How Will Working Affect Social Security Benefits? By Ken earwood

In a recent survey, 67% of current workers stated they plan to work for pay after retiring.¹ And that possibility raises an interesting question: How will working affect Social Security benefits? To answer that question requires an understanding of three key concepts: full-retirement age, the earnings test, and taxable benefits.

Full Retirement Age

Most workers don’t face an “official” retirement date, according to the Social Security Administration. The Social Security program allows workers to start receiving benefits as soon as they reach age 62—or to put off receiving benefits until age 70½. “Full retirement age” is the age at which individuals become eligible to receive 100% of their Social Security benefits. For example, individuals born in 1955 can receive 100% of their benefits at age 66 years and 2 months.²

Earnings Test

Starting Social Security benefits before reaching full retirement age brings into play the earnings test. If a working individual starts receiving Social Security payments before full retirement age, the Social Security Administration will deduct $1 in benefits for each $2 that person earns above an annual limit. In 2015, the income limit is $15,720.³

What’s Your Full Retirement Age?

Those born in 1942 or before are already eligible for full Social Security benefits at age 65. For those born between 1943 and 1960, full retirement age increases incrementally until it reaches 67.

During the year in which a worker reaches full retirement age, Social Security benefit reduction falls to $1 in benefits for every $3 in earnings. For 2015, the limit is $41,880. ⁴ For example, let’s assume a worker begins receiving Social Security benefits during the year he or she reaches full retirement age. In that year, the worker earns $65,000. The Social Security benefit would be reduced as follows: Earnings above annual limit $65,000 – $41,880 = $23,120 One-third excess $23,120 ÷ 3 = $7,707 In this case, the worker’s annual Social Security benefit would have been reduced by $7,707 because he or she is continuing to work.

Taxable Benefits

Once you reach full retirement age, Social Security benefits will not be reduced no matter how much you earn. However, Social Security benefits are taxable. For example, say you file a joint return and you and your spouse are past the full retirement age. In the joint return, you report a combined income of between $32,000 and $44,000. You may have to pay income tax on as much as 50% of your benefits. If your combined income is more than $44,000, as much as 85% of your benefits may be subject to income taxes. There are many factors to consider when evaluating Social Security benefits. Understanding how working may affect total benefits can help you put together a program that allows you to make the most of all your retirement income sources—including Social Security.

CAREFUL GIVING: Suggestions for Passing Gifts to Care Providers

PRACTICAL GUIDELINES In the common sense area, don’t take the health provider to see the lawyer, and don’t let your health provider be your trustee; get someone else. If you don’t have a family member or friend for the job, find a Private Fiduciary to take you to the lawyer and serve as your Trustee/Executor/ Agent. Otherwise, it looks like you’ve been steam-rolled, and it makes the government nervous.

By Donald A. Hunsberger

According to the Orange County Register, a 91-year-old woman from Brea died in mid-2014, and left her entire estate to her caregiver. After officials discovered the transfer, they removed her assistant as trustee and took away the home she left him.

Likewise, don’t discuss your plan with your caregiver, ever. This is something which leads to problems. Keep the process to yourself; don’t let him or her see the documents when they are done. Instead, spend time with outside persons other than your provider. Remember that the image of the isolated senior is what the courts and the agencies are looking for when they examine these cases for people who may have been victimized.

W

hy? According to the District Attorney and the ruling Judge, the facts of the case demonstrated “Fraud and Undue Influence” by the caregiver.

A caregiver has worked for you in your later years; How can you to safely include this person in your will or your trust? Can you avoid the accusations that destroyed this 91-year-old’s wishes?

Further, ask your lawyer about a “life estate” instead of making an outright transfer to the caregiver. This helps the provider during his or her life, and then delivers the asset to your friends, family or a charity following the caregiver’s death. Name a third party (not the provider) as the trustee (perhaps a private fiduciary) instead of the caregiver. This level of control on your part is not consistent with “undue influence,” and should calm the courts and placate the prosecutors.

To avoid the Brea woman’s result, consider a few specific rules from both the Probate Code and from common sense experience to show that the senior receiving the health care –the employer- has not been unduly influenced to name the care provider as an estate beneficiary. Probate law in California reflects years of problems with caretakers of the elderly. Following these rules may help to avoid both the appearance of – and actual—undue influence by the targets of the law.

At the same time, don’t name the provider as the sole beneficiary, or even the largest beneficiary. It just looks too suspicious. Name family, a Church, a School or a Hospital as beneficiaries along with the provider.

STATUTORY GUIDELINES

Remember, there are both good and bad people in every profession. The Probate Code is not there to punish the elderly, it exists to protect us. If there is a caregiver who objects to any of the suggestions above, it may be time to search for a new health care provider: this one may only be looking after his or her own well being, not for yours.

First, the statutes provide that a senior changing her planning documents to benefit a caregiver should not act either 90 days before or 90 days after the provider comes to work for her. That makes sense: nobody gives her estate to a person she just met three months ago unless she had somehow been “hypnotized” by being with the caregiver 24 hours a day. The senior in this situation should see an attorney and wait for the time to pass; don’t present the image of being hustled by a new health care giver.

Hunsberger Law is a full-service estate and business planning legal firm, with estate planning attorneys and a licensed, professional fiduciary on staff. Connect with Don Hunsberger by calling (714) 663-8000 or visit www.hunsbergerlaw.com

Second, make sure your lawyer introduces you to an attorney who is not a part of her firm to interview you and complete the form provided in the Probate Code. Keep this Independent Review with the documents you receive.

United American Mortgage Company Trusted, tried and true. That’s the reason why United American Mortgage has been successful for over 20 years. Since 1993, UAMC specializes in providing mortgage-financing solutions to homeowners and homebuyers.

Y

ou’re almost tired of reading about it but it’s true: the mortgage meltdown hurt more than a few and people are still trying to recover. It’s hard to know whom to trust. With many mortgage companies competing and failing, UAMC has found itself expanding into 9 states. Originally formed to provide for the specific needs of Southern California borrowers seeking mortgage products tailored for them, UAMC has made it their personalized business to get to know and help you.

