One Community Services An Independent Charity One Community is an independent charity, established in 1978, to support the voluntary and community sector. We provide a range of care, transport and information services aimed at improving the quality of life for those we serve and helping our communities to flourish. We are not “the council”, though we do receive funding for some of our services through contracts and service level agreements with local authorities. Fundraising is vital to the continuation of our services, so donations and legacies are warmly welcomed to help us to meet the needs of local people. One Community Information, Support and Care Services Care Navigators A team of six Care Navigators works across the Eastleigh Southern Parishes, which has five GP practices. They work with the practices to ensure that patients receive personalised and coordinated care, linking them to services within the voluntary and community sector. This is a non-clinical service and is a supportive role to the health and social care professionals. Tel 023 8202 0363

Care & Respite Service The Care & Respite Service, registered with the Care Quality Commission, offers carers a few hours respite from their caring responsibilities each week. Trained staff provide stimulating and therapeutic activities for the cared for person, or a trip out in the car, perhaps for coffee and cake, depending on their capabilities and interests. Carers Centre One Community’s Carers Centre offers support to people over the age of 18 in the Borough of Eastleigh who care for someone with an illness, disability, mental health condition or addiction. It also undertakes research into the needs of carers and the impact that their caring role has on their lives.  Telephone signposting service, helping carers navigate to the right source of

Young Carers Young Carers offers support to young carers aged 8 to 18 years who live in the Borough of Eastleigh. The activities organised give young carers time off from caring and the opportunity to make friends with other young people in the same situation. We aim to empower young people and provide them with positive experiences in a safe and fun environment where they are free to be themselves. Tel 023 8090 2462 Email eastleighyc@1community.org.uk

Day Centre Service One Community has six day centres, which offer a wide range of activities for older people to help them to live independently, meet others and socialise, while providing respite to carers. The day centres cater for people who have dementia or physical disability. Refreshments and lunch are provided and transport can be arranged. Referrals can come through Adult Services or can be booked directly through the day centre.
