BREAST CANCER – A GENERAL SURGEON’S PERSPECTIVE
By Brett Howard, M.D.Nearly seven years ago, I returned to Tallahassee after five years of surgical training. When I began my practice, it was easy to reflect on some of the major medical centers I had been exposed to and identify the need for a comprehensive breast center in our region. I realized we already had many of the pieces necessary to achieve this goal: an advanced radiology practice, experienced surgeons, specialized pathology, enthusiastic nursing and therapy staff, and highly trained medical and radiation oncologists. The initial and most important piece, of course, is our supportive local community led by the Walker family. Sharon Walker died in 2005. She inspired the Tallahassee community as an active volunteer and advocate for women with breast cancer. To honor her memory, Sharon’s husband, Claude, and friends, family members, and supporters, established the Walker Breast Center at Tallahassee Memorial HealthCare (TMH). This dedicated cancer treatment center strives for a progressive approach to individualized care.
It has become standard practice to provide a fullservice clinic that offers the latest in preventive care, support, and cancer education/information for breast cancer patients. In this single location, patients are provided with comprehensive services from a cohesive team of doctors and other experts who specialize in breast cancer. These centers have proven to shorten the time from diagnosis to treatment, improve outcomes, and result in better patient satisfaction.
Why not Tallahassee? Why not the Big Bend region? Our community expands across many counties with a near total inclusive population of 1 million. Now that the incidence of breast cancer has risen to 125 cases per 100,000, we must be prepared to care for over 1,000 new cases of breast cancer annually. This expectation is what has motivated us to build our region's first state-of-the-art comprehensive breast center.

effective treatment to beat their cancer. A patient leaves this clinic with an individualized plan, as well as a support system that not only monitors, but also treats the disease. Our patient nurse navigator Amy Anderson and support staff provide a family atmosphere during consultation and treatment throughout a patient's journey– from the diagnosis to the ringing of the bell.
I must explain here that attending and participating in these weekly clinics has expanded my knowledge and sparked an interest in the data and science of breast cancer. As a surgeon I understand how important it is to work with a team; how physicians and caregivers can learn from each other and find answers through collaboration. There is still so much we can learn from our expert faculty who are constantly studying and analyzing this new data. The context of these sessions allows for differences in opinions, which is so vital. The discussions that result from the back and forth of ideas results in the most comprehensive assessment and analysis for each case. Our patients will be confident and secure in knowing that all options have been carefully considered before being presented to them.
We strive to continue our center's expansion and hope to include a high-risk screening clinic with the expectation of participating in clinical trials and research. Furthermore, this team approach gives us the power to bring a community together by breaking many of the stigmas, as well as financial barriers that have unfortunately led to delays in treatment. Early detection and expeditious treatment are crucial to better outcomes.
Therefore, as I write this article, I want it to be a Thank You! Thank you to the medical community and all the specialists who have made this possible. Thank you to Dr. Shelby Blank, our medical director, whose determination and leadership has brought this project to its fruition. Thank you to TMH for the opportunity to help develop this program. Most importantly, thank you to the Walker family. Everyone’s tireless efforts have laid the foundation of this research-based cancer center. The advanced screening and diagnostic imaging, cutting-edge surgical care, including localization technology, will be led by this region's first fellowship trained breast surgical oncologist, Dr. Shlermine Everidge. This is a program that can and will grow with our community and make the difference in so many lives. It’s important to note that while we are all proud of what has been accomplished thus far, we know that there is more hard work ahead of us. We now stand ready with the treatment and support our patients need and deserve. And the best news is that we can all look forward to seeing improved outcomes and enhanced quality of life for our patients. Go Pink!

On September 23, Dr. Tracey Hellgren was inducted into the FSU Medical Alumni Hall of Fame as a member of its 2023 class during a ceremony at their fall reunion in Orlando. Dr. Hellgren was among four alumni chosen to be honored this year and joined 31 previous inductees. The purpose of the FSU Medical Alumni Hall of Fame is to recognize graduates of the College of Medicine and Program in Medical Sciences for their outstanding contributions to the health of all people through patient care, medical education, research, and/or health services administration and the demonstration of the high principles of the medical profession.
“Dr. Hellgren served as a preceptor for students in the COM since its inception and joined our faculty full-time as one of the anchor physicians at our FSU PrimaryHealth faculty practice,” said Alma Littles, M.D., Interim Dean of the FSU College of Medicine. “Serving as medical director of that practice, she has overseen its growth since it opened in May 2019 including helping ensure that patient care continued through the COVID-19 pandemic. She continues to teach students as she provides patient-centered care to her own patients, most of whom have been with her for decades. Having participated in her training as a resident in the TMH Family Medicine Residency Program, I was especially proud as dean of the COM to present this well-deserved recognition to her.”

