Welcome to our 2019 Summer Monitor Dear neighbors, Summer is in full swing and I hope it finds you and your family in good health. It’s my pleasure to welcome you to our combined Summer Monitor and 2018 EvergreenHealth Community Annual Report.

We’ve made significant strides toward our health system’s vision to be your most trusted source for health care solutions —and on behalf of our board and entire organization, we are honored to share our highlights with you from the past year in our Community Annual Report.

Among the many things I’ve learned, as a physician and as a health care administrator, is the importance of being a good listener. As an organization, intently listening to our community provides us with meaningful insight that helps us continually learn what’s most important to you, and what you value most in a health care partner.

Listening to your feedback has taught us a great deal, and influences both the care and service we provide. As we continue to evolve, we remain steadfast in our vision to strengthen and expand EvergreenHealth as your public hospital, so that we can continue to serve Eastside families with the highest quality health care and services in an environment of absolute safety now, and well into the future.

Communication Is Key: Empowering Patients Throughout the Expected and Unexpected When a mother’s childbirth experience didn’t go as planned, her care team stepped in to advocate for her goals and desires.

When Kaley returned to Dr. Wittman’s office the next morning, the team detected that the baby could be in distress. Kaley was escorted directly to the Family Maternity Center for immediate care—and possibly, an emergency C-section.

Throughout Kaley’s prenatal care, she and Dr. Wittman talked extensively about Kaley’s desire for an unmedicated childbirth. Their conversations were guided in part by a new program, Team Birth Project, piloted

by EvergreenHealth’s Family Maternity Center designed to empower patients, foster teamwork and communication and support decision-making that helps women and families play an even more active role in their birth stories.

As the situation quickly escalated, it became apparent that Kaley’s birth experience would follow a different path than she’d planned.

Kaley Porter-Perry was nearly 40 weeks pregnant with her first child when a record-setting snowfall blanketed the Puget Sound area earlier this year, bringing the region to a standstill.

So when she stopped feeling her baby move, it was especially concerning for the soon-to-be mom. Since her OB practice was closed for the day due to the snow, her physician, Dr. Mary Wittman with Eastside OB/GYN, advised her to go to the hospital for fetal monitoring.

Luckily, tests showed small movements from the baby, and Kaley went home with peace of mind knowing she had a regularly scheduled OB appointment the following day.

“We tried several interventions for helping the baby, but he showed signs of distress early on in the process. Though things were happening quickly, it was important to pause and explain what was happening and include Kaley and her husband Travis in the decision-making for our next steps,” Dr. Wittman explained.

“I expected to have to advocate for myself in order to have the experience I envisioned—which is difficult even under the best circumstances of labor and childbirth,” Kaley described.

However, thanks to Dr. Wittman’s attentiveness, she was there to advocate on Kaley’s behalf, inviting Kaley as a collaborator and a partner in the decision-making process as the delivery progressed. Together, Kaley’s care team confirmed that an emergency C-section was the best option for delivering the baby safely, and a new birth plan was set in motion, still with Kaley’s desires front and center.

“As a physician, I try to see the situation through the patient’s eyes and ask myself, ‘What will she remember about this experience?’ It’s my goal to make sure we’ve done everything we can to ensure they have the experience they desire, while keeping everyone’s safety at the forefront,” Dr. Wittman said.

“Our birth plan had gone out the window, but I was able to trust my OB even more because I knew she had exhausted all possible options for sticking with an unmedicated birth. When we found ourselves in an emergent situation, I could let go of that fear of needing to advocate for myself because I knew Dr. Wittman had my needs and preferences in mind,” Kaley added.

Grayson Philip Perry was delivered safely on February 5, 2019 at 12:24 pm, weighing 9lbs. 5 oz., and the whole family is healthy and living well.

