DON’T MISS THE SESSION ABOUT: Building for the future - the national perspective Simon Dukes, Chief Executive, Pharmaceutical Services Negotiating Committee Sunday 6th October 09:20 - 10:05 Keynote Theatre

Community pharmacies that provide needle exchange will be asked to test patients for hepatitis B as part of the drive to eliminate hepatitis B. Further services that support the drive on reducing antibiotic resistance may be introduced. This might include testing for strep A and CRP. All pharmacies will have to progress to healthy living pharmacy level 1 as an essential element of the new contract. The reach of the mandated annual health campaigns that community pharmacies take part in will be extended and

People are getting more used to a digital age and obtaining more goods through internet shopping and apps. There are several NHS compatible apps that support patients to order their repeat prescriptions online and pharmacies offering on-line consultations. Many people expect added convenience and home or work delivery.

EPS has also supported the increase in robotic dispensing. Automation of dispensing is thought to free up pharmacy team time and reduce errors. Although automation is common in large volume dispensaries, smaller pharmacies do not gain the benefits. The Government, however, has committed to pursue legislative change that will allow all pharmacies to benefit from the efficiencies of hub and spoke dispensing.

These two factors; the introduction of EPS; and the drive

aligned to the equivalent campaigns in general practice as part of effective integration across PCNs. This will open the opportunity to manage smoking cessation and weight management with local partners. The Pharmacy Integration fund and the PCN Testbed programme will be used to test a range of additional prevention and detection services, which if found to be effective and best delivered by community pharmacy, could (with appropriate training) be mainstreamed within the CPCF over the course of the settlement period. Every year these new elements of the pharmacy contract will need to be agreed and implemented. There is almost £250million of unallocated funds that will need to be diverted into new services. Continued on p2

NHS Digital is making great progress with the implementation of electronic prescription service (EPS) and no wonder as it is predicted to save the NHS £130 million. The first CCG has now implemented EPS in all practices meaning that all prescriptions can be sent digitally. As more patients nominate their preferred pharmacy then all paper prescriptions can be avoided. From generation, through dispensing, pricing and reimbursement to information to commissioners, every element is paper free. for on-line convenience have come together to drive the uptake in on-line pharmacies. If people nominate an on-line pharmacy, they usually get their medicines delivered to their home a couple of days after ordering. There is no doubt that this sector is growing, with many of the more traditional pharmacies offering a similar service.

THE EFFECT OF DIGITALISING CARE Nominations will become an important factor for the success of a pharmacy. The volume of nominations will determine the volume of prescriptions as the old-fashioned collection of paper prescriptions from doctors’ surgeries is lost. Nominations will drive the competition between on-line pharmacies and the more traditional bricks and mortar pharmacies.

Much has been said about the maintenance of the global sum at the current level for the next 5 years, but little has been said about the opportunities to fundamentally change the format of community pharmacy that the contract opens. The community pharmacy consultation service will replace the NHS Urgent Medicine Supply Advanced Service as well as local pilots of the Digital Minor Illness Referral Service. Patients can be referred to community pharmacy from NHS 111 for minor ailments and emergency supply of medicines. This will be expanded to include referrals from GPs, urgent care centres, on-line NHS 111 and perhaps A&Es.

PHARMACY CONTRACT: THE GATEWAY TO THE CLINICAL COMMUNITY PHARMACIST We have heard the rhetoric of community pharmacies being a ‘vital and trusted part of the NHS’, but now we have a contract that will test both sides of the table to provide and to deliver. There is little doubt that the new contract is bold, but much of it is unwritten and will depend on the developing relationship between pharmacist, the negotiators and their paymasters.

The contract recognises the role of community pharmacy in medicines safety. A medicines reconciliation services will be introduced to ensure that changes made within secondary care are implemented in primary care. In addition there is support for structured medication reviews, although it is not clear how these will be delivered. The new medicines service will be maintained and expanded to include other medicines. The pharmacy quality payments scheme has a new name the pharmacy quality scheme (PQS). As expected elements of the previous scheme will move into essential services as new elements are brought in. The most surprising is the sweetened beverages element where the NHS is clearly directing what can and can’t be sold in a pharmacy. This may have significant implications for some.

Many pharmacies are experiencing financial difficulties. This new contract does little to relieve this, of funding over the next 5 years. However, this is dependent on a change in community pharmacy, where a larger element of the funding will depend on clinical services delivered on direct referral or within new services. Many would applaud the direction of travel and welcome a contract that facilitates a change that has been required for many years. But the success will depend on continued effort of both sides and the willingness of community pharmacy to embrace the change in direction.

Most importantly some enablers are being put in place. Part of the PQS pharmacies will receive a payment to engage with PCNs. Education and training will be supported by Health Education England and money will be spent on the pharmacy IT enablers. There are also plans to review the reimbursement process to make this more efficient and to look at different dispensing models to free up pharmacy staff at the front line. These are, perhaps some of the most important elements of the contract agreement and important to support the integration of community pharmacy, both digitally and through enhanced capability.

