STAFF Peter M. Ashmore, Managing Attorney University of Missouri (B.A., Political Science, 1991). DePaul University College of Law (J.D., 1994). Hired as a staff attorney upon graduation from law school, Peter had already interned, externed, and volunteered with CVLS. As Managing Attorney his duties include directing CVLS’ IT and database needs, organizing staff and management meetings and overseeing CVLS’ CLE programs. As director of CVLS’ Guardian Ad Litem for Adults Program, Peter trains, recruits and supervises volunteers who serve as GALs for disabled adults and represent parties in contested cases. In addition, Peter advises volunteers on an eclectic variety of cases, including family law and minor guardianship, tort defense, eviction, contracts, real estate, bankruptcy, probate, tax and public benefits. Peter formerly served as President of the Illinois Center for Law & Technology, now known as Illinois Legal Aid Online and as a member of the Illinois State Bar Association’s Delivery of Legal Services Committee. He is a member of the Chicago Bar Association’s Probate, Elder Law, Mental Health and Consumer Law committees. Margaret C. Meg joined CVLS in 1982 to set up the Panel Referral Program, became Deputy Director in 1983 and was elevated to Executive Director in 2003 when her predecessor retired. While responsible for overall program management and funding, Meg also coordinates CVLS’ bench, bar and law firm relations. An experienced family law litigator, Meg helped draft substantive changes to the Illinois Probate Act and still steps in on minor guardianship cases from time to time. Meg regularly speaks on child custody, client and pro bono issues for the ISBA, CBA and IICLE and writes a bi-monthly column on pro bono for the Chicago Lawyer. In 2012, she was honored with Association of Corporate Counsel-Chicago Chapter’s Thurgood Marshall Lifetime Achievement Award. Prior honors include the Illinois State Bar Association’s Board of Governors Award in 2007; the Chicago Bar Foundation’s Thomas H. Morsch Public Service Award in 2001; the Lawyers Trust Fund of Illinois’ Esther Rothstein Award in 1999

and Loyola University School of Law’s Public Service Award in 1994. She is an active member of ISBA’s Legislation Committee and former chair of the ISBA’s LawPac Board. She co-chairs the Chicago Bar Foundation’s Advisory Committee Legal Aid Academy and serves on its Pro Bono Week Committee. Past activities include: chair of the Illinois State Bar Association’s Delivery of Legal Services Committee, charter member of the Illinois Pro Bono Center’s Board of Directors, speaker at the Midwest Region SPAN Conference on the Future of Legal Services, Board of Directors of the Women’s Bar Association of Illinois and two-term ISBA Assembly member. Meg’s husband, Art Simon, is an active CVLS volunteer. Eric S. Donahue, Controller Purdue University, (B.A., School of Liberal Arts 1996). Eric joined CVLS in 2002 after working for law firms in Indiana. As controller, he oversees all accounting, financial reporting and the financial internal control operations of CVLS. He handles CVLS’ day-to-day financial activities and manages all outsourced functions. Eric also manages various aspects of CVLS’ human resources including the 403(b) retirement plan and serves as staff liaison to the CVLS finance committee and CVLS junior board. Susan L. DeCostanza, Staff Attorney, Guardian ad Litem for Minor and Adult Programs Boston College (BA in English Literature & Elementary Education, 2001), DePaul University College of Law (JD, 2008). Before joining CVLS as a staff attorney in 2010, Susan was a CVLS law student intern. In addition, she volunteered while working in her early post-graduation jobs at Chicago-Kent College of Law as a public interest career counselor and DePaul as pro bono program coordinator. Susan also has experience with civil rights litigation in both not-for-profit and law firm settings. Now she serves as Probate staff attorney, serving as guardian ad litem for children and disabled adults and providing extensive volunteer support to both programs. A member of the Chicago Bar Association, the Illinois State Bar Association and the PILI Alumni Network Leadership Council, Susan received the esteemed Kimball R. Anderson and Karen Gatsis Anderson Public Interest Law Fellowship in 2012 for her demonstrated commitment to public interest work,