More about Team Birth Project The Team Birth Project is an innovative program led by Ariadne Labs, a joint center of Brigham and Women’s Hospital and the Harvard T.H. Chan School of Public Health. Team Birth is focused on empowering mothers and all members of the birth team to initiate communication at any time during delivery, and inspire more collaborative teamwork to achieve better outcomes for mothers and babies. EvergreenHealth and nearby Overlake Medical Center are among the four hospitals in the nation piloting the program, which aims to reduce unnecessary C-sections and increase the rate of vaginal deliveries, while enhancing safety and patient experience for laboring mothers and their families through improved communication tools and strategies.

According to Kaley, though Grayson’s birth was completely different than she had envisioned, she says the experience stands out to her because of the exceptional care, service and collaborative partnership from her team.

“Even though it was overwhelming at first, my experience was really positive. I felt like all my needs, desires and wishes were respected by the whole team—and most importantly, my son was delivered safely,” Kaley said.

Part of my goal when treating younger patients is to make a visit to my office not only comfortable, but also an enjoyable experience that will encourage them to return for their next preventive care appointment. This is one of the reasons why I helped launch Reach Out and Read™ at EvergreenHealth’s Redmond primary

Reach Out and Read is a national program that partners with 5,800 health providers across the country to provide nearly 7 million books to children annually. Research has shown that parents who participate in the program are more than twice as likely to read to their children on a regular basis, while children’s language development increases by three to six months.

At each well-child visit for children between the ages of 6 months and 5 years old, I am now able give my patient a new book to take home—which equals up to nine books by the time the child is 5 years old! Having access to this program helps me encourage parents and children to incorporate reading time into their daily routine, which is so important in a child’s development and early academic success. It also serves as another way to connect with my patients, in addition to traditional touchpoints like vaccination updates and vision tests. It’s a win-win for everyone involved.”

Alberto Rodriguez, EvergreenHealth Food Services Respect & Accountability “Hospitals are not generally known for the quality of their food service, but at EvergreenHealth, we’ve established a dining program that resolves that issue. Our unit hosts help enhance the overall patient experience by helping patients select customized meals that align with their dietary needs and preferences, and delivering the meals at a time specified by the patient. We have also eliminated the use of disposable packaging for utensils, which makes them more accessible to patients with reduced motor function while supporting our commitment to environmental sustainability.”

I want to know who they are, how they enjoy spending their time, and any thoughts or anxieties they might have about needing surgery. EverHealthy: Blueprint for Advancing Our Community’s Health Proposition 1, included on the August 6 primary election ballot, will provide funding to support new projects vital to the ongoing health of Eastside families.

When EvergreenHealth first opened its doors as a public district hospital by strengthening our infrastructure and upgrading our services with nearly 50 years ago, it did so with the community’s vision to serve this the newest technology, fostering even safer environments for the area with access to the highest quality health care and services. best possible outcomes.

Since then, both the region and our health system have experienced tremendous growth. In response to this growth, the EvergreenHealth Board of Commissioners approved a master facilities plan called EverHealthy, created to ensure we can continue to serve our patients’ needs now, and well into the future. With EverHealthy-Proposition 1, we can continue to meet the health needs of our growing community

On occasion, we’ve come to the community to ask for your support to help with community growth and evolving health needs. Thanks to that support, we were able to open a hospice center and hospice home care program in 1991 as well as an expanded emergency department and eight-story inpatient tower in 2007.

What specific projects within the EverHealthy plan will Proposition 1 support, if approved?

As with everything we do, our focus is on safety and growth that meets the clinical and service needs of our patients and community.

On August 6, our district’s residents will be presented with the opportunity to vote on the future health of our community through the following projects in the upcoming election:

DISASTER READINESS

CRITICAL CARE UNIT

Life-saving seismic strengthening to core areas of the original hospital structure and parking structures. Due to our geographical location, experts have named EvergreenHealth as one of the only viable medical centers to sustain service and provide care in the event of a major earthquake.

Construct a new critical care unit designed to support the best possible outcomes and experience for patients and their families during the most critical situations.

OUTPATIENT MEDICAL BUILDING

FAMILY MATERNITY CENTER

NEW LIFE-SAVING MEDICAL EQUIPMENT

Construct a medical building for programs such as outpatient mental health.

