Members are invited and encouraged to contribute to the CSPD Bulletin. Articles of general or specific interest, Letters to the Editor, and commentary on issues of public policy and matters of pediatric oral health are welcome. Items intended for publication may be submitted to Managing Editor Dr. Chanel McCreedy at chanel.mccreedy@gmail.com or to admin@cspd.org

The mission of the Bulletin of the California Society of Pediatric Dentistry is to communicate with members on issues affecting the specialty of Pediatric Dentistry and the oral health of infants, children, and adolescents. The Bulletin will provide information on Society activities, initiatives, governance, and member services, as well as information and notice of the accomplishments and actions of its members.

The views and opinions expressed by individual members in articles and commentaries are those of the author and do not necessarily represent policy or viewpoint of the California Society of Pediatric Dentistry.

PRESIDENT'S MESSAGE

WE’RE GOING UP, UP, UP...

“It’s our moment / You know together we’re glowin’ / Gonna be, gonna be Golden (EJAE)“ This year on Halloween, you may have seen many kids sporting purple and pink hairdos dressed up as their favorite KPOP Demon Hunter character. This record-breaking movie has dominated music and streaming charts for the past few months. A noteworthy scene at the end - spoiler alert - shows the heroes triumphant in defeating the demons only when the collective voices and beliefs of the community join in to help them. It illustrates the timeless message that individual efforts alone cannot enact change without support from all. The strength of the CSPD lies in our membership and whether it is through member voices, skills, time or money, every single contribution towards the common good helps. Thank you for your continued membership and support of our organization and mission. Here’s what the board has been working on this quarter to advance the practice of pediatric dentistry, patient safety, and advocacy.

Looking UP - planning ahead

CSPD is increasing our effectiveness by investing in technology and legislative expertise. We will move to a new digital home next year and plan to unveil the platform at the 2026 Annual meeting in San Diego! Stay tuned for updates and messages to prepare you for a new level of engagement and information. The board voted unanimously at a virtual meeting in October to approve an increase in the operational budget for implementation and transfer of CSPD’s member database and Online Continuing Education to NOVI, a best-in-class membership management system that will serve as a hub for other integrations.

Additionally, we have spent several months researching and deliberating on fortifying advocacy on behalf of our members and the patients we serve. The board approved that the CSPD utilize the services of a lobby firm for the upcoming year to enhance the voice of pediatric dentistry in California. The lobby will work in concert with our PPA to ensure continued alignment with the CDA and elevate issues relating to the practice of pediatric dentistry. Members will not experience a dues increase to retain professional representation throughout the state and partner organizations.

It is difficult to look up and see the big picture when our lives are exponentially more complex and busy. Yet we actually gain more control by looking beyond our immediate needs to learn about things occurring on a broader level. Because if we are not planning for the future, we will be left behind following a path that others have planned for us.

Speaking UP - giving perspective

We celebrated alongside the AAPD with some recent wins at the ADA House of Delegates meeting in Washington DC, from October 25-28. Our board personally reached out to colleagues who were delegates representing California, wrote statements to the ADA, and stood in solidarity with the AAPD to provide in-person testimony at the hearing. The well-orchestrated grassroots effort to mobilize members in opposition of Resolution 407 resulted in pediatric dentistry being heard on a national stage. The ADA reference committee amended the resolution to include verbiage emphasizing collaboration with pediatric specialists, the AAPD, and American Academy of Pediatrics to develop the ADA guideline for pediatric sedation.

The 2 upcoming legislative issues in 2026 that we will keep members closely informed about are:

• A clean-up bill to amend SB501- Anesthesia and sedation law

• Renewed interest from mid-level provider advocates to bring dental therapists to CA

These issues have the ability to cause substantial disruption in the delivery of care and we will need to hear your voices to amplify the pediatric dental perspective. Now is the time to call and write to the board to make sure your questions are answered and opinions are heard!

