Eligibility & Enrollment

What you need to know

Who is eligible?

If you’re a full-time employee at Hankin Group you’re eligible to enroll in the benefits outlined in this guide. Full-time employees are those who work 30 or more hours per week. In addition, the following family members are eligible for benefits:

⚫ Spouse or Domestic Partner, subject to the conditions outlined on the “Spousal Affidavit”

⚫ Child(ren), stepchild(ren), adopted child(ren), and child(ren) whom you are a legal guardian for under age 26

⚫ Child(ren) over age 26 who are not able to support themselves due to a physical or mental disability

Enrolling in benefits

Are you ready to enroll? The first step is to review your current benefits. Verify all of your personal information and make any necessary changes. Once all your information is up to date, it’s time to make your benefit elections.

All enrollment elections and changes will be made in Employee Navigator:

⚫ Login with your username and password

⚫ New hires will be sent an enrollment link from Employee Navigator to enroll

I’m a new hire – when do I enroll?

Coverage will begin on the first of the month following or coinciding with 30 days of employment

You have from your hire date until one week before your benefits effective date to complete your enrollment. If you do not enroll within this timeframe, you will need to wait until the next Open Enrollment period unless you experience a Qualifying Life Event.

Qualifying Life Events (QLE)

Unless you experience a Qualifying Life Event, you cannot make changes to your benefits until the next Open Enrollment period. QLEs include:

⚫ Marriage, divorce or legal separation

⚫ Birth or adoption of a child

⚫ Change in child’s dependent status

⚫ Death of a spouse, child or other qualified dependent

⚫ Change in employment status or a change in coverage under another employersponsored health plan

You must submit a Qualifying Life Event through Employee Navigator. This must be completed within 30 days of the event.

About the medical plans

Hankin Group offers the following Cigna medical plan options. Each plan includes prescription drug benefits through CVS Caremark (outlined on the next page). Prescription Benefits

About the prescription plans

If you enroll in one of the Cigna medical plans, you are automatically enrolled in the corresponding prescription drug coverage, outlined below. Using the mail order program for your maintentence medications will save you money! You will recieve a 90-day supply of medications for the cost of two retail copays. In addition the savings, your medications will be delivered right to your home.

Dental Benefits

About the dental plan

Eligible employees and their eligible family members may enroll in the Cigna dental plan, which includes 100% coverage for preventive services such as routine dental exams, cleanings and X-rays

Dental care is vital for overall health, as poor oral hygiene can affect other systems, like increasing the risk of heart disease. Taking care of your teeth and gums plays a key role in preventing such issues.

If you need help finding a dental provider, please visit mycigna.com > select Find a Doctor, Dentist, or Facility > Follow prompts to search by type of dentist or dentist name > When prompted to select a plan choose DPPO/EPO > Total Cigna DPPO

Cigna Dental Virtual Care

If your dentist is unable to assist with your urgent dental care need, simply log on to myCigna.com and follow the prompts to the virtual care portal.

Vision Benefits

About the vision plan

Eligible employees and their eligible family members may enroll in the Cigna vision plan. Take care of your vision and overall health while saving on your eye care and eyewear needs. Vision insurance can help you maintain your vision as well as detect various health problems. Health conditions such as diabetes and high blood pressure can be detected early through a comprehensive eye exam.

Employee Contributions

Medical, Dental, and Vision Plans

Your contributions for medical, dental, and vision coverage are deducted from your paycheck and based on a pay period of 26 pays. Your specific cost will be determined based on the plan you choose, salary range, and selected coverage level.

Health Savings Account

HealthEquity

If you participate in a qualified High Deductible Health Plan (HDHP) you may be eligible to participate in a Health Savings Account. To access your HSA account, visit healthequity.com

An HSA is a great way to save money by allowing you to set aside pre-tax dollars, via payroll deductions, to efficiently pay for qualified healthcare, dental and vision expenses. The funds in your HSA never expire; you may utilize the money you accumulate in your account for future healthcare expenses, even if you change jobs or retire. Enrollees will be issued an HSA debit card for use from HealthEquity.

HSA Contributions

The maximum amount that can be contributed to the HSA in a tax year is established by the IRS and is dependent on whether you have individual or family coverage in the HDHP plan. For 2025, the contribution limits are:

⚫ $4,300 for individual coverage

⚫ $8,550 for family coverage

⚫ The annual catch-up contribution for age 55 and older is $1,000.

Hankin contributes to your HSA!

For first time HSA enrollees, Hankin Group will drop $250 into your HSA account if you elect single coverage, or $500 if you include dependents, as long as you elect to contribute at least $20 per pay. For single coverage, Hankin will match 50% up to $1,300, if dependents are included Hankin will match 50% up to $2,100.

