2016 Annual Repor t

We Put Patients First

We Work as One Team

We Focus on Quality

We Act with Integrity

2016 ANNUAL REPORT Letter from President/CEO and Board Chair: As we look back upon our accomplishments in 2016, we think first and foremost about the countless supporters and partners who helped to advance our mission. Through your support, Charles B. Wang Community Health Center has earned a reputation for providing high quality primary health care services and programs to help patients achieve their highest level of health. In 2016, we provided medical, dental, mental health, vision, social work assistance, health education, and care coordination services to more than 52,000 patients.

Jane T. Eng, Esq. President & CEO

Our care team strives to provide compassionate and effective care to all patients regardless of their ability to pay, the language they speak, or their country of origin. We are very proud that in 2016, we were once again recognized by the Bureau of Primary Health Care as a National Quality Leader for exceeding national clinical benchmarks, including Healthy People 2020, for preventive care, chronic disease management, and prenatal and perinatal care. Less than 5% of nearly 1,400 community health centers in the United States received this award.

Victor Lee Board Chair

Thanks to you, our community is becoming healthier and our future is becoming stronger. Sincerely yours,

Jane T. Eng, Esq.

Victor Lee

I really appreciate my primary care doctor Dr. Douglas Chen. He always answers my questions carefully and helps me improve my health physically and mentally. I also appreciate the patience and sincerity of Irene Yuan so I was able to continue my medication.

This is the only organization I know of willing to work with your current economic situation, what other place allows you to bring in your taxes and adjusts your payments and fees accordingly? My mom came in for a breast mammogram and physical, was only charged $20-$30 for the physical and was given the breast mammogram for FREE. - Mrs. B, patient

Dr. Cheng Tou was so friendly and answered all of my questions clearly. She didn’t run and took my questions seriously. I never had this experience in my entire life. Also the staff are fantastic. They are friendly and there’s a comfortable atmosphere here. I highly recommend her if you are looking for a good female GYN doctor. - Mrs. W, patient

Pediatrics 86,635 patient visits

“

I have been going to pediatrics since the third grade and now I am 21 and I still come here. I choose to come here not only because the nurses and doctors tend to my patient needs, I appreciate the quality time spent on me. Dr. Soohoo, you rock.

Dr. Loretta Au has been my daughter’s doctor for many years. She has great medical skills and cares for my daughter’s physical and social development. On top of that, Dr. Au referred my daughter to the social worker who helped me a lot. I would like to express my appreciation to Dr. Au.

“

19,264 patient visits On behalf of my father this review is for the work of art that was delivered to him as a patient. Dr. Valerie Lo had granted him with a pair of dentures that had given him his second opportunity to bite and smile. It was a well-fitted pair of dentures for my father. He was indeed very happy to see teeth in his mouth. Words may not express how thrilled he was. Thank you Dr. Lo for the work of art that you had delivered.

Mental Health

“

14,001 patient visits I only have good things to say about the clinic. Drs. Jianping Chen and Teddy Chen are patient, understanding, and knowledgeable, and when I was in a poor state Teddy constantly called me to make sure I was okay, and he said I could call him any time if I need. They also reached out to a clinic in Boston, where I go to school, and tried to arrange care for me there. This type of care is rare in general, especially in Chinatown. - Mr. M, patient

Nutrition Counseling

“

Thank you to Mrs. Yung Hua Ursula for teaching me how to eat right. I never met a nutritionist like her. She is the best. I am also happy with the receptionist at the 5th and 2nd floor. They are nice, polite and helpful, and especially my Doctor Wu. He is the best doctor.

Social Work Assistance

I am writing to express my appreciation to social worker Josephine Tong for her assistance in communicating with the education department regarding special education services for my daughter. Her polite attitude, compassion and diligence really touched me. Ms. Tong coordinated with schoolteachers, the principal, and pediatric specialists to learn about my daughter’s condition. Ms. Tong’s assistance means a lot to me, especially since I have language barriers and a busy schedule at work. - Mrs. H, patient

Health Insurance Enrollment

“

Many thanks to the Social Work Department for all the help. My social workers have devoted much time and energy to connecting me with financial assistance, insurance benefits and community resources. They helped me so much with money, medication, and nutrition so that I can live until today. I will remember this for the rest of my life. - Mr. L, patient

Pharmacy Benefits

“

My doctor told me that I needed to start medication to treat hepatitis B. I did not have insurance and could not afford the medication due to its high price. My doctor referred me to Hep B Care Program, through which I was informed that I could apply for the Patient Assistance Program to get medication for free. The staff in your health center are really taking very good care of me, and my hepatitis B is getting better controlled now. - Mrs. T, patient

I live in Flushing. Years ago, I came here once and felt that the doctors and staff here treat their patients like family. Ever since, I have been traveling from Flushing to the clinic for my check-ups each year. Dr. Gong is very friendly, careful and caring. Last year, Dr. Gong invited another doctor and they both explained my special medical conditions in detail to me. The staff here also helps me make medical appointments, which removes the language barrier. I really appreciate all the help. -Mr. P, patient