Significantly upgrade and expand the Family Maternity Center to continue to offer the best childbirth experience and outcomes, preserving our legacy as America’s first Baby-Friendly Hospital and the area’s most preferred hospital for care.

Provide life-saving medical equipment, technology and facility upgrades to support these projects, and poise our organization for ever-evolving technology and the next generation of care.

How is EverHealthy Funded?

On August 6, the community will have the opportunity to vote on a $345 million bond levy to fund enhancements to the EvergreenHealth medical center campus, helping the health system continue to meet the health care needs of its growing community. Tax rates will not increase for district homeowners.

Evergreen Gala Raises $1.2 Million The 2019 Evergreen Gala presented by Denali Advanced Integration raised an outstanding $1.2 million to support EvergreenHealth’s Neurological Surgery Program.

On Saturday, April 27, more than 600 physicians, community leaders and friends of EvergreenHealth gathered in our signature tent at King County’s Marymoor Park to support EvergreenHealth’s Neurological Surgery Program. The theme of the 18th annual gala, Rat Pack Bash, lent itself to a fun night full of entertainment, bidding and philanthropy.

The tremendous community support at this year’s Gala will enable the purchase of state-of-the-art equipment, which provides advanced visualization for neurosurgical procedures and an enhanced patient experience.

2018 Highlights Welcoming New Leadership EvergreenHealth Monroe Welcomes New Leadership EvergreenHealth Monroe’s executive leadership team welcomed Brenda Sharkey, MSN, RN, as chief nursing officer. With 25 years of nursing experience, Sharkey is responsible for overseeing day-to-day and strategic operations for inpatient nursing areas, emergency services and overall nursing practice at EvergreenHealth Monroe.

EvergreenHealth Monroe also welcomed John Green, CPA, as chief financial officer. Green is an experienced administrator and brings nearly 40 years of health care industry financial expertise to the executive team.

Growing to Meet the Needs of Our Community Introducing Biplane Imaging for Neurovascular Disorders EvergreenHealth introduced a biplane imaging machine as part of an entirely new neurovascular treatment suite which, in addition to a procedure room, features five preparation and recovery rooms for patients. The biplane machine offers the most advanced interventional imaging technology available, enabling physicians to treat life-threatening neurovascular conditions with minimally invasive procedures.

EvergreenHealth Opens Primary Care in Mill Creek EvergreenHealth expanded its existing Mill Creek urgent care location to include a new primary care practice, led by Dr. Jinat Parveen. In 2018, EvergreenHealth also welcomed 25 new providers across 15 specialities to continue serving the health needs of the growing Eastside community.

EvergreenHealth Monroe Upgrades Emergency Department with New Equipment and Online Wait Times Thanks to funds raised at the 2017 Blue Jeans and Boots Gala, in 2018 EvergreenHealth Monroe added a range of advanced equipment to the hospital’s emergency department. The new safety and comfortfocused tools include a mobile ultrasound machine, Accuvein digital vein viewing system, high-capacity digital rolling scales and weighing gurneys, and CO-Oximeter hand-held kits to test patients’ oxygen levels.

Additionally, EvergreenHealth Monroe launched a new online scheduling tool to help reduce wait times by giving patients the option to reserve their spot in line for emergency care, and to check live wait times on EvergreenHealth Monroe’s website before their visit. Patients can also compare EvergreenHealth Monroe’s wait times with those of the health system’s Redmond emergency care location, to provide even greater choice for seeking immediate care.

Levy Program Spotlights EvergreenHealth’s Nurse Navigator & Healthline Expands with 24/7 Dedicated Nurses for New Parents

STEM Global Health Education Partnership with Lake Washington, Northshore and Riverview School Districts

With the support of the community’s levy funds, EvergreenHealth’s 24/7 Nurse Navigator & Healthline offers 24/7 on-call nurse triage; provider referral and scheduling assistance; community health class registration; postdischarge follow-up; health care services and insurance resource navigation, and more.