HSA Eligibility

In order to qualify for an HSA, you must be an adult who meets the following qualifications:

⚫ You have coverage under an HSA-qualified, high deductible health plan (HDHP)

⚫ You (or your spouse, if applicable) have no other health coverage (excluding other types of insurance, such as dental, vision, disability or long-term care coverage)

⚫ Are not enrolled in Medicare or Tricare

⚫ You cannot be claimed as a dependent on someone else’s tax return

HSA-Qualified Healthcare Expenses

You can use the funds in your HSA to pay for qualified healthcare expenses such as:

⚫ Doctor visits

⚫ Dental care, including extractions and braces

⚫ Vision care, including contact lenses, prescription sunglasses and LASIK surgery

⚫ Prescription and over-the-counter medications

⚫ Chiropractic services and acupuncture

⚫ Hearing aids and batteries

⚫ Menstrual care products

Flexible Spending Accounts (FSA)

The Harrison Group

Flexible spending accounts, or FSAs, provide you with an important tax advantage that can help you pay health care and dependent care expenses on a pre-tax basis. By anticipating your family’s health care and dependent care costs for the next plan year, you can lower your taxable income.

Healthcare FSA

The Healthcare FSA allows you to set aside pre-tax dollars via payroll deductions to reimburse qualified healthcare expenses for you and your dependents. The annual maximum amount you may contribute is $3,300 per calendar year. Enrollees will be issued an FSA debit card for use from Harrison Group.

The Healthcare FSA can be used for:

⚫ Doctor office copays

⚫ Non-cosmetic dental procedures (crowns, dentures, orthodontics)

⚫ Prescription contact lenses, glasses and sunglasses

⚫ LASIK eye surgery

Note: You cannot contribute to the Healthcare FSA if you choose to contribute to the HSA

Limited Purpose FSA

If you are enrolling in the High Deductible Health Plan (HDHP) and elect the HSA, the IRS rules prohibit you from participating in the Healthcare FSA. However, you may elect up to $3,300 in a Limited Purpose FSA, which can be used for dental and vision expenses only.

Healthcare & Limited Purpose FSA $660 Carryover

Hankin Group allows up to $660 of unused Healthcare FSA funds to carry over into the 2026 plan year. Any amount over $660 in the account will be forfeited.

Dependent Care FSA

The Dependent Care FSA lets you use pre-tax dollars toward qualified dependent care expenses. The annual maximum amount you may contribute is $5,000 (or $2,500 if married and filing separately) per calendar year.

The Dependent Care FSA can be used for:

⚫ The cost of child or adult dependent care

⚫ The cost for an individual to provide care either in or out of your house

⚫ Nursery schools and preschools (excluding kindergarten)

Life and AD&D Insurance

The Standard

Basic Life and AD&D Insurance

Life and Accidental Death & Dismemberment (AD&D) insurance provides protection to those who depend on you financially, in the event of your death or an accident that results in death or serious injury.

Hankin Group provides full-time employees with group life and AD&D insurance. Hankin Group pays for the full cost of this benefit.

Voluntary Life Insurance

While Hankin Group offers basic life insurance some employees may be interested in additional coverage based on their personal circumstances.

Are you the sole provider for your household? What other expenses do you expect in the future (for example, college tuition for your child)? Depending on your needs, you may want to consider buying supplemental coverage.

With voluntary life insurance, you are responsible for paying the full cost of coverage through payroll deductions. You can purchase coverage for yourself, your spouse, or your dependent child(ren) as outlined in the chart.

Disability Insurance

The Standard

In the event that you become disabled from a nonwork-related injury or sickness, disability income benefits will provide a partial replacement of lost income.

Short-Term Disability (STD)

Short-Term Disability is a type of disability insurance coverage that can help you remain financially stable should you become injured or ill and cannot work. Hankin Group pays for the full cost of this benefit

Voluntary Long-Term Disability (LTD)

Long-Term Disability insurance protects workers in the event they become disabled for a prolonged period prior to retirement.

LTD provides you with income continuation in the event your illness or injury lasts beyond 180 days. This helps ensure you have a continued income if you are unable to work due to a covered sickness or injury.

Employee Assistance Program (EAP)

The Standard

There are times in life when you might need a little help coping or figuring out what to do.

Take advantage of the Employee Assistance Program (EAP) which includes WorkLife Services and is available to you and your family in connection with your group insurance from Standard Insurance Company (The Standard).

It’s confidential — information will be released only with your permission or as required by law.

