LOOKING AT THE WHOLE PICTURE
Acocording to the World Health Organization, “social determinants of health are the conditions in which people are born, grow, live, work and age, including the health system. These circumstances are shaped by the distribution of money, power and resources at global, national and local levels, which are themselves influenced by policy choices. The social determinants of health are mostly responsible for health inequities...”
Clinical care only makes up 20% of a person’s health. This is where most of health care dollars are spent. The other 80% of health care funding can be found in community agencies, government policies and society itself.
As a community agency, you are in the unique position to improve health in a holistic way. By referring your clients to an evidence based program, they will be more likely to manage their health and address more than the clinical care a person receives to stay healthy

LIVING HEALTHY WITH CHRONIC CONDITIONS WHAT IS PREDIABETES?
UNDERSTANDING PREDIABETES
A person with prediabetes has a blood sugar level higher than normal, but not high enough for a diagnosis of diabetes. According to Ann Albright, Ph.D., RD., director of the Center for Disease Control’s Division of Diabetes Translation, “The CDC estimates that more than 79 million Americans ages 20 years and older have prediabetes. Without intervention, 15 to 30 percent of those men and women will develop type 2 diabetes within 5 years.”
The Diabetes Prevention Program is an effective lifestyle change program proven to prevent or delay the onset of type 2 diabetes in participants by 58 percent.

The Diabetes Prevention Program is mirrored after the National Diabetes Prevention Program and supports participants in making lifestyle changes to reduce their risk for type 2 diabetes including weight loss, increased physical activity and healthy eating.
The primary focus of this initiative is diabetes prevention. However, many of your community members already have chronic conditions such as diabetes or high blood pressure. This toolkit will also provide the information you need to assist community members in managing or preventing other chronic conditions.

There are several programs to help engage community members to be active partners in their care. The two evidence based programs that have been shown most effective are the Diabetes Prevention Program and Living Healthy management programs.

The Diabetes Prevention Program is a structured lifestyle change program. The program helps participants make lifestyle changes to reduce their risk for type 2 diabetes. The Lifestyle Coach works with groups of participants to:
• Lose weight through healthy eating
• Be more physically active
• Learn to recognize and overcome barriers to healthy eating and physical activity

Living Healthy is a chronic disease self-management program. The program aims to increase the knowledge of people living with chronic conditions and teach them the skills to better manage those conditions. The program encourages people knowledgeable about their health to take that extra step to fully engage in their health care. There are specific Living Healthy programs for Diabetes and Chronic Pain
Subjects covered in the workshops include:
• Treatment decision-making and problem solving
• Communication with family, friends and physicians
• Making healthy eating decisions
• Using your mind to manage pain, stress and other negative emotions
Living Healthy does not conflict with existing programs or treatment. It is designed to enhance regular treatment and disease-specific education such as Better Breathers, cardiac rehabilitation or diabetes instruction.

NY Connects is your trusted place for information and assistance about long term services and support, whether they are paying for services themselves, through insurance, or are eligible for a government program.
Calling 2-1-1’s professionally trained staff, community members receive referrals to services for solutions to a variety of problems such as:
I am out of heart medication, can someone help?
· My mother’s snowed in and has no food.
· What if my daughter can’t pay her medical bills?

YOU are the most important person on your health and wellness team. You can make a difference in the kind of health care you receive. You know your body and know when something is going wrong. By being active in improving your health, you can make a real difference in your quality of life.
You may see your doctor once a year, every few months or after an emergency. By being active in your health care and wellness you are better able to make health related day-to-day decisions but you can also play an important role in working with your health care team in what is called Shared Decision Making.
Shared Decision Making is a team approach to making health related decisions together with your doctor. Working with your doctor and other members of your health and wellness team, like pharmacists and dietitians, can go a long way in helping to make decisions that lead to disease prevention and control.

