SHOULDER PAIN! COULD IT BE THE ROTATOR CUFF? Shoulder pain has no limits for who it affects! From young adolescents and adults competing in high school, collegiate, and professional athletics to those retirees living out their golden years, shoulder pain will affect every individual to some extent. Often times, the exact cause may not be blatantly obvious, whereas other episodes clearly delineate the exact event that may have precipitated the pain. It is important to realize that there is a multitude of possibilities that may have provoked the symptoms such as degeneration, impingement, overload, or a combination of these elements. It is imperative that a thorough physical examination is performed by a physician to evaluate for potential shoulder injuries that may involve structures such as the rotator cuff. It is important to identify the purpose of the rotator cuff so that a better comprehension may be attained in identifying this as a potential cause of shoulder pain. Essentially, the rotator cuff is a group of four muscles used to stabilize the shoulder joint. These muscles also function in allowing the arm and forearm to move away from, and towards the body in a very characteristic manner. Once one or all of these muscles, including the tendons, have been compromised, the stability of the shoulder joint, along with its kinetics will be impaired. It has been theorized that tension overload contributes significantly to rotator cuff injuries which has been evident in athletes who consistently engage in throwing motion activities. Repetitive movements, as seen with athletes, may contribute to weakness of the rotator cuff muscles which increases the

instability of the shoulder joint. This may result in increased laxity of the stabilizing muscles resulting in impingement which may eventually evolve into a partial or complete tear. Rotator cuff injuries may be asymptomatic, but could also present with generalized shoulder pain over the outer most region of the shoulder along with weakness. It is imperative that a full physical examination is performed by a physician if the symptoms persist, or significantly interfere with daily activity. Three classical physical examination techniques may be performed by a physician that may identify whether or not the rotator cuff is involved, and if radiological evaluation is necessary. The presence of a painful arc sign, drop arm sign, and weakness with external rotation are clinical findings that may indicate the need for further evaluation by a physician. Individuals who have a positive painful arc sign, drop arm sign, and weakness with external rotation may require radiologic evaluation with x-rays and/or MRI. Plain x-rays may be utilized to identify large, chronic rotator cuff tears, as well as mal-positioning of the shoulder joint. which mainly consists of physical therapy and glucocorticoid (steroid) injections. Physical therapy has three main goals which focus on shoulder mobility, increasing muscle strength, and overall shoulder function. Glucocorticoid injections can be used to treat severe pain, but are only effective for short-term pain relief. Non-steroidal antiinflammatory drugs and acetaminophen may be just as affective as injections and should be considered as initial treatment prior to the use of glucocorticoids. If surgery is necessary, patients must remain in a brace for six weeks to relieve stress on the repair, and to prevent any further damage. Three months post surgery patients may return to light activities that do not involve above the shoulder action, and by six months may engage in more strenuous activities. Shoulder pain can be caused by a number of different possibilities, it is important to be seen by a physician for a full evaluation. More often then not, conservative treatment is sufficient to relieve the symptoms, but if the pain persists it may be necessary to perform a more extensive evaluation. Pain involving the shoulder should not be ignored; receiving a thorough examination by your primary care physician will result in appropriate identification and treatment.

Earnhardt Fed Up With Talladega

Dale Earnhardt Jr. has won five races at Talladega Superspeedway. His father won 10. When a NASCAR driver named Earnhardt says "enough is enough," he deserves an airing of his grievances. The Good Sam Roadside Assistance 500 ended with the track strewn with as many burnt-out vehicles as tanks left smoldering at El Alamein. This wasn't a battle, though. It only resembled one. Everyone walked away. In the fourth turn of the final lap -- which was one too many, owing to overtime -- 25 cars crashed, making Matt Kenseth's victory elementary. That crash left Earnhardt 20th in the race and, more importantly, 11th among the 12 in the Chase for the Sprint Cup. He is 51 points behind Brad Keselowski, and since Talladega will host no other races this fine autumn, the odds of an Earnhardt championship are as long

• On Oct. 31, 1950, 21-year-old Earl Lloyd becomes the first black man to play in an NBA game when he takes the court in the season opener for the Washington Capitols. The Capitols had picked him in the ninth round of the draft. After seven games with the Capitols, Lloyd was drafted into the military and sent to Korea for two years.

• On Oct. 30, 1893, the World's Columbian Exposition closes in Chicago. Fairgoers were offered a chance to see the first gas-powered motorcar, an alternating-current power plant, a 46-foot-long cannon, a 1,500-pound Venus de Milo made of chocolate, and Juicy Fruit gum. • On Nov. 4, 1922, British archaeologist Howard Carter and his workmen discover a stone sarcophagus containing three coffins nested within each other. Inside the final coffin, which was made of solid gold, was the mummy of the boy-king Tutankhamen, preserved for more than 3,000 years. • On Nov. 3, 1930, the Detroit-Windsor Tunnel between the United States and Canada is officially opened to auto traffic. Each end of the tunnel had a 100foot-tall ventilation tower; each tower held 12 huge fans, six for pumping fresh air into the tunnel and six for exhaust. The tunnel's ventilation system still works just as well today as it did 80 years ago. • On Oct. 29, 1948, a killer smog continues to hover over Donora, Pa. The town's steel mills and a zinc smelting plant had released excessive amounts of sulfuric acid, carbon monoxide and other pollutants into the atmosphere.
