Who We Are Arizona’s Primary Care Alliance The Arizona Alliance for Community Health Centers (AACHC) has served as Arizona’s Primary Care Association (PCA) since 1985 and strives to promote and facilitate the development and delivery of affordable and accessible community-oriented, high quality, culturally effective primary healthcare for everyone in the state of Arizona through advocacy, education, and technical assistance.

AACHC comprises the state’s largest network of primary care providers and is committed to working with a variety of partners to expand the availability of tools that health centers and organizations serving those in need can utilize to address the needs of their patient populations and improve health outcomes while continuing to demonstrate cost savings.

AACHC is committed to serving as a resource for organizations providing primary health care to the underserved, including Health Center Program Grantees, Federally Qualified Health Center (FQHC) Look-Alikes, Rural Health Clinics, Tribal organizations, behavioral health facilities, and others with a vested interest in the primary care safety net.

AACHC also has strong local, state, regional, and federal partners with which it leverages resources to accomplish goals. AACHC provides a variety of educational opportunities for members, including peer networking committees, which provide a forum for sharing of best practices among member colleagues for optimal healthcare delivery.

Dear Members, Partners & Affiliated Organizations, As AACHC celebrated its 30th Anniversary in 2015, we reflected on its rich history and embraced incredible change as a number of Arizona health center founders, leaders and long-term AACHC board members retired or moved on to other opportunities. We were sad to say goodbye to many of our mentors and colleagues while at the same time showing our appreciation for the legacy they have helped build. We welcome incoming health center CEOs and are excited about the new perspectives and energy they bring to their communities, their organizations, and AACHC. As we close 2015 and enter into 2016, we are reminded how quickly our industry has changed and the possibilities ahead. I am honored to have served as your Board President and to be part of a dynamic team that continues to demonstrate unwavering commitment to meeting the diverse needs of the communities we serve. The work of AACHC and its members is to ensure the vision of a healthier Arizona at the individual and community level. We know this requires much more than the provision of primary healthcare services alone. We are the pacesetters in addressing the social determinants of health and focusing on patient experience. Seventy five percent of Arizona health centers have already achieved Patient-Centered Medical Home accreditation, marking the results of our model in meeting the needs of our patients in ways that make care more personal, accessible, and affordable. Each year, I am more impressed with the collaborations across health centers, community organizations, and safety net providers that allow us to continue delivering cost-effective, high-quality, culturally-sensitive and integrated primary healthcare services in innovative ways. Through AACHC, our health centers share best practices, learning from each other on an ongoing basis so that we may continue to differentiate health centers in the marketplace, both locally and nationally. The strong partnerships we forge, collectively and individually, with other organizations committed to access to healthcare help to support this transformation. As we look to the future, knowing that health delivery reform is well underway, we must all strive to work together to promote the effectiveness of our programs and demonstrate the value we bring to patient care. Health centers will strengthen their efforts around payment reform, value-based reimbursement, provider of choice recognition, innovative team practice models, health informatics, workforce development, outreach and enrollment, and fully integrated patient-centered care void of health disparities and the negative impacts of the social determinants of health. All the while, AACHC will align its members and help us demonstrate the value of integration through advocacy, training, technical assistance and innovative partnerships that promote primary healthcare for all. We are delighted to continue to support Arizona health centers in leading the provision of exceptional population-based healthcare for all those we serve. Our Mission: To promote and facilitate the development and delivery of affordable and accessible community oriented, highquality, culturally effective primary healthcare for everyone in the state of Arizona. This will be accomplished through advocacy, education, and technical assistance.

Our Purpose: To support Arizona health centers in leading the provision of exceptional population based healthcare for the communities we serve.

Our Strategic Goals:

• Value Based Reimbursement: Lead payment reform developments that position and prepare Arizona health centers to adopt new reimbursement models that maintain or improve quality and support fiscal sustainability. • Position Health Centers as Providers of Choice: Increase visibility and credibility of the Alliance to position health centers as an essential primary care network. Strengthen ties with current and potential patients, among community partners and payers, and with policymakers/ stakeholders. • Innovative Practice Models: Lead and support health centers in implementing and refining care and service coordination models that promote patient-centered comprehensive care. • Health Informatics: Effectively use data to demonstrate evidence of value based care that AACHC members provide. • Position Health Centers as Employers of Choice: Support health center workforce development efforts through advocacy, partnerships, and training and technical assistance that position health centers as employers of choice in their communities.

A Message from Our CEO During the past year, the healthcare landscape in Arizona and across the country has continued to change rapidly due to the ongoing implementation of the Patient Protection and Affordable Care Act. In addition, health center networks remained engaged in activities that promote and demonstrate their long history of providing high-quality, costeffective, primary care with an added focus on valued-based care and associated payment reform efforts. Centered around the shared value that all people should have access to individualized care and an understanding that we must demonstrate the value of that care, health centers continue to meet patient needs in extraordinary ways. As the largest network of primary care providers in the state, AACHC is poised to best demonstrate and strengthen valued-based care efforts in this transformative time. As we welcome 2016, we celebrate health center achievements in 2015, including: • Eight New Access Point awards to our members totaling more than $3.5 million to serve a proposed 42,389 new patients • Over 44,000 individuals identified as being part of a special population were served by health centers • 47,177 people assisted with AHCCCS and Marketplace applications across the enrollment programs in which AACHC and health centers participate • 75% of Arizona health centers having achieved Patient-Centered Medical Home status Going forward, we recognize that there are many challenges and opportunities that await us in 2016 and beyond. It is apparent that we have come to a fork in the road on our journey to demonstrate the value of care that community-based health centers provide. AACHC, as a Primary Care Association, remains committed to assisting our members in preparation for payment reform and/or valued based purchasing (or as I like to say valued-based care). We will continue to widen our scope of work to include support for our membership with the goal of assisting in efforts to demonstrate the value of care that you provide every day and will continue to build upon that foundation. With the completion of our new strategic plan, AACHC members and staff are working collaboratively to address the needs of our health centers in order to position them for future viability and growth. Whether the focus is patient-centric care, service integration, and/or analytics to demonstrate value, AACHC is well-positioned to support health centers to succeed in this competitive environment. While we focus on value-based reimbursement (care), establishing health centers as providers and employers of choice, innovative practice models, and health informatics, we also recognize that health center outreach and enrollment efforts, healthcare needs of the diverse special populations we serve, provider recruitment and retention, service integration, and many other key issues of importance to our health centers and partners will require ongoing attention. AACHC recognizes that collaboration and partnership are integral to our work to support access to care for all Arizonans. Your Alliance team will continue to support you through activities such as state and federal advocacy, content-specific seminars, facilitation of peer or best practices networking committees, development and promotion of key partnerships, and other programs and tools designed to meet health center need while remaining true to our mission to promote and facilitate the development and delivery of affordable and accessible community oriented, high-quality, culturally effective primary healthcare for everyone in the state of Arizona. This will be accomplished through advocacy, education and technical assistance. In 2015, AACHC’s financial position was further strengthened, and we continued to see our membership grow to encompass a wider range of health service providers and other organizations that relate to our mission and desire to benefit from our work. We were also pleased to create a sister organization, Collaborative Ventures Network, a new provider network that will offer additional support and services to health centers and your patients. Please join me in acknowledging the hard work of members, staff, and partners which has resulted in national recognition of AACHC and its health center members. We look forward to continuing to work collaboratively with you to ensure all Arizonans have access to healthcare.
