Welcome

The Arizona Alliance for Community Health Centers (the Alliance) works to create a more just and equitable healthcare system, so every Arizonan has the opportunity to thrive. We are the Primary Care Association for Arizona, dedicated to supporting and growing the healthcare safety net that includes health centers, look-alikes, and Federally Qualified Health Centers (FQHCs) — collectively referred to as Community Health Centers (CHCs). We also work with organizations that are interested in becoming health centers.

A nonprofit member organization, the Alliance supports the vital work of CHCs by providing health center staff with training, technical assistance, programmatic support, and advocacy. We also drive health center growth by assessing unmet needs at both local and statewide levels and identifying opportunities for CHCs to be part of the solution. We work collaboratively with health centers to develop strategies that leverage their unique community-driven model to meet those needs.

Canyonlands is a private, nonprofit corporation providing consumer-directed, high-quality, comprehensive, cost-effective primary and preventative care. Canyonlands offers sliding fee programs designed to improve access to healthcare for those who face financial barriers and need assistance.

Chiricahua Community Health Centers, Inc. (Chiricahua) is a mission driven Federally Qualified Health Center and a tax-exempt not-forprofit 501(c)(3) organization. Founded in 1996 as a small, rural health clinic operating in a community center, Chiricahua has since grown to become the largest primary care organization in southeastern Arizona, serving more than 33,000 patients annually. Chiricahua has developed and operates a comprehensive primary care model that includes medical, dental, and behavioral health carefully integrated with an approach that addresses every aspect of life (physical, emotional, social, and environmental) for the uninsured, underinsured, veterans, migrant and seasonal farmworkers. Chiricahua operates fixed-site medical, dental, pharmacy and behavioral health facilities, as well as mobile medical and mobile dental clinics that serve patients throughout the more than 6200 square mile borderlands of Cochise County. Chiricahua is not only the largest primary care provider in Cochise County but also offers the only true Sliding Fee Discount Program in the region.

Health Care for the Homeless Program Grantee/Federally Qualified Health Center

Circle the City (CTC) is a dynamic, Arizona-based, 501(c)(3) nonprofit organization that provides integrated, holistic healthcare to all individuals facing homelessness in Maricopa County. CTC has outpatient clinics, where patients can receive primary, preventive healthcare, behavioral healthcare, and assistance with connecting to community resources and services. CTC also operates respite centers for people healing from an acute illness or injury. CTC’s respite care program began in 2012 to meet the needs of people who were not ill enough to be in the hospital, but too ill to be on the street. Each 50-bed respite center provides patients facing homelessness, a safe place to rest, heal, and recover. Respite patients receive a bed, meals, daily medical evaluation, 24/7 nursing support, behavioral healthcare, and case management to help connect patients to resources, including stable housing. Approximately 70% of respite patients are connected to affordable, long-term housing upon discharge. CTC also operates mobile medical units that provide healthcare to people in all parts of Maricopa County. Additionally, our street medicine teams provide backpack medicine and mental health services to people facing homelessness in encampments throughout the county.

Since its inception as the Prescott Free Clinic, Inc. in 1993, it has stayed true to its mission to contribute to the well-being of the people of Yavapai County by offering integrated primary health services including adult medical, pediatrics, reproductive health, behavioral health, dental, and care coordination. Creek Valley Health Clinic provides comprehensive, quality healthcare to the rural community that surrounds Colorado City, Arizona. The nonprofit organization was created by local community members, and has a vision of creating a healthier, stronger, more connected community. As one of Arizona’s newest health centers, Creek Valley Health Clinic has been delivering services since December 2019. Creek Valley Health Clinic delivers services in person and through telehealth, and offers a robust line of health services including general primary medical care, diagnostic laboratory, chronic disease management, well-child services, voluntary family planning, immunizations, preventative dental, gynecological care, prenatal and postpartum care, behavioral health, psychiatry, occupational therapy, speech therapy, contract pharmacy services, as well as enabling services such as case management, eligibility assistance, health education, outreach, transportation, and translation.