In 2018, EvergreenHealth completed its sixth year of partnership with Juanita High School’s Global Health STEM program. The joint collaboration is open to students in 11th and 12th grades, and provides them opportunities to job shadow EvergreenHealth providers, attend the hospital’s Grand Rounds, learn from EvergreenHealth guest speakers and present information on important global health topics to their fellow peers.

In 2018, Healthline served more than 100,000 callers across the region and launched a new service in partnership with EvergreenHealth’s Welcoming Baby program, a one-stop resource that offers everything new moms and their families need to help them prepare for the little one’s arrival. The Welcoming Baby line is a dedicated phone line reserved just for expecting and postpartum mothers to: • Select an OB or midwife • Register for personalized childbirth education classes • Find a lactation consultant for breastfeeding assistance • Ask questions of on-call nurses, who are available day or night Learn answers to all your pre- and postnatal questions by calling the Welcoming Baby line at 425.899.BABY (2229).

Juanita High School senior Isabel Cleofe credits the class and her job shadow experience for her decision to pursue a career in the health care industry.

Beyond learning about physical therapy from a provider perspective, Isabel also lived her own experience as a patient.

A lifelong competitive soccer player, Isabel suffered a severe injury to her clavicle in March 2018 that required reconstructive surgery and months of physical rehabilitation.

“I returned to EvergreenHealth four days after my surgery to shadow a physical therapist for class,” Isabel said. “I spent an entire day listening to other patients who were just like me, and witnessed how their eyes lit up when working with the physical therapist. I realized that I could be that same source of hope for people recovering from injuries or other debilitating conditions, and I knew right away that’s what I wanted to do.”

This fall, Isabel is attending the University of Hawaii at Mānoa to study health and exercise science in preparation for a career as a physical therapist.

Community Healthcare Access Team

EvergreenHealth Geriatric Care

EvergreenHealth’s Community Healthcare Access Team (CHAT) is comprised of a group of compassionate social workers who support community members facing barriers to affordable, high-quality health and social services.

EvergreenHealth Geriatric Care is a multidisciplinary practice that, with the support of community levy funds, provides high-quality, coordinated care to the community’s most vulnerable patients. The team of specialists, including physicians, nurses, social workers and pharmacists, collaborates to assess and develop treatment plans for geriatric patients with complex health needs.

Supporting Students in Crisis Through a unique partnership, Lake Washington School District and EvergreenHealth established a program to place EvergreenHealth social workers in local high schools to assess students with high-risk behavioral and mental health conditions, and assist them in seeking affordable treatment that doesn’t require them to leave school for an extended period.

CHAT social workers were able to provide 793 crisis interventions to students, including contact with those expressing suicidal thoughts. In one case, a 15-year-old student was referred by his school’s guidance counselor to an EvergreenHealth social worker who provided the student with a safe space to share his feelings, while assessing his level of safety, and working in partnership to develop a safety plan.

Together, they also decided to include the student’s parents in the conversation so they could be involved in the plan and learn how to best support their son during this challenging time.

Because the student’s family did not have health insurance to cover the costs of counseling, EvergreenHealth’s social worker helped his mother navigate Apple Health options and select a therapist, and continued to follow up until he could begin treatment to ensure he was staying on track. By having access to resources through CHAT, the student received real-time support in a familiar environment and is living well today.

In 2018, CHAT helped 5,358 community members access appropriate resources for insurance, financial assistance, behavioral health services, information and help with general health care questions.

As part of its chronic care management program, in 2018 EvergreenHealth Geriatric Care implemented a transitional care management program offering expanded support and resources for patients as they transition from the hospital to home, or in between clinic visits. The goal of the program is to reduce hospital readmission rates and address any health issues as they arise, preventing symptoms from worsening and potentially lessening the need for another clinic or hospital visit.

Through these coordinated efforts, EvergreenHealth Geriatric Care reduced its readmission rates to 9.28% in 2018— significantly lower than the state’s average readmission rate of 13.8%.