Shared Decision Making (SDM) has been shown to improve patient knowledge and clarity about medical preferences, and to reduce conflict in the decision making process. It also has the potential to:

New York State Diabetes Prevention Program (NYS DPP)
Prediabetes Identification and Intervention Algorithm
Screen or test patient if age ≥ 45 OR Patient is overweight or obese (BMI ≥ 24 kg/m2 or BMI ≥ 22 kg/m2 if Asian) AND positive for at least one of the following:1 (See back page for risk information collection options.)
· Physical inactivity
· Family history of diabetes in parent, brother or sister
· History of gestational diabetes mellitus (GDM) or baby over nine pounds
· Member of a high-risk ethnic population (African American, Latino, Native American, Asian American, or Pacific Islander)
· Hypertension (≥140/90 mmHg or on therapy for hypertension)
· HDL cholesterol <35 mg/dL and/or triglyceride level >250 mg/dL
· History of cardiovascular disease (CVD)
· A1C ≥ 5 7%, IGT or IFG on previous testing

Diabetes Prevention: Proven, Possible and Powerful!
The NYS DPP is an evidence-based program for adults with diagnosed prediabetes or who are at high risk for developing type 2 diabetes The program is led by a trained Lifestyle Coach and meets one hour per week for 16 weeks, followed by at least six monthly follow-up sessions. The program is delivered in community or health care settings, in groups of 10-15 people, where personal lifestyle goals are set by each participant. The sessions cover healthy eating, physical activity, and lifestyle changes to help participants achieve the goals that lead to the prevention or delay of a diabetes diagnosis, including a 5-7% weight loss and maintenance, and a gradual increase in physical activity to 150 minutes per week.
The NYS DPP is based on clinical research trial led by the National Institutes of Health, which showed that people with prediabetes can prevent or delay type 2 diabetes by 58% overall, and 71% in people 60 years of age and older.

Q: Who should participate in the Diabetes Prevention Program lifestyle intervention?
The Diabetes Prevention Program lifestyle intervention is specifically designed for persons that have prediabetes or are at high risk for type 2 diabetes.
Q: How long is the Diabetes Prevention Program?
There are 16 core sessions that generally take place once a week for about four months. Those sessions are followed up by post-core sessions that are held monthly for 6-8 months.
Q: How long is each session?
Each session lasts one hour.
Q: Do I have to attend every session?
To be successful in preventing diabetes, it is important to commit to the program. Attending the 16 sessions is critical if you are going to achieve your goals. If you must miss one or two sessions, we will provide you with the materials from the missed sessions and schedule a makeup session. However, if you know you will miss more than two sessions, we advise that you wait to join until your schedule frees up to get the most out of the program. The monthly post-core sessions are to help you maintain the healthy changes that you made.
Q: I am worried about the physical activity involved in the program. I’m not very active. What kind of physical activity will I have to do? Do I have to join a gym or buy any special equipment?
You will start out slowly to work up to your physical activity goals. If you are not active now, it is important to check with your doctor before increasing your level of physical activity. Many people use walking as their main physical activity, but there are many ways to be active. You will learn about how to be active in the program. You will not need to join a gym or buy any special equipment, although we suggest that you wear a good pair of shoes that are appropriate for the activity you are doing.
Q: How successful are participants?
Studies have shown that people with prediabetes who lose a modest amount of weight (5% to 7%) and increase their physical activity to 150 minutes a week can reduce the risk of type 2 diabetes by more than half. Participants in the Diabetes Prevention Program get help and support to make and sustain those lifestyle changes needed to prevent type 2 diabetes.

How the Vacation Request Process Will Work in 2018
Employees will be eligible to schedule time off up to 180 days in advance. This will be a “rolling 180-day” calendar of open availability. For example, anyone viewing available benefit time in Empower on Friday, January 12th, 2018 could make time off requests for dates through Wednesday July 11th, 2018.
Time off will be approved on a first-come first-served basis.
NOTE: Holidays will show (0) time available and be handled through a separate process.
Anyone requesting time for a period that has already met the maximum amount of timeoff possible will automatically roll into “wait list” status. If things change to permit additional time off, the group allowance balances will be updated with increased allocations and requests in the wait list will be processed in the original order in which they were requested.

8 oz. cream cheese, softened 2/3 c. white Cheddar
1 tsp. garlic powder
kosher salt
Freshly ground black pepper
12 slices bacon, cooked and crumbled, divided 1/2 c. toasted pecans, chopped and divided 1/2 c. chives, chopped and divided
DIRECTIONS
In a large bowl, combine cream cheese with cheddar, garlic powder, half the bacon, half the pecans, and half the chives. Season with salt and pepper and stir until combined.
Empty bowl out onto a piece of plastic wrap and form into a log shape with your hands. Place in the refrigerator to harden slightly.
On a large plate, combine the remaining chives, bacon, and pecans. Stir so that they are evenly distributed on the plate. Remove cheese log from refrigerator and roll in mixture, pressing more toppings on with your hands if needed. Place on a serving dish with crackers or vegetables for dipping.
