Estate Information Checklist Legacy and Estate Planning is an important part of the 8 Wealth Management Issues embedded within the Invest With A Plan process. Putting in place your key documents helps to make sure that what you want to happen after you pass away, actually happens. But, how do people know what documents you have and where they are? Would they know where your bank accounts, insurance policies, etc. are? Would they know who your key advisors are? This is the purpose of assembling an Estate Information Document. This document should be updated on a regular basis and put in the hands of the people who will have a role to play in your estate (e.g., your executor, successor trustee, etc.). This Guide is intended to give you a checklist of the important information that should be included: Important People/Roles Identify anyone named as an executor or trustee Name and contact information of your financial advisor Name and contact information of your estate attorney Name and contact information of your tax professional Your name(s), birthdate and social security number Organizations Name, address and online login for any firm where you have investment assets Name, address and online login of any employer/co where you have retirement plan assets Name, address and online login of any banking institution where you hold accounts Name, address and online login of any insurance companies where you have a policy Investments and Insurance List of investment and bank accounts; for each account, list: o Owner and Type of account (e.g., Joint, Individual Name, Trust, etc.) o Firm/Organization where the account is held o Account Number List of all insurance policies that are in force; for each policy, list: o Insured and Type of policy (e.g., 20-year term, Individual Name, etc.) o Insurance carrier that issued the policy o Policy Number o Date the policy was issued o Face amount of the policy Digital Assets Identify master password to password helper, if used Identify passcode or PIN used to unlock your computer Other Information List all key estate documents such as wills, powers of attorney, medical directives, etc. and the date they were most recently executed or updated. List the location of original documents and any copies. Identify any safe deposit box(es) you maintain along with the branch/location and the location of the keys to the box(es). This is not intended to be an exhaustive list of the information to include; you might have other assets to document such as real estate holdings, collectibles, etc. The idea is to take inventory and document your assets and relationships so your executor and/or trustee will know where to start. Once completed, give a copy to all primary and successor executors/trustees and review the document annually for any updates and changes. And finally, provide a copy to your financial advisor so it is available when needed.