Stepping UP - taking action

Our CSPD - AAP liaisons have been busy working to reinforce the importance of maintaining the medical-dental connection in health care. Chapter 1 liaison and past president, Jonathon Lee helped our CE committee put on a CSPD/AAP Nutrition webinar series in November featuring Dr. Roger Lucas, author of “More chocolate, no cavities” and Dr. Sushma Chamarti, a pediatrician who spoke about obesity. Similar efforts by Dr. WaiYin Chan in San Diego are materializing to partner with AAP Chapter 3 for a Wellness event. We challenge pediatric dentistry to step up and join AAP today to collaborate with pediatricians within your communities and represent our profession at large. Join the Section on Oral Health in your local chapter to strengthen integrated care.

The upcoming federal cuts to Medi-cal will bear long-term impacts to our most vulnerable populations. California ranks among the worst states for pediatric dental disease due in large part to low utilization and lack of providers accepting Denti-cal. We are investigating partnerships and studying models of successful implementation initiatives to rally fellow pediatric dentists to step-up and sign on as providers to face this moment head on and help alleviate the pain and disease progression that will occur.

Yes, you can

I’ve had the privilege of hearing CDA president, Dr. Max Martinez, report on the success and activities of the largest state dental association in the nation. In his inspiring speeches, I can feel his spirit of humility and service. His parents would often tell him that “Sí se puede!” YES, YOU CAN. His conviction is balanced with kindness and reminds me of how similar our organizations are. CSPD is the largest state chapter of the AAPD and kids’ oral health is at the core of what we do. In this season of giving and reflection, make sure to include pediatric dentistry in your todo lists and ask, “What is it that I CAN do - today and tomorrow for our profession?” We are celebrating CSPD’s Golden anniversary this year and with your unwavering support, will undoubtedly keep going up, up, up for another 50 years.

DR. JACOB LEE - Q & A

Dr. Jacob Lee, Current AAPD Vice President, CSPD Past President (2017-2018) Interviewed by: Dr. Pardis Farhadian, CSPD Secretary

1. What year did you join CSPD?

“A long, long, time ago, I can’t remember…”

2. What has been your favorite aspect of your membership?

When I attend CSPD functions, I enjoy broadening my circle of friends and connecting with and making new acquaintances. When I meet and listen to colleagues, I always discover something refreshing from their life journeys. When discussing certain issues, I learn perspectives I may not have considered. Such colorful and interesting moments enlighten and add new dimensions to my life.

3. What initially drew you to pediatric dentistry, and what keeps you passionate about working with children?

Graduating from University of Toronto Faculty of Dentistry, I was selected as a dental intern at the Hospital for Sick Children in Toronto. It was a Level 1 pediatric trauma center. As I saw and treated dental trauma at all hours, I immediately became fascinated with that field; thus, I decided to become a pediatric dentist. To this day, whenever I see or hear about a dental trauma, my heart skips a beat with curiosity.

During my life as an academician, I became enthralled with dental materials. I became obsessed with finding the best material that is easy to use, reliable, long lasting, and for a reasonable cost. As with medications, not all materials are suited for all patients. In my practice, I enjoy the process of matching what material would meet the needs for that particular patient. It’s like selecting a certain lure in fishing or reaching for that specific club for an optimal shot in golf. It’s problem solving of a different kind.

4. Can you describe a particularly memorable or rewarding moment that illustrates what you love most as a member of CSPD?

In 2018, I was given an opportunity to produce a video tribute to the past presidents of CSPD & WSPD. At the time there were 43 CSPD past presidents and 11 WSPD past presidents. I researched and reached out to them. I even chased down a few at the dental meetings to take their current photograph. While studying how they successfully steered through difficulties they encountered, I had a renewed respect and gratitude for their time in office. For example, each CSPD president was trained in the School of Titans to inspire and unify our members to overcome the challenges in caring for children. I am truly grateful to be part of CSPD, as well as for having served as the 43rd CSPD president (2017-2018).

5. Which cartoon character would make the best dental assistant staff and why?

I can’t think of a singular cartoon character, but I often overhear my office manager reminding staff, especially the front desk staff, “Be nice!” These are the words used by the late Patrick Swayze in the movie Road House when teaching bouncers how to manage patrons kindly while also setting firm boundaries. We can do what we do thanks to the intelligence of our staff and their innate skills in interacting with parents, along with their unique ability to connect with pediatric patients. But don’t let Pikachu-like innocence and sweet appearances fool you!