“We’ve grown substantially because we have the ability to adapt to the ever-changing marketplace,” says UAMC President Al W. Hensling. “We are in the home business, the helping-people business. Yes, it requires us to at times work around the clock. Yes, we have to keep up-to-date on the latest products and regulations. We take our clients and what we do seriously because at the end of the day, everyone should either be in the home of their dreams or keep the one they love so much. To some people, it’s just paperwork and numbers. But to us, it’s personal. It’s home.” Their incredibly impressive, rare, referral-only business model may also be the key to understanding their success, as customers flock just to see if UAMC truly walks the walk. To them, offering you efficient, cost-effect and timely service is just a day’s business. These days, most companies feel the need to plaster those words all over their advertisements. UAMC doesn’t need to express that they go above and beyond, they just think that it should come standard. “I guess you could say we just want to help how we can. Since our business is referral based, we truly get to know your family and friends. UAMC continues to evolve with new products and financing solutions to the ever-changing Southern California real estate market. While many have given up the idea of owning a home or even refinancing, UAMC holds to the knowledge that today’s new and innovative programs are making it easier than ever to purchase or refinance. “After a period of time in which it was extremely difficult to obtain a mortgage, things have started to normalize and lending guidelines are starting to loosen,” says Al. “The ‘Mortgage Meltdown’ created a great deal of uncertainty in the marketplace and made it difficult for so many to secure financing. We understand that a lot of people are scared to see where they stand and that’s ok. We just want you to know that it doesn’t have to be this way. Most of our clients feel the same way. Some people tell us they didn’t even want to find out about

your unique situation. We’re ready to meet your challenges and assist you into getting or keeping your home! “As the Economy continues to grow and we experience job growth, more families will have an opportunity to experience the benefits of home ownership. Clients always ask what is going to happen to interest rates. I really wish I had that crystal ball to answer them accurately. It is difficult to say what rates will do, as markets have not reacted to this economy as they had previously. I counsel clients to borrow based on need and what they can afford. Loan programs are changing almost daily. As we move farther away from the mortgage meltdown of 2008, more and more programs and financing options will open up. But always be wary of the deal that sounds too good to be true. Even though recent changes have cleaned up the lending industry, there are still people out there that are not looking out for the client’s best interest. Families should always determine the experience level of their mortgage professional, along with checking references from previous clients.”

Al Hensling is President of United American Mortgage of Irvine. Connect with Al and his team by calling (800) 708-5626 x110 or by emailing him at alhensling@uamco.com. The Spirit of Wertz & Company Tax, Accounting and Business Solutions for the Next Generation of Entrepreneurs By Betsy sanz PhotograPhy by Lana FarFan, Caught in the MoMent PhotograPhy

Starting and running a business can be a risky, difficult thing. It takes a certain kind of person to even think about doing it, much less to actually execute. All the leading business publications, and any voice competing to be an authority on the entrepreneurial spirit, have a published list of personality traits that are common to entrepreneurs.

C

ourage. Passion. Vision. Tolerance of ambiguity. Tenacity. Flexibility. This is the heart of the entrepreneur. If entrepreneurs share common personality traits, it follows that they also face common challenges. Many entrepreneurs who start a business are not totally prepared for the reality of, well, business. Passion for baked goods doesn’t exactly equal passion for tax prep; a vision for new technological inventions isn’t always accompanied by a vision for corporate structure.

Paperwork. Bookkeeping. Tax strategizing. Investor relations. Growth and exit planning. This is the life of the entrepreneur. Fortunately for Orange County-based visionaries, there is Wertz & Company, the accounting firm that thrives on helping entrepreneurs turn their idea-driven ventures into businesses that can boom.

Entrepreneurial Roots Russ Wertz has the entrepreneurial spirit. Thirty years ago he took a leap of faith and cofounded an accounting firm of his own. Russ is a people lover and a problem solver. His clients

– particularly other entrepreneurs – would come to him for accounting and tax services, but would have many additional needs. “Entrepreneurs are a special breed,” says Russ. “They are hard workers. They are driven by belief in their product or service. But they don’t always see all the factors that can make or break their business. I have just always loved assisting and guiding them in all the financial logistics that go into making a business successful.” Because Russ cared about the success of their ventures, he found himself helping his clients in matters of cash flow, credit, banking, trust formation, staffing and more. He opened his door in a way that many accounting firms don’t. And his clients stayed. Over the course of thirty years, Wertz & Company grew to be one of Orange County’s premier tax accounting firms. Today, Wertz & Company employs over thirty professionals, half of whom are in their fifties, and half of whom are in their twenties and thirties, providing a wealth of both experience and energy to service entrepreneurs at every stage in their business’s growth.

A Next Generation for the Next Generation Russ matured along with his clients, like many accountants do. But unlike most accountants, who run the business lifecycle with their clients and don’t develop a strong case for succession in their own business, Russ created Wertz & Company to be a legacy company. To serve the next generation of entrepreneurs, Wertz & Company needed its own next generation of leadership. He and his company have found it in Katie Sterling. Katie has worked in public accounting for eight years. Her experience includes working with closely held businesses and their owners, as well as working with startup companies from their initial launch through all the various stages of growth. Katie is passionate about the work that Wertz & Company does, and she is commit-

Katie Sterling

14

A recent case illustrates how Russ, Katie and their team helped a local company take advantage of their potential. A high-end baby toy wholesaler came to Wertz & Company in 2013 when they were experiencing exponential growth but didn’t have the resources to manage such rapid growth. On top of that, they were getting no support from their old accountant in terms of what Wertz & Company deems basic accounting services (i.e. tax planning, accounting support, assistance with bankers).

Katie with Company Founder Russ Wertz ted to taking Wertz & Company into the future right alongside the entrepreneurs they are serving today. “Katie is the next generation of Wertz & Company for the next generation of Orange County entrepreneurs,” says Russ. “They want their own accountants familiar with the issues of their generation, but they also want the experience, expertise and horsepower of a seasoned professional. That’s Katie. Not only is she now a part-owner at Wertz & Company, we are grooming her to be its next CEO.” “I truly love what I do,” says Katie. “Working with small businesses and entrepreneurs to solve business issues, simplify processes and plan for the future are the areas I enjoy the most.”

