It’s Time for 2024-2025 Open Enrollment!
OPEN ENROLLMENT DATES: JULY 29 - AUGUST 11, 2024
OPEN ENROLLMENT
Phoenix Global will be holding a PASSIVE Open Enrollment this year. Passive enrollment means your current coverage (with the exception of the FSA) will remain in place and roll over into the new plan year, unless you want to make changes to your elections and/or covered dependents. Please review your enrollments and costs.
IMPORTANT INFORMATION
• Great News! There will be no changes to employee payroll contributions for medical/Rx, dental or vision for 2024/2025!
• There will be no changes to the current medical and prescription plans. These benefits will continue to be offered through Independence Administrators (IA) and Optum.
REMINDER: The benefits you elect during Open Enrollment will be effective from September 1, 2024 through August 31, 2025. If you miss the August 11th enrollment deadline, you will not be able to make changes/enroll until the next Open Enrollment period unless you experience a Qualifying Life Event.
ENROLLMENT SYSTEM
You MUST complete your enrollment through the ADP enrollment system at www.workforcenow.adp.com by August 11, 2024. Please see your benefits guide for instructions on how to register.
• Employees and covered dependents who are enrolled in one of the IA medical plans have access to three wellness reimbursement programs for participation in tobacco cessation, weight management, and fitness programs.
• If you wish to participate in the Flexible Spending (FSA) and/or Dependent Care (DCA) plans for 2024/2025, you MUST make a new election in the system. Your current elections WILL NOT roll over.
• If you do not make your elections in the ADP system before Sunday, August 11, 2024, your current elections will remain in place beginning September 1, 2024, and you will not have an opportunity to make changes until the next open enrollment period.

Medical & Prescription Drug Plans
There are no changes to the current plans or carriers. Employees have a choice between the PPO Low and PPO High Plans.
Employees enrolled in the medical plan will have access to the Optum Rx Price Edge benefit. Through Price Edge, when you present your ID card, the system will automatically look for any available discounts to ensure members pay the lowest possible copay on covered medications.
Vision Plan
The vision plan will remain with VSP. There will be no benefit plan changes.
Dental Plan
The dental plans will remain with Delta Dental. There will be no benefit plan changes, employees will have a choice between two PPO plans.
Health and Dependent Care Flexible Spending Accounts
Flexible Spending Accounts are administered by Flex Facts. As noted previously, if you wish to participate in either of these plans, you MUST make a new election for the September 1, 2024 through August 31, 2025 plan year.
For 2024, the annual maximum amounts are as follows:
• Healthcare FSA: $3,200
• Dependent Care FSA: $5,000 (or $2,500 if married and filing separately)
Voluntary Benefits
• Phoenix will continue to offer Voluntary Accident, Critical Illness and Hospital Indemnity coverage through Prudential.
• Pre-Paid Legal and Identity Theft coverage will continue to be offered through Countrywide.

Employee Assistance Program (EAP)
Phoenix Global will continue to offer an EAP to all employees and their immediate family members; no enrollment through ComPsych is necessary.
Life and Disability
Life and Disability benefits are offered through Prudential.
• Full-time employees working 30 hours or more per week will continue to be eligible for Basic Life and Accidental Death and Dismemberment (AD&D), Short Term Disability (STD), and Long Term Disability (LTD).
• Eligible employees have the option of purchasing Voluntary Life and AD&D coverage. When you enroll yourself and/or your dependents in this benefit, you pay the full cost through payroll deductions
− Employees: Increments of $10,000 to a maximum of $500,000 not to exceed 5x your annual earnings.
∗ Guaranteed Issue Amount: $150,000
− Spouses: Increments of $5,000 to a maximum of $100,000, not to exceed 100% of the employee’s amount. Spouse premiums are based on the employee’s age.
∗ Guaranteed Issue Amount: $35,000
− Dependent Children: (Live birth to age 26): Flat $10,000
∗ Guaranteed Issue Amount: $10,000
IMPORTANT REMINDERS
• Guaranteed Issue Amounts only apply to new hires.
• If you or your spouse are making a new election but were previously eligible or are currently enrolled and would like to increase your coverage amount, you will be required to submit a health questionnaire, which is more commonly known as Evidence of Insurability or EOI, for all elected amounts. Guarantee Issue amounts do not apply.
• If you enroll in Voluntary Life coverage, you will automatically be enrolled in Voluntary AD&D for a matching amount.
• Employees must elect Voluntary Life and AD&D insurance for themselves in order to elect coverage for their spouse and/or child(ren).