Horizon Health and Wellness has developed an innovative practice model that embodies our philosophy of whole-person care while promoting wellness. This approach to health care addresses the whole person by integrating behavioral health with primary health care while promoting wellness and prevention. In this model, our medical and behavioral health staff can work with patients in the same exam rooms to ensure integrated care. Both are involved in assessments, brief interventions and follow-up with patients. Where appropriate, patients may also be referred for wellness activities (e.g. smoking cessation, nutritional education, physical activities, etc.), as well as traditional psychotherapy, and psychiatric services. Horizon Health and Wellness is excited to implement this innovative practice model at a time when health care is undergoing transformation nationally. It is our hope that our integrated model will improve outcomes for people and reduce healthcare costs for society.

Mariposa Community Health Center was founded in 1980 in Nogales, Arizona. Mariposa is the largest primary care provider of medical, dental and health education services in Santa Cruz County. As a patient-centered health care home, MCHC’s purpose is to ensure access to culturally appropriate primary care and community-based health education and assistance, regardless of an individual’s ability to pay. MHC Healthcare is a Federally Qualified Community Health Center providing care to the Tucson, Marana, Catalina, Oro Valley, and Picture Rocks areas. MHC Healthcare is the longest serving FQHC in Arizona, providing continuous health care since 1957. MHC Healthcare has since grown to a network of 17 Health Centers, employing over 700 staff, and serving over 60,000 patients. Our commitment and dedication to our community is to remove barriers to ensure everyone has access to healthcare, no matter their ability to pay. MHC has an integrated care model, offering family medicine, internal medicine, pediatrics, OBGYN, WIC, Women’s Health, Integrated Behavioral Health, Dental, and Same Day Care. Pharmacy, Lab and Radiology services are offered at many MHC clinics, offering “One Stop” care. Native Americans for Community Action (NACA)

Urban Indian Health Program/Federally Qualified Health Center

Native Americans for Community Action has provided over 50 years of services to the Native American and non-Native American peoples of Flagstaff and Coconino County, both on and off the reservation. Since 1971 the agency has provided services such as: capacity building in Native American communities by assisting future political and community leaders, fighting stereotypes of Native American individuals and organizations, and advocating as a knowledge base on Indigenous issues to governments and other non-Native organizations. Since 1991 NACA has offered a variety of health and human services to urban Native Americans, low-income, and other underserved families, as well as those who experience health and socio-economic disparities at rates disproportionate to the general population. Their core program is the Family Health Center, which provides health and medical services to Native Americans and non-Native Americans. Health Promotion programs include health related prevention programs such as: Tobacco Education, Diabetes Prevention, Health Promotion, and Chronic Disease Prevention.

NATIVE HEALTH provides integrated primary medical, dental, behavioral health, WIC, and wellness services provided by an interdisciplinary team for Native Americans living in the greater Phoenix urban area and all persons in the general community. NATIVE HEALTH has 3 clinics, NATIVE HEALTH Central, NHW Community Health Center, and NATIVE HEALTH Mesa. NATIVE HEALTH is accredited as a PatientCentered Medical and Dental Home and as a Quality Ambulatory Health Care provider by AAAHC. AHCCCS, SNAP, TANF, and Health Insurance Marketplace assistance is available for patients and the community. Sliding fee scale for uninsured individuals. Starting in 1996 with a mobile unit and a single nurse practitioner traveling between schools, Neighborhood Outreach Access to Health (NOAH), has expanded to a community health center operating sites in Glendale, North Phoenix, and Scottsdale. NOAH provides accessible, affordable, and confidential care for individuals and families. NOAH’s integrative medical, dental, and behavioral health services are provided by an interdisciplinary healthcare team, which is supported by community outreach programs and patient-specific education programs. North Country HealthCare’s roots reach back to the Flagstaff Community Free Clinic, which was founded in 1991 by a group of local healthcare providers and administrators. The founders were concerned about the large number of uninsured people who were presenting at the Flagstaff Medical Center Emergency Room because they had no medical home. In 1996, North Country Community Health Center became a federally qualified health center (FQHC) and moved into an old roller skating rink in Flagstaff’s Sunnyside neighborhood. Today, North Country HealthCare serves 12 communities across northern Arizona. North Country HealthCare established the premier family medicine residency in the region in 2020, called the NARBHA Institute Family & Community Medicine Residency Program.

