You are eligible for benefits if you are an active, full-time employee working an average of 30 or more hours per week. If you are a new hire, your coverage will be effective on the first of the month following 30 days of employment.
You may also enroll eligible dependents for benefits coverage. The cost for coverage depends on the number of dependents you enroll and the benefits you choose. When covering dependents, you must select and be on the same plans.
Your legal spouse
Children under the age of 26 regardless of student, dependency, or marital status
Children over the age of 26 who are fully dependent on you for support due to a mental or physical disability, and who are indicated as such on your federal tax return
Once you elect your benefit options, they remain in effect for the entire plan year until the following Open Enrollment. You may only change coverage during the plan year if you have a Qualifying Life Event, some of which include:
Marriage, divorce, legal separation, or annulment
Birth, adoption, or placement for adoption of an eligible child
Death of your spouse or child
Change in your spouse’s employment status that affects benefits eligibility
Change in your child’s eligibility for benefits
Significant change in benefit plan coverage for you, your spouse, or your child
COBRA event, judgment, or decree
Becoming eligible for Medicare, Medicaid, or TRICARE
If you have a Qualifying Life Event and want to change your elections, you must notify Human Resources and complete your changes within 30 days of the event. Our dental plan helps you maintain good oral health through affordable options for preventive care, including regular checkups and other dental work. Coverage is provided through MetLife
Two levels of benefits are available with the DPPO plan: innetwork and out-of-network. You may see any dental provider for care, but you will pay less and get the highest level of benefits with in-network providers. You could pay more if you use an out-of-network provider.

Type A – Preventive Care
Oral examinations, full mouth X-rays, bitewing X-rays (adult/child), prophylaxis – cleanings, emergency palliative treatment (children to age 14): sealants, space maintainers, topical fluoride applications
Type B – Basic Restorative
Amalgam and composite fillings, prefabricated crowns, endodontics root canal, periodontal surgery, periodontal scaling & root planing, periodontal maintenance, oral surgery (simple extractions), oral surgery (surgical extractions), other oral surgery, general anesthesia
Type C – Major Restorative Crowns/inlays/onlays, repairs, bridges, dentures, consultations, implant services
Type D – Orthodontia

1In-network benefits refers to benefits provided under this plan for covered dental services that are provided by a participating dentist. Out-of-network benefits refers to benefits provided under this plan for covered dental services that are not provided by a participating dentist. Utilizing an out-of-network dentist for care may cost you more than using an innetwork dentist.
2Negotiated fees refer to the fees that participating dentists have agreed to accept as payment in full for covered services, subject to any copayments, deductibles, cost sharing, and benefits maximums. Negotiated fees are subject to change.

4Out-of-network benefits are payable for services rendered by a dentist who is not a participating provider. The Reasonable and Customary (R&C) charge is based on the lowest of:
The dentist’s actual charge (the ‘Actual Charge’);
The dentist’s usual charge for the same or similar services (the ‘Usual Charge’); or
The usual charge of most dentists in the same geographic area for the same or similar services as determined by MetLife (the ‘Customary Charge’).
For your plan, the Customary Charge is based on the 80th percentile. Services must be necessary in terms of generally accepted dental standards.
Our vision plan offers quality care to help preserve your health and eyesight. Regular exams can detect certain medical issues such as diabetes and high cholesterol, in addition to vision and eye problems. You may seek care from any vision provider, but the plan will pay the highest level of benefits when you see an in-network provider. Coverage is provided through MetLife

Life and Accidental Death and Dismemberment (AD&D) insurance through MetLife are important to your financial security, especially if others depend on you for support or vice versa. With Life insurance, you or your beneficiary(ies) can use the coverage to pay off debts, such as credit cards, loans, and bills. AD&D coverage provides specific benefits if an accident causes bodily harm or loss (e.g., the loss of a hand, foot, or eye). If death occurs from an accident, 100% of the AD&D benefit would be paid to you or your beneficiary(ies). Life and AD&D coverage amounts reduce by 35% at age 65, 60% at age 70, 75% at age 75, and 85% at age 80.
Basic Life and AD&D insurance is provided at no cost to you. You are automatically covered at $15,000 for each benefit.
A beneficiary is the person or entity you elect to receive the death benefits of your Life and AD&D insurance policies. You can name more than one beneficiary, and you can change beneficiaries anytime. If you name more than one beneficiary, you must identify how much each beneficiary will receive (e.g., 50% or 25%).
You may buy more Life insurance for you and your eligible dependents. If you do not elect Voluntary Supplemental Life insurance when first eligible, or if you want to increase your benefit amount at the next Open Enrollment, you may need to show proof of good health. You must elect Voluntary Supplemental Life coverage for yourself before you may elect coverage for your spouse or children.

Disability insurance provides partial income protection if you are unable to work due to a covered accident or illness. We offer Short Term Disability (STD) insurance for you to purchase through MetLife
STD coverage pays a percentage of your weekly salary if you are temporarily disabled and unable to work due to an illness, non-work-related injury, or pregnancy. STD benefits are not payable if the disability is due to a job-related injury or illness. If a medical condition is job-related, it is considered Workers’ Compensation, not STD
