FGM) con pasta para pulido Diamond Excel (FGM). Se combinó por lo tanto óptimo costo X beneficio, resistencia mecánica, estética (Figs. 27 y 28), excelente adaptación/asentamiento de las piezas y óptima respuesta de los tejidos peri-implantares (Fig. 29).

Se concluye que el planeamiento, la técnica y los materiales utilizados son condecentes con un excelente y previsible resultado para tal indicación.