National pharmacy NHS 111 referral service to launch on October 29 Pharmacies across England will receive £14 for each consultation resulting from a referral from NHS 111 under a new advanced service launching at the end of October. Pharmacies in England can from September 2 register their interest in providing the Community Pharmacist Consultation Service (CPCS), which will see them receive referrals from NHS 111 from October 29 for minor illnesses – such as rashes, constipation and vaginal discharge – and urgent medicines supply. The advanced service follows on from two pilots – the Digital Minor Illness Referral Service (DMIRS) and the NHS Urgent Medicine Supply Advanced Service (NUMSAS) – and aims to establish community pharmacy as the “first port of call” for low acuity conditions, as well as reduce demand on urgent care services, NHS England said. Pharmacies must meet a set of requirements to provide the service and will receive £14 for every consultation completed – either over the phone or face-to-face – following a referral from NHS 111. As set out in the five-year funding contract in July, pharmacies signing up to provide the CPCS by December 1 will receive a £900 “transition payment”, and those who do so by January 15 will get £600.

How will the service work? Under the CPCS, an NHS 111 advisor will refer a patient to a participating pharmacy via a dedicated IT system – which pharmacies must check at regular intervals – and advise them to call the pharmacy, so a pharmacist can assess their need for an urgent medicine supply, NHS England explained. The consultation can be conducted entirely over the phone in certain circumstances, such as if a patient does not need to pick up a prescription. However, the pharmacist may deem it necessary to invite the patient to attend a face-to-face consultation before making an emergency supply, it added. As well as emergency supplies, referrals may lead to pharmacists giving advice, providing over-the-counter medicines, referring patients to locally commissioned

pharmacy services, or to other appropriate services and healthcare professionals, NHS England said. The service must not be actively promoted to the public and should not be used as a replacement for the normal repeat prescription ordering and dispensing processes, the commissioning body stressed.

Service requirements Pharmacies participating in the service must have a consultation room and from April 2020 be able to access the CPCS IT system from the room. Pharmacists delivering the consultations must have access to the summary care record and the pharmacy’s shared NHS mail inbox. It will initially fund a CPCS IT system to manage the referrals and log the outcomes. However, by 2021 contractors will be expected to have decided on the IT system they wish to use to continue to deliver the service. From October 29, pharmacies signed up to provide the CPCS will only receive referrals from NHS 111, but NHS England expects to add additional “strands” to the service –such as GP and urgent care referrals – following pilots, it said. Pharmacies can sign up to the service – and claim their £900 transition payment – on the NHS Business Services Authority website.

FLARIN Unique Lipid Formulated Ibuprofen Improving pain relief choice in pharmacy What makes Flarin ibuprofen different?

Managing Joint pain

Flarin is the only ibuprofen available with a unique lipid formulation. Flarin® utilises a lipid-based drug delivery system, which means the lipids in Flarin® encase the ibuprofen1. This has important effects on the absorption of ibuprofen and unique clinical benefits have been demonstrated with Flarin. The ibuprofen stays encased in the lipids rather than dissolving in the stomach acid, helping to shield the stomach from damage. The lipid ibuprofen then travels to the small intestine where the lipids are broken down for absorption into the gut lymphatic system2.

Joint pain is a common complaint, with around 10 million people in the UK experiencing joint pain almost daily4. Some will experience pain and discomfort that limits movement; in some cases this can be debilitating. While there are many potential causes, joint pain is frequently caused by injuries or arthritis. Pain in the knee is particularly common. Lifestyle measures such as maintaining a healthy weight and keeping active can help to reduce the impact of joint pain. Medication such as ibuprofen can also be an important part of the pain relief approach as it helps to reduce inflammation. It is important that consumers, together with their pharmacist, understand the range of medication options available so that they can choose the medication that is right for them.

®

Improving pain relief choice in pharmacy Flarin® is a new generation of ibuprofen, providing consumers and pharmacists with a new treatment option for joint pain. It is the first and only OTC ibuprofen (200mg soft capsule) medicine with this unique lipid formulation. Because of this lipid formulation, it is also the only OTC ibuprofen (at a maximum dose of 1200mg/day) clinically proven to be as effective as prescription-strength ibuprofen (2400mg/day) at reducing joint pain3. No other ibuprofen is more effective for joint pain. A recent study (FLARE) found that taking two lipid ibuprofen capsules (2x200mg) three times a day for five days compared to standard ibuprofen provided 86 per cent of patients with improvement in their knee pain flare-ups3. Patients also reported a reduction in stiffness and swelling. They had a lower incidence of gastrointestinal (GI) side effects than with prescription strength ibuprofen dose3 as Flarin’s unique lipid formulation helps shield the stomach from damage.

