DOWN – BUT NOT OUT, AS THE PHARMACY SHOW CONSIDERS THE ‘WHAT NEXT?’ FOR COMMUNITY PHARMACY Imposed funding cuts and punitive reimbursement price reductions are “damaging blows” to contractors’ cash flow which may be “impossible to manage”, PSNC has warned.

Speaking to contractors following the latest changes to pharmacy income, the negotiating body has urged contractors to make whatever provisions they can for the funding reductions and to continue to take advantage of any additional locally commissioned services that they can afford to provide.

In her last presentation to the Pharmacy Show before she retires at the end of the year, PSNC chief executive Sue Sharpe will open the Keynote Theatre conference programme on day one of the event with a presentation on the outlook for community pharmacy 2018-2020.

Other presentations during the two days of the Pharmacy Show will also discuss where pharmacy should go next.

Presenting views from the “outside” are James Kingsland, president of the National Association of Primary Care and Rob Darracott, director of McIntosh Health Partners and former chief executive of Pharmacy Voice.

However, from the day for Local Pharmaceutical Committee (LPC) leaders organised by PSNC and held at the end of September, it is clear that community pharmacy may be down because of the funding and reimbursement cuts – but that it is far from being ‘out’.

Funding in 2016/17 was cut by £113 million, a reduction of 4 per cent over the year, but because of the timing of the announcement, this led to a squeeze of 12 per cent for pharmacies in the last four months, from December

2016 to March 2017.

In 2017/18, a further cut of £95 million was introduced, which saw funding levels from April 2017 drop by around 7.5 per cent compared with funding before the December 2016 cut.

Pharmacies with lower than average There are still many questions dispensing volumes facing community pharmacy in urban areas have going into 2018. been specifically targeted by the new funding model and from August, all pharmacies will experience additional downward pressure on income - £15 million per month for 12 months - from the reduction in Category M reimbursement prices needed to redress an over-delivery of margin income last year. Just two weeks into the month, pharmacies had already taken to the digital air waves to complain about their inability to buy pregabalin at its new average reimbursement price of under £3.

Stretch, director of prescribing at the Argyle Health Group, on the topic of medicines optimisation in care homes, and in the public health and wellbeing forum at 10am from Deborah Evans, managing director of Pharmacy Complete, speaking on the subject of Healthy Living Pharmacy: beyond ticking the box.

DEVELOP YOUR CAREER AS A GP PHARMACIST According to PDA Chairman Mark Koziol, it is a welcome development that pharmacists in much larger numbers than ever before are now to be encouraged to work in GP surgeries, and this is being driven by the capacity shortages in the GP service. For pharmacists working in a GP surgery-based role, the benefits include additional clinical roles and skills, including independent prescribing, as well as an improved relationship with the GP surgery. The effect of this emerging career choice on the number of pharmacist independent prescribers is evident: at the beginning of November 2015, there were 2,567 independent prescribers (5 per cent of all registrants). At mid-August 2017 this has risen to 5,279 independent prescribers (9.6 per cent of all registrants).

The PDA sees the development of GP pharmacists as going some considerable way towards addressing some of the supply and demand issues currently being faced by the profession. But, as with all of these new developments, there are risks along the way; there is concern from a risk management perspective about the lack of focus upon the different levels of experience required from the prospective GP pharmacist, particularly if placed in patient-facing roles. FAKE MEDICINES AND BREXIT. HOW UK PHARMACY IS AFFECTED Medicines regulator the Medicines and Healthcare Regulatory Agency was involved in seizing £13.6 million worth of potentially dangerous medicines in 2016.

As part of the annual Operation Pangea IX global initiative to tackle the illegal trade in medicines and medical devices, the MHRA also worked to close down or suspend 4,700 websites. In its annual report for 2016-17, the MHRA says its inspectorate also conducted 1,850 inspections, with over 90 of them at overseas companies.

The annual report also considers Brexit – a topic being considered in some depth by the Pharmacy Show Business Theatre this year - and how this might impact on the agency and its public health protection role. In his foreword to the annual report, MHRA chief executive Ian Hudson says the MHRA is still considering two main models for the post-Brexit regulation of medicines: one in which the MHRA operates in partnership of some form with the EMA regulatory network; the other where the MHRA becomes a standalone regulatory agency, outside European procedures.