Serving the Needs of the Entrepreneur So what can Wertz & Company offer the Orange County entrepreneur? Tax preparation and planning, accounting, auditing, management consulting, financial advising, estate planning, and transition planning are the core services offered by Wertz & Company, and they have a whole division dedicated to serving the unique needs of entrepreneurs and small businesses. They call it their Entrepreneurial Services Division. “While it’s true that some of our clients now have large, mature businesses – they didn’t start that way,” says Katie. The Wertz & Company Entrepreneurial Services Division (ESD) has the flexibility to work with the unique cash constraints that start-ups face, while building financial and business foundations that can support growth. The ESD suite of services includes accounting system design and implementation, bookkeeping, accounts receivable/accounts payable management, bank reconciliations, preparation of monthly financial statements, periodic reports and analysis, budgeting and projection assistance, cash flow review and planning, cost control analysis, independent contractor compensation reporting (i.e. 1099’s, etc.), tax return preparation, and, perhaps most importantly, business advice and solutions to complex problems. “Most start-ups and small businesses don’t have internal financial professionals,” says Russ. “Entrepreneurs need a sounding board, someone they can call with questions. They often need advice on the structure of the business; sometimes they need help cleaning up their financials or tax documents; often they need to recruit good accounting personnel, or generally get the right banking, insurance and legal advisors in place. They don’t know what to do or where to go outside of their expertise. We love to help them with all of that. Of a hundred emails I get, seventy to eighty of them aren’t about taxes; they’re about business.”

FortunateLy For orange County-Based visionaries, there Wertz & CoMPany, the aCCounting FirM that thrives on heLPing entrePreneurs turn their idea-driven ventures into Businesses that Can BooM. is

Wertz & Company immediately put together a plan of action to get the accounting services shored up by providing ample accounting support under their Entrepreneurial Services Division service – not just making adjustments, but teaching the client about how to do it better so their financial statements became more meaningful to them. They put a tax plan together to help the client feel at ease about where they actually stood and how much to set aside for taxes; they also provided planning ideas to minimize such taxes where possible. They helped them get more meaningful financial statements to provide their bankers so they could maintain their vital lines of credit and obtain new letters of credit as they continued to expand. More than just providing financials, they engaged with the bankers to make sure their questions were answered and they had what they needed to make faster credit decisions. On top of all the financial and banking assistance, Wertz & Company rolled its sleeves up and helped manage the growth in a much more physical sense by successfully selecting, interviewing and hiring accounting personnel. Today, that company has expanded their operations overseas as a result of a planned growth strategy centered on a solid operational foundation.

Partners for the Long Term You might say that passion for the entrepreneurial spirit defines the spirit of Wertz & Company. With its own succession plan in place, an unabated enthusiasm for the work it does, and little to no client or employee turnover, Wertz & Company is here for the long haul, no matter what stage a business is in – whether it is starting up, refining its established operations, or strategizing its growth and expansion as mature companies do. Wertz & Company services everything from start-ups to $500 million dollar companies, and individuals earning from $150,000 to millions of dollars each year. The only rule? “We only service nice people,” says Russ.

Return on Investment: Understanding how fitness can make you rich

O

By Paul and Carolyn Fetters

n a beautiful spring day back in 1975, 5,042 sets of twin brothers would be recruited to take part in a landmark study—the results of which would not surface for another 29 years. Then, in 2004, the secrets of this research project was uncovered and reported in the January/ February issue of Pacific Standard. This historical and first-of-its-kind study aimed to determine the direct effects and differences on long-term income and earnings of these twin brothers.

A FREE RESOURCE FOR MEN OF ORANGE COUNTY CALIFORNIA IN COOPERATION WITH THE TRAINING SPOT, BALANCED HABITS, AND

THE RECLAIMING YOUR METABOLISM PROJECTTM

MEN, MONEY & MUSCLE

The researchers, insistent on removing all possible variances that may affect results took the due diligence to adjust for: Health differences, genetic differences, family factors, labor market and related absenteeism difference, individual factors such as marital status and education and even physical attractiveness. Amazingly, one particular segment of these men generated up to 17% more wealth than their twin - enough to increase their wealth an estimated $153,000-$510,000 over their biological brothers.

WHAT MADE THE DIFFERENCE Have you ever wondered, as I have, what makes this kind of difference in people’s lives? In the case of this twin study, it isn’t native intelligence, talent, environment or dedication. It isn’t that one brother wanted success and the other didn’t. And it isn’t that one twin was more attractive or better looking than the other. The difference lies in a single choice each made: to work out or not to work out, to have an active lifestyle or to be sedentary. That single choice—to work out or not—is the underlying reason for the increasing wealth.

How A Lack Of Muscle Is Costing You A Fortune

PAUL FETTERS

EDITOR OF THE ORANGE COUNTY JOURNAL OF HEALTH & FITNESS

Skeptical that working out can make men smarter, savvier and ultimately richer? I understand your skepticism, and it’s why I’m writing this book. Over the next few pages, I’ll offer you a preponderance of proof, uncovering the science, knowledge, and practical application. If it’s true that working out helps increase wealth, the information in this book could potentially change the course of your financial life.

THE GREATEST RETURN ON ANY INVESTMENT YOU’LL EVER MAKE Naturally, anyone invested in Wall Street is concerned about the growth of stocks, and if their investments will improve anytime soon. And while I can’t guide you in the right direction regarding your stocks, bonds or international investments on the Forex market, I can tell you this: None of it amounts to a hill of beans if you are not making the most important investment of all. OF COURSE, I’M TALKING ABOUT YOUR HEALTH AND FITNESS Most men take their health for granted until a crisis finally catches up with them. Years of neglect, poor nutrition, and finding time to stress but not to exercise, are sacrificed on the altar of excuses, like… “I’m too busy and don’t have time to workout,” or… “I only have so much energy in a day, and I can’t waste in a gym,” or my personal favorite… “My granddad smoked two packs of cigarettes a day, drank like a sailor, ate fried food his whole life, and still lived

16

to be 95 years of age.” If that’s an attitude you’d like to stick with, all I can say is, “Good luck with that strategy,” and you might as well stop reading now. Because the rest of this book is designed to help you build up your health and fitness portfolio… beef up your overall immunity…and suggest some hedges against heart disease, memory loss, diabetes, muscle wasting and more.