Home comforts – are pharmacists best placed in a community setting? NHS England’s latest initiative to improve the quality of patient care has re-opened discussions concerning where pharmacists are most effective – in a community pharmacy setting or from within a GP practice? The ‘NHS Long-Term Plan’, published on 7 January 2019, has pledged £4.5bn in funding over the next decade for community healthcare. The document proposes to fund “expanded community multidisciplinary teams aligned with new primary care networks” of GP surgeries covering between 30,000 and 50,000 patients. Although the new plan outlines making greater use of pharmacists’ skills and opportunities to offer better healthcare outcomes for patients, there is some ambiguity as to where they should reside. Location, location, location? In 2016, NHS England pledged to invest £100m to fund placing 1,500 additional pharmacists into GP practices. However, with the funding for practices non-recurring three years later, some GPs are now struggling to sustain the employment of pharmacists and are looking to shed costs.

This trend has prompted senior influencers, such as the president of the National Association of Primary Care (NAPC) Professor James Kingsland, to suggest that community pharmacies and general practices need to work collectively in a different way. Funding challenges To make the NHS Long-Term Plan’s lofty ambition a reality, pharmacy bodies have called for clarification and assurances over the availability of funding. In October 2018, the Pharmaceutical Services Negotiating Committee (PSNC) ensured that community pharmacy funding will remain unchanged for the 2018/19 financial year at £2.59bn, despite attempts by the Government to cut spending. While this news has been welcomed, most pharmacists would suggest that a funding overhaul is still needed to allow the sector to achieve the NHS’s vision.

An annual survey* published by Chemist+Druggist has revealed the impact of funding cuts on pharmacy owners in England in 2018. 97% of English contractors made one or more financial cutbacks to their pharmacy business, with almost two-thirds of respondents admitting they had postponed plans for investment. In addition, 63% conceded that funding cuts were the biggest threat to their business, followed by category M clawbacks (13%) and increased competition from other providers (5%). Community pharmacies for the 21st century Community pharmacies could hold the key to adopting a more clinically focussed approach and engaging more closely with patients. But this utopia requires forward-thinking pharmacies to invest in modern technologies, from patient medication record software and robotic dispensers to cloudbased portals for logging dispensing errors and electronic prescription services. Notwithstanding the continued uncertainty surrounding funding, pharmacy owners do not have to put their investment strategies on hold indefinitely. Specialist and alternative finance providers can assist pharmacists in their professional finance requirements with a range of flexible solutions to facilitate their technology needs, whether you a first-time buyer or an existing business owner looking to enhance the premises you already own. 2019 is set to be a year of increased responsibilities and opportunities in the pharmacy sector. With the right support, community pharmacists can be trailblazers for the NHS Long-Term Plan through offering a wider variety of pioneering services and more personalised interactions to make a tangible difference to their patients’ lives. *Revealed: impact of funding cuts on pharmacy owners in England in 2018, Chemist+Druggist, December 2018. https://www.chemistanddruggist.co.uk/ feature/revealed-impact-funding-cuts-pharmacy-owners-england-2018

Eczema is a multidimensional skin condition Eczema is a multidimensional skin condition which can affect patients and their families psychologically, socially and sometimes financially. Characterised by red, inflamed and itchy skin, it is estimated that up to 15 million people in the UK could be living with eczema. Affecting up to 20% of children and 6% of adults, eczema has many forms, with Atopic Eczema being the most common. Although the exact cause is unknown, eczema is a combination of genetic and environmental factors which result in a defective skin barrier, making the skin of sufferers more susceptible to sensitisers, irritants, allergens and pathogens. Common triggers which can cause eczema to flare-up include irritants and sensitisers, such as SLS, parabens and fragrances, as well as soaps, clothing, stress, food, hormonal changes and exercise, including swimming. Identifying a patients trigger can help them avoid it, seems simple when you’re changing from a soap to a soap substitute however not so feasible with hormonal changes. Irrespective of severity, emollients form the basis of managing dry skin conditions such as eczema however patient adherence is crucial to successful emollient therapy. Finding the right emollient that works for a patient can be a lengthy process. An important part of this journey is making sure that they are aware of how to apply the emollient correctly, in terms of technique, quantity and frequency. Incorrect technique can sometimes lead to a patient’s skin becoming more irritated and a worsening of their condition. Under these circumstances they may stop using the emollient believing that the treatment is not working for them when in fact, with correct application it may actually have worked. Pharmacists are in a prime position to help educate patients on correct emollient application – smooth gently onto the skin in the direction of hair growth, liberally and frequently at least 2-4 times a day. Those with very dry skin may require more frequent applications or a greasier emollient. When looking at adherence patient choice and circumstances should also be taken into account, for example, how many patients with very dry skin are going to want to wear a greasy ointment to school or work? Not many! Sometimes, a number of different products of varying greasiness may