A CVLS 711 intern during law school, after graduation Keri worked as a staff attorney and then pro bono coordinator for Lawyers’ Committee for Better Housing, where she supervised volunteers and defended tenants in eviction actions. She joined CVLS’ busy foreclosure mediation program in the fall of 2011, handling cases, supervising and assisting volunteers, and performing a myriad of other essential tasks for this demanding program. She is a member of the Chicago Bar Association’s Legal Aid Committee and, in a prior life, taught English as a foreign language to Spanish speakers. Jessica Engle, Office Manager Jessica joined CVLS in 2009 after working as a customer service representative for Graphics Suisse, a paper supply company. In charge of all of CVLS’ administrative functions from ordering and monitoring supplies to handling the paperwork for CVLS’ CLE program, Jessica also maintains office equipment and the volunteer, donor and friends databases, opens new files, arranges volunteer orientations, seminars and meetings, handles bookkeeping chores and, in her spare time, assists with client intake. Allegra Cira Fisher, Staff Attorney, Cook County Foreclosure Mediation Program University of Miami, Coral Gables, Florida, (B.S., Communications, 2002). Kristen Harrison, Panel Program Coordinator Kristen, CVLS’ former intake worker and receptionist, does her best to get cases to our hundreds of panel attorney volunteers. Reaching out by phone and email, Kristen encourages volunteers to find the perfect case on CVLS’ list of pro bono opportunities or offers them one selected just for them. In addition, Kristen supports panel volunteers and their clients, to provide them with the perfect pro bono experience. Sarah D. Hollibaugh, Volunteer Coordinator, Cook County Foreclosure Mediation Program Northwestern College (B.A., Theatre, 2008), Roosevelt University (M.A., Women’s and Gender Studies, 2010) Sarah started at CVLS in February of 2014 as the Volunteer Coordinator for the Foreclosure Mediation Program. As cases become available, she contacts volunteer attorneys to ensure that the cases get placed in a timely manner, while providing any needed support for the volunteers, as well as the clients. She works closely with The John Marshall Law School’s Pro Bono Program to ensure 711 students are provided with mediation cases and have all the information and documents that are necessary. She acts as a liaison for volunteers by providing information about the program, CVLS, and upcoming Foreclosure Mediation Training. She provides administrative

Matthew Hulstein, Staff Attorney, Chancery Court Access to Justice Program Northwestern College (B.A. Writing and Rhetoric; Theater and Speech 2009) University of Iowa (J.D. 2012) Matt first joined CVLS as a college intern in 2008. He then worked for CVLS as a PILI intern during the summer of 2011 and stayed on as a volunteer while he finished his third year of law school at DePaul. Graduating from law school in 2012 with highest service honors, Matt was recognized for volunteering the most pro bono hours in his graduating class. By the time he joined CVLS as a staff attorney in September, 2012, he already had a substantial CVLS caseload. Matt was honored in 2012 with the Chicago Bar Foundation’s Chicago Sun-Times fellowship in recognition of his commitment to the public interest. When he isn’t handling foreclosure, insurance, and administrative review cases, he loves to groove to classical music. Adel Johnson, Development Associate Columbia College Chicago (B.A., Marketing & Public Relations, 2011) Adel joined CVLS in January of 2014 as a Development Associate. She works on supporter outreach and efforts to promote CVLS’ 50th anniversary, with a focus on using digital media to improve overall communications. Adel’s previous experience includes work in digital communications with a public affairs firm, promotion and design with a music venue, copywriting with an online marketing agency and development with a nonprofit food pantry. In her spare time, she volunteers at the bookstore run by youth literacy organization Open Books. Kathy Koester, Administrative Assistant, Chancery Court Access to Justice Program Kathy started working at CVLS as a paralegal with the Cook County Foreclosure Mediation Program. Moving to the Chancery Court Access to Justice Program this year, Kathy assists program staff and volunteer attorneys filing motions, scheduling appointments, scanning documents, contacting clients and performing any variety of administrative