A CRITICAL ADVANTAGE IN THE TOUGHEST OF TIMES Turn on any television news station, read any online newsletter, or even hold a discussion around the water cooler at work, and it’s probable all roads will lead to Wall Street. Everyone is concerned about the world economy, the implication of the financial impact of refugees entering the country, the

war against ISIS, and even the upcoming election of the next President of the United States. If that’s you, just imagine how the 5,042 twins in this study felt over the course of the 10 costly wars that took place between 1975 and 2004: Lebanon (1982–1984), Grenada (1983) Panama (1989), Gulf War (1991), Somalia (1993), Bosnia (1994–1995), Kosovo (1999), Global War on Terror (2001– ), Iraq War (2003–2010), and Afghanistan (2001–2014). At the same time, they also faced three U.S. recessions: 1980-1982, 1990-1991, and in 2001. And they lived through the election of six different U.S. Presidents: Gerald Ford (1974-1977), Jimmy Carter (1977-1981), Ronald Regan (1981-1989), George H. W. Bush (1999-1993), Bill Clinton (1993-2001), and George W. Bush (2001-2009). Despite it all, the men in this study who exercised grew their wealth up to 17% above their biological twin. This wealth increase wasn’t by chance, good luck or any other variable - it was by the deliberate and purposeful commitment to invest in exercise. The researchers concluded that exercise enhances a person’s performance at work by improving their perseverance when facing obstacles, fostering goal-oriented behaviors, and developing social skills. Not only that but working out encourages participation in competitive situations within the workplace by enhancing self-esteem, self-confidence, and the belief of being in control. In other words, because they stayed active and fit, they received the greatest ROI in both health and monetary gain in some of the toughest times in recent history.

FOUR SCENARIOS THAT REVEAL HOW YOUR BODY CAN BE A PERSONAL DEFLATIONPROOF/RECESSION-PROOF/DEPRESSION-PROOF MONEY MACHINE Buy a Certificate of Deposit from your bank right now, and you’ll be lucky to get a 2% or 3% yield. For most people, that’s nowhere near enough to live on—and it’s sure not enough to grow richer! But by investing in your health and fitness in as little as three days as week, you are giving yourself a serious competitive edge over your peers. Consider this: If you had been one of the exercising twins in the study that started back in 1975, and you increased your long-term income by 17% over your brother like these men did, then… Scenario #1: Assuming an average $30,000 annual income over 30 years… …Your twin could have earned $900,000…but you would have earned $153,000 more than your brother…bringing your total income to $1,053,000. Scenario #2: Assuming an average $50,000 annual income over 30 years…Your brother could have earned $1,500,000…but you would have earned an additional $255,000…bringing your income to $1,755,000. Scenario #3: Assuming an average $75,000 annual income over 30 years…your sibling could have earned $2,250,000…but you would have banked an additional $382,500… giving you a windfall income of $2,632,500. Scenario #4: Finally, assuming an average $100,000 annual income over 30 years… your brother could have earned $3,000,000…but you would have pocketed an additional $510,000…for a total nest egg of $3,510,000. Now, if you’re finding all this hypothetical math to be a bit hyperbolic and difficult to swallow, then simply adjust the math any way you want. Come up with the figure you feel is more realistic, and then add 17% to the total, to reflect the gain these men enjoyed. Heck, for that matter, tabulate all the income you’ve made over the last 10, 20, even 30 years, and then add 17% to it. Instantly you’ll see the profit/loss statement favors those who decide to workout throughout whole careers. Listen, even if you completely reject this massive, three-decade-long research project’s conclusions, is there any logic in arguing that getting your health in prime condition is a bad investment?

Wouldn’t it be in your best interest to get in shape, so that steady flow of increased oxygen your brain would help you think better, make better decisions, enjoy the benefits of health, and ultimately make more money? The bottom line is that taking care of your health and fitness through a consistent exercise program is the easiest, fastest, and most certain way to become your own, personal moneymaking machine.

IF YOUR BODY WERE A STOCK OFFERED ON WALL STREET WOULD YOU INVEST IN YOURSELF? If that IPO were the condition of your body… the sharpness of your mind…and 100% dependent upon your health…would you risk your money to invest in your current level of fitness? Would you feel confident asking others to risk their investment dollars for you, with the expectation of gaining 17% growth on their long-term investment? If not, you have some work to do. Because listen, if making money is a core motivation for you…if you the ardent desire to make as much money as you can, safely as possible…then the greatest investment you’ll ever make is in your health. Your ability to think clearly, your high energy levels, the overabundance of strength stored in your muscles, and emotional control to make rational decisions is the most reliable money making advantage you’ll ever possess. Ultimately, strength training, cardio conditioning, and proper nutrition can help you bridge the gap between sickness and health on your way to true fitness. CLINICAL RESEARCH STUDIES:

Finding New Hope for Alzheimer’s Disease By Samuel Henderson, PhD

Alzheimer’s disease is a progressive neurological disease that primarily affects the elderly. In 2015 it was estimated that 5.3 million Americans were suffering from the disease. As the baby boomers age this number is predicted to more than double, with over 13 million Americans affected by 2050. Without significant new treatments, the rise in the number of Alzheimer’s patients will place a tremendous economic and social burden on the United States.

U

nfortunately, the development of new drugs for Alzheimer’s disease has not gone well. In the last 10 years no new drug substance has been approved. Between 2005 and 2015, there has been a 100% failure rate for new drug substances. The only new drugs that have been approved have been reformulation of existing drugs.

that have attracted the attention of those developing drugs to combat Alzheimer’s. While researchers identify new targets and develop potential new therapies, there is an opportunity for those with Alzheimer’s and their caregivers to help in the fight against this terrible disease. By participating in a clinical research study for Alzheimer’s disease you can directly help researchers in their efforts to find a cure.