Pediatric dentists have two speeds when treating children: fast and very fast! When treating a very difficult patient, I swear I thought my assistant had 5 or 6 hands to help me complete the treatment.

Our staff is, indeed, the juggernaut of our office!

2017-2018 CSPD Board during presidential term of Dr Jacob Lee

REMEMBERING DR. MEL ROWAN, DDS

August 7, 1934 – November 6, 2025

A giant in our profession, a great mentor and friend. I shall miss him! We shall miss him!

I met Mel as I became actively involved in CSPD leadership in the early 2000’s as a South Director to the Board of Directors. I had little experience in governance in a prestigious organization such as ours.

Unimaginable now, but at the time when a report was due I submitted my report in a typed format via “snail mail”! I received a call from Mel. Oh, oh I thought! “Santos”, he said in his deliberate and direct manner. “This is not going to work. I have many reports to collate for the board packet and a written report on paper is just not going to cut it.” That was Mel, to the point. Direct, but not making me feel badly with his straight message. I understood clearly not only about what he needed me to do but that he would guide me and many others to higher performance under his tutelage. I learned so much about governance, how to run a committee or board meeting and the nuances of dental politics. ..and the game of golf!

Mel graduated from Long Beach Polytechnic High School and was matriculated and was a member of the golf team at Stanford University before being admitted to the DDS program at the University of Southern California. He established practice in pediatric dentistry in the city of San Pedro and became an active member of the Harbor Dental Society. He loved his chosen profession and dedicated decades of volunteer leadership to enhance it. He was one of the first chairmen of the then-newly founded, The Dentists Insurance Company (TDIC), and we know what a gem on the crown of CDA that has become.

For complete article visit the CSPD website.

I am one of many hundreds of people who had the great pleasure of knowing Mel Rowan in his long and productive life during his career in Pediatric Dentistry, as a devoted father, loving husband, friend and mentor. Although my friendship with Mel was limited to the last three decades of his life, I did experience what everyone who knew of him from childhood: a wonderful friend and beautiful human being. His sense of humor and wit were readily apparent to everyone he met as was his love for the game of golf and his adventurous nature, which prompted him to travel the world in retirement with his wife Linda.

Between the mid 1950’s and late 1960’s, the specialty of Pediatric Dentistry was developed and received recognition by the American Dental Association as one of eight specialty organizations in dentistry. In California, this movement was championed by the likes of Mel Rowan, Weyland Lum and David Good, and other colleagues, who in 1975 incorporated an organization then called “The California Society of Pediatric Dentists” (CSPD). The stated purpose of this infant organization was to represent the interests of its’ members in their role in developing the specialty within California. During Mel’s role as CSPD Executive Director the CSPD was renamed “The California Society of Pediatric Dentistry” in recognition of the change in focus of an organization whose primary goal it was to advocate for the oral health and welfare of the pediatric population we serve rather than the welfare and benefit of the member dentists.

Mel also served on the Board of Directors of CSPD in its formative years and as CSPD President prior to becoming the second CSPD Executive Director at his retirement from private practice (following Tom Barber) and served in the position for over a decade. As a result of these critical roles, he will join the pantheon of those who will be remembered as the “fathers” of the specialty of Pediatric Dentistry in California.

It was during his tenure as CSPD Executive Director, that I, like so many other of my colleagues, had the privilege and good fortune to get to know Mel as a close friend and as one who would be considered to be one of the most significant personalities in our lives. It was under Mel’s tutelage and mentorship as I navigated my roles as CSPD president and then as AAPD Trustee.

STRENGTHENING PEDIATRIC DENTISTRY IN CALIFORNIA: ADVOCACY, ACCESS, AND ACTION

Dr. Evelyne Vu-Tien, DDS, MBA-Public Policy Advocate

CSPD has continued to promote children’s access to care and support for pediatric dental professionals by working collaboratively with the California Dental Association (CDA), American Academy of Pediatric Dentistry (AAPD), and the California Partnership for Oral Health (CPOH). CSPD was at the table to discuss ways to enhance Kindergarten Oral Health Assessment (KOHA) training, promote fluoride education, expand workforce development, and improve sedation safety. Legislative achievements, sponsored by CDA, included the passage of SB 386 (virtual credit card reform) and SB 351 (protection of clinical autonomy). CSPD remains committed to ensuring equitable, safe, and high-quality oral health care for all children and empowering the success of pediatric dentists.