be a better option than just one product. Patients should also be reminded to continue using their emollient after their skin improves to prevent further flare-ups. AproDerm® is a complete range of emollients which are free from SLS, parabens, colours and fragrances as well as being cruelty free and suitable for vegetarians. Available on the NHS and to buy the whole range, consisting of creams, a gel and ointment, is suitable from birth and includes AproDerm® Colloidal Oat Cream, a paraffin free emollient which also doubles up as a soap substitute. AproDerm® have an extensive website for both patients and pharmacists containing a variety of downloadable PDFs on eczema, psoriasis and how to apply emollients. Check

out their kids zone, a whole area dedicated to helping children learn about their eczema in a fun way through various games and activities with the help of friendly little characters called the AproDites. Read all about their adventures in their recently launched e-book, a must for all kids with eczema (and their parents). For more information visit www.aproderm.com Eczema is not contagious however sufferers still feel segregated and so, as Pharmacists, you are ideally placed to help dispel the myths, improve outcomes and make sufferers feel more comfortable in their own skin.

The effect of digitalising care Continued from p1 The increase in digital communication will help to drive patients into bricks and mortar pharmacies. The new NHS community pharmacy consultation service will refer patients from NHS 111 direct to a pharmacy. As this service expands to include GP referrals, urgent care centre referrals and other sources this will increase the clinical role of community pharmacies. Further developments based on the ‘refer to pharmacy scheme’ and point of care diagnostics linked to hepatitis B, antibiotic resistance and cardiovascular disease management will increase the flow to some pharmacies. These pharmacies will develop in partnership with neighbouring pharmacies and the GPs within primary care networks to provide a responsive clinical service.

NHS England and NHS Improvement is the national commissioner for NHS community pharmacy services and makes sure the NHS provides safe, high quality patient care and services within primary care while ensuring that the NHS lives within its means. The Pharmacy and Medicines bulletin brings you the latest news, information and resources from NHS England and NHS Improvement, and other partners. The Government appears clear in its wish to drive digitalisation and thereby drive a fundamental differentiation in the future of community pharmacy. One element driven by high volume, robotic dispensing of repeat prescriptions, either direct to consumer or via hub and spoke systems with neighbourhood pharmacies. And a second element of bricks and mortar neighbourhood pharmacies delivering urgent acute prescriptions and more clinical care direct to patients and in association with local GPs within a clinical network and digital referrals.

Learn more about cannabis research and CBD products with GreenLight Medicines at the Pharmacy Show GreenLight Medicines was formed in 2014 with a vision to establish a cutting-edge R&D programme investigating the therapeutic potential of plant-based compounds. Today, this journey has progressed significantly, with GreenLight becoming the largest company in Ireland specialising in cannabinoid research. From its inception, GreenLight has focused its research on biologically active compounds from flowering plants in the family Cannabaceae, genus Cannabis, with a specific focus on a series of related compounds called cannabinoids. These plant compounds (phytocannabinoids) have shown an effect, in a plethora of studies, on the human endocannabinoid system, making them promising therapeutic drugs. GreenLight’s aim is to turn the anecdotal evidence surrounding cannabis into rigorous scientific evidence through a range of basic research on compounds of interest, advancing them through preclinical models, then taking only the most promising candidates to clinical trials. A large number of principal stakeholders, from some 13 internationally recognised research institutes (Ireland, UK

and the US), have been drawn together to form a global research network unlike any other. With the power of science and clinical research, GreenLight Medicines will become a world leader in plant-based medicines. This research will be instrumental in the development of industry leading solutions and therapies.

GreenLight Supplements will be exhibiting at the Pharmacy Show on Stand PG51. Dr. David Gibson, CSO with GreenLight Medicines, will deliver his talk on ‘Cannabinoids and Health – Supplements to Medicines’ on Sunday 6th October, 3.40pm in the Public Health & Wellbeing Theatre.

A sister company to GreenLight Medicines, GreenLight Supplements is committed to the development and supply of high-quality plant-based supplements. With growing demand for plant-based medicine and supplements in the Irish and wider European and UK markets, GreenLight intends to build a strong export market with a view to expanding into these countries towards the end of 2019. GreenLight Supplements will be exhibiting at the Pharmacy Show at Stand PG51. Chief Scientific Officer at GreenLight Medicines, Dr. David Gibson, will also be delivering a talk on ‘Cannabinoids and Health – Supplements to Medicines’. The talk will overview: the endocannabinoid system as a pharmacological target; current CBD supplements; MedCan prescribing regulatory frameworks and available MedCan prescribing recommendations; key messages from GreenLight’s online CPD training course for pharmacists. Through this speaker session, Dr. Gibson hopes to create a clear understanding around the biological basis of phytocannabinoid effects and drug interactions and the current UK supplement and prescribing recommendations, as well as sharing current clinical evidence of cannabinoid efficacy.

Experience the SynMed® Difference in Pharmacy Automation Synergy Medical, an expert manufacturer of automated pharmacy systems, says that pharmacies using its SynMed® technology for high throughput dispensing of solid oral medications are freeing up technical and professional pharmacy staff’s time for more patient facing activities. This is resulting in pharmacies offering health-driven services including completing Medicines Use Reviews (MURs) and public health campaigns. with two operators. This is a fraction of the time it would take to manually fill packs and means that pharmacies can increase productivity and confidently grow their business to take on new patients without compromising safety.