“With an average age of 85, the patients we treat typically have been diagnosed with at least five or six different health conditions. Every member of our care team collaborates regularly—from the pharmacists who organize and package daily medication dosages, to our social workers who follow up on a potentially unsafe living situation—to ensure we are providing the appropriate care to treat the complex needs of our patients with compassion, at the right time and in the right place.”

Levy Program Spotlights Northshore Senior Center

Youth Mental Health Task Force

With support from EvergreenHealth levy funds, Northshore Senior Center (NSC) served 2,344 seniors in 2018 with evidence-based health and wellness programs. In April, Cindy Tang, MSW, joined NSC as the organization’s first community mental health specialist, a new role designed to serve individuals seeking support for mental health issues and assistance navigating social services resources.

With the support of community levy dollars, EvergreenHealth’s Youth Mental Health Task Force promotes awareness of the mental health challenges facing young adults, encourages the exchange of information and resources among community members, and fosters grassroots solutions.

“By adding a mental health specialist to our staff, we were able to expand and bolster our programs and service offerings to better accommodate those with mental health, housing, caregiving and family issues, all of which are commonly experienced among the population we serve,” Janet Zielasko, NSC Wellness Manager, explained.

“For example, we launched a new grief and loss support group to help participants through the grief process after experiencing a loss. And with funding from EvergreenHealth, we can now provide access to counseling services for those in need.”

Christine’s Story 70-year-old Kirkland resident Christine (Chris) Hanley has taken many classes at NSC over the years—such as fitness and art classes—but the eight-week “Matter of Balance” course listed in NSC’s newsletter, immediately caught her eye. The Matter of Balance class is an evidence-based group program that teaches seniors to view falls and the fear of falling as manageable obstacles, while offering physical exercises to improve strength and balance to reduce the risk of falling.

“The course was as much of a physical learning experience as it was an emotional support group,” Chris recalled. “Not only did a physical therapist teach us exercises and demonstrate what to do if we did fall, but the other participants and I also used it as an opportunity to exchange ideas and resources, truly supporting each other through the process. By the end of it, we were all much more confident in our own ability to keep ourselves safe from falls.”

Since 2016, the task force has expanded its reach beyond EvergreenHealth, forging partnerships with other like-minded community organizations to develop programming and resources. One such collaboration is with National Alliance on Mental Illness (NAMI) Eastside, which hosts monthly educational forums at EvergreenHealth featuring guest speakers who share information and resources on various topics relating to mental health.

In 2018, Dr. Brenda Butterfield led mindfulness workshops at the joint forum, which taught participants how to develop mindful self-care practices for managing chronic stress and coping with life’s challenges. EvergreenHealth also teamed up with Studio East and various community partners, including NAMI, to host a screening and teen panel discussion of “Angst,” as well as a resource fair. “Angst” is a documentary designed to help viewers identify and understand the symptoms of anxiety, and encourage youth and families to reach out for help.

“I was inspired to join EvergreenHealth’s Youth Mental Health Task Force after being touched by mental health issues within my own family from a young age, and more recently, bearing witness to the tragic deaths of youth in our community due to opioid overdoses and suicide. I’m a strong advocate for breaking the stigma around seeking mental health care, and feel passionate about encouraging other parents to talk openly with their children about mental health.”

EvergreenHealth Hospice and Palliative Care Programs Many patients with advanced cancer have significant symptoms that can be caused by the cancer itself, by cancer treatments, or both. Additionally, making decisions about the type of treatment and care that’s best for each individual can be difficult for patients and their families. Many studies have shown that providing care focused on managing symptoms and clarifying treatment decisions can improve both quality and quantity of life for those living with advanced cancer.

In 2018, EvergreenHealth was included in a report compiled by the Hutchinson Institute for Cancer Outcomes Research (HICOR), which evaluated hospitals and clinics across Washington on the intersection of clinic-level quality metrics Hope Wechkin, MD, EvergreenHealth Hospice and Palliative Care programs medical director

These metrics are a positive reflection of that work, but most importantly, we remain focused on partnering with patients and families to understand what’s most important to them so that we can provide care that is truly meaningful.”

and their associated costs. The report’s goal was to encourage providers to share best practices, and ultimately to improve the quality of cancer care for patients while lowering costs.