SCHOOL-BASED DENTAL PROGRAMS AND EXPANSION OF PREVENTIVE MEASURES

With the collaboration of former CSPD President and CDA Foundation Board Member, Dr. Santos Cortez, Jr,

and Dr. MyLinh Ngo, CSPD advocated for statewide and national clinical guidelines for school-based dental clinics to ensure a high standard of care, including access to emergency services. Within California, over 5.4 million children are enrolled in Denti-Cal; yet only 44.9% of those enrolled utilize services.¹,² AAPD was asked to consider outlining clinical guidelines for other states to follow, to help ensure that children establish a dental home with continuity of care and transparency regarding their child’s dental services, along with viable referral sources.

Preventive efforts are underway with Assembly Bill 350, which extends fluoride varnish coverage up to age 21, and remains active in the Senate. CSPD also consulted with Dr. Kristin Hoeft (UCSF/Director of the Community Oral Health Technical Assistance Center) and CDA leadership to enhance the statewide Kindergarten Oral Health Assessment (KOHA) training by incorporating a pediatric dental perspective.

CONTINUED JOINT ADVOCACY

CSPD participated in the bi-monthly California Partnership for Oral Health, led by Ms. Mary McCune, J.D., Executive CDA Director for Policy Analysis and Community Outreach, and Emergency Department/Dental Ad hoc Meetings and CA Dental Board Meetings with Ms. Tooka Zokaie, MPH, MAS, CPH, CDA Senior Health Policy Analyst. Fluoride advocacy, addressing dental staff shortages, and navigating an increasing number of dental emergencies in the emergency department were discussed.

Community Health Worker (CHW) Benefit

The approval of California’s State Plan Amendment allowing Medi-Cal reimbursement for Community Health Worker services marks a significant milestone. UCLA and California Northstate University (CNU) are leading the CHW certification efforts. These CHW programs will extend preventive dental care education and patient navigation services to families in underserved regions.

Fluoride Advocacy

CAPOH’s Community Water Fluoridation Workgroup is currently exploring an Early Notification Law that would require public notice before local water systems terminate fluoridation. CSPD supports focusing on education and community awareness campaigns for fluoridation.

Emergency Department Physician, CDA, and CSPD Collaboration

Communication between medicine and dentistry is fostered with collaboration between Dr. Alicia MikolaycikGonzalez, the Board Member and Public Policy Specialist for the California division of the American College of Emergency Physicians, and Ms. Tooka Zokaie and CSPD. Plans for an upcoming joint article and webinar on dental emergencies are in progress.

DENTAL WORKFORCE DEVELOPMENT

CSPD is actively addressing statewide shortages in the dental workforce, particularly among Registered Dental Assistants (RDAs) and hygienists. At the Dental Assisting Stakeholder Meeting, CSPD advocated for affordable pathways to ensure dental assisting certification through alternative routes, aiming to maintain a high level of training and safety for dental assistants while supporting in-office dental assistant and private dental assistant training programs. A California Dental Board-sanctioned virtual infection-control course is in process, enabling rural dental team members with transportation barriers to obtain certification and fulfill mandated training requirements.

LEGISLATIVE AND REGULATORY ADVOCACY

CSPD remains involved in policy discussions that shape the delivery of pediatric dental care. Regarding the sedation bill, SB 501, a stakeholder interdisciplinary group meeting will be held in the future and will include representation from CSPD. A discussion will ensue regarding the accessibility of licensure, the processes for obtaining minimal and moderate sedation licenses, and the regulatory challenges associated with them. Future discussions will include evaluating the pertinence of PALS certification for dental assistants during sedations, considering that PEARS certification is more relevant and emphasizes airway management.