With more patients using monitored dosage systems (MDS), pharmacies are struggling to keep pace. One solution is to use automated dispensing systems, such as SynMed® ULTRA and SynMed® XF to reduce dispensing costs and enhance safety and precision. The combination of embracing technology and redeploying resources is increasing capacity to meet greater health demands. For example, pharmacy branches whose MDS are produced by SynMed® technology state that they now have time to run a range of private services such as diabetic screening, travel vaccination, blood pressure monitoring and weight management clinics.

Unlike many pharmacy automation systems, SynMed® has the capacity to hold 400 to 500 drug containers, covering 98% of the total drug requirement for MDS production. Since pharmacists using the system only need to dispense 2 to 5% of their drugs manually, this reduces labour time and cost, as well as the risk of making dispensing errors. Versatility is another benefit and SynMed® systems can prepare single and multi-dose blister packs using more than 35 pack formats from all the popular manufacturers. Pharmacies have freedom from being locked into a single manufacturer’s proprietary pack, as well as flexibility to adapt to new target markets and negotiate contract prices. Additionally, utilising SynMed® technology, pharmacy staff can calibrate containers on-site within five minutes, eliminating the downtime and expense of sending them to a third-party for calibration.

SynMed® ULTRA and SynMed® XF systems are helping pharmacies boost productivity by using pick and place technology to dispense tablets via suction pipettes. This improves precision and patient safety by avoiding cross contamination, as well as system downtime for staff to clean excess powder inside systems, which regularly occurs with gravity container equipment that dispense drugs from cannisters using a common chute.

Michel Guerra, Vice President, Sales and Business Development Europe at Synergy Medical stated: “The latest National Pharmacy Association’s quarterly update, reports 8.5% dispensing error rates in England1. Some of these may have occurred due to time pressures staff face with filling more MDS and the number of different medications they have to put in them.”

Using a SynMed® XF system, just one technician can accurately dispense solid medication in 240 blister packs every eight-hour shift (over 1000 packs/week) while SynMed® ULTRA, can produce over 3000 blister packs/week

Guerra added: “By visiting us on Stand PD22 at the Pharmacy Show pharmacists can find out how the precision, speed and flexibility of our SynMed® Ultra and SynMed® XF systems could help safely overcome this filling problem and

increase their pharmacy’s capacity to accurately dispense solid dose medication in a large range of MDS blister packs.” Synergy Medical is the manufacturer of SynMed® systems and works directly with pharmacies without third-party involvement, acting rapidly to make system adjustments or improvements when needed. The technology is fully integrated with all major Patient Medication Record (PMR) systems and is fully supported for a minimum of 10 years, offering a longer life cycle than most automated pharmacy technologies. Expanding your Private Vaccination Service Vaccines offer protection throughout our lives from birth to old age Vaccination is considered to be the second most effective intervention on reducing global burden of infectious diseases, the first being clean water. Despite this, in 201718, childhood vaccination coverage declined in England compared to the previous year.

Pharmacy’s growing role in vaccinating the public

MSD attends the Pharmacy Show! MSD are pleased to run two events at the Pharmacy Show to help support pharmacists in delivering vaccination services. Keep an eye on the events calendar for more details! These promotional events have been organised and fully funded by MSD. MSD products may be discussed at these events. Prescribing information will be available at the MSD stand. This is promotional content from MSD.

Commissioning pharmacy to deliver vaccinations makes sense and the sector has demonstrated its significant contribution in the national NHS Flu programme delivery. In 2018/19, 78.2% of all community pharmacies in England administered an average of 158 per pharmacy, (more than 1.4 million doses administered across all pharmacies) according to a report published by NHS Digital. With many pharmacists now able to administer vaccinations, the sector is starting to deliver a much broader range of vaccination services.

The future of Pharmacy in England The NHS Long Term Plan sets out its ambitions for system transformation to ensure the NHS is fit for purpose for the future amongst growing demand. Alongside this, the new Community Pharmacy Contractual Framework 201924 has further underlined pharmacy’s role in prevention. This includes continued commissioning of the National Flu Vaccination Service and also a testbed programme which could look at recommendations from the ongoing review of vaccinations and immunisations and if successful, extend an enhanced role for community pharmacy at a national level.

Private vaccination services are expanding, particularly with pharmacies looking to reduce their reliance on NHS funding. They are getting involved in offering a wide range of vaccinations including travel and wellness vaccines. There is no doubt that community pharmacy makes a real difference protecting the nation’s health.