Earlier this year health secretary Jeremy Hunt and business secretary Greg Clark wrote to the Financial Times stating that in developing a new regulatory system post-Brexit, the best interests of patients and support for the UK life

science industry would be central to negotiations. As well as securing the medicines supply chain, central elements in this discussion include the implementation in the UK of the European Falsified Medicines Directive (FMD) by February 2019. This topic can also be found in the Business Theatre education programme at the 2017 Pharmacy Show.

FMD is the European approach to dealing with the rising global issue of falsified medicines. Like other major markets, including China and India, it involves individual serialisation of medicine packs and an associated authentication system.

The European Directive and the Delegated Regulation are binding on all EU and EEA Member States. As it currently stands, it is likely that FMD scanning will come in to effect before the UK leaves the European Union. The current position of the Department of Health and the MHRA is that it is “business as usual” for the FMD. FMDSource, www.fmdsource.co.uk, is the UK FMD working group for community pharmacy, and it says that given the global nature of falsified medicines and the fact that the UK remains a major part of the pharmaceutical sector, the UK has no other option but to adopt pack serialisation in order to maintain its reputation as a safe part of the medicines supply chain. “At present, it seems likely that this would be the European FMD system,” it says.

WHY PHARMACY TEAMS SHOULD WALK IN GPS’ SHOES The Pharmaceutical Services Negotiating Committee (PSNC) has developed a toolkit to help pharmacy and GP teams better understand each other’s roles. PSNC’s Services Team explain.

It is hard to escape headlines about the pressures facing GPs at the moment; but just how difficult can running a practice be? “From the very start of the start of the day I realised just how much pressure the practice is under,” says community pharmacy contractor Chris Fernandes, who has spent a number of days working in practices near his pharmacy in Lewisham. The biggest surprise was the amount of administrative and paperwork that goes on behind the scenes, he explains.

Pharmacy contractor Al Patel had a similar experience on his ‘job swap’ with local GPs: “We both realised just how much extra work is involved in the jobs we each do. We don’t see all the paperwork, but there is a huge amount for all of us.”

Chris and Al were both taking part in a ‘Walk in my Shoes’ project commissioned by Lewisham Clinical Commissioning Group (CCG) two years ago. The Inter-Professional Exchange project gave staff from GP practices the chance to visit community pharmacies, and community pharmacy staff visited GP practices in return. The experience helped both teams to better understand the pressures facing the other and to identify solutions to local problems together.

All of which is very much in line with wider NHS objectives to get health professionals working more closely together to tackle increasing demand and financial pressures.

In Lewisham between January and April 2015 more than 160 exchange visits took place. Then earlier this year, PSNC published a toolkit to help others set up a Walk in my Shoes programme in their area. The toolkit can be used by either a Local Pharmaceutical Committee (LPC) who would like to work with the Local Medical Committee (LMC), NHS England or CCG to get a programme implemented across

their area, or by a pharmacy team looking to improve relationships with their local GP practice. It builds on the documentation from the Lewisham project and includes everything you will need to do the service from template promotional letters and presentations to a confidentiality agreement, checklists, reflective logs, action plans and more.

So what can you expect to get out of if you do take part in a similar project?

One useful outcome for Chris was that the GPs saw how much work was involved in making up dosette boxes. The scheme can also lead to better long-term communications.

Chris says: “Talking to the doctors used to be very difficult; the receptionists could be a real barrier and if we had a problem we went into the queue system with everyone else.

We now have bypass numbers and some email addresses which can mean we get a response very quickly.”

There can definitely be some perks to spending a day in general practice too. “Lunchtime was bliss,” says Al. Although for the doctors who were busy signing prescriptions and reviewing repeat requests, he says the two hours without patients was still pressing. But he did like the fact that the doctors were very well supported by staff on the administrative side of things. “This was quite unexpected as I assumed the doctor would take charge and look at all medical related paperwork.”

In general the process made both GPs and pharmacists more sympathetic to the challenges faced by the other.

As Al concludes: “Working together as a team with the local surgery is enhanced, and you can get a better understanding of each other’s problems, making life easier for both sides.”