The path to finding new hope for Alzheimer’s disease is dependent on clinical research studies. Drug development in Alzheimer’s has focused largely on the accumulation of a sticky protein called amyloid beta. This sticky protein is thought to be toxic to neurons and to interfere with brain function. Various attempts have been made to either prevent the formation of amyloid beta, or clear it from the brain. Unfortunately, to date these attempts have failed to demonstrate efficacy in clinical research studies. As outlined in previous articles in this series, there are other characteristic changes that occur in Alzheimer’s disease

While researchers identify new targets and develop potential new therapies, there is an opportunity for those with Alzheimer’s and their caregivers to help in the fight against this terrible disease. 18

and Drug Administration and ethics boards. Regulatory authorities review clinical trial design for scientific merit, as well as to ensure that appropriate patient safety and protection measures are in place. During the clinical trial, the safety of the participants is the primary concern. Any adverse reactions to the drug are carefully monitored, followed, and steps are taken to ensure the safety of the participants.

Myth: I need to stop my current medications to join a clinical research study Another common myth is that participants in a clinical research study will be required to stop taking current medications in order to test the new one. This is not always the case. In many cases, because the clinical trial drug being tested is unproven, it would be unethical to stop a working medication to test a new one. Generally, participants in a clinical study are asked to be on stable dosing of their current medications, and are asked not to change the dose during the course of the study.

Myth: I will lose my primary care physician if I join a clinical study Participants in clinical studies for Alzheimer’s will be seen by a clinical trial physician who is an expert in Alzheimer’s disease and has treated many other patients with the same condition. A high standard of care for those who participate in a clinical trial is expected, and there is no requirement for you to stop seeing your regular doctors. Participants are encouraged to continue seeing their regular physician according to their normal schedule.

In this article we will dispel some of the common myths and misconceptions around clinical research studies and discuss what it means to participate in a clinical research trial.

Myth: There are plenty of volunteers

Myth: If I join, I may not get the drug being tested

One of the most common misconceptions about joining a clinical trial is that “There are plenty of volunteers and I’m not needed”. This couldn’t be further from the truth. Studies in Alzheimer’s disease often involved hundreds and sometimes thousands of patients and can take years just to recruit enough people for one study. Slow enrollment in Alzheimer’s disease trials is recognized as one of the major hurdles to new drug development. The more people that participate in clinical research studies, the faster progress will be made.

Typically, in a clinical trial, participants are randomly assigned to either the active study drug or the placebo, which is an inactive substance that looks like the active study drug. This is called a double-blind study, and it is the best way for researchers to test whether the new drug is safe and effective. While there is a chance that participants will receive placebo, rather than the investigational drug, many trials are designed with what is known as an “open label phase”. During this open label phase all participants receive the active study drug at no cost for a period of time. In some cases, the open label phase can last until the new drug is approved and available to the public.

Myth: Participants are guinea pigs that will be exposed to dangerous untested drugs Some people are afraid to participate in clinical trials because they think they may be dangerous. They say they would rather let someone else be the guinea pig, and prefer to wait until the drug is approved. While it is true that clinical trials do carry some risk because drugs being tested have often not been fully evaluated, it is important to remember that before a new drug can be tested in people, it has to be extensively tested in animals and other models. Prior to starting a clinical research trial the design is reviewed by regulatory authorities, including the Food

A high standard of care for those who participate in a clinical trial is expected, and there is no requirement for you to stop seeing your regular doctors. 19

Joining an Alzheimer’s disease clinical study represents a considerable commitment.

After you have signed the informed consent, a full examination will be performed. This examination will likely consist of a complete review of your medical history, medications you are taking, a physical exam, blood work, and, in the case of studies for Alzheimer’s disease, testing of your memory and thinking, as well as other tests required by the specific research study. This will all be done at no cost to you. Based on the results of these screening tests, the study site will let you know if you qualify for the study. Note that although you may be healthy, it is possible that you may not qualify for the study for other reasons such as specific genetic requirements, or particular lab values that may be out of range for that specific study.

Myth: It costs too much to participate in a clinical research study It should not cost participants anything to take part in a clinical research study. The costs of a clinical trial are fully covered by the study sponsor. In fact, many studies provide modest compensation for participating, which may include reimbursement for travel and other expenses incurred during the study.

Once you have qualified for the study, you will be scheduled for your “baseline visit”, where additional study procedures are performed, and you will be randomly assigned to either the study drug or the placebo. No one will know if you receive the study drug or placebo – not you, or even the study doctor or his/her staff will know until after the study is complete.

Myth: I need health insurance to participate Health insurance is not required to participate in a clinical research study. The costs of the procedures and related medical care received as part of the clinical trial will be covered by the study sponsor.

Clinical research studies may last from a few weeks, to a few months, to a few years, depending on the design of the study. During this time, you will be required to attend study visits at regular intervals to check on your health and to measure how the study drug is working.

What to expect when you join a clinical trial If you are interested in being a part of finding new hope for Alzheimer’s disease by participating in a clinical research study, the first step is to find a clinical study in your area. In many cases, your current doctor may not be well-versed in what clinical research studies are currently underway. There are several online resources that can be used to locate potential studies, including www.clinicaltrials. gov or www.trialreach.com. On both of these websites, you can search for Alzheimer’s disease studies that are enrolling in your area.

Joining an Alzheimer’s disease clinical study represents a considerable commitment. By participating in clinical research you will have access to the study drug well before the general public, in addition to the benefit of the study doctor’s expertise. Although there are no guarantees that the study drug will improve Alzheimer’s symptoms, you will have taken an essential step to help advance new drug discovery, and most importantly, you will be a part of the fight against Alzheimer’s disease.

The next step would be to contact the clinical trial site and ask about the study. The study site may be enrolling for more than one study, so if you are interested in a specific study it is important that you relay that information to the study site at the beginning of the conversation. The personnel at the study site may ask you some preliminary questions to gauge your eligibility for participation in a particular study. This may include questions about your medical history or current medications.

Dr. Henderson received his PhD in Molecular Genetics and Cell Biology from the University of Chicago, and has a BA in biology from Washington University. For more information on this topic, Dr. Henderson can be reached at shenderson@accerapharma.com

Finally, if it appears that you may qualify for the study, the study site personnel will schedule a time for you to come in for a more comprehensive evaluation called a “screening visit”. At this first study visit, the site will explain the study in detail, including any risks or benefits. They will ask you to sign a document stating that you understand the study and the potential risks or benefits. This document is called an “informed consent”. It is very important that you fully understand what is expected of you during the course of the study and that you have all your questions addressed. You always have the option to take the informed consent home to discuss the study with your family and/or your doctor.