Decisions, decisions

VISIT US ON STAND PC50

In the first of a four part series on automation in community pharmacy, we start at the beginning of the process, with the decision. Why are community pharmacies powering up the robots? A drive for greater efficiency, a refit, a relocation prompting a fresh look at how to run a pharmacy, or concerns over safety. There are many reasons why the robot pioneers in community pharmacy have opted for automation. automated MDS machine,” she says, “but then I thought if I fix that it doesn’t fix everything. We then looked at trying to deal with the footfall through the door.” But a trip to Lincolnshire with pharmacist Usmaan Hafiz to view an automated collection point in action had an unexpected outcome. “We walked in, and we thought ‘this isn’t a busy pharmacy’. It was calm, people weren’t running around. When I asked how many items they do, he said 25,000.” She indicates the 11m BD Rowa VMax behind her. “Then we walked past one of these.” Videos taken, Jacinta and Usmaan spent the journey home in the car “plotting and planning. We went to look at one more, then we started designing. Once we locked down a design, I took that to my partners, and it was more a case of ‘we are having one of these’, not can I have one of these,” she says.

“If it wasn’t for automation, we wouldn’t be here,” says Amit Pattni, at Sonichem in Leicester. “I had this vision over five years ago. There was a lot of muttering about the future of pharmacy and the shape of the NHS going forward. We had to look to expand our horizons, and get away from the basic dispensing model and look more at services. We needed to make ourselves available for anything else that opened up in the market. “Our problem was that me and my uncle, two pharmacists, were managing all the dispensing with two counter assistants. We hit 10,000 items a month five years ago and it felt like we were in McDonalds not a pharmacy. On Thursday and Friday we’d hardly look up. When the orders came in it took us three working hours to put them away. You cut your day in half doing menial things.” The big idea. “Bring in automation. We knew we couldn’t have a robot in our existing layout because we were both big lads at the time, we had no space. When other pharmacies were thinking of slowing down or stopping, we went the other way. We thought ‘go for it’”. An 8m extension to accommodate a 7.5m robot later, the pair have never looked back. They saved the time they were expecting – their delivery driver now checks off orders and sets the robot’s ProLog autoloader going in 15 minutes. And we’ll find out how they’ve reinvested that time later in the series. For Kaleem Ahmed of A+ Pharmacy in Birmingham, the merger with a neighbouring pharmacy that had some legacy automation prompted a fresh look at the proposition. “We were running a 100-hour pharmacy, and we got the opportunity to buy the normal hours contract over the road. There was already a robot in there, but the feedback from the staff was that the 15 year old model wasn’t very good.” Kaleem knew that he was facing a huge increase in items in the merged business, to be delivered over what was going to be, for him, a shorter time. “We were looking to make things more efficient and because there was already a robot there, I started to think there was some merit in having one. So I started researching it.” He wanted something that was fully automatic, so chose a 3.5m BD Rowa Smart. They started thinking about workflow immediately. “We were moving across the road, so it was easy to set up new systems for that. The staff I took with me saw the robot working from new and were able to learn

“It was the biggest decision of my life, but the decision was the easy bit. It all came down to safety for us. It got to a point where there are only so many things you can do, so many procedures you can put in to keep you feeling safe and less fraught.” Usmaan agrees. The robot, Ezrah, named in a customer competition, arrived in February. There are no picking errors out “Errors are massively down. It gives you confidence of the robot. It’s so much safer. The when you are checking.” Now Kepple Lane is calm too, like the pharmacy they saw almost a year ago. only errors now are human errors “We’ve had to put radio speakers in so patients can’t hear the banter. It’s weird that.” from that experience and work with it from the beginning.” The tricky bit? Needing to manage stock better Of course, at the point of deciding automation is the Efficiency, safety and stock control were the main right decision for the pharmacy, there is the small matter considerations for Albert Wilde in Fleetwood. The manager of finance to consider. There are three basic routes: from of the large health centre pharmacy, Jon Flitcroft says: “We the banks, from third party finance companies and forms basically had a person employed to put the order away, of lease/purchase arrangement, which may be simpler to and then put the splits away. Literally looking after stock organise if the automation provider offers the arrangement, for eight, nine hours a day. We’re a very busy pharmacy. but may be more expensive overall. Supporting market leading self-care in your community In 2018 the NHS issued new guidance for Clinical Commissioning Groups regarding prescribing for conditions considered to be self-limiting or which lend themselves to selfcare and for items which can be routinely purchased over-the-counter (OTC).1 This guidance encourages doctors to reduce prescribing for conditions such as minor pain and fever, mild dry skin, dermatitis and athlete’s foot, warts and verrucae.