Pharmaceutical Care: a more difficult route in England Achieving excellence in Pharmaceutical Care: a strategy for Scotland is an excellent and well thought through document. The strategy recognises the baseline Minor Ailment Service (MAS), Chronic Medication Service (CMS) and Public Health Service (PHS), underpinned by Serial Prescribing and Dispensing and then wants to drive these forward. Scotland has committed to better clinical training, more independent prescribing pharmacists, enhanced MAS, community pharmacy led clinics, a bigger role in long term conditions and an expanded role in public health. It has not been easy and it is not all there, but ambition is everything.

In England we have a system that is slowly moving towards place based capitation based budgets within an accountable organisation (system) structure. The English NHS doesn’t want to pay for otc medicines, wants to make savings on the cost of dispensing medicines and encourage self-care and perhaps self-pay for those who can afford choice and convenience.

We do, however have the quality payment scheme which may be unfairly criticised as a tick box exercise rather than creating a platform from which to create excellence. Community Pharmacy, similar to optometrists and dentists seem to sit on the outside of plans and watching everything develop in front of our eyes.

I have no doubt that the English NHS will be unable to achieve its objectives without the inclusion and participation of community pharmacy. However, whereas pharmacies in Scotland have clarity about funding and support, English pharmacies will have to tread a similar pathway without central funding, without direct support and with limited certainty of the future. NHS England is on a savings spree and local authorities are skint. On the bright side, some CCGs have small amounts of money as do some hospitals; welcome as it is, it just drives further inequity of provision and more confusion as to what service is available where.

The mantra ‘free at the point of delivery’ has been weakened. If you cut the number of points of delivery, then people have to pay. There is a clear correlation between waiting times and private provision. As waiting times go up and access is challenged then people seek other sources of healthcare and have to pay. It is a flow- whether we agree with it or not – it is happening. We either object or we run with it. There are a number of things that community pharmacy should consider: •

Look wider than your current customer base and consider the whole local population

•

Identify their health needs and priorities and what is important to them

•

Develop and provide new services and ensure that you match need with provision

The purpose of all businesses is to fulfil customer need profitably. It is becoming harder to service the main NHS customer. Look elsewhere and look differently.

When you visit the Pharmacy Show this year you will be faced with an incredible variety of speakers and stands. It is the most important year to think differently and look at the exhibitions in a different way. The future is out there – find your path.

AIM HOST SUPERINTENDENT FORUM AT PHARMACY SHOW AIM, the Association of Independent Multiple Pharmacies, is making its final preparations for its bi- annual Superintendent’s Forum at this year’s Pharmacy Show.

ARE YOU A PRE-REGISTRATION PHARMACIST? The pre-registration year is filled with opportunity. It is a very important year and each pre-reg trainee will experience their own unique journey. This is a journey which you should prepare well for, as well as receiving the support you need.

This year, a conference which will be taking place at The Pharmacy Show can help you. The British Pharmaceutical Students’ Association (BPSA) and Pharmacists’ Defence Association (PDA) have collaborated with ProPharmace, to produce ‘Aiming High’ – a high-quality, twoday conference which aims to help get your pre-reg year off to a great start.

COMMUNITY PHARMACY AND SEXUAL HEALTH There is a sexual health crisis. We have failed to encourage people to look after their sexual health and test regularly. The incidences of Gonorrhoea and syphilis have been increasing steadily. In 2015 only 13% of young men were tested for STDs and 32% of young women. The diagnosis of chlamydia is falling, but so is the rate of testing – the two are probably linked.

With all of this happening, sexual health services are experiencing a funding crisis.

Smaller outreach clinics are closing in favour of centralised services and postal services. Commissioned Community Pharmacy based services are few and far between and lack the support of a proper persuasive campaign.

We are failing to persuade sexually active people to take responsibility for their sexual health and test regularly. Safe sex is not safe – just safer. Some STDs can be present without troubling symptoms – the only way of knowing is to test.

Community Pharmacies are ideally placed to take a lead with their local communities and provide a range of simple tests for STDs. Testing should become part of a normal routine – a way of life – a sexual health check, as well as a mechanism for testing when symptoms are present. Community Pharmacy has the ability to understand the need for routine testing and the support required if tests are positive. Condoms are no longer hidden in the third draw on the left – they are on open display.

Community Pharmacy is the right professional service to advise on safer sex and support routine sexual health testing.

