It’s hard to believe that 75 years ago, on March 13, 1948, the American Academy of Neurology was incorporated in Minneapolis, MN, by a mere 50 neurologists that included “the Four Horsemen”: Abraham B. Baker, MD, FAAN; Adolph L. Sahs, MD, FAAN; Francis M. Forster, MD, FAAN; and Russell DeJong, MD, FAAN. By contrast, today the Academy is a global community of 40,000 neurologists and neurology professionals hailing from every corner of the world and representing every interest and subspecialty. The story of the Academy’s wonderous 75-year history is—like many great organizations— one that reflects a commitment to embracing continued growth and change in order to respond to members’ unique needs. Indeed, the AAN, like neurology itself, has evolved over a rich and fascinating past, and in 2023 we took the opportunity to recognize and celebrate its significant growth and accomplishments.

As you’ll see in this Annual Report, in 2023 the AAN saw overall growth in membership, especially among international members, advanced practice providers, business administrators, and researchers. We also saw success in our advocacy efforts at the federal level, including new regulations aimed at reducing the burden of prior authorization, the extension of telehealth flexibilities through 2024, and a new add-on code that significantly increases reimbursement for E/M services associated with providing ongoing care for people with complex conditions—like many of our patients. Our support for critical research also remained strong as we continued our long-standing partnership with the American Brain Foundation in its pursuit to promote and invest in research across the spectrum of brain disease.

Along with the biographies of each AAN president, significant accomplishments during their terms, and revelatory interviews with key leaders of the AAN and the field of neurology, the page hosts a monthly series of AANnews ® articles that were run in 2023 that explored the circumstances behind the founding of the AAN and how it has supported its growing membership through the years in many areas. You’ll also find fascinating photographic archives, letters, and reports dating back to as early as the Academy’s founding, and newsletters dating back to 1970 that we’re certain you’ll find interesting.

With the severity of the COVID-19 pandemic ever further in the rearview mirror in 2023, members exhibited even greater engagement with our world-class conference opportunities. Both the 2023 Annual Meeting and the 2023 Fall Conference saw record attendance, with 15,347 making their way to Boston or online this past April, and a combined 1,000+ convening in Las Vegas and online for October’s Fall Conference. Even A.B. Baker and his colleagues who planned the first Annual Meeting in 1949 in French Lick, IN, knew that socializing and networking was as important as it should be fun. 2023 Annual Meeting attendees experienced no shortage of either, with a

weeklong array of fun-filled opportunities designed to celebrate our 75-year milestone, including the fabulous 75th Anniversary Celebration at the Museum of Science in Boston.

Inclusion is the reason the AAN was founded back in 1948—quite literally, to be an organization that is the home for all neurologists. Indeed, it is inclusion that makes us stronger, and why our values reflect our commitment to intentional action to be a fully inclusive, deliberately diverse, and anti-racist organization that respects and values our membership, our staff, and the communities we serve. To this end, we increased our diversity, equity, and inclusion efforts over the past year by elevating our Diversity, Equity, and Inclusion Subcommittee to a committee, with the chair now serving as an ex officio member on the AAN Board of Directors.

Of course, one can’t honor and celebrate the past without embracing the future—something the AAN has always done. And your Academy continues to seek new and innovative ways to best deliver ever greater value to you, our members. One way we are doing this is by making investments in technology and making improvements that are based on hearing directly from you about your expectations and preferences, identifying gaps in our offerings, and ensuring we continue to meet your needs. These exciting investments are anchored to the AAN’s new education vision, which calls for content that is easy to find, easy to use, and tailored to the user. In 2023 we lived up to our promise of unveiling a new online learning platform on AAN.com that is easier to navigate with improved search functionality, enhanced video capabilities, and the ability to start a course on one device and pick up right where you left off when you go to another device. This is just the beginning as we double down and prioritize being a digital-first organization to position us for the future demands of the ever-evolving world of neurologic health care.

We are also looking toward the future with a much wider lens. In 2023, we released our new position statement on and vision for national brain health by 2050 that includes improving brain health at every stage of life, from a person’s earliest years of development to their oldest years. As champions of brain health, neurologists will be leading efforts with a broad range of other providers to work toward this vision, with our AAN Brain Health Action Plan calling for research, education, public policy, and direct-to-public messaging to achieve this future of brain health for everyone.

As you read through this report, you’ll also see how your Academy continues to be a financially solid organization poised to move us forward with our recently unveiled five-year strategic plan.

So, as we look back in this report on our many successes in 2023, let us simultaneously look forward with great excitement to how our ongoing efforts will continue to position us to better support our truly diverse, global neurology community and take us into the next 75 years and beyond.