tasks. In addition, she creates and updates protocols and procedures for this ever-changing and growing program. Phillip J. Mohr, Deputy Director University of Illinois, Champaign, (B.A., Economics, 1987). De Paul University College of Law (J.D., 1991). Formerly a CVLS volunteer, Phillip joined staff in 1991, became the Family Law Director in 1996, and was named the Deputy Director in 2006. His own large caseload and the work of his volunteer panel does much to meet our greatest need: representation of clients on contested family law matters. In addition to trial work, he also handles appellate work and argued, along with a CVLS volunteer, before the Illinois Supreme Court. As an experienced family law practitioner, Phil spends hours tutoring volunteers, both individually and through seminars, gradually guiding them from simple to complex cases. Recognizing his talent for training, Phil runs the summer student and in-house volunteer programs. Phil is a regular speaker on family law and pro bono issues, speaking at NLADA Equal Justice Conference, statewide Legal Advocates Conferences, the ISBA mid-year meeting, and on the ISBA’s cable production. Former chair of the Chicago Bar Association’s Legal Aid Committee, Phil also chaired a legal service’s consortium committee that created pro-se family law forms and materials and assisted with the creation of the Domestic Relations Division’s Self-Help Center. He also is a Legal Content Editor of the Family Law and Adoption practice area of the Illinois Legal Aid Online’s websites and co-chair of the Clerk of the Circuit Court Pro Se Litigants Advisory Committee. Phil is a member of the Illinois State Bar Association and a former ISBA Assembly and ISBA Family Law Section Council member. CVLS staff and volunteers represent clients in mortgage foreclosure litigation and mediation, in Chicago Housing Authority terminations, in title fraud, mortgage fraud, and predatory lending cases and in other Chancery litigation matters. Under her leadership, CVLS has represented thousands of homeowners in mediation and litigation. In addition to recruiting, training and supporting more than 400 volunteers, Patty handles her own challenging caseload. She also arranges ongoing seminars and training workshops in these areas. Patty has been a featured speaker on mortgage foreclosure defense and title and mortgage fraud for Metropolitan Family Services, the Illinois Paralegal Association, the Chicago Bar Association, the Chicago Daily Law Bulletin and Illinois Continuing Legal Education. She is the author of a foreclosure defense instruction manual, and the co-author of an eviction guide for low income pro se landlords. Previously, Patty spent eight years as our utility attorney. She is experienced in real estate litigation, foreclosure litigation and mediation, probate, landlord tenant, contract disputes, adoption, DCFS administrative hearings and acting as guardian ad litem in contested custody disputes. Patty also has experience drafting legislation, lobbying for its passage, and testifying in support of that legislation to the Illinois House of Representatives Judiciary Committee. Honored as Advocate of the Month by Illinois Legal Aid Online, Patty serves on the CBA’s Legal Aid and Real Estate Law Committees, is an active member of the Illinois State Bar Association, and serves on several task forces with Illinois Legal Aid Online. Norma Perez, Intake and Receptionist Norma screens clients for financial and case eligibility, books appointments for clinic and panel interviews and handles the walk-in clients sent directly from the courts. She also excels as the CVLS receptionist. Matthew Pollock, Director of Communications Arizona State University, (B.S., Mechanical Engineering, 2005). Before joining CVLS in July 2007, Matthew worked for six years as a mechanical engineer in Chicago and Phoenix. While toiling in the world of big business he developed a desire to give back to the community and found his way to CVLS. Eventually, Matt merged his creative instincts with his solid background using computer systems for management and design, to become CVLS’ Director of Communications. Matt’s primary responsibilities include: planning and executing CVLS’ fundraising and social events such as Race Judicata®, Vino & Van Gogh, and Law & Disorder; expanding and improving relationships with existing and prospective donors and sponsors; developing and managing CVLS’ print, electronic, social media and web-based marketing; developing and updating the design, features and content of CVLS’ website at cvls.org. In his spare time, Matt serves as liaison to CVLS’ Junior Board of Directors Rebekah Rashidfarokhi, Director, Guardian Ad Litem for Minors Program The University of Georgia (BBA—Marketing & B.A-Spanish, 2003). Joining CVLS as a staff attorney in 2007, Rebekah has handled family law, guardianship, immigration, foreclosure litigation and foreclosure mediation. Rebekah is also responsible for recruiting and training new pro bono GALs. Prior to joining CVLS, Rebekah worked at an immigration clinic at the US-Mexico border, the United National Office on Drug and Crime (UNODC) in Bogota, Colombia, and two international law firms in Madrid. She is fluent in Spanish and has represented clients in a