Dr. MyLinh Ngo represented CSPD at the American Dental Association House of Delegates. ADA Resolution 407 regarding pediatric sedation was passed. Through collaborative efforts among local, state, and national advocacy efforts, the caucus recognized the importance of preserving the AAP/AAPD partnership on pediatric safety guidelines.

Recent Legislative Wins by CDA Co-Sponsored Bills

CSPD celebrated two key legislative victories sponsored by the CDA, SB 386 (Virtual Credit Card Reform) and SB 351 (Clinical Autonomy Protection).

SB 386 ends dentists’ automatic enrollment in VCC payments, which can account for up to 10% of plan payments. Effective April 1, 2026, the law ensures that dentists can opt in or out of virtual credit card payments and requires full disclosure of all fees.3

SB 351 also passed, restricting corporate influence on patient care decision-making in medicine and dentistry. Effective January 1, 2026, it prohibits private equity and corporate entities from dictating patient quotas, procedures, or clinical decisions.4

Through unified advocacy, CSPD ensures that every child in California has access to compassionate, high-quality, and safe dental care while empowering and protecting our profession. We are grateful for our incredible advocates and partners, who are crucial to the future of our children and specialty.

WSPD PRESIDENT REPORT

My name is Harlyn Susarla, and I am honored to serve as President of the Western Society of Pediatric Dentistry (WSPD). Born and raised in the greater Los Angeles area, my curiosity and sense of adventure led me to the East Coast, where I earned degrees from Wellesley College, Boston University (MPH), and the Harvard School of Dental Medicine, followed by a pediatric dental residency at the University of Maryland School of Dentistry (UMSOD). The West Coast has always been home, and I now live in Seattle with my husband and two daughters, practicing at Seattle Children’s Hospital, where I care for children with complex medical, developmental, or behavioral differences, as well as those requiring emergency dental care.

My journey in organized dentistry began in residency, when a mentor brought me to a Maryland Academy of Pediatric Dentistry board meeting and I realized I could have a meaningful impact. Attending the Pediatric Oral Health Advocacy Conference soon strengthened my commitment to equitable care for every child and to advocacy on behalf of my colleagues. After serving in several leadership roles in Washington State, I joined the WSPD Board as the Washington representative, which further deepened my involvement. I also serve as a Trustee of the AAPD Foundation, President-Elect of the College of Diplomates of the American Board of Pediatric Dentistry, and participate in multiple AAPD councils and committees.

Two years ago, WSPD finalized a strategic plan to enhance communication, expand member services, and strengthen advocacy and resident support. Since our founding in 2005, WSPD dues have remained at $40, which is minimal compared to other districts. To sustain the initiatives set forth in our strategic plan, a modest increase to $60 will take effect next year.

For California members, this increase allows WSPD to maintain its highest-level sponsorship of the Annual CSPD Meeting and continue supporting Resident Poster Awards. For smaller states within our district, dues revenue allows us to support advocacy and leadership development, ensuring equitable benefits across the district. By collaborating and sharing experiences across our district, we strengthen our profession and better serve our patients—an effort I am proud to advance as WSPD president this year.

At our Fall Board Meeting in October, we welcomed Leadership Intern Sandy Huang. In fact, of the five interns selected since the program’s inception, three now serve as state representatives— Aarika Anderson (WA), John Warner (OR), and Nadiene Wu (HI)—a testament to the program’s success in cultivating future leaders. The Board also approved funding for five district

residents to attend POHAC Advocacy Days, a cornerstone of resident engagement and professional growth.

I am deeply grateful to the WSPD Board and the Washington State Academy of Pediatric Dentistry for their support, and to CSPD for modeling the strong leadership and organizational excellence that guide our work. As WSPD President, I will continue collaborating with CSPD to strengthen our shared efforts while fostering leadership, inclusion, and a strong voice for future generations. I would also like to thank Immediate Past President Dr. Rob McAlpine (Alaska) for his leadership and mentorship, and our amazing Executive Director, Dr. Christine Roloafs. I look forward to supporting President-Elect Dr. Tom Tanbonliong, whose long-standing service to CSPD many of you will remember.

