TREATMENT & THERAPEUTIC INTERVENTIONS - PHASE APPROACH

Phase Approach: Intensive In-Home Services utilizes a phase approach to provide structured support for families and help them progress towards less intensive interventions.

Phase I

Initial Assessment and Stabilization

The team begins working with the family to assess their strengths and needs.

Crisis planning and stabilization of the living environment are prioritized.

Initial skills training is introduced

This phase typically lasts at least four weeks and requires 4 in-person sessions per week

Phase II

Intensive Intervention

The team continues assessing needs and providing tools to strengthen family skills

Therapeutic counseling and interventions are implemented. The focus is on empowering the family through education and skill-building.

Treatment components may include individual and family counseling, crisis intervention, and life skills training.

Phase III Transitions and Step Down

The team encourages and supports the youth and family as they practice newly learned skills.

There is a focus on building social and community support

The family is assisted in transitioning to less intensive services and natural supports

The intensity and frequency of contacts typically decrease to 2 contacts per week

Throughout these phases, the Intensive In-Home Services team utilizes a multi-systemic approach, tailoring interventions to meet the individual needs of each youth and family.

Progression through the phases is not always linear and should be based upon clinical assessment of the family’s needs.