Straightforward Skincare

This change in emphasis from doctors prescribing has been driving more and more customers to pharmacy to get your help in managing minor ailments. At RB, we’re committed to giving your team the knowledge and confidence to make the right self-care recommendations. That’s why we’re hosting the Self-Care Zone at The Pharmacy Show. RB is the OTC market leader in several therapeutic categories, with number one brands including Nurofen,* E45† and Scholl‡ in our portfolio. Our series of talks in the Self-Care Zone will use our market leading products to demonstrate what you can implement to stimulate self-care in your own community. We’ll also give you details of the dedicated training and support we can offer your business. *Nielsen unit sales data L4W 15/06/19. †Nielsen RMS ScanTrack Data to WE 18.05.2019. ‡Nielsen MAT February 2019 Total Footcare Value Sales Total Coverage. Visit the RB Self-Care Zone to learn about the major opportunities in key minor ailments: MINOR PAIN AND FEVER Fin Mc Caul Fin’s talk will focus on: • Common misconceptions in OTC pain relief • Recommending the right relief for your customers. Two important criteria when making a pain recommendation are efficacy and safety. For the patient, efficacy is paramount. Taking a new approach to pain recommendations means considering the patient’s priorities first and making efficacy and safety the basis for analgesic selection. Fin Mc Caul is Managing Director of Prestwich Pharmacy, a committee member of the Greater Manchester LPC and the PSNC Regional Representative North West. Fin was awarded fellow of the Royal Pharmaceutical Society in 2016 for his service to pharmacy.

MILD DRY SKIN AND DERMATITIS Rod Tucker Rod’s talk will focus on: • Skincare in the modern world • How to make your pharmacy a specialist destination for skincare. A survey of consumers has found that 81%* of the UK population have experienced at least one skin condition (excluding acne).2 In England alone, consultations for skin problems account for 13.2 million GP visits per annum at a cost of £723 million to the primary care budget.3 Rod Tucker is a pharmacist with over 30 years’ experience and is currently an independent researcher and writer focussing on dermatology. He developed his interest in dermatology while working in prison and established clinics for patients with skin problems and in 2003 won the RPS Pharmaceutical Care award for primary care for his dermatology clinics. Rod was also a member of the group that developed the NICE psoriasis guidelines. *E45 Straight Up Skincare Project Survey of 5,009 UK adults. September 2018.

ATHLETE’S FOOT, WARTS AND VERRUCAE Sunil Kochhar Sunil’s talk will focus on: • Why common foot conditions are a target for self-care • How your pharmacy could benefit from a dedicated footcare service. Focussing on footcare in pharmacy should be a priority for all pharmacies, not only to look after their patients & to be seen as a trusted source of local advice on footcare problems, but to ensure they are getting their share of sales in this important category. Sunil Kochhar is a consultant pharmacist for the online patient information platform, www.dearPHARMACIST.info. He has over 20 years’ hands-on experience of helping local communities improve their health and well-being. Within all his work, Sunil has demonstrated that marketing is an essential part of gaining public engagement to enhance their health and well-being.

The NPA urges us to go ‘forwards in partnership’ The road to success is rarely a road taken alone. That is why the vast majority of independent pharmacies choose to be members of the National Pharmacy Association. For nearly a 100 years, we have brought community pharmacists together, to pool resources, to learn from each other and to move forwards together. In tough times - as now – the sector needs to stick together or it will surely sink together. That’s why Forwards in Partnership has been one of our key themes this year, as you will gather if you attend any of the NPA’s sessions at the Pharmacy Show. On the NPA stand PG40, that theme is explicit, because several of the NPA’s business partners are there with us: • CDSmart – The all-in-one app that saves you time, money & stress •

EMT Healthcare – ‘Big enough to deliver, small enough to care’. Over the past 55 years EMT Healthcare has become a leading independent wholesaler supplying pharmacy and health and beauty retailers with a comprehensive range of products.

• FMD I.T. – a custom built solution created specifically to enable its users to be fully compliant in line with the Falsified Medicines Directive legislation • MedicSpot - Medicspot installs its technology into consulting rooms to convert pharmacies into mini private GP clinics; which gives patients easy access to timely GP consultations, health assessments and blood tests. • TravelJab.co.uk – a unique platform which enables NHS and Private patients to search, compare and book vaccinations across the UK by searching for their nearest or most convenient pharmacy travel clinic. Partnership operates at many levels: from team working in your pharmacy, to providing multidisciplinary care with fellow health care professionals, and the most important clinical partnership of all - partnership with patients – empowering people to get the best use from their medicines. Partnership also extends to working collaboratively with the NHS in pursuit of shared, patient-focused objectives. What does an effective partnership with the NHS - the

sector’s biggest paymaster - actually look like? Firstly it has to be about shared objectives. We all share an interest in developing community pharmacy as the front door to health – an indispensable component of the urgent care pathway, as well as a prevention focused community asset that helps people to stay well as much as it helps people to feel better when they are poorly. It must also be about giving the NHS a return on every NHS Pound spent with us – whilst not being afraid to ask to be paid for that value! Community pharmacists need a return on the investments they make and the risks they take. Which brings us, inevitably, to the new contractual framework in England. We independents must fully embrace the contract’s clinical service agenda, which is our best hope for a change in fortunes from our currently precarious situation and the right thing to do for our patients. Yet many pharmacies, worn down by funding cuts and escalating costs, are not in a position to invest in new services that would greatly benefit patients and the NHS.

achieve the transformational improvements the NHS and pharmacists would like to see. For some pharmacies, it won’t even be enough to keep the doors open, unless other substantial sources of income can be found. To ensure the service levels required for patients, the £2.59bn core funding needs to be a floor not a ceiling. Nevertheless, the journey to a more clinically focused future in community pharmacy is one we independents must, and should wish to take. We have a better chance of getting there if we take the journey together, in partnership.

