Many families prepare for the start of a new school year by focusing on what children need to begin classes: a new backpack, new clothes and numerous supplies.

While these are all important, what cannot be overlooked during this busy time are vaccinations.

Some of history’s most serious illnesses, such as polio and smallpox, have been eradicated through immunization, and we’ve significantly reduced the prevalence of other serious and potentially deadly conditions.

Influenza, or “flu,” is another example of the tremendous benefits of vaccination. Each year, seasonal influenza results in more than 36,000 deaths and Gaurov Dayal, M.D. 200,000 hospitalizations. While hand washing and staying home can help prevent the spread of flu, an annual vaccination is the single best defense against this potentially serious illness.

In Maryland, children are required to have four doses of a vaccination called DTaP prior to kindergarten.

It protects children against three serious illnesses: diphtheria, which can lead to breathing problems, paralysis and death; pertussis (whooping cough), which can lead to pneumonia, seizures and brain damage; and tetanus (lockjaw), which prevents victims from opening their mouths or swallowing and can lead to death.

Despite the demonstrated benefits of immunizations, each year thousands of Americans forgo vaccinations. While some question ingredients in certain vaccinations and the common practice of giving children multiple vaccinations simultaneously, the safety of these vaccines has been demonstrated over several decades. The risk of not vaccinating your children significantly outweighs the minimal risk that they will have a rare side effect from vaccination.

Those diagnosed with type 1 diabetes do not produce insulin. In type 2 diabetes, the body does not produce enough insulin or the cells ignore the insulin. The body breaks down sugar and starches into glucose, which fuels our cells.

The insulin carries the glucose to the cells. The good news is that there are ways to manage both types of diabetes.

Dissecting VASCULAR DISEASE Doctors at Shady Grove Adventist Hospital are using modern medicine to treat life-threatening vascular conditions

n estimated 25 million Americans are at risk for vascular disease, which results from the narrowing of arteries or veins that carry blood throughout the body. Vascular disease can range from minor conditions, such as varicose veins, to life-threatening situations that include strokes.

Shady Grove Adventist Hospital is the only hospital in Montgomery County to offer a comprehensive continuum of care for patients with vascular diseases.

Lorena deLeon, Adventist HealthCare’s manager of vascular services, says, “In addition to prescreening and diagnosing, we can perform minimally invasive interventional treatments as well as open surgical repairs if needed.”

Getting to the Diagnosis Peripheral artery disease (PAD) is one of the most common vascular conditions, affecting one in 20 Americans older than 50, according to the Department of Health and Human Services. PAD, which increases the risk for a heart attack or stroke, occurs when arteries that carry blood to the legs become clogged with plaque.

“Some people with PAD don’t experience any symptoms,” says Robert Fox, M.D., vascular surgeon at Shady Grove Adventist Hospital. “The most common symptoms are fatigue in leg muscles, leg or foot pain, sores or wounds on the feet or legs, color changes on the foot and a lower temperature in one leg.”

Dr. Fox encourages anyone with these symptoms to speak to their doctors about a vascular screening test.

Albert Katoski, 88, of Gaithersburg remembers the excruciating pain.

Jeane, his wife of 65 years, remembers their vascular surgeon’s directive: Get him to Shady Grove Adventist Hospital’s emergency department immediately.

Katoski’s thoracoabdominal aortic aneurysm (TAAA) was rupturing.

“A rupture of this type of aneurysm is usually fatal,” says Katoski’s vascular surgeon, Jeffrey Wang, M.D. “Here, the aneurysm started in his chest area and extended into the abdomen. When it ruptured it resulted in massive bleeding and he needed immediate surgery.”

Dr. Wang and his partner, vascular surgeon Robert Fox, M.D., performed Shady Grove Adventist Hospital’s first minimally invasive repair of a ruptured TAAA, a groundbreaking and complex procedure.

“With endovascular surgery, we prevented a large incision,” Dr. Wang says. “We were able to use the latest technology to preserve blood flow to the intestines, kidneys, spleen and liver while fixing the aneurysm.”

Katoski was discharged from the hospital just 36 hours after surgery. “I was surprised how quickly I was able to go home,” he says. “I was lucky they caught it in time, and happy to have the surgeons I did.”

Once patients are diagnosed and treated for a vascular condition, they may require rehabilitation through monitored exercise or physical therapy, also provided on the Shady Grove Adventist Hospital campus.

Certain populations are at greatest risk for developing vascular conditions, including the elderly and individuals with diabetes. Other factors include family history, smoking, high cholesterol, obesity and lack of exercise.

“Risks for vascular diseases can be minimized with treatment and prevented with a healthy diet, daily exercise, reduced stress and a tobacco-free lifestyle,” Dr. Fox says.

Gearing up to prevent—and treat— concussions among student-athletes Approximately 300,000 Americans suffer sportsrelated concussions each year. Last winter, Leah Hunsinger of Gaithersburg became one of them.