This year, I challenged the WSPD Board with the guiding theme: “How can I help?” I now ask each of you to reflect on the same question. Whether you are a new resident or a seasoned practitioner, your involvement strengthens our profession and benefits the children and families we serve. Together, we can make meaningful progress—not just for our profession, but for the children and families we serve.

Thank you for your dedication. I look forward to all we will accomplish, side by side.

On Friday October 24th, 2025, in Washington DC, Dr. Christine Roalofs (WSPD Executive Director and its past president) was inducted as a Fellow of the American College of Dentists, with Dr. Jacob Lee as the first sponsor and Dr. David Okawachi as the second sponsor.

Dr. Roalofs was nominated for her many years of dedicated service in organized dentistry, demonstrating integrity, high professional ideals, ethical conduct, always placing the needs of the organization above hers. She thus embodies the mission of the American College of Dentists to advance excellence, ethics, professionalism, and leadership in oral healthcare.

Congratulations Dr. Roalofs!

AAPD FOUNDATION EARMARKS $4.75 MILLION FOR 2026 GRANTS

Dr. Donald Schmitt - AAPD Western District Trustee (2024-2027)

The AAPD Foundation Board of Trustees met to discuss how to help counteract the upcoming changes to federal Medicaid funding. In response, the Board approved allocating $4.75 million in new grants for the 2026 fiscal year ($4 million earmarked for nonprofit clinics and dental schools facing revenue losses due to Medicaid changes and other federal cuts). “Unfortunately, these anticipated losses are not a matter of if, but when,” said AAPD Foundation President Dr. Mario Ramos. “And, with federal funding comprising on average two-thirds of nonprofit clinics’ annual revenue, the consequences of these changes are brutal: millions of powerless children will suffer.”

Your financial support for the Foundation is needed now more than ever. Please Join the Fight to Protect All Little Smiles and Become a Healthy Smile Hero by donating to the Foundation today by using the QR code or going to aapdfoundation.org/donate.

AAPD Ad Interim Board Meeting

The Board met in September in Edinburgh, Scotland in conjunction with the Smiles Across the Globe meeting. Some of the actions of the board are summarized below:

1. Approved that the AAPD adopt AAP’s 2025 immunization schedule for inclusion in the 2026 AAPD Reference Manual, in place of the 2025 CDC-ACIP recommendations.

2. Reaffirmed the AAP/AAPD joint guidelines on sedation and anesthesia with updated references, and to allow flexibilities as needed from P&P to proceed with the joint publication with AAP.

3. Approved the Early Career Pediatric Dentistry Committee to host an in-person meeting in September 2026 in conjunction with the biennial AAPD Chapter Leadership Summit.

4. Approved new/revised AAPD State Chapter Logos.

5. Approved submitting a 15% increase in AAPD member dues across all member categories to the General Assembly for final approval.

6. Motion was Not Approved to support the AAOMS request for the Oral Health Milestones 1, 7, and 15 Campaign for up to $60,000 per year for three years.

7. Approved AAPD Headquarters providing members with information on ADA Resolution 407 via e-media.

AAPD Leadership Update

As most of you are likely aware, our beloved CEO for the past 26 years, Dr. John Rutkauskas is taking well deserved retirement in June of 2026. A search committee completed a very thorough process to determine the best candidates to be AAPD’s next CEO. The AAPD Board met in August for the final interviews and chose a new CEO. Negotiations with that individual are ongoing and an announcement is expected in January.

AAPD Annual Session Las Vegas

Registration will open in December for AAPD’s first ever session in Las Vegas, May 21-24, 2026 at the Ceasars Forum Convention Center. I look forward to seeing you there!

WELCOME LOMA LINDA RESIDENTS

Dr. Daria Farkhondeh was born in the Netherlands and raised in Northern California. She earned her Bachelor of Science in Genetics & Genomics from UC Davis, and graduated with her DMD degree from Boston University. Her passion for pediatric dentistry stems from her love of working with children and her desire to make dental visits a positive and empowering experience. She is especially interested in early childhood prevention, patient education, and the oral-systemic connection. In her free time, Dr. Farkhondeh enjoys traveling,

Dr. Isabella "Bella" Idea was born in Puyallup, Washington and raised in Camarillo, CA.
