The Arizona Alliance for Community Health Centers (AACHC) is the Primary Care Association (PCA) for Arizona. We exist to support and grow the healthcare safety net that includes health centers, Look-alikes, and Federally Qualified Health Centers (FQHCs), collectively referred to as Community Health Centers (CHCs), as well as organizations interested in becoming health centers.

We believe every Arizonan deserves quality primary healthcare. As a nonprofit member organization, AACHC supports the vital work of CHCs by providing health center staff with training, technical assistance, programmatic support, and advocacy. We also drive health center growth by assessing unmet needs at both local and statewide levels and identifying opportunities for CHCs to be part of the solution. We work collaboratively with health centers to develop strategies that leverage their unique community-driven model to meet those needs.

The Directory is published as a guide to the community for low-cost primary healthcare options. It is intended to serve as a reference for state, local, public, and private health and human service agencies, as well as students and potential healthcare providers searching for practice opportunities in Arizona. Community Health Centers (CHCs) are local, nonprofit, healthcare providers serving people of all income levels in medically underserved communities. Chiricahua Community Health Centers, Inc. (Chiricahua) is a mission driven Federally Qualified Health Center and a tax exempt not-for-profit 501(c)(3) organization. Founded in 1996 as a small, rural health clinic operating in a community center, Chiricahua has since grown to become the largest primary care organization in southeastern Arizona, serving more than 30,000 patients annually. Chiricahua has developed a multi-dimensional model that includes medical, dental, and behavioral health carefully integrated with an approach that address every aspect of life (physical, emotional, social, occupational, intellectual, and environmental) for the uninsured, underinsured, veterans, migrant and seasonal farmworkers. Chiricahua operates fourteen fixed-site medical, pharmacy and behavioral health facilities, as well as seven mobilemedical and mobile-dental units that serve patients throughout the more than 6200 square mile borderlands of Cochise County. Chiricahua is not only the largest primary care provider in Cochise County, but also offers the only true Sliding Fee Discount Program in the region.

CCHCI has 14 sites throughout Benson, Bisbee, Douglas, Elfrida, Sierra Vista, and Willcox, and a mobile medical and dental program.

Circle the City (CTC) is a dynamic, Arizona-based, 501(c)(3) nonprofit organization that provides integrated, holistic healthcare to men, women, and children facing homelessness in Maricopa County. CTC has two outpatient clinics, where patients can receive primary, preventive healthcare, behavioral healthcare, and assistance with connecting to community resources and services. CTC also operates two recuperative care, respite centers for people healing from an acute illness or injury. CTC’s respite care program began in 2012 to meet the needs of people who were not ill enough to be in the hospital, but too ill to be on the street. Each 50-bed respite center provides patients facing homelessness, a safe place to rest, heal, and recover. Respite patients receive a bed, meals, daily medical evaluation, 24/7 nursing support, behavioral healthcare, and case management to help connect patients to resources, including stable housing. Approximately 70% of respite patients are connected to affordable, long-term housing upon discharge. CTC also operates four mobile medical units that provide healthcare to people in all parts of Maricopa County. Additionally, our street medicine teams provide backpack medicine and mental health services to people facing homelessness in encampments throughout the county. Most recently, CTC expanded its patient outreach to hospitals with its innovative Health Navigator Program, where a CTC employee is embedded in 7 hospital systems within Maricopa. CTC has received national recognition for its seamless continuum of healthcare for people facing homelessness in Maricopa County.

Creek Valley Health Clinic provides comprehensive, quality healthcare to the rural community that surrounds Colorado City, Arizona. The nonprofit organization was created by local community members, and has a vision of creating a healthier, stronger, more connected community. As one of Arizona’s newest health centers, Creek Valley Health Clinic has been delivering services since December 2019. Horizon Health and Wellness has developed an innovative practice model that embodies our philosophy of whole person care while promoting wellness. This approach to health care addresses the whole person by integrating behavioral health with primary health care while promoting wellness and prevention. In this model, our medical and behavioral health staff can work with patients in the same exam rooms to ensure integrated care. Both are involved in assessments, brief interventions and follow-up with patients. Where appropriate, patients may also be referred for wellness activities (e.g. smoking cessation, nutritional education, physical activities, etc.), as well as traditional psychotherapy, and psychiatric services. Horizon Health and Wellness is excited to implement this innovative practice model at a time when health care is undergoing transformation nationally. Mariposa Community Health Center was founded in 1980 in Nogales, Arizona. Mariposa is the largest primary care provider of medical, dental and health education services in Santa Cruz County. Starting in 1996 with a mobile unit and a single nurse practitioner traveling between schools, Neighborhood Outreach Access to Health (NOAH), has expanded to a community health center operating six sites in Glendale, North Phoenix, and Scottsdale. North Country HealthCare’s roots reach back to the Flagstaff Community Free Clinic, which was founded in 1991 by a group of local health care providers and administrators. The founders were concerned about the large number of uninsured people who were presenting at the Flagstaff Medical Center Emergency Room because they had no medical home. In 1996, North Country Community Health Center became a federally qualified health center (FQHC) and moved into an old roller skating rink in Flagstaff’s Sunnyside neighborhood. Today, North Country HealthCare serves 12 communities across northern Arizona. North Country HealthCare established the premier family medicine residency in the region in 2020, called the NARBHA Institute Family & Community Medicine Residency Program.

North Country HealthCare has 14 sites throughout Bullhead City, Flagstaff, Grand Canyon, Holbrook, Kingman, Lake Havasu City, Payson, Seligman, Show Low, Springerville/Round Valley, Williams, and Winslow, and 2 mobile units.

Each year, Valle del Sol touches the lives of thousands of patients through its programs and services. The agency has served Arizonans for more than 50 years. Opening their doors in 1970, the first programs offered focused on methadone and behavioral healthcare services, predominately for monolingual, Spanish speaking families. In 2011, Valle del Sol introduced primary care to its portfolio of medical services. More recently, they added mobile health, telehealth, and 24/7 behavioral health services to their portfolio of services. Today, Valle del Sol integrates Behavioral Care and Primary Care in an approach that serves patients holistically: mind, body, and spirit. This approach has shown that it helps achieve better health outcomes, allowing staff to work directly with patients in a culturally respectful manner, developing individual treatment plans and facilitating the least invasive level of care. The Maricopa County Special Health Care District, doing business as Valleywise Health, is the only public teaching hospital and health care system in Arizona. VH provides primary and specialty care services predominantly to underserved, low-income and ethnically diverse populations.