Helping Children of the Homeless By Joanne Coontz

Over 25 years ago, members of Orange Rotary recognized a need for children of the homeless, who were being educated at Project Hope School, co-sponsored by the Orange County Department of Education and various non-profit organizations.

R

otarians wanted to provide an opportunity for the children to purchase gifts for their family members during the Christmas holidays, with the goal of helping the children learn about the value of giving to others. Each volunteer Rotarian was given a predetermined amount of money and accompanied the children to a major store, where the children made their own individual choices of gifts for their family.

cember, Rotarians ventured throughout the store, holding each child’s hand and a shopping cart as they walked aisle to aisle hunting for special gifts. When the children returned to the school, at 343 E. Grove Ave. in Orange, Rotarians provided a lunch and helped the youngsters wrap their family donations. Project Hope School, now known as Skyview School, was created to meet the educational needs of children of homeless families living in motels or shelters. Serving children in kindergarten through elementary and middle school, the Orange County Department of Education pays for the school facility and teachers; however needs of the children for food and clothing and office supplies for the center are sought through organizational donors.

The yearly Christmas program is high on the list of Orange Rotary’s community programs as twenty-five to thirty members look forward to accompanying the children through Target in Orange to make their choices of presents with utmost sincerity and delight. In early De-

Personalized Regenerative Medicine

Stem cells treat heart disease and stroke – and change the way we age By Kristine emerson

F

The “Usual” Treatments for chronic diseases treat symptoms with drugs that have side effects, are costly and typically don’t fix anything!

or this reason, regenerative medicine has seized the spotlight thanks to its focus on helping repair, and even regenerate, body parts and tissues damaged by disease, injury or aging using stem cell therapy and innovative treatments such as hyperbaric oxygen therapy.

One of the trailblazers in regenerative medicine and especially stem cell therapy is physician David Steenblock. Since 2000, Dr. Steenblock has treated over 7,000 patients with stem cells and also coauthored a groundbreaking book on the subject titled “Umbilical Cord Stem Cell Therapy” (2006, Basic Health) which details impressive clinical responses in children and adults who’d been treated using pure umbilical cord blood-derived stem cells in Mexico.

regenerative medicine can be explained as simply as ‘out with the bad, in with the good.’ 26

As you might expect, Dr. Steenblock’s education, life and career journey uniquely prepared him to become a distinguished expert in doing personalized regenerative medicine. Having grown up on a farm in Iowa where his natural talent for finding or creating innovative solutions was encouraged because it saved time and money, Dr. Steenblock decided early that this plus his desire to help others pointed to a single best career path: science and medicine. Dr. Steenblock’s subsequent trek into medicine included earning a B.S. degree from Iowa State University, then an M.S. in biochemistry and Doctor of Osteopathy (D.O.) degree from the College of Osteopathic Medicine and Surgery (Des Moines, Iowa). His post-doctoral training included three years at Case Western Reserve Univer-

As the blood flow resumes, the patient begins to improve. This process takes from three weeks to three months to complete, during which time the patient needs to follow certain lifestyle guidelines. Today, Dr. Steenblock recommends stem cells as the best treatment there is for the alleviation of suffering and regaining the functions that were lost due to a stroke or heart attack (and many other conditions, injuries and even aging). In his practice, Dr. Steenblock uses both therapies together because “hyperbaric oxygen allows for an increased invasiveness of the stem cells into the tissue and helps activate the stem cells and also helps mobilize the stem cells from the bone marrow into the circulation and into damaged tissues.”

Hyperbaric Oxygen Therapy (HBOT) is used to treat a variety of conditions such as cerebral palsy, stroke, traumatic brain injury, post-plastic surgery patients, non-healing wounds, neuro degenerative diseases and more! sity, one year at the Oregon Health & Sciences University and a clinical Rotating Internship at Providence Hospital in Seattle, Washington. In addition, he did an externship at the University of Washington Department of Hematology/Oncology in 1969 which included work with stem cell-rich bone marrow. This program was run by Dr. Edward Donnall Thomas (who in 1990 was awarded the Nobel Prize in Medicine & Physiology). He then put in a number of years studying and working in the field of pathology before launching out into the world of holistic medicine (Better known now as “complementary-alternative medicine” or “integrative medicine”). In 1979 he founded the first holistic medical clinic west of the Mississippi and began making inroads into the treatment of chronic diseases, including working out methods for reversing blood vessel blockage and heart disease that was decades ahead of what other doctors were doing. Then, in 1989, Dr Steenblock began working with hyperbaric oxygen therapy (HBOT) as a means of flooding his patients bodies with oxygen in ways that rev up natural healing mechanisms. It was a year or so later that a patient who’d had a stroke came to him asking if there was something he could do to reverse his brain damage, as the best conventional medicine had to offer had done very little for him. Steenblock prescribed a course of HBOT treatments which brought about swift improvements. By the mid 1990s Dr. Steenblock’s success in turning around chronic stroke had earned him the appellation “The Stroke Doctor”. Says Dr. Steenblock, “HBOT uses stem cells and growth factors to make new blood vessels to the stroke brain area. HBOT was a great step forward in our quest to help stroke patients but it was a daily program for two months that was hard to do for most. However, now the same or better results can be achieved through the use of just stem cells. Once you give the stem cells, they go directly to the damaged areas where they become lodged in the small blood vessels that were injured because of the stroke. They enter the damaged tissues, proliferate there and secrete growth factors that cause new blood vessels to grow into the injured areas.”

hyperbaric oxygen therapy (hbot) proved to be so successful that dr. steenblocK developed a clinic around that particular therapy

