Program Overview

Program Overview

The Facility-Based Crisis (FBC) program provides short-term, intensive crisis stabilization for youth ages 6-17 experiencing mental health crises, intellectual/developmental disabilities, or substance use disorders. The program operates 24/7, 365 days a year, with a capacity to serve up to 16 children. The average length of stay is between 7-14 days.

LicensedClinicians:Provideclinicalassessments,developindividualized treatmentplans,offertherapeuticinterventions,andleaddischargeplanning efforts

RegisteredNurses(RNs):Monitorthehealthandsafetyofallclients,manage medicationadministration,andcoordinatemedicalcare,includingemergency services.

LeadBehavioralHealthCounselors:Coordinatedirectcareactivities, superviseBehavioralHealthCounselors,andensureadherencetosafetyand interventionprotocols.

BehavioralHealthCounselors:Providedirectcaretoclients,monitorsafety, andimplementtherapeuticinterventionsunderthesupervisionofthe ProgramManagerandLeadBehavioralHealthCounselors.

Qualified/AssociateProfessionals(QP/AP):Supporttheprogramby handlingintake,maintainingmedicalcharts,assistingwithdocumentation, andensuringthatadministrativetasksanddailyoperationsrunsmoothly

Keypoints:

IncidentReporting

Reportingprocess:

Notifysupervisor/QPwithin4hours.

Completeanincidentreportassoonaspossible. Supervisor/QPtakesappropriateaction.

Incidentsclassifiedasminor(I),moderate(II),orserious(III). Criticalincidents(IIorIII)requireadditionalreportingintheNCIRIS system.

Additionalnotificationsmayberequiredforspecificincidents,such asmedicationerrorsorassaultallegations.

Documentation

FacilityBasedCrisisprogramprovidesdailyservicenotesandmaintainsa logofallreferrals,includingtheyouth'sname,dateandtimeofreferraland admission,andrationaleforanyadmissiondenials.

ProgressNotes:CompletedinEchoVantageforeachshift,describing behaviors,interventions,andprogress.