This conference will give you a head-start with preparing for the registration assessment, with sessions such as ‘An Introduction to Calculations’ and ‘The Registration Assessment Framework’; as well as helping you to develop some important professional skills and providing practical tips on planning your year, writing ‘evidence’ and much more.

The NPA also acknowledges the intrinsic value to NHS commissioners of the “hundreds of thousands [who] walked through the doors of their local pharmacy for the vaccination” last year. Mr Strachan says: “The community pharmacy network is a source of unexploited value. There are lots of things we could do, that we don’t at present. We are part of the fabric of people’s lives and that means we could really make a huge difference, if the network was invested in. People just need to give the pharmacy network a chance to show what it can do.”

In a report issued earlier this month , researchers from Manchester, Nottingham and London Universities used economic modelling to conclude that the service had saved the NHS potentially over £500 million in its first five years. Savings arose through improved medicines adherence that has quality of life benefits and which is associated with reductions in the overall cost of treating diseases covered by the service.

Opportunity knocks At the Royal Pharmaceutical Society, chair of RPS England Sandra Gidley has called for the list of medicines covered by the New Medicine Service to be extended to cover all long-term conditions including mental health issues.

Commenting on the results, PSNC chief executive Sue Sharpe said: “With the current pressures on the NHS it is vital to use community pharmacists to help support GPs and other parts of the health care system, using their expertise in medicines and the relationship they have with their patients.”

She says: “We’d also like to see more patients referred into the service by primary or secondary care providers to ensure the service is used as widely as possible. All patients prescribed new medication should be encouraged to take part in the New Medicine Service by their prescriber.”

NPA chairman Ian Strachan adds that week on week community pharmacies work many tens of extra hours each week, in addition to those contractually provided. He says this shows the “unrecognised value and importance of community pharmacy to commissioners and NHS decision-makers”.

At this year’s Pharmacy Show, PSNC will again host the Pharmacy Services Innovation Theatre in which pharmacy teams can see and hear a whole host of ideas for commissioned services, additional income streams and ways to connect with clinical commissioning groups, local authorities and the voluntary sector. The conference education stream has been created to give delegates the tools and the confidence they need to make a difference within the communities they serve.

The flu opportunity One area in which community pharmacy can readily excel is flu vaccination. From the beginning of September, England launches the 2017-18 flu immunisation season and with this gives community pharmacies another chance to show the value of their service in local neighbourhoods.

Last year, GPs fell well short of flu immunisation targets with uptake figures for the over-65 age group actually falling compared to the previous year to reach a 70.5 per cent uptake rate. Overall, for all people in the clinical at-risk groups uptake reached just under 49 per cent. Taking minimum 2017/18 season vaccination rates into account, the exact extent of the challenge facing immunisers is clear. Targets for 2017/18 include a 75 per cent coverage rate in the over-65s and a 55 per cent uptake for all clinical risk groups overall.

THE FUTURE OF PHARMACY IS ALL ABOUT DELIVERING CLINICAL VALUE AND SAFER BETTER PATIENT OUTCOMES Medicines are the most common healthcare intervention. £16bn investment from the NHS and rising. Pharmacy is so well placed to ensure the best return on investment for patients and for the NHS as a whole – we can and should be ensuring that all of that investment delivers the best possible care in the safest possible way. Jeremy Hunt has highlighted medication errors as a real priority for the NHS and we are the people who should be leading on improving this.

with prevention support, early diagnostics, treatment of common conditions and signposting to others when needed. As well as the strength of MURs and NMS and how they can be used much more strategically as part of care pathways so that other clinicians can rely on the patient receiving support with medicines use and adherence.

There is some amazing work going on by pharmacy teams around the country but there is unwarranted variation in outcomes and pharmacy needs to coordinate and integrate to improve this. In other words, we need to take responsibility making sure that every patient is getting the care they deserve. Including some of the most vulnerable – the housebound, the care home residents, those with learning disabilities and everyone who has to try and manage polypharmacy.

