

The Arizona Alliance for Community Health Centers (the Alliance) works to create a more just and equitable healthcare system, so every Arizonan has the opportunity to thrive. We are the Primary Care Association for Arizona, dedicated to supporting and growing the healthcare safety net that includes health centers, look-alikes, and Federally Qualified Health Centers (FQHCs) — collectively referred to as Community Health Centers (CHCs). We also work with organizations that are interested in becoming health centers.
A nonprofit member organization, the Alliance supports the vital work of CHCs by providing health center staff with training, technical assistance, programmatic support, and advocacy. We also drive health center growth by assessing unmet needs at both local and statewide levels and identifying opportunities for CHCs to be part of the solution. We work collaboratively with health centers to develop strategies that leverage their unique community-driven model to meet those needs.

Circle the City (CTC) is a dynamic, Arizona-based, 501(c)(3) nonprofit organization that provides integrated, holistic healthcare to all individuals facing homelessness in Maricopa County. CTC has outpatient clinics, where patients can receive primary, preventive healthcare, behavioral healthcare, and assistance with connecting to community resources and services. CTC also operates respite centers for people healing from an acute illness or injury. CTC’s respite care program began in 2012 to meet the needs of people who were not ill enough to be in the hospital, but too ill to be on the street. Each 50-bed respite center provides patients facing homelessness, a safe place to rest, heal, and recover. Respite patients receive a bed, meals, daily medical evaluation, 24/7 nursing support, behavioral healthcare, and case management to help connect patients to resources, including stable housing. Approximately 70% of respite patients are connected to affordable, long-term housing upon discharge. CTC also operates mobile medical units that provide healthcare to people in all parts of Maricopa County. Additionally, our street medicine teams provide backpack medicine and mental health services to people facing homelessness in encampments throughout the county.

Native Americans for Community Action has provided over 50 years of services to the Native American and non-Native American peoples of Flagstaff and Coconino County, both on and off the reservation. North Country HealthCare’s roots reach back to the Flagstaff Community Free Clinic, which was founded in 1991 by a group of local healthcare providers and administrators. The founders were concerned about the large number of uninsured people who were presenting at the Flagstaff Medical Center Emergency Room because they had no medical home. In 1996, North Country Community Health Center became a federally qualified health center (FQHC) and moved into an old roller skating rink in Flagstaff’s Sunnyside neighborhood. Today, North Country HealthCare serves 12 communities across northern Arizona. North Country HealthCare established the premier family medicine residency in the region in 2020, called the NARBHA Institute Family & Community Medicine Residency Program.

Each year, Valle del Sol touches the lives of thousands of patients through its programs and services. The agency has served Arizonans for more than 50 years. Opening their doors in 1970, the first programs offered focused on methadone and behavioral healthcare services, predominately for monolingual, Spanish-speaking families. In 2011, Valle del Sol introduced primary care to its portfolio of medical services. More recently, they added mobile health and telehealth services to their portfolio of services. Today, Valle del Sol integrates Behavioral Care and Primary Care in an approach that serves patients holistically: mind, body, and spirit. The Maricopa County Special Health Care District, doing business as Valleywise Health, is the only public teaching hospital and health care system in Arizona. VH provides primary and specialty care services predominantly to underserved, low-income and ethnically diverse populations. Over a nearly 150-year history, VH has built a reputation of quality health care by placing compassion at the forefront of care. The mission of VH is “To provide exceptional care, without exception, every patient, every time.” Accordingly, the System employs a comprehensive approach to care, clinical training, and population health management. Our facilities include the Valleywise Medical Center, three inpatient and three outpatient behavioral health clinics, an Assertive Community Treatment Team, the First Episode Center, the largest Ryan White HIV/ AIDs program in the state, and 11 Federally Qualified Health Centers.

Since 1950, Wesley Community and Health Centers (Wesley) has been serving the Phoenix area by providing key community services. Health services originated in 2002 with the establishment of a volunteer clinic for the uninsured. This was followed by the opening of our first Federally Qualified Health Center in 2009 at our original location at 10th St. and Buckeye Rd. Since then, we have added two additional full-time sites at the Golden Gate Community Center and on the campus of AT Still University in Mesa. We also operate a part-time location within the Coffelt-Lamoureaux public housing project and two mobile units. Wesley is committed to providing an array of programs and services alongside our primary healthcare services to focus on the wellness of our community. These include a DES Certified After-School Program, ESL and financial literacy classes, daily fitness programs like Zumba, no-cost diabetes self-management education, nutrition education and prenatal classes, and a variety of community events designed to meet the diverse needs of our community.
Wesley Health Center has sites in Phoenix and Mesa and has mobile medical units.