CELEBRATING 75 YEARS AS THE HOME FOR ALL NEUROLOGISTS

From its incorporation on March 13, 1948, with only 50 neurologists to today’s global neurology community of 40,000+ members representing every interest and subspecialty, the story of the Academy’s 75-year history of growth, change, inclusion, and accomplishments was enthusiastically celebrated throughout 2023 with numerous events and opportunities. CONTINUED GROWTH: BY THE NUMBERS

Membership continued to strengthen in 2023, reflecting another encouraging year with total membership growth of approximately 3%. We saw another consecutive year of nearly 95% retention of US dues-paying neurologists, and growth across both US neurologist and APP members. We saw a second year of growth in our Researcher membership. We also had another record year for academic membership in terms of recruitment and retention with our students and members-in-training. Another year of growth in international members means we are holding steady with nearly one in four members being international.

INTERNATIONAL PROGRAMS

There are no borders for neurologic disease. The AAN continues to collaborate with global and international neurology organizations to build relationships and contribute to the exchange of knowledge for the benefit of the field.

• The AAN hosted 35 international scholars from 24 countries around the world at the 2023 Annual Meeting in Boston.

• The AAN sponsored Jeremy Tanner, MD, AAN delegate at the 14th Annual EAN Regional Teaching Course in sub-Saharan Africa.

MEMBERSHIP

The AAN celebrated 75 years of service to neurology. The AAN was incorporated on March 13, 1948. We celebrated our 75th anniversary throughout the year, including a grand celebration in Boston at the AAN Annual Meeting and launching the AAN Digital Archive Collection, a growing collection of historical correspondence, photos, publications, and images of artifacts.

DIVERSITY, EQUITY, AND INCLUSION

• The IDEAS Subcommittee was elevated to the DEI Committee and the chair now serves as an ex officio member on the AAN Board of Directors.

• AAN DEI grants supported projects, events, and activities that fostered DEI within academic neurology departments or the communities they serve, including:

• Two DEI Innovator Grants funded new, untested, and innovative approaches to fostering DEI.

• Two DEI Project Grants supported those who have experience and are already working in the DEI space within their academic neurology departments.

• One DEI Changemaker Award honored a member who made a meaningful contribution within their academic institution by increasing diversity, equity, and inclusion efforts within the neurology department.

WELLNESS

The Wellness Hub at the 2023 Annual Meeting in Boston provided a welcome opportunity for attendees to take part in 35+ rejuvenating sessions during the busy week and empower them to fight burnout and find joy in their careers.

CAREER CENTER

• Java, Juice & Jobs, a new on-site career fair, debuted at the 75th AAN Annual Meeting in Boston with 137 attendees exploring opportunities with 30 employers and enjoying interactive stations, prize giveaways, and a free breakfast.

SECTIONS AND SYNAPSE

AAN Sections and Synapse SM Member Communities continue to grow and provide opportunities for member engagement. 17 AAN sections collaborated to host 11 Section Spotlight Sessions at the 2023 Annual Meeting, providing all meeting attendees the opportunity to participate. Synapse continues to grow, hosting vibrant conversations for more than 75% of AAN members who belong to one or more of the 60 member communities. The Synapse mobile app, introduced in 2022, saw significant growth in 2023.

• The AAN launched a new learning management system to provide members with improved content curation, usability, and accessibility and a learning portfolio that is flexible, extensive, and customized to a user’s personal pace.

• A new NeuroSAE: Medical Student Edition launched that is designed for the medical student neurology clerk.

• The Neurology Question of the Day app received a major system update making for smoother login and a more stable user experience.

Research plays a pivotal role in advancing our understanding of neurological disorders. Scientific abstracts, awards, and scholarships are all indispensable in fueling this progress and ultimately contribute to the collective effort in pushing the boundaries of neuroscientific knowledge.

The Neurology ® journal saw several significant enhancements:

• Moved to a new, streamlined system for submitting research manuscripts and decreasing turnaround times.

• Migrated to a new web platform that embraces enhanced digital features and search, increasing accessibility and discoverability of article content.

• Shifted to continuous publishing across all journals, decreasing final publication of an accepted article by six weeks.

• Continuum ® named Lyell K. Jones, Jr., MD, FAAN, as new editor-inchief, succeeding Editor-in-Chief Steven L. Lewis, MD, FAAN, who completed his 10-year term in December 2022.

The AAN continued to focus on resources that help members enhance the efficiency and daily practice operations while also providing guidelines and quality measurements to empower members in delivering optimal care to their patients.

AMYOTROPHIC LATERAL SCLEROSIS GUIDANCE

• The Quality Measures Subcommittee updated the ALS measures set—a critical tool in encouraging an improved standard of patient care.

