One Community Services Together we make a difference

Celebrating 40 Years One Community is an independent charity, established in 1978, to support the voluntary and community sector. We also provide a range of services that help people to live independently. Our aim in all we do is to improve the quality of life of those we serve. We are the proud recipients of the Queen’s Award for Voluntary Service. We are not “the council”, though we do receive funding for some of our services through contracts and service level agreements with local authorities. Fundraising is vital to the continuation of our services, so donations and legacies are warmly welcomed to help us to meet the needs of local people. One Community Information, Support and Care Services Care Navigators A team of Care Navigators works across the Eastleigh Southern Parishes and the New Forest, attached to GP practices. Their role is to ensure that patients receive personalised and coordinated care, linking them to services within the voluntary and community sector. This is a non-clinical service and is a supportive role to the health and social care professionals. Tel 023 8202 0363

Care & Respite Service The Care & Respite Service, registered with the Care Quality Commission, offers carers a few hours respite from their caring responsibilities each week. Trained staff provide stimulating and therapeutic activities for the cared for person, or a trip out in the car, perhaps for coffee and cake, depending on their capabilities and interests. The service also provides encouragement for individuals to maintain independence at home, whether that be assistance to access the community, administration or support to carry out daily tasks. Day Centre Service One Community has six day centres, which offer

a wide range of activities for older people to help them to live independently, meet others and socialise, while providing respite to carers. The day centres cater for people who have dementia or physical disability. Refreshments and lunch are provided and transport can be arranged. Referrals come through Adult Services or can be booked directly through the day centre. An initial assessment will be made at home to ensure that the day centre can meet the individual’s needs and aspirations. Tel 023 8090 2407 Tel Adult Services 0845 6035630

Eastleigh Museum One Community is responsible for the day-to-day running of Eastleigh Museum, in an innovative partnership with Hampshire Cultural Trust and Eastleigh Borough Council. Our aim is to preserve the museum and its record of Eastleigh’s heritage while providing a community hub with outreach for One Community’s services. The museum, staffed by One Community volunteers, offers a shop selling local produce, locally themed goods and locally produced crafts, and a welcoming café serving hot and cold drinks, cakes and snacks. To the rear of the museum is One Community’s Information Centre (see below) and a temporary exhibition centre
