Description “Moving toward an inclusive society with equitable access to the fruits of prosperity and development” is one of the ultimate goals of Thailand’s 4.0 policy. This goal challenges such local higher institutes as Chiang Mai University which is located among diverse ethnic group communities in the northern part of Thailand, and immigrants from neighboring countries in upper ASEAN. This roundtable will focus on the quality of life of children from ethnic group communities and immigrant families who suffer from craniofacial conditions such as cleft lip and palate.

The children born with cleft lip and cleft palate require special care from a team of different health professionals. The series of treatments is planned, managed, and monitored from infancy to young adulthood. Theoretically, the best outcome should be achieved when the treatment is done in the right time of growth and development.

The ethnic group and immigrant patients often fail to accomplish the series of treatments as shown in the treatment protocol. That is because of financial problems, difficulty of 4-­‐5 hours travelling from their remote communities, repeated surgical corrections, long-­‐term treatment, too frequent doctor’s appointments etc.

What can CMU scholars do to increase the quality of life of children with cleft lip and cleft palate and their parents? Thanks to the CMU team an initiative has been created for patients with cleft lip and cleft palate especially for the disadvantaged ethnic group communities and migrant laborers.

The team consists of expertise from many professionals including a surgeon, dentist, ENT doctor, nurse, speech and language pathologist, social worker, and IT person. They address the same problems and use their expertise to find answers for each issue.

The goal of the project is that every patient could achieve the necessary standard of treatment for patients with cleft lip and cleft palate.

The surgical outcome of cleft lip and cleft palate repair affects the quality of life of the patient.

If surgeons wish to achieve success, not only must surgical technique be improved, but a collaboration network must be formed.

These induce psychosocial problems for the patients and their family.

Back in 2013, Nasoform was invented with the aim of solving the problems of the collapsed nose after surgical correction.

10 Since then, it has been used in the patients who have undergone cleft lip-nose correction at Maharaj Nakorn Chiang Mai Hospital.

They often fails to accomplish the aim patients with cleft lip Âą palate of treatment that lasts up to 20 years.

Based on the data of Maharaj Nakorn Chiang Mai hospital (2017) 13

was designed to overcome their difficulties in accessing the treatment.

When the long-term treatment was planned and distributed to the proper hospital,

with systematic monitoring and effective collaboration, most of them were treated as planned.

There are approximately ten babies born with cleft lip and cleft palate each year.

Most of the population in this area is the disadvantaged ethnic groups and immigrants.

Prior to this project, all babies born with cleft lip and cleft palate were referred for investigation and treatment at the higher-potential hospital in the city.

After collaborative network was set up, most treatment is done at Fang hospital.

This allowed them to access the treatment as planned.

What is the next step for implementation this pilot system We found the excellent results from this pilot project; one stop network for cleft lip and cleft palate.

1) good clinical outcomes for immigrant patients have to take care by a high experience surgeon or care giver because they have just one or two chances for their treatment.

2) new concept of the health collaborative network of care may be the answer from them

This concept is University hospital are network monitoring and coaching for information and knowledge, on the other hand public hospital is center of health provider.