How it works

Your program includes up to three face-to-face assessment and counseling sessions per issue. EAP services can help with:

⚫ Depression, grief, loss and emotional wellbeing

⚫ Family, marital, and other relationship issues

⚫ Life improvement and goal-setting

⚫ Addictions such as alcohol and drug abuse

⚫ Stress or anxiety with work or family

⚫ Financial and legal concerns

⚫ Identity theft and fraud resolution

401(k) Retirement Savings Plan

About the Retirement Plan

Hankin Group matches the first 6% you contribute to your retirement plan. Eligibility for participation is the first of the month following 90 days of continuous employment

How to log on to and set up your account for the first time:

⚫ Visit secure.ascensus.com and enter the default, firsttime login information.

⚫ User ID: Your Social Security number in 999009999 format (no dashes)

⚫ Password: Your birthday in MMDDYYYY format (e.g., 06041982 for June 4, 1982

⚫ You will then be guided through the steps to keep your online account secure and complete the brief enrollment process

Changing Contribution Elections

⚫ Click on My Account.

⚫ Click on Manage Contributions.

‒ Use the top box to make your elections. Please ensure your elections total 100% then, click Save when you are finished.

‒ To add an available fund from the bottom box to your portfolio, click “Select” to the left of the fund you want to add. The fund will appear in the top box.

‒ To remove a fund from your portfolio, click “Remove” to the left of the fund’s name. The fund will be removed from the top box and will be available in the bottom box.

‒ Total must equal 100%, no decimals.

Education Benefits & Grants

Hankin Group

Education Expense Reimbursement

Hankin Group encourages the continuing development and education of its staff. The Company provides reimbursement for undergraduate and graduate level courses that are relevant to the duties performed as a Hankin Group employee.

Hankin Group will reimburse tuition when the employee completes and presents proof of completion in the form of the final grade report. Requests must be pre-approved by the Department Manager/Director, COO and Human Resources.

The employee will also submit a tuition bill showing the cost of the class along with a receipt for any applicable lab fees and books.

Hankin Group agrees to reimburse its employee for costs up to $5,250 per calendar year, minus any student loan assistance received by employee per calendar year, according to the following:

Student Loan Assistance

Hankin Group supports the financial well-being of Hankin employees by aiding with their student loan debt on principal and interest on an employee’s qualified education loans.

Eligible loans include federal or private student loans or loans taken out for the employee’s education (not for dependents or spouses).

Hankin Group agrees to reimburse or remit payment for student loan assistance in advance to eligible employees up to per calendar year:

Eligibility

Full-time employees with a minimum of one (1) year of service, in good standing and employed by Hankin Group at the completion of the course. Employees with less than one (1) year of service are eligible for reimbursement of specialized seminars only.

Eligibility & Deadlines

This policy applies to full-time employees who have successfully completed a degree program (e.g., Associate’s, Bachelor’s, Master’s, Doctorate) with a minimum of one year (1) year of service and are in good standing with the company. This policy applies to current and future student loans.

Hankin Group will provide student loan assistance reimbursement or remit payment in advance when the employee completes student loan assistance application with required documentation.

Requests must be received by December 1st for current calendar year student loan assistance. Applications must be approved by the Department Manager/Director, COO and Human Resources.

Education Benefits & Grants

Hankin Group

Hankin Group 529 Plan Contribution

Have a new family member? Hankin Group will contribute $500 to a 529 savings plan for that child.

Note: If you already have a 529 plan, please provide HR with the Ugift Code

How it works:

⚫ Go to pa529.com & click Open an Account

⚫ Choose your desired account type: Guaranteed Savings Plan (GSP) or Investment Plan (IP)

⚫ Enter your information & follow the prompts. You will need the DOB & SSN for both the account owner (parent) & account beneficiary (child)

⚫ Once open, the Ugift Code may be shared with anyone who wants to contribute

⚫ Send the Ugift Code to Carolyn Van Fleet and a $500 contribution from Hankin Group will be made.

‒ If you have an account with a different institution, please send instructions on how to contribute a gift.

Hankin Group Children Education Grant

Hankin Group offers a Post-Secondary Education Grant for employees children to help pay for their education.*

The eligibility requirements are as follows:

⚫ Visit HankinGroupBenefits.com to complete the application and submit it to HR.

⚫ Employees must be employed for a period of at least 1 year.

⚫ Applies to all post-secondary education such as colleges, universities, tech schools or trade schools.

⚫ Must be an accredited institution or trade school, with a 2-4 year program.

⚫ Children must be 23 years of age or under and be classified as a full-time student (carry at least 25 credits for the year or 12 credits per semester).

Please attach a copy of the student’s transcript including grades to this form and submit upon completion to the HR Department. Course must be completed with a passing grade to receive grant money.

* Reimbursement is subject to a maximum of $1,000 per school year.