MEET THE TEAM ON STAND PG40

So let’s be clear that the core funding for England’s pharmacies for the next five years is very unlikely to be enough to

Who wants to talk about fraud? Fraud can be an uncomfortable subject, but talking about it is vital to protect both individuals and organisations. That’s why the NHS Counter Fraud Authority will be at the Pharmacy Show 2019 (stand PE45) and are keen to speak to as many people as possible about their work. Fraud is not always the easiest conversation topic. Individuals who have become a victim of fraud can be ashamed to talk about it, and organisations can be reluctant to acknowledge that fraud is a threat for them. However, talking about fraud is vital. Since fraud is essentially deception carried out for financial gain, it is typically a hidden crime. This means that fraud can prosper on a lack of awareness or a reluctance to acknowledge it. Fraud is also a growing threat for both individuals and organisations in the UK, and the crime you are most likely to experience during your lifetime. At the NHS Counter Fraud Authority (NHSCFA) we are always keen to talk about fraud. Our task is to identify, investigate and prevent fraud and other economic crime, such as bribery or corruption, affecting the NHS and the wider health group. As a special health authority focused entirely on counter fraud work, we are independent from other NHS bodies and directly accountable to the Department of Health and Social Care (DHSC). Understanding the threat How big is the problem in the NHS? According to our latest estimates, based on 2017-18 data, fraud costs the NHS £1.27 billion every year. This is enough to fund over 5,000 new frontline ambulances, or over 40,000 community nurses.

All health systems are exposed to the risk of fraud, and the NHS is no exception. We recently published a new fraud reference guide on our website which details the different types of NHS fraud. A quick look at the guide is enough to realise that a wide range of people commit fraud against the NHS (including a dishonest minority of staff, patients, suppliers and contractors) and that no sector of the health service is immune from the threat. The guide is there to help those who are unsure of what constitutes NHS fraud and want to access information about different types of fraud. Along with the guide, our growing list of fraud prevention resources is designed to further inform NHS organisations and the wider health service of the different risk areas and what measures can be put in place to mitigate those risks. At the NHSCFA we strongly believe that collaboration and engagement with our stakeholders and the public are vital to the success of the fight against fraud. That is another reason why we are eager to speak to professionals and specialists in different sectors, to raise awareness of fraud and better understand the impact it has on their work. Meet us at the Pharmacy Show Wanting to engage with the community pharmacy sector, we are attending the Pharmacy Show 2019. We will be at stand PE45 and hope to speak to pharmacy professionals about what we do and how we want to work with and support the sector in preventing fraud and protecting NHS funds which are meant for patient services. Over the course of the two days, we will be engaging with pharmacy colleagues to talk about how we can work together to fight fraud. NHSCFA colleagues attending the conference will also be doing a survey to improve our understanding of the fraud risks facing NHS community pharmacy. We will also talk to colleagues about our priority areas for action this financial year, which include pharmaceutical contractor fraud along with procurement and commissioning fraud, GP contractor fraud and improving counter fraud outcomes in the NHS. Our priority areas are set each year

We’re with you every step of the way.

based on our latest evaluation of available intelligence on the fraud risks facing the NHS. Fighting fraud together Our aim in attending the conference is to improve our knowledge of the challenges faced by the community pharmacy sector in identifying, preventing and tackling fraud. We also hope to get a better understanding of how all our work – not only on pharmaceutical contractor fraud, but also on other areas such as fraud involving the misuse of prescription forms – affects NHS community pharmacy. The NHSCFA is clear that the vast majority of pharmacists and other pharmacy professionals providing services to their communities are honest. As in all sectors, it is only a minority who commit fraud, but their actions have a significant impact on NHS resources. We want to work with the pharmacy sector and the wider NHS to ensure that strong measures are in place, for example in relation to payment processes, to reduce the risk of community pharmacy being exploited by fraudsters. However, no matter how strong preventative measures are within an organisation or system, fraudsters will sometimes succeed in targeting it. What should you do if you have any suspicions or concerns about fraud against the NHS? There are a couple of ways you can report fraud. One is to contact NHS England or your local clinical commissioning group – they have their own counter fraud function with a Local Counter Fraud Specialist (LCFS) who can look into your concerns and provide counter fraud advice. Another way is to call our national, 24-hour fraud and corruption reporting line on 0800 028 4060 (operated by Crimestoppers) or make an online report to us at cfa.nhs. uk/reportfraud. All reports are handled in confidence and you have the option to report anonymously.

We hope to see you at the Pharmacy Show 2019, and hope you’ll be up for a chat about fraud – make sure you stop by stand PE45!
