Guided Care is a private pay model inclusive of Medicare reimbursement for eligible patients. Remote Patient Monitoring devices are covered by Medicare regardless of Medicare eligibility. Some long-term care insurance policies may provide additional coverage.
Guided Care offers a one-time investment in a 4-month intensive program designed to reduce your risk of complications from high-risk chronic conditions and guide you to self-manage your care. After the 4 months, you can choose to customize ongoing services.
Upon discharge Guided Care offers the Invisible Safety Net program enabling patients unlimited access to a Guided Care nurse as needed 24/7.
