INTRODUCTION
Mansion House Care Home, a small care facility with 15 residents, implemented a holistic patient monitoring application ‘Cyril’ to improve the quality of life and overall health of its residents. The application combined physical health tracking, emotional wellbeing assessments, social engagement, and cognitive health monitoring. This case study explores the methods used, challenges faced, and the benefits achieved
APPROACH
Each room was equipped with Tele Health devices to monitor heart rate, blood pressure, oxygen levels, and IOT devices to monitor mobility This allowed caregivers to quickly respond to any sudden changes in physical health
Caregivers adopted frequent check-ins to assess mood, mental state, and stress levels, using a structured electronic questionnaire and observational tools
We recognised the importance of social connection; we monitored each resident’s engagement in social activities and interactions We implemented a schedule of social events and encouraged residents to participate, tracking attendance and the effects of social interactions on residents’ well-being
Cognitive assessments were conducted regularly, using a combination of puzzles, memory exercises, and tasks designed to gauge mental acuity and cognitive stability Data was collected from these activities and shared with families and used to adjust individual care plans
OUTCOMES & BENEFITS
Early detection of health changes, allowed for faster intervention and reduced emergency incidents by 25%
Weekly mental health check-ins reduced depression & anxiety symptoms by approximately 30% among residents. Families felt reassured by the transparency of monitoring. Holistic monitoring provided a comprehensive view of each resident, enabling caregivers to personalise care and focus on both physical and psychological needs.