Rebekah is a member of the Junior Leadership Council of the National Immigrant Justice Center and a former Director of the Young Lawyers Section of the Chicago Bar Association. Daniel P. Santrella, Clinic Coordinator Dan began his journey at CVLS in 2008 as an administrative assistant in the Chancery Court Access to Justice Program. His responsibilities expanded in 2010 when CVLS began working with the Foreclosure Mediation Program. Now in 2014, after placing countless mediation cases with volunteers, running its student clinics, and maintaining all of the statistical and administrative data program-wide, Dan has stepped into CVLS’ Clinic Coordination role where he’ll provide a vital link between CVLS’ offsite clinics and our administrative offices. NOTES TO THE FINANCIAL STATEMENTS NOTE A – NATURE OF ACTIVITIES AND SUMMARY OF SIGNIFICANT ACCOUNTING POLICIES Organization and Nature of Activities Chicago Volunteer Legal Services Foundation (the “Foundation”) is organized as an Illinois not-for-profit corporation. Its purpose is to administer programs to assist in securing legal assistance and related service for persons of low income in the Chicagoland area. The Foundation’s support comes primarily from individual and corporate contributions, foundations, and law firms, as well as fundraising activities. SIGNIFICANT ACCOUNTING POLICIES Financial Statement Presentation The financial statements are presented using the accrual method of accounting, which recognizes revenue when it is earned and expenses when they are incurred. Financial statement presentation follows the recommendations of the Financial Accounting Standards Board’s Accounting Standards Codification (“FASB ASC”). As specified in FASB ASC 958, Not-for-Profit Entities, the Foundation is required to report information regarding its financial position and activities according to three classes of net assets: unrestricted, temporarily restricted, and permanently restricted net assets. Unrestricted net assets are net assets not subject to donor-imposed stipulations on their use in the Foundation’s operations, as reported in the statements of activities. Temporarily restricted net assets are donor contributions with stipulations that are to be met by actions of the Foundation or others and/or the passage of time. When a donor restriction expires, temporarily restricted net assets are reclassified to unrestricted net assets and reported in the Statement of Activities as released from restrictions. Restrictions fulfilled in the same period as the funds were received are classified as unrestricted net assets. Permanently restricted net assets are subject to donor-imposed restrictions that they be maintained permanently by the Foundation. Generally, the donors of these assets permit the Foundation to use all or part of the income earned on related investments and/or unrestricted income for the Foundation’s operations. Contributions and Other Receivables Contributions and grants are unconditional promises to give cash and other assets to the Foundation that are expected to be collected within one year at fair value at the date the promise is received. Unconditional promises to give that are expected to be collected in future years are recorded at the present value of their future cash flows. Conditional promises to give are reported at fair value when the conditions are substantially met. There were no conditional promises to give at June 30, 2014. In the absence of donor stipulations, the Foundation classifies contributions as unrestricted. Donated goods and services are recognized as revenue and expense at the fair value of the donated goods or services. Contributions, grants and promises to give, are recorded as restricted support, if they are received with donor stipulations that limit the use of the donated funds. When the stipulated purpose of the restriction imposed by the donor is met, these funds are transferred from temporary restricted to unrestricted. Total unconditional promises to give, grants and other receivables were $317,283 and $614,288 at June 30, 2014 and 2013, respectively. At June 30, 2014, $316,807 of contributions receivable and other receivables in the amount of $476 are expected to be paid within one year. The contributions receivable are composed primarily of contributions and grants and are deemed to be fully collectible by management based on historical experience.