MIDLIFE
By Michelle Miller, M.D.No, I’m not buying a Porsche, having an affair, or filing for a divorce. Though thinking about it, Porsches have been replaced by Teslas now. I, too, am interested in looking into electric cars when my husband's 2013 Ford finally breaks down. When I suspected I was pregnant, my 2017 Prius was also a large purchase. Thankfully, I paid that in full without financing.
Looking back at the first half of my life, on the horizon of the next half (or more) of life, I feel like I am at a halfway point, i.e. “midlife,” but don't feel alarmed or as though I am in an existential crisis. I reflect with contentment, while trying to come up with some future goals. When mentioning being “midlife” to my husband, he immediately dismissed it, saying off hand that we were too young. However, we are both approaching 40 quickly!
Some may have considered my midlife crisis when I moved to New Zealand for a year in 2021 with my family. However, personally, I consider it fateful. It was an alignment of the stars. With COVID-19, my child being preschool age, and having paid off both my mortgage and student loans debt, my family was able to take the pay cut so I could be a locums GP for a year in New Zealand. This was an experience my husband still looks back at fondly more than a year after our return to the States. It was an adventure that helped me to recharge and recover. At the time, I had begun immunosuppressive therapy for my multiple sclerosis; due to finding a small lesion in my spine after having neuritis that I thought was shingles without the rash (Zoster Sine Herpete). I had called a medical school alumni, one that now specializes in multiple sclerosis, and his comment, “your spine has very limited space,” was my catalyst to make the change in my treatment plans.
Most of you may not realize but I had made it a priority to pay off all debt as quickly as possible, while still traveling and concurrently working full time as a primary care physician prior to this trip. I had just applied for both disability and life insurance prior to having my first symptomatic attack of multiple sclerosis. This episode and that diagnosis resulted in denial of disability insurance. Realistically I knew I could have progressive disability and thus my family would not have a reliable source of income, so my goal was to become as financially independent as soon as possible while still enjoying life.
With the news of my diagnosis, my husband and I had discussed that we didn't want to wait until retirement to travel, but wanted to do so economically in the present. In the spring after my diagnosis, my husband and I planned international cruises going trans-Atlantic to Barcelona with a stop in London on the way home. We quickly followed it the next year with a cruise through the Suez Canal to Venice with a stop in Istanbul on the way home. We had planned to cruise the next year around Australia and New Zealand but had to cancel due to becoming pregnant, hence another reason to take the locums GP job in NZ when the opportunity arose.
As for my current level of debt, my family is still living in our first home that my husband and I had found while in residency, prior to physician mortgages. At the time, I had a high debt to income ratio and didn't qualify for much. It is a modest 1,200 square foot, 3 bedroom, 2 bath in a lovely neighborhood near Tom Brown Park that we have no plans to move away from yet. The only current debt is the financing for my solar panels. This being another goal I decided to finally complete after returning from NZ. Now I plan to pay it off when I finish my three months locums primary care assignment in November locally.
I am looking forward to this new phase in my life. For the past year, I have been working part time from home with telemedicine urgent care companies treating patients from Florida and now Michigan. I added the additional state as the demand has slowed due to an influx of providers in the tele-med market. Further needs slowed more this spring and summer, hence the locums primary care assignment. I will likely add more state licenses and continue with it after the locums is finished. I suspect I will not have the energy to return to full time work. Presently, in my new position, I will be .8 FTE during the three months, taking Wednesdays off to recharge. In all honesty, I am dreading how it will affect my energy levels, though I may be pleasantly surprised.
Goals looking forward include continued travel, spending more time with family and friends, and potentially, writing more seriously. I love reading and the fictional worlds it has the potential to open up. I have given myself a reading challenge this year for 100 books and have already completed 70 books. I also enjoy writing articles for Cap Scan and would recommend others to give it a try. It is fun and personally rewarding. I have found research supports reading fictional books and narrative writing to mitigate burn out or moral injuries in physicians.

The Capital Medical Society Foundation’s community of donors strategically help fulfill its mission “to support the charitable efforts of physicians and others, increase access to healthcare, promote education and serve the community’s health needs through innovative projects that are exemplary, affordable and dignified.”
➢ Access to healthcare is increased through gifts to the We Care Network and the We Care Network Endowment Fund. More than $129 million in donated specialty medical and dental care has been provided to low-income, uninsured patients since the program was created in 1992. In fiscal year 2022-23, which ended June 30, 2023, more than $5 million in healthcare was donated. Gifts support the operational expenses of the program including salaries and benefits of the case management team who coordinate the donated care.
➢ To provide incentives to keep medical students trained at the FSU College of Medicine in the Big Bend after completion of their residencies and help meet the growing demand for physicians in Florida, gifts to the CMS Foundation Scholarship Fund provide scholarships to FSU College of Medicine students who have expressed an interest in returning to the Big Bend to practice after completion of their residencies. $61,000 was awarded in 2023. An additional $1,000 was awarded from The Anesthesiology Associates of Tallahassee Scholarship Fund. The Julia Revell St. Petery, M.D. Scholarship in Pediatrics and The J. Orson Smith, M.D. Scholarship in Internal Medicine were also awarded in 2023.
➢ Gifts to the Capital Medical Society Foundation help provide operational resources for the Capital Medical Society Foundation. In addition, modest grants are awarded to area non-profits whose programs improve access to healthcare for the underserved. In 2023, the CMSF awarded a total of $14,000 in grants to the Big Bend Hospice Foundation for bereavement services, Elder Care Services for in-home services to seniors, Lighthouse of the Big Bend to provide services to the visually impaired, and to Lutheran Social Services of North Florida, Inc. for the KidSafe program.
