Editor’s Note

Although in many parts of Australia it is chilly and cold, there are lots of activities available for you to warm up and engage with the pain community this winter. There are opportunities available for networking, career advancement, and translation into practice for members to warm up with in this edition of the APS newsletter.

Ranging from audio visual volunteers needed for the one thing, to putting in your SPC scholar position applications, and call outs to be part of the painSTAR inaugural event in SA 14-18th Nov 21. There is also an opportunity to gain a real-life view of the ICD-11 chronic pain classification by testing the algorithm developed by researchers from Philipps University Marburg, Germany.

If these are not enough to inspire or motivate you all, save the date, as next year the APS 42nd annual meeting will be held 10-13th April at the Hotel Grand Chancellor in Hobart, Tasmania.

There are two upcoming webinars to put in your calendar diaries - first up on the 27th July is Prof Allan Basbaum who will be presenting on imaging cortical activity during a general anesthetic and providing preclinical insights into cortical pain processing. Pain and anhedonia is the topic of Prof Siri Leknes on the 31st of August.

Plus, our latest publication in the European Journal of Pain by an APS member is on the mediating effect of pain catastrophizing on pain intensity and the influence of the timing of assessments by Rodrigo Rizzo and colleagues. To clarify the causal role of pain catastrophizing on pain intensity, they tested whether the mediating effect of pain catastrophizing on pain intensity depended on the time that pain catastrophizing was assessed.

Lots of activities to get involved in and keep you warm and toasty this winter edition. Keep sharing with us your latest publications and reviews.

The mediating effect of pain catastrophizing on pain intensity: The influence of the timing of assessments

Thank you to APS members Rodrigo Rizzo, Aidan Cashin and James McAuley and their colleagues Hopin Lee, Leonardo Costa and Sylvia Gustin for sharing the following recent publication.

Article first published online: 28 May 2021

DOI: 10.1002/ejp.1810

Link: https://doi.org/10.1002/ejp.1810

ABSTRACT

Background

Pain catastrophizing underpins several psychosocial theories of pain, but there is limited evidence to support the proposal that changes in pain catastrophizing cause changes in pain. Results from mediation analyses have conflicting results, and one reason for these might be the timing of the assessment of pain catastrophizing. This study aimed to test the effect of the timing of pain catastrophizing on pain intensity.

Methods

Causal mediation analysis using data from a randomized controlled trial which included 100 participants with chronic low back pain. The trial found that clinical hypnosis, compared to pain education, reduced worst pain intensity and pain catastrophizing. In model 1, we used data from 2-week follow-up for pain catastrophizing and 3-month follow-up for pain. In model 2, we used data from 3-month follow-up for both pain catastrophizing and pain.

Results

The intervention had a significant average total effect on pain (-1.34 points, 95% CI -2.50 to -0.13). The average causal mediation effect was significant when pain catastrophizing and pain were measured at the same time (-0.62 points, 95% CI -1.30 to -0.11), but not significant when pain catastrophizing and pain intensity were measured at different times (-0.10 points, 95% CI -0.42 to 0.09).

Conclusions

The timing of the assessment influenced the mediating role of pain catastrophizing on pain intensity. These results raise questions on the casual role that pain catastrophizing has on pain intensity. Psychosocial interventions such as clinical hypnosis can reduce pain intensity even when there has been no change in pain catastrophizing.

Declaration

The authors have no conflicts of interest to declare.

APS Members have Discount Rate to Pain Management Workshop through the Australian Physiotherapy Association

In 2019, the Australian Physiotherapy Association developed a foundational pain workshop called Pain Level 1. The course presents the key concepts of pain and pain management, using the IASP core curricula as its basis.

Key areas discussed in the course are Concepts of Pain, Epidemiology, Mechanisms and Contributors to pain, and Assessment and Management approaches to pain.

The course can be accessed in 2 different forms, either as a face-to-face workshop or as a hybrid online/webinar series. Both options provide for a strong “workshop” delivery model, where attendees discuss the relevant concepts in a group situation and develop evidence-based clinical skills for the assessment and management of pain.

Whilst the course was developed for the APA, its construction around the foundational concepts of pain always allowed for a wide range of possible professions to attend.

Get a real-life view of the ICD-11 chronic pain classification and help with this study!

An IASP Task Force created a classification of chronic pain for ICD-11. To help pain specialists use the new classification, researchers from Philipps University Marburg, Germany, have developed a decision algorithm guiding through the criteria. The Principal Investigator is Dr Antonia Barke and study coordinator is Ginea Hay, email: ginea.hay@uni-marburg.de

In this important validation study, pain specialists are invited to be the first to test the algorithm. Using the online-platform participants will be able to interact with virtual patients and describe which ICD-11 diagnoses they have. This allows participants to see the new ICD-11 diagnoses at work while testing the algorithm. For each participant, the researchers will donate a tree!

Important facts in brief

1. Duration of the study: approx. 60 - 80 minutes (pausing the study is possible)

2. Certificate of participation if desired

3. As an independent university, monetary compensation cannot be offered for participation; however, the university will donate a tree for each completed participation

4. Participation requirements: Good English language skills, experience with chronic pain patients

5. Anonymous data collection