Donated Assets Donated marketable securities are recorded as contributions at their estimated fair value at the date of donation and adjusted to market value at the end of the year. Donated securities at June 30, 2014 and 2013, were valued at $6,271, and $5,178, respectively. Donated Services The programs of the Foundation utilize volunteer legal services, which are provided directly to beneficiaries of the Foundation. No amounts have been reflected in the financial statements for donated volunteers’ time because the criteria under FASB ASC 958, Not-for-Profit Entities, have not been satisfied. However, a substantial number of volunteers have donated significant amounts of time towards activities of the Foundation. Cash Cash includes highly liquid debt instruments purchased with an initial maturity of three months or less which approximates market value. Cash includes petty cash and amounts deposited in the checking account and custodial account. Amounts deposited in the custodial account are required to be maintained in a separate account. In certain situations, cash held as short and long-term investments is classified as such. Property and Equipment Property and equipment are reported at original cost. All property and equipment purchases in excess of $750 for the years ended June 30, 2014 and 2013, were capitalized, while lesser amounts were charged to expense. Depreciation is calculated on the straight-line method over the estimated useful lives of the assets (5 to 8 years). Depreciation expense was $7,672 for the year ended June 30, 2014, and $5,427 for the year ended June 30, 2013. Functional Allocation of Expenses The costs of providing various programs and other activities have been detailed in the statements of functional expenses. The costs have been summarized in the statements of activities. Accordingly, certain costs have been allocated among the programs and supporting services benefits. Comparative Financial Information The financial statements may include in certain cases prior-year summarized comparative information in total, but not by detail and net asset class. Such information does not include sufficient detail to constitute a presentation in conformity with U.S. generally accepted accounting principles. Accordingly, such information should be read in conjunction with the Foundation’s financial statements for the year ended June 30, 2013 from which the summarized information was derived. Tax Status The Foundation is not-for-profit entity as described in Section 501(c)(3) of the Internal Revenue Service Code (“IRC”) and is exempt from federal and state income taxes for related income. It qualifies for a charitable contribution deduction in regard to its donors, and is subject to income taxes only to the extent of unrelated business income. In addition, per letter dated March 1972, the Internal Revenue Service has determined that the Foundation is a public foundation within the meaning of Section 509(a) of the Internal Revenue Code.

NOTES TO THE FINANCIAL STATEMENTS Use of Estimates The presentation of financial statements in conformity with generally accepted accounting principles requires management to make estimates and assumptions that affect certain reported amounts and disclosures. Accordingly, actual results could differ from those estimates. NOTE B – INVESTMENTS Investments in marketable securities with readily determinable fair values and all investments in debt securities are reported at their fair values, certifications of deposit are valued at the bank valuation, in the statements of financial position. Unrealized gains and losses are included in the change in net assets. Investment income and gains are reported as increases in unrestricted net assets in the reporting period in which the income and gains are recognized. NOTES TO THE FINANCIAL STATEMENTS NOTE C – FAIR VALUE OF FINANCIAL INSTRUMENTS Effective January 1, 2008, the Foundation adopted FASB ASC 820, Fair Value Measurements and Disclosures. FASB ASC 820, which establishes a single definition of fair value and framework for measuring fair value, sets out a fair value hierarchy to be used to classify the source of information used in fair value measurements, and expands disclosures about fair value measurements required under other accounting pronouncements. It does not change existing guidance as to whether or not an instrument is carried at fair value. Fair value is the price that would be received upon sale of an asset or paid to transfer a liability in an orderly transaction between market participants at the measurement date. The following fair value hierarchy is used in selecting inputs, with the highest priority given to Level 1, as these are the most transparent or reliable. The valuation techniques required by FASB ASC 820 are based upon observable and unobservable inputs. Observable inputs reflect market data obtained from independent sources, while unobservable inputs reflect the Foundation’s market assumptions. These two types of inputs create the following fair value hierarchy. Level 1

Quoted prices for identical instruments in an active market.

