This legend lists the services offered by the organizations catalogued within this membership directory.

PURPOSE OF MEMBERSHIP DIRECTORY The Arizona Alliance for Community Health Centers’ (AACHC) Membership Directory is published as a guide to the community. It is intended to serve as a reference for state, local, public and private health and human service agencies, as well as students and potential medical providers searching practice opportunities in Arizona. Due to the possibility of expansion of sites and services, and changes in personnel in the future, please refer to our website (www.aachc.org) for the most current information.


ARIZONA COMMUNITY HEALTH CENTERS AT A GLANCE The Arizona Alliance for Community Health Centers (AACHC) has served as the Primary Care Association for the state of Arizona since 1985. AACHC has consistently strived to address the needs of Arizona’s Community Health Centers (CHCs), as well as the diverse populations that they serve. Arizona’s CHCs are local, nonprofit, healthcare providers serving people of all income levels in medically underserved communities. Arizona’s CHC network has evolved into a healthcare delivery system of 19 Health Center Program Grantees, three Federally Qualified Health Center Look-Alikes, and many other healthcare organizations. Health centers are located in 14 of the 15 counties in Arizona, and our CHCs and Look-Alikes serve over 540,000 patients and provide almost 2,000,000 annual visits. The number of patients and patient visits continues to grow. Nationwide, health centers serve as the primary health care home for almost 23 million people a year. Arizona’s health centers provide high-quality, affordable primary care and preventative services and often provide onsite dental, pharmaceutical, and behavioral health services. Health centers’ costs of care rank among the lowest, saving taxpayers millions of dollars. CHCs help remove common barriers to healthcare access because they: •Are located in high-need areas, identified by the federal and state government •Provide enabling services that helps ensure access to quality, comprehensive healthcare is available, including transportation, translation, case management, health education, and chronic disease management •Are open to all residents regardless of insurance and income status •Tailor services to fit the special needs of their local communities and provide services based on the recommendations of local residents CHCs focus on improving the health of individual patients, as well as improving the health status of the entire community. The medical home model, which is already widely used in health centers today, has been identified as the future of healthcare delivery.

(CCHCI) is a diverse not-for-profit health center program grantee/federally qualified health center providing a variety of services. It has been developed on a multi-dimensional model that includes medical, dental, and behavioral health carefully integrated with an approach that address every aspect of life (physical, emotional, social, occupational, intellectual, and environmental) for the uninsured, undersinsured, veterans, migrant and seasonal farmworkers, and all residents of Cochise County. Nies, RN, BSN, MHSA www.chcy.info The Community Health Center of Yavapai (CHCY) has been serving all of Yavapai County since 2001. Since its inception as the Prescott Free Clinic, Inc. in 1993, it has stayed true to its mission to contribute to the well-being of the people of Yavapai County by offering integrated primary health services including adult medical, pediatrics, reproductive health, dental and care coordination. Copper Queen Medical Associates Rural Health Clinic Chief Executive Officer: James J. Dickson www.cqch.org Copper Queen Medical Associates (CQMA) has three clinics that service southeast Cochise County, Bisbee, Palominas and Douglas. CQMA Rural Health Clinics partner with the Copper Queen Community Hospital in Bisbee to provide primary care, diagnostic services including lab and diagnostic imaging, physical therapy, home health, and telemedicine. In June 2004, the Douglas and Bisbee Clinics were established as part of the largest hospital-based Rural Health Clinic network in Arizona. The Palominas Clinic was added in October of 2007 in the growing Palominas-Hereford area. All three clinics provide specialty-supported primary medical care to over 30,000 patients in the US/Mexico border region.

Maricopa County Health Care for the Homeless (HCH) was established in 1984 and offered only basic health care to the homeless in the downtown Phoenix area. In 1988, the Maricopa County Department of Public Health was awarded McKinney Homeless Assistance Act funds, and this enabled HCH to provide an integrated system of health care services throughout the county for homeless individuals and families. Vanaskie, MS Executive Director of Maricopa Health Centers Governing Council: Laurie Wood www.mihs.org Maricopa Integrated Health System (MIHS) includes Maricopa Medical Center, the Arizona Burn Center, the Comprehensive Healthcare Center, the McDowell Healthcare Clinic, 11 community-oriented family health centers, and an attendant care program. Maricopa Medical Center is a premier training center for the nation’s physicians. The medicine, surgery, pediatrics and OB/GYN programs, in particular, contribute to the body of knowledge of patient care. Maricopa Medical Center ensures that the health care is available to all area residents giving paying and non-paying patients access to top quality medical care. MIHS is the health care safety net for citizens of Maricopa County. Annually, MIHS has nearly 20,000 inpatient admissions and 500,000 outpatient visits.

Wesley Community Center Health Center Program Grantee/Federally Qualified Health Center Chief Executive Officer: Betty Mathis www.wesleycenterphx.org Since 1950, Wesley Community Center, located in south central “downtown� Phoenix, AZ, has provided programs, services, and activities for primarily Hispanic families through its family/ youth services, community development, adult education, and healthcare. Wesley Health Center, opened in 2003 for uninsured patients, became an FQHC in 2009 through a grant from the American Recovery and Reinvestment Act (ARRA) and HRSA. In 2011, primary healthcare services and preventive/health management classes for everyone were provided to over 8,000 unduplicated patients. Southwest Center for HIV/AIDS Executive Director: Cindy Quenneville www.swhiv.org SWHIV brings 25 years of experience to support those living with and at risk for HIV/AIDS. The agency’s mission is to reduce HIV infection, improve quality of life, and contribute to worldwide research. We are co-located with MIHS’ McDowell Health Clinic on light rail in Central Phoenix. Our focus is HIV prevention, HIV care, Lesbian, Gay, Bisexual, Transgender and nonbinary gender individuals. Services are offered in English and Spanish.