The 17-year-old field hockey player didn’t see the hard hit to the head coming.

“I don’t remember falling. I just remember hurting on the ground and then opening my eyes,” says Leah, who graduated in June from Quince Orchard High School.

It happened during a January tournament for her club field hockey team.

Her father, Dan, saw it all from the stands.

“Normally when you fall, you put your hands up

to brace yourself. But when her teammate accidentally knocked her shoulder into Leah’s head, she just fell flat and smacked her head on the ground,” he says. “Basically, she had two blows to the head within a two-second period.”

Though it was not clear at the time, Leah had suffered a concussion.

“A concussion is a mild traumatic brain injury.

A blow to the head, like in Leah’s case, or a hard tackle in football, can cause signs such as loss of consciousness or confusion, headache, dizziness and cognitive impairment,” says Marc DiFazio, M.D., director of pediatric neurosciences at Shady

In fact, other than seeming dazed, Leah’s serious symptoms did not appear until the next day at school. “My head really, really hurt,” she says. “It was just overwhelming.” The headache lasted about four months.

Taking Concussions Seriously It was the first concussion for the standout player and multisport athlete who excels at field hockey and lacrosse. It was something to monitor, especially since Leah asked her club coach to return to the game.

“As a competitor, Leah wanted to keep playing, but as her dad and a pediatrician, I knew to take her home,” Dan Hunsinger says. He also knew what to look for, and kept a close eye on her mental and physical symptoms following the game.

Head injuries are concerning in children; their brains and bodies are still developing, Dr. DiFazio says. “A brain injury not only affects balance and coordination, but it can also affect the educational experience,” he says.

Leah, a straight-A student, admits she had trouble concentrating in school, especially on complex math problems. In fact, for the duration of her four-month-long headache, she had to “rest” her brain and refrain from activities. Dr. DiFazio provided a number of behavioral interventions to help her cope with the increased stress related to schoolwork and after-school activities.

Although Leah was eager to resume exercise and team activities, Dr. DiFazio cautioned against too rapid a return to contact. “Increasingly progressive, strenuous activity is recommended for

athletes,” he says. “We took our time to ensure full recovery and finally, I cleared Leah for play.”

That clearance, along with concussion education for youth coaches, parents and students, became Maryland law in July (Education— Public Schools and Youth Sports Programs— Concussions, Education, Section 7-432, and Health—General, Section 14-501, Annotated Code of Maryland).

This is critical, Dr. DiFazio says, because second and third concussions typically exacerbate the initial symptoms and could result in rare but deadly brain swelling known as second impact syndrome.

Local Safety Measures Montgomery County Public Schools have long practiced such sports safety policies. Similar to the state law, “Montgomery County Public Schools have a concussion program that addresses four components: education, recognition of concussion, responsibility—who determines the injury—and procedures on when a student can return to practice or play,” says Duke Beattie, the county’s director of school systemwide athletics. Specifically, Beattie adds, coaches must take annual courses on sports and concussions and how to prevent and care for athletic injuries. Likewise, parents and students are informed about the risks and symptoms of head injuries.

The school system also offers student-athletes baseline testing, which measures normal brain function and activity. The testing is something the entire Hunsinger family strongly encourages.

The day after the injury, Quince Orchard’s athletic trainer performed the follow-up test that helped establish the severity of Leah’s concussion.

Leah has fully recovered and the recent graduate is preparing to play field hockey for York College in Pennsylvania this fall.

Dr. DiFazio is a proponent of her decision to remain active.

“Balance must be struck,” he says, “between reducing the risk of concussion and encouraging participation in sports to ensure cardiovascular fitness and develop life skills through team and individual play.”

doctors; I didn’t want to go,” says Michelle Rheams, a 45-year-old mother of three from Aspen Hill, explaining why she waited nearly six months after discovering a lump on her breast to see a doctor.

“I should have gone the moment I thought something was wrong, but I was scared. I knew it wouldn’t be good.”

On Nov. 10, 2010, Cynthia Plate, M.D., a breast surgeon at Washington Adventist Hospital, diagnosed Rheams with breast cancer. Further tests confirmed it was Stage IV. “Stage IV is advanced, invasive cancer that has spread beyond the breast to other areas of the body,” Dr. Plate says. “Surgery is usually the first line of attack against breast cancer, but because Michelle was already in this late stage, she needed chemotherapy first to destroy the cancer that had spread.”

Upon hearing her diagnosis, the first thought that raced through Rheams’ mind was of her children. “I thought of Meshach, Alexis and Annie, and how no one can raise them or be there for them like I can. I’m their mother,” she says.