Level 2

Quoted prices for similar instruments in active markets, quoted prices for identical or similar instruments in markets that are not active, and model-derived valuations in which all significant inputs are observable in active markets.

Level 3

Valuations derived from valuation techniques in which one or more significant inputs are not observable, or supported by little or no market activity.

Following is a description of the valuation methodologies used for assets measured at fair value. There have been no changes in the methodologies used at June 30, 2014 and 2013. Mutual Funds: Valued at the closing price reported on the active market on which the individual funds are traded. Money Market: Valued at cost plus interest earned, which approximates fair value. The preceding approach described might produce a fair value calculation that may not be indicative of net realizable value or reflective of future fair values. Furthermore, although the Foundation believes its valuation methods are appropriate and consistent with other market participants, the use of different methodologies or assumptions to determine the fair value of certain financial instruments could result in a different fair value measurement at the reporting date.

Temporarily restricted net assets consist of two major multi-year grants benefiting the Access to Justice Program which are part of receivables at June 30, 2014 and 2013. Permanently Restricted Permanently restricted net assets of $326,549 at June 30, 2014 and 2013 were restricted to investment in perpetuity, the income from which is expendable to support the Foundation’s operations and activities.

NOTE E – PERMANENTLY RESTRICTED NET ASSETS AND FSP FASB ASC 958 The Foundation received an initial endowment (the “Endowment”) in 1993 in the amount of $200,000; and the Foundation has raised additional funds from various contributions to this endowment fund in the amount of $126,549. The Endowment has been established as a permanently restricted fund for which the income may be used to support the Foundation’s operations and activities, per the donor gift instrument. In August 2008, the Financial Accounting Standards Board issued Staff Position FSP FASB ASC 958, Endowments of Not-for Profit Organizations: Net Asset Classification of Funds Subject to an Enacted Version of the Uniform Prudent Management of Institutional Funds Act, and Enhanced Disclosures for All Endowment Funds (“FSP FASB ASC 958”). This standard provides guidance on classification of donorrestricted endowment funds for a not-for-profit organization that is subject to an enacted version of the Uniform Prudent Management of Institutional Funds Act of 2006 (“UPMIFA”) and provides other disclosures about an organization’s endowment funds.

Description of the Governing Board’s Interpretation of the Law(s) that Underlie the Organization’s Net Asset Classification of Donor-Restricted Endowment Funds The Board of the Foundation has interpreted the Uniform Prudent Management of Institutional Funds Act as requiring the preservation of the fair value of the original gift as of the gift date of the donor-restricted endowment funds, absent explicit donor stipulations to the contrary. As a result of this interpretation, the Foundation would classify as permanently restricted net assets (a) the original value of the funds donated to the permanent endowment, (b) the original value of subsequent gifts to the permanent endowment, and (c) accumulations of the permanent endowment made in accordance with the direction of the applicable donor gift instrument at the time the accumulation is added to the funds. The remaining portion of the donorrestricted endowment fund not classified in permanently restricted net assets would be classified as temporarily restricted net assets until those amounts were approved for expenditure by the organization in a manner consistent with the standard of prudence prescribed by UPMIFA.