So what can we do better and how can we work together as a more integrated profession to join up care and make sure people stay safe and with the best possible health outcomes? My suggestion is to get more involved with fellow clinicians locally – pharmacy teams, doctors, nurses, OTs, physios as well as social care. Talk to your colleagues locally and don’t give up if you get the brushoff at first, keep going with messages that you know will improve patient care. We need to be a key part of emerging multidisciplinary teams and step forward to participate and be seen as the medicines experts. At the same time we need to integrate our own profession more successfully with specialist hospital pharmacists available to support those working in GP surgeries, community pharmacy and other settings. Communication is key here and technology can help, especially if we join campaigns to ensure that full

Community Pharmacy holds a trump card in the patient care pathway where medicines are involved. The most frequent clinical contact with patients for the whole time they are needing to continue with medication and we need to be making more of this contact opportunity to add more value. In my view community pharmacy needs to take much more responsibility for patient outcomes and be seen in that key role by other clinicians in the local health care system. Also we need to be smarter about collating data and shouting about the amazing work that community pharmacy already does to support local communities

TIME TO BE ASSERTIVE AND RESILIENT! Many of us are aware of the importance of looking after ourselves physically, but how many of us allocate time to looking after our mental health? With figures from the charity Mind indicating that 1 in 4 people in the UK will experience a mental health problem each year, a shift in focus toward mental wellbeing may be in order.

A wellbeing legacy Over the past five years the team at Pharmacist Support – the profession’s independent charity – has been assisted by a substantial legacy to develop pro-active ways to assist those in the profession to manage their wellbeing. The forward thinking vision of the benefactor enabled the creation of the Wardley wellbeing service. Practical support is provided, free of charge, through workshops and a range of online resources which focus on the provision of information, tools and techniques to help recognise the signs and symptoms of stress and deal with everyday pressures.

New workshops, new focus Building on the success of this service to date, Pharmacist Support is now expanding its wellbeing support with the launch of brand new workshops focussing on three specific areas: Assertiveness, Building resilience and Time management.

The new sessions will allow attendees to explore these aspects of manging their wellbeing in more detail. The aim being to provide effective techniques to help equip those in the profession to build the resilience and confidence they need to deal with the day-to-day and unexpected stresses of pharmacy. Attendees will explore a range of tried and tested tools, techniques and ideas, with activities including role play, case studies, cognitive behavioural therapy (CBT) and group discussion.

“These three new workshops are an ideal next step for people who have already attended the ‘Introduction to wellbeing’ workshop and want to broaden their knowledge, but they are also perfectly accessible for anyone new to the service” commented Pharmacist Support Chief Executive Diane Leicester-Hallam. “Each session is discussed and planned in advance with the host to ensure it meets the specific needs of each group, and is adapted whenever necessary.”

Having to try to combat this funding loss, we are now in a position of reducing recruitment into our business, reducing innovation to help better our community’s health, but we are spending more time isolated in our pharmacies.

We were spending time out in the community bringing pharmacy into the lives of those that needed it the most. The vulnerable, isolated, and in the outreach of our communities that are need of health care; are all finding out now what it’s like not to have pharmacy at their sights.

The elderly & housebound have been neglected of the care of prescription deliveries, as most of community pharmacy are reducing the unpaid costs to their business plans.

Key strategies have been shared on our forum including purchasing efficiencies, extra income generation from services, proactive retailing and staff training. Cash flow has been very challenging for us all. Reducing costs through reducing staffing levels, or even changing opening hours have all been our first plan of action.

All this put together has made pharmacy owners feel very isolated, and there is no where for us to get any guidance and support.

PHARMACY - OVERCOMING CHALLENGING TIMES Since the Pharmacy Show in 2016, life has not got any easier for contractors and the future is looking even more challenging with funding cuts biting hard. It is however a chance for everyone to be the best that they can be in these difficult times, operating at the top of their licence.

It is not a time to be tied to the dispensary bench, labelling, dispensing, ordering and completing the mass of additional paperwork that is increasing daily. Rather, it is a time to delegate these tasks to the well trained pharmacy team and pharmacy technicians and to seek out all available opportunities.

There is still money on the table for MURs, NMS and Flu and not forgetting the recently introduced Quality Payments that allows us all to up or game with the quality agenda.

At our best, we will improve standards and quality outcomes, introduce safer working practices and be accredited Healthy Living Pharmacies which will make a true difference to our local communities. Use of Summary Care Record will also prepare us for the digital future where we can be fully integrated with our GP and hospital colleagues. This is not a time for the faint hearted; resilience is the name of the game and I am very confident that our sector will come out of this much stronger, it may just be in a different shape to what we have grown up with.