When stroke patient Bob Ziachkowsky came to Dr. Steenblock, he was in “such bad shape that he couldn’t walk and “could hardly move.” Having completed a painless, safe course of bone marrow stem cell and hyperbaric oxygen therapies, Bob credits “The Stroke Doctor” with the tremendous improvement in his motor function and cognitive ability – a fact that his wife Melanie says is evidenced by the fact that Bob can now complete an entire word search book in one sitting. In addition to the use of stem cells and hyperbaric oxygen to “regenerate the body from the inside out,” Dr. Steenblock also uses customized IVs and chelation, External Counter Pulsation (ECP), extracorporeal shock wave therapy, lasers, pulsed electromagnetic therapy and many other “safe, non-toxic” therapies. These gently stimulate the body’s natural healing and restoration mechanisms for stroke, cardiovascular disease, Parkinson’s Disease, Alzheimer’s and dementia, ALS, arthritis and antiaging. According to the American Heart Association, nearly 800,000 Americans suffer a stroke each year, with two-thirds surviving and requiring some type of rehabilitation, while heart attacks strike over one million people in the U.S. annually. The widely accepted “standard of care” treatments are not meant to “cure” the effects of stroke or heart attack because they cannot substantially reverse the damage. Rather, its goal is to help a survivor become as independent as possible—unfortunately often with only partial success. Although Dr. Steenblock’s practice encompasses therapies for many degenerative dis(continued on page 14)

Bone marrow is taken, usually from the pelvis bone (iliac crest), using a needle and syringes. Patients often ask how painful the procedure is and while every individual has a different pain threshold, 50% of patients need only a local anesthetic and others may also need a small amount of valium intravenously. Question: What is Hyperbaric Oxygen Therapy (HBOT)? Dr. Steenblock: Hyperbaric oxygen therapy alleviates oxygen deficiencies, reduces brain swelling, repairs damaged cell membranes, stimulates the growth of neural stem cells (important for neural re-growth), neutralizes toxins, enhances immune defenses, renews ‘sleeping’ or ‘dormant’ neurons and stimulates the growth of new blood vessels into damaged tissues. External Counter Pulsation Machine (EECP) is used to help treat heart conditions and much more. orders and diseases, we asked that he address frequently asked questions about regenerative medicine as it specifically pertains to the treatment of stroke and cardiovascular disease. Question: What is regenerative medicine? Dr. Steenblock: Regenerative medicine focuses on assisting or augmenting the body’s own ability to heal by utilizing powerful, leading edge medical therapies that enable the body to repair, replace, restore and regenerate damaged or malfunctioning cells and tissues to help people return to better health. Instead of managing symptoms with prescription drugs, devices and invasive medical procedures, all physicians will someday use regenerative cell therapies to replace dam-aged tissue and restore function of a failing organ caused by disease, injury, congenital defects, aging or other circumstances.

During an HBOT session, the patient lies or sits in a special pressurize chamber and breathes in 100 percent pure oxygen. Hyperbaric oxygen increases the concentration of oxygen in the blood TEN TIMES NORMAL. This high concentration of blood oxygen forces oxygen into the wounds and areas of tissue damage, reduces swelling and builds new blood vessels.

Question: How are stem cells and HBOT used to aid in recovery for patients who have suffered a stroke (or suffer from other neurological conditions like Traumatic Brain Injuries and Cerebral Palsy)? Dr. Steenblock: Very simply, bone marrow is taken from the hip bone and given back to the patient intravenously. HBOT is then done daily for a few days. In patients with a stroke, heart attack, cerebral palsy or a traumatic brain injury, injured tissues are ‘dormant’ and nonfunctioning, but still alive. Stem cells and/or HBOT rescues these ‘sleeping’ tissues and return them to more normal function. In children, cognitive function and spasticity are improved in 70% to 80% of cases. Paralysis may improve or even disappear; children have gained their eyesight for the first time while people blind from their stroke have been known to regain their eyesight. Over 90% of stroke victims will have at least some degree of improvement in their problem areas if treated even many years after their stroke. Question: What is stem cell therapy and how does it work? Dr. Steenblock: Healing begins naturally in the body when healthy stem cells – which are responsible for growing new tissues - are made. With regard to cardiovascular disease, for example, transplanting healthy stem cells into the heart, can help restore or regenerate unhealthy cells and repair damaged tissue so that the heart can start functioning better. Stem cells generally come from umbilical cord blood, adult blood, fat or adult bone marrow. In the bone marrow, billions of new blood cells are made every day from blood-forming stem cells.

How to

BUILD BETTER BONE HEALTH After Age 50 By Thomas J. Moffett

Most people don’t think about bone health until they’re at risk for developing osteoporosis, a condition that causes bones to become porous, brittle, and weak.

A

Osteogenic loading sessions only take about 10 minutes a week and can be done in regular street clothes because people don’t break a sweat.

ccording to the National Osteoporosis Foundation, 54 million Americans age 50 and older are affected by osteoporosis and low bone mass. The statistic came from a 2014 study published by the Journal of Bone and Mineral Research that also said 10.2 million adults have osteoporosis and another 43.4 million have low bone mass.

When the body’s natural rebuilding responses are triggered during high-resistance osteogenic loading sessions, the body continues to build bone for days after the osteogenic loading event. Because it stimulates the whole body, osteogenic loading also leads to improved muscle strength and better balance.

Decreased bone mass affects people over age 50 more dramatically because the body naturally loses bone and rebuilds it more slowly as people age. Doctors often prescribe medications to slow the rate of bone loss and encourage the body to rebuild bone, but medications are costly and come with lots of uncomfortable and even painful side effects.

Osteogenic loading is beneficial at every age, regardless of physical condition. For people who are facing bone loss after age 50, it has had a significant impact on reversing the effects of bone loss. Many people who have done osteogenic loading sessions just once per week have completely reversed their osteoporosis and stopped taking osteoporosis medication.

Doctors also advise exercise to combat bone loss, but emerging research has shown that diet and exercise alone won’t build significant enough bone mass later in life to reverse the effects of osteoporosis.

If you are at risk of osteoporosis, or if you already have osteoporosis and are concerned about breaking a bone, consider osteogenic loading. It’s a natural solution that lets your body increase its bone density at any age.

A proper diet and regular exercise are always good for overall health, but when it comes to improving bone density, many people are turning to osteogenic loading, an emerging technique that is proven to build bone density at any age.