The oncology team at Washington Adventist Hospital addresses every aspect of cancer treatment from prevention, early detection, pretreatment evaluation, treatment, rehabilitation and post-treatment monitoring. mother’s fight Care Beyond the Illness In addition to helping Rheams tackle the physiological side of her illness, the cancer team also provided a range of resources to support the other areas of her life affected by cancer, including her children. Traudi Rose, R.N., OCN, MBA, cancer care navigator at Washington Adventist Hospital, was there the day Rheams was diagnosed.

“I’m there to answer patients’ questions about their diagnosis, and help ease their fears,” Rose says. “Michelle was worried about telling her kids, so I gave her educational material on understanding cancer, and talking to kids about cancer.”

Rheams says she realized she had an important decision to make: “It was to live or die,” she says. “I chose to live.”

Following her diagnosis, Rheams was referred to Alida Espinoza, M.D., medical oncologist at Washington Adventist Hospital, for chemotherapy.

“We started chemo soon after her diagnosis, and knew we had to be aggressive since we were catching her cancer at such a late stage,” Dr. Espinoza says.

“Her treatment lasted nearly five months, and Michelle responded very well.

She had great resolve to beat her cancer and get her health back on track.”

The Rheams family

Support Makes All the Difference Rheams credits the support from her friends and family, and her caregivers at Washington Adventist Hospital, for helping her stay positive.

“Traudi was more than my cancer nurse; she was my go-to person,” Rheams says. “Whenever I had a question, she was there to help. If I needed someone to talk to, she was there to listen. She made my life easier during difficult times.”

Rose helped Rheams with transportation to and from her appointments, gave her support information, and even arranged to have Rheams’ house cleaned when she was going through chemo.

“I talked with Michelle throughout her treatment, and made sure she knew that no question was too great, too personal or too difficult,” Rose says.

Rheams says she could feel her faith getting stronger throughout her course of treatment. “My children were my daily reminder of why I was doing this,” she says. “We’ve always had a special bond, but this made it stronger.”

On May 9, 2011, Dr. Plate performed a modified radical mastectomy on Rheams, a surgery that involves removing the entire breast. This didn’t mark the end of her cancer journey, though, and Rheams continues to meet with Dr. Espinoza to monitor her health and decide if or when more treatment may be needed.

“Michelle’s case helps stress the importance of early detection and early treatment,” Dr. Plate says. “When breast cancer is caught in the early stages, treatment is much faster and simpler. I’m glad Michelle can help spread that message.”

As she continues to battle her illness, Rheams is the first to admit she wasn’t smart to initially ignore her lump. “A lot of people make the same mistake I did,” she says. “I’ve come a long way, and couldn’t have done it without the support of my kids and my wonderful doctors and nurses who I truly consider part of my family.”

Hospital officials will break ground for the Aquilino Cancer Center next year. The facility, which will sit on the Shady Grove Adventist Hospital campus, is scheduled to open in 2013 as Montgomery County’s first community hospital-based, integrated, comprehensive cancer care program.

“The Aquilino Cancer Center will offer our patients and families access to expert clinicians in medical, radiation and surgical oncology, as well as essential cancer support services,” says Dennis Hansen, president of Shady Grove Adventist Hospital. “This is an exciting opportunity to provide patient-centered and quality-driven care in one convenient location.”

Technology and Support Under One Roof The Aquilino Cancer Center will house the most advanced technology to fight a range of tumor-site-specific cancers alongside an array of resources, such as a shop for wigs and prosthetics, an education library and space for support groups, nutrition counseling and cooking demonstrations.

Local resident Michael Aquilino has generously supported the building of this center after losing his first wife to cancer. “It was a difficult time, but the treatment she received at Shady Grove was phenomenal,” he says.

The Aquilino Cancer Center will allow local cancer patients to avoid shuttling from appointment to appointment during anxious times and will provide space for wellness programs, health screenings, research, treatments, palliative programs and survivorship support.

“When you’re going through cancer, time is so valuable,” Aquilino says. “You don’t want to spend that time commuting while maintaining a routine with your children, your work and the hospital. I truly believe upper Montgomery County deserves and needs a convenient, high-caliber cancer center like the one being planned, with all the critical services in one place and top medical researchers in close proximity.”

Better Care Through Collaboration As part of the Great Seneca Science Corridor in the Gaithersburg/Rockville area, the center will provide opportunities for collaboration on cancer treatments, research and clinical trials that will result in expanded access to exceptional and cutting-edge cancer care for the community.

“This will truly mean coordinated care for patients, with their caregivers conferring on treatments and management of their disease,” says Joseph Haggerty, M.D., chair of Shady Grove Adventist Hospital’s Cancer Care Center Committee. “And for physicians, the center will give them access to the latest technology and the most advanced research and studies on care.”

Physicians at the Aquilino Cancer Center will be able to treat their patients with state-of-the-art equipment that can target the cancer tumor with radiation while preserving the healthy tissue. Chemotherapy patients will undergo their infusion treatments in a soothing and private setting that overlooks the hospital’s healing garden.