North Country HealthCare has sites in Bullhead City, Flagstaff, Grand Canyon, Holbrook, Kingman, Lake Havasu City, Payson, Seligman, Show Low, Springerville/Round Valley, Williams, and Winslow. They also have mobile units.

Sunset Health provides a comprehensive array of primary care services such as medical, dental, behavioral health, pharmacy, and women’s health. The emphasis in the provision of primary care services is on the prevention and health maintenance of patients with chronic conditions. Sunset also provides health education and disease self-management education. The center employs approximately 285 healthcare professionals, of whom 35 are physicians, dentists, pharmacists, or mid-level practitioners serving 29,000 patients (inclusive of over 6,000 migrant workers). Sunset Health is an important part of the healthcare safety net and continues to expand sites and services to meet the needs of the communities they serve.

Each year, Valle del Sol touches the lives of thousands of patients through its programs and services. The agency has served Arizonans for more than 50 years. Opening their doors in 1970, the first programs offered focused on methadone and behavioral healthcare services, predominately for monolingual, Spanish-speaking families. In 2011, Valle del Sol introduced primary care to its portfolio of medical services. More recently, they added mobile health and telehealth services to their portfolio of services. Today, Valle del Sol integrates Behavioral Care and Primary Care in an approach that serves patients holistically: mind, body, and spirit. This approach has shown that it helps achieve better health outcomes, allowing staff to work directly with patients in a culturally respectful manner, developing individual treatment plans, and facilitating the least invasive level of care. Valle del Sol has six locations throughout the metropolitan Phoenix area and over 45 school and community co-locations offering behavioral health services to individuals where they live, work, and play.

The Maricopa County Special Health Care District, doing business as Valleywise Health, is the only public teaching hospital and health care system in Arizona. VH provides primary and specialty care services predominantly to underserved, low-income and ethnically diverse populations. Over a nearly 150-year history, VH has built a reputation of quality health care by placing compassion at the forefront of care. The mission of VH is “To provide exceptional care, without exception, every patient, every time.” Accordingly, the System employs a comprehensive approach to care, clinical training, and population health management. Our facilities include the Valleywise Medical Center, three inpatient and three outpatient behavioral health clinics, an Assertive Community Treatment Team, the First Episode Center, the largest Ryan White HIV/ AIDs program in the state, and 11 Federally Qualified Health Centers.

Since 1950, Wesley Community and Health Centers (Wesley) has been serving the Phoenix area by providing key community services. Health services originated in 2002 with the establishment of a volunteer clinic for the uninsured. This was followed by the opening of our first Federally Qualified Health Center in 2009 at our original location at 10th St. and Buckeye Rd. Since then, we have added two additional full-time sites at the Golden Gate Community Center and on the campus of AT Still University in Mesa. We also operate a part-time location within the Coffelt-Lamoureaux public housing project and two mobile units. Wesley is committed to providing an array of programs and services alongside our primary healthcare services to focus on the wellness of our community. These include a DES Certified After-School Program, ESL and financial literacy classes, daily fitness programs like Zumba, no-cost diabetes self-management education, nutrition education and prenatal classes, and a variety of community events designed to meet the diverse needs of our community.

Wesley Health Center has sites in Phoenix and Mesa and has mobile medical units.