Thomas J. Moffett is franchise owner of OsteoStrong of Laguna Woods specializing in building bone density naturally through osteogenic loading. OsteoStrong of Laguna Woods is located at 23601 Moulton Parkway, Suite B in Laguna Woods. TO BOOK A FREE BONE DENSITY TEST AND FREE FIRST SESSION, call (949) 916-0916 or visit www.OsteoStrongLagunaWoods.com

Osteogenic loading is a brief set of intense resistance movements that naturally triggers the body’s internal rebuilding responses. When it comes to increasing bone mass, osteogenic loading can be more powerful than exercise because monitored sessions allow the user to safely push four or more times their own body weight, the amount of pressure required to build new bone. Patented osteogenic loading technology closely monitors sessions so that even a massive amount of pressure will never become uncomfortable and won’t cause injury.

Your Vulnerable Brain

Bumps to the head can impact your brain for years to come

H

by Mary McPherson

ow many times have you bumped your head throughout your lifetime? My granddaughter slipped and fell on a marble floor running through the kitchen in her socks when she was three years old - the blow to the back of her head sent blood flying in all directions. Rushing her to the hospital, we realized she was going to have a concussion. She will never remember that incident, yet in that moment the Neuro Net of her brain went into shock and her brain activity ramped up to start the healing process.

Concussions or head injuries from sports, automobile accidents, hard blows to the head, falling and hitting your head – almost everyone either has a similar story or knows someone who experienced a head injury. When I was a young girl I was hanging upside down from the top of the swing set and fell straight on my head. What we don’t often realize is these blows to the head can create a number of conditions that will develop in the months and years following the injury - ADD, headaches, anxiety, depression and many more. In fact we’re seeing it now with our professional athletes where research studies are finding that so many repeated blows to the head from playing football, soccer, etc. has a lasting adverse effect on the Neuro Net.

Often times the impact of the injury isn’t apparent immediately after the injury – in the weeks and months after a blow to the head, look for signs of stuttering, lack of concentration, possibly drifting off (or what we would call daydreaming), irritability, defiance, disassociating with friends and even feeling anxious and argumentative. If you notice yourself or your child exhibiting any of these symptoms or behaviors after a blow to the head there is help. Having a brain assessment after an injury is a helpful tool to see if the injury has actually created damage to the neural pathways.

Mary McPherson is the founder of the San Juan Capistrano-based Awoken Life Brainwave Optimization, where she helps adults and children achieve brain balance and harmony in order to live healthier, happier, and more abundant lives. Neurotechnology “Brainwave Optimization” helps the brain to reconnect the neural pathways after an injury. Connect with Mary and Awoken Life by calling (949) 661-6909 or visit www.awokenlife.com.

Stay and Play at the West Inn and Suites

One of Carlsbad’s best-kept secrets serves up serious family fun

W

By Ann LAurence

ith miles of charming seaside communities straddling the coast from Oceanside to La Jolla, San Diego County has no shortage of laid-back beach and ocean lifestyle destinations. But for our family's recent "staycation," we were looking for a locale where the mild and mellow Southern California beach scene blends with plenty of action and attraction-rich kid-friendly fun. We quickly settled on the idyllic beach town of Carlsbad and thanks to the West Inn and Suites (westinnandsuites.com) and its central location literally minutes from everywhere you'd want to be, we were able to indulge in a bit of both.

A favorite with business and pleasure travelers alike, this 86-room (including 36 suites) boutique hotel offers many special touches and thoughtful amenities, including a complimentary hot breakfast, complimentary evening pouring of featured wines, complimentary WiFi - and

a particular hit with the kids - a nighttime nightcap of milk and fresh baked cookies. With its relaxed and contemporary accommodations, we felt miles away from our otherwise hectic lives within minutes of arriving. Our spacious room was exactly as described on the hotel's web site - a place where "casual elegance meets coastal comfort characterized by a breezy "Casablanca meets Cape Code" design and vibe. To start our weekend away, we headed over to Bistro West, the more casual of the two dining establishments attached to the hotel property (the other is the upscale West Steak and Seafood) where we enjoyed farmto-table fare where the herbs, fruits and vegetables that define the dishes are sourced from The West Farm a few miles away. Because of its proximity to the hotel, the chefs at both restaurants are able to plant, review and harvest the season's produce daily. My roasted beet salad was delicious and arrived with roasted red and golden beets, baby greens, apples, walnuts, goat cheese and an orange tarragon vinaigrette while my husband thoroughly enjoyed Mary's Rotisserie Chicken which arrived accompanied by mashed potatoes, seasonal vegetables and roasted garlic lavender jus. The kids went

with penne pasta and a cheese pizza - pretty much their go-to dinner choices when eating out. After our meal, we strolled the streets of nearby Carlsbad Village reminiscent of a quaint European village - defined by funky beachinspired shops, unique boutiques and popular bistros.

Preserving Your Stone's Natural Beauty

After fueling up for the morning at the hotel's complimentary breakfast buffet, we were off to explore the San Diego Zoo Safari Park (sdzsafaripark.org) - an sprawling semi-arid 1,800-acre wildlife preserve and botanical garden that is home to over 3,500 animals representing 260 species from the continents of Africa, Asia, Europe, North and South America and Australia. One of the highlights for the kids (and their parents after walking the entire park to view its many exhibits) is the Africa Tram (included in the price of admission) which explores the expansive, free-range enclosures for "open-air" experience of antelopes, giraffes, cranes, buffalo and rhinoceros.

Locally Owned and Operated STONE & GRANITE REPAIR Granite Countertop Polish and Sealing

For an additional fee, the park offers several signature experiences - one of the most popular being the Jungle Ropes Safari where kids (wearing a safety harness) can climb, balance, swing and navigate their way through the trees - crossing rope bridges and moving platforms, walking across tightropes and negotiating swinging log steps - at times, the 90-minute adventure takes up more than 35 feet off the ground.

CONCRETE STAINING Marble/Natural Stone CLEAN & POLISH Flagstone & Pavers Tile and Grout Maintenance

Then it was back to our "home base" at West Inn and Suites, where my husband and I relaxed in the outdoor jacuzzi while keeping a watchful eye on the kids as they expended a bit more of their seemingly boundless energy in the heated swimming pool.

10 + Years Experience Maintaining and/or Restoring Your Flooring to Like New Condition

With proper care, we can extend the life of your flooring investment