The HICOR report measured four end-of-life care metrics including, administering chemotherapy in the last 14 days of life, emergency department visits and intensive care stays in the last 30 days of life, and hospice care provided for three or more days prior to death.

In these key metrics, EvergreenHealth outperformed the regional average in both quality and cost.

In 2018, EvergreenHealth’s Hospice Care team also recognized the need for a more affordable inpatient care option for short-term, acute care patients.

With the help of levy dollars, patients receiving inhome hospice care who are treated for an acute event at the Hospice Center, can now continue inpatient treatment for a reduced daily rate, making this service more accessible to patients and families across the region while enhancing comfort and quality of care for those nearing the end of life.

EvergreenHealth Financial Stewardship In 2018, EvergreenHealth invested in several initiatives to expand and enhance patient care, including its Master Facilities Plan project to upgrade the health system’s infrastructure and facilities, to meet the community’s needs as they grow and evolve.

Message from the Board Chair & Director Dear Neighbors, The EvergreenHealth Monroe Foundation strives to improve the health and well-being of the greater Sky Valley and East Snohomish County with the support of our philanthropic partners.

Through our generous supporters, including staff, patients, local foundations and service clubs, we met our 2018 goal to renovate the EvergreenHealth Monroe Cancer Care Center, thanks in part to the record $194,000 raised at our annual Blue Jeans & Boots Gala in September. At this event, we also embraced the opportunity to honor community member Katy Woods for her exemplary community service to Monroe and the Sky Valley community.

Fran Wockner, on behalf of the William E. Wockner Foundation, gifted a transformational contribution to support the EvergreenHealth Monroe Cancer Care Center, as well. This generous donation enabled the purchase of six new infusion chairs equipped with a heating element and massage to offer additional patient comfort during infusion treatments.

The new Cancer Care Center accommodates advanced cancer care services through a collaborative partnership with Seattle Cancer Care Alliance. Since January 2019, patients have benefited from access to cutting-edge cancer treatments by expert physicians right here in Monroe. Thank you to everyone who came together to make this possible!

2018 EvergreenHealth Monroe Foundation Highlights New Cancer Care Center at EvergreenHealth Monroe In 2018, the EvergreenHealth Monroe Foundation set its sights on helping to bring the highest quality of cancer services to Monroe. The 2018 Blue Jeans & Boots Gala brought in a record-breaking $194,000 to support the renovation and redesign of the new Cancer Care Center at EvergreenHealth Monroe. Today, in collaboration with expert physicians from Seattle Cancer Care Alliance, the Cancer Care Center is open to see patients right here in the Sky Valley.

EvergreenHealth Earns Five-Star CMS Overall Hospital Quality Star Rating Earlier this year, EvergreenHealth became one of only six hospitals statewide and two within the Puget Sound region to receive a five-star overall rating from the Centers for Medicare and Medicaid (CMS). Achieving a five-star rating represents EvergreenHealth’s high scores across a range of quality measures and places it among the top 293 hospitals in the nation for quality.

EvergreenHealth Receives “A” Rating for Hospital Safety The Leapfrog Group, a leading nonprofit watchdog organization on hospital quality and safety, has once again presented EvergreenHealth with an “A” rating for patient safety, reflecting the health system’s commitment to keeping patients safe from medical errors, injuries, accidents and infections. The Leapfrog Hospital Safety Grade uses 28 measures of publicly available hospital safety data to assign grades to more than 2,600 U.S. acute-care hospitals twice per year, and EvergreenHealth has historically been one of only 11 hospitals in Washington state to earn the highest “A” grade.

Vote on Proposition 1 in the Primary Election by August 6 This summer, the community will vote on Proposition 1, a $345 million bond levy to fund enhancements to the EvergreenHealth medical center campus to help the health system continue to meet the health care needs of its growing community without increasing tax rates for district residents.