As provided by UPMIFA, its guidelines for expenditure of a donor-restricted endowment funds apply in the absence of overriding, explicit donor stipulations. Specific provisions of donor gift instruments will take precedence over the general default provisions of UPMIFA, and these have been specified in the first paragraph of this note. Description of the Foundation’s Endowment Spending Policy The Board of the Foundation has interpreted the income and the changes in realized and unrealized value of investments as unrestricted, and available to be spent per the terms of the donor instrument gift referenced above. Description of the Foundation’s Investment Policies The Endowment has been invested in cash and the investments described in Notes B and C, with the objective of maintaining its funding power over time. It relies on a total return strategy in which investment returns are achieved through both capital appreciation (realized and unrealized) and current yield (interest and dividends). Nature and Types of Restrictions Per the terms of the Endowment instrument, the original gift is held in perpetuity. Subsequent gifts are treated in the same manner. Information on Deficiencies The Endowment was not deficient as of June 30, 2014 or June 30, 2013. NOTE F – LEASES The Foundation previously leased its office space under a 10-year, non-cancellable operating lease, which expired November 14, 2013. The Foundation was also responsible for its share of common area maintenance costs and real estate taxes. The Foundation entered into a rental agreement for new office space in April 2013 under a 12-year, non-cancellable operating lease, which expires November 30, 2025, with lease payments commencing on December 1, 2013. The Foundation has provided the landlord with an irrevocable letter of credit in the amount of $160,000 expiring on November 30, 2016 and is subject to renewal at reducing amounts per the rental agreement. The Foundation is also responsible for its share of common area maintenance costs and real estate taxes. The landlord has provided a credit for the buildout and purchase of furniture and fixtures in the amount of $517,870. Committed future minimum lease payments are as follows. Total building rent expense under all leases was $247,894 and $191,175 for the years ended June 30, 2014 and 2013, respectively. The Foundation leases certain office equipment under a non-cancellable operating lease, which expired April 2011; and the Foundation has not entered into a new lease. Lease expenses were $11,044 and $11,587 for the fiscal years ending June 30, 2014 and 2013, respectively.

NOTE H – DEFERRED REVENUE Deferred revenue includes amounts received prior to year-end related to the Foundation’s annual 5K run/ walk special event held in July. Deferred revenue for the years ended June 30, 2014 and 2013, totaled $136,870 and $99,783, respectively.

NOTE I – REDEPOSITS PAYABLE Redeposits payable represent the Foundation’s client retainers for case settlement and court costs. Redeposits payable for the year ended June 30, 2014 and 2013, totaled $7,860 and $8,171 respectively. NOTE J – EMPLOYEE 403(b) PLAN The Foundation sponsors a defined contribution 403(b) plan for its employees. Eligible employees are responsible for their investment in the plan and may make elective deferrals of their compensation, on a pre-tax basis, to the plan. The Foundation makes a discretionary contribution. The Foundation’s contributions to the Plan for the years ended June 30, 2014 and 2013, were $50,000 and $43,000, respectively. NOTE K – SIGNIFICANT CONCENTRATIONS Donations The Foundation had two contributors who contributed approximately 70.9% of the total Foundation support for the year ended June 30, 2014, and three contributors who contributed 70.8% for the year ended June 30, 2013. Cash The Foundation maintains its cash balances at financial institutions in the Chicago metropolitan area, which at times may exceed federally insured limits. Management believes that the Foundation is not exposed to any significant credit risk related to their accounts.

NOTES TO THE FINANCIAL STATEMENTS NOTE L – UNCERTAIN TAX POSITIONS As of June 30, 2014, Chicago Volunteer Legal Services Foundation did not have any uncertain tax positions that it is obligated to disclose in accordance with FASB ASC 740, Income Taxes. Additionally, there were no returns open to review or under review by taxing authorities in excess of annual statutory periods. The Foundation follows a conservative approach of research and open disclosure in regard to its tax-exempt status and positions held.

NOTE M – SUBSEQUENT EVENTS The management of the Foundation has evaluated events subsequent to the statement of financial position date of June 30, 2014, through October 14, 2014, the date the financial statements were available to be issued. It has concluded that there are no effects that provide additional evidence about conditions that existed at the statement of financial position date that require recognition in the June 30, 2014, financial statements or related note disclosures in accordance with FASB ASC No. 855, Subsequent Events.