• The Guidelines Subcommittee initiated two new guidelines initiatives: Management of Functional Seizures and Care of ALS Patients.

PRACTICE MANAGEMENT

• The Telehealth Subcommittee released a series of bitesize webinars aimed at improving confidence to perform a high-quality neurologic exam via telehealth, highlighting possibilities, limitations, and common misconceptions about telehealth for general neurologic evaluation, as well as for specialty exams such as pediatric headache.

“Determination of Brain Death/Death by Neurological Criteria” was published. This comprehensive update to a critical need in the health care space was the result of coordination efforts among three additional medical societies in an effort to guarantee broader acceptability and inspire greater confidence in the process.

The Care Delivery Subcommittee provided analysis and recommendations for AAN members on CMS’ new Guiding an Improved Dementia Experience (GUIDE) Model

• Three articles were published on Axon Registry ® data: 1) “Examining the National Representativeness of the Axon Registry, A Neurology-Specific Patient Registry,” in Neurology; 2) “Measuring Ambulatory Racial and Ethnic Neurologic Disparities With the Axon Registry” in Neurology: Clinical Practice; and 3) 2022 Axon Registry Annual Registry on AAN.com.

• The AAN is a designated Qualified Clinical Data Registry (QCDR) for Merit-based Incentive Payment System (MIPS) reporter from the Centers for Medicare & Medicaid Services (CMS).

We remained fully committed to advocacy at the federal level to ensure that the needs of neurologists and the field of neurology were represented and we saw many of our priorities successfully implemented by decision-makers.

NEUROLOGY ON THE HILL

The 21st Neurology on the Hill was held March 5–7 in Washington, DC, with 184 participants from 45 states meeting with 278 congressional offices to discuss reform to the Medicare physician payment system; overly burdensome barriers to care; and support for research and care within National Institutes of Health (NIH), BRAIN Initiative, and Veterans Affairs Neurology Centers of Excellence in fiscal year 2024.

WORKFORCE

Conrad 30: The AAN leads a coalition of organizations in support of the Conrad State 30 and Physician Access Reauthorization Act (S.665/H.R. 4942). The AAN has taken the lead on hosting coalition meetings to keep organizations abreast of developments in this space, coordinating coalition group meetings with Senate and House offices to urge them to cosponsor the bill, and creating resources to facilitate support and increase visibility.

PRIOR AUTHORIZATION

Heeding the AAN’s feedback, CMS published new regulations aimed at reducing prior authorization burden. The AAN submitted robust comments, leading to the finalization of requirements for Medicare Advantage plans and hopes that the agency will finalize policies impacting electronic prior authorization shortly.

TELEHEALTH

Many critical flexibilities that were in place during the COVID-19 Public Health Emergency were extended through December 31, 2024. Key extensions include reimbursement for services temporarily added to the Medicare Telehealth Services List; reimbursement for telehealth furnished anywhere in the country, including the beneficiary’s home; coverage of audio-only telehealth; and a continued ability to prescribe controlled substances via telehealth.

E/M REIMBURSEMENT

CMS implemented a new add-on code that significantly increases reimbursement for evaluation and management (E/M) services associated with providing ongoing longitudinal care for patients with complex conditions. This new code is a key longstanding element of the AAN’s efforts to ensure fair reimbursement for neurologic care.

CAPITOL HILL

On July 18, AAN President Elect Natalia S. Rost, MD, MPH, FAAN, FAHA, testified on behalf of the AAN before the House of Representatives Energy and Commerce Health Subcommittee at a hearing titled “Innovation Saves Lives: Evaluating Medicare Coverage Pathways for Innovative Drugs, Medical Devices, and Technology.” Rost discussed the AAN’s work regarding access to monoclonal antibodies directed against amyloid for the treatment of Alzheimer’s disease.

2023 saw us emerge further from the pandemic and it was with much joy that we saw members engaging with each other at AAN hosted events while continuing to collaborate with public and private stakeholders in shaping the future of brain health.

BRAIN HEALTH SUMMIT

The AAN Brain Health Summit held on September 21, 2023, in Washington, DC, brought together over 150 leaders in brain health science, policy, and education. During the summit, the AAN and American Heart Association (AHA) announced a new joint research program, the Ralph L. Sacco Scholarships in Brain Health, also known as the Sacco Scholars program. The program was made possible by a generous bequest to the AAN and AHA from the late Ralph L. Sacco, MD, MS, FAAN, FAHA, who was the only neurologist who has served as president of both the AAN and the AHA.

SUMMER CONFERENCE

Emergency and Hospital Neurology was the focus of the 2023 Summer Conference, which was held July 28–29 in Minneapolis. Of the 505 registrants who attended either in person or online, 96% rated the conference good or excellent.
