What is Tecarforesi Transdermal Delivery mode Tecarforesi is transdermal delivery associated with Tecar (Capacitive and Resistive Energy Transfer). A painless method without side effects. What is the Pressure Shock Wave method Shock waves are acoustic waves (sound pulses) that carry high energy characterized by a particular wave shape (rapid peak of positive pressure followed by a rapid phase of negative pressure), able to produce a direct mechanical stimulation. What is Tecar method It is the application of an energetic transmission system in radiofrequency, applied to the human body as a curative method, which controls and quantifies the quality and quantity of energy accumulated by the body.

Laser biological effects

The laser radiaton, penetrating the tissues, causes biochemical reactions on the cell membrane and within the mitochondria inducing several effects including: •

Vasodilation, with a consequent increase of local heat, increase in cell metabolic demands, neuro-vegetative stimulation and modification of the intracapillary hydrostatic pressure;

increase of lymphatic drainage, increase of the effectiveness of the sodium/ potassium pump, ionic transmembrane stabilizations, more absorption of interstitial liquids; activation of microcirculation thanks to a more nutritional intake with rebalancing of the functional balance;

•

Metabolic stimulation, accelerates the process of transformation of ATP into ADP+P+E and of electrolytic exchange between intra- and extra-cellular environments, increasing the conentration of nucleic acids and amino acids;

•

Slight modification of intracellular and extracellular pH.

The pain management is obtained thanks to the analgesic action that is induced by the perception thresold of the algotrope nerve endings and the release of endorphins.

The anti-inflammatory, anti-edema, eutrophic and stimulating effect for the cellular tissue is given by the increase in blood flow due to capillary and arteriolar vasodilation.

Depth and Absorption Each laser emits its own beam at a well defined wavelength and, obviously, each different type of laser has the ability to reach a certain depth in target tissues.

Laboratory experiments conducted in several Universities have shown an increase of oxygen absorption by cell cultures subjected to irradiation with laser energy of different wavelenths. In particular, the activation occurs in two areas of the “wavelength spectrum”: between 600 and 1200 nm”.

Absorption and Dispertion From “Physics“ point of view, the laser beam, passing through the biological structure, interacts with two mechanisms: absorption and dispertion.

In fact, the cell of living tissues is shown as a semitransparent system (a sort of glass bowl with a semi-opaque content) that holds a certain amount of energy leaving the excess, that hits partly for “reflection“ and partly for “crossing“ and transfer towards the underlying layers.

Although the impact of the laser beam with the skin is perpendicular (and the photons are parallel), as the tissue penetration gradually occurs (due to dispertion effect) the section of the irradiated tissue becomes conical and irregular because of the different density of the structures crossed.

These graphics allow to notice that the increase of the wavelength is not strictly linked with an increase in the level of absorption by the tissues. As can be seen, most of the absorbed energy stops in the first skin layer (epidermis and dermis). An extremely important factor is given by the chromophores, or particles that absorb light radiation and make the laser a device of high selectivity and precision for the pathology treatment.

For the same amount of energy delivered, the differences of application by virtue of the wavelength determine the inteaction of the chromophores. The 810 nm wavelength is mainly absorbed by the melanin instead of the 975 nm and 1064 nm that are mainly recognized by the hemoglobin. For the same amount of energy absorbed by tissues and depth, the wavelength inteaction will determine specific and different therapeutic responses.

The biostimulation

The biostimulation is an interaction that occurs at molecular level on cells tissues, changing and influencing the intercellular state and extracellular responses occurring during the inflammatory phases, in order to obtain the furthest expression of wounds healing physiological potential, all aimed for the morphofunctional restoring of an organ.

The term has been introduced in medicine with the discovery of laser properties in order to enhance the outcomes of diseases and healing. These lasers, in order to be distinguished from those with surgical skills (cutting or coagulation), are called “therapeutic lasers“. The term has never been introduced in dictionaries, but is more and mroe used in the current language.

The use of the term comes from the attention of medicine on a surgery biologically guided, where technologies and biomaterials are means to influence the physiological processes in order to obtain faster healing, stimulating tissues regenerative processes. For instance, in dental surgery is important to choose procedures that allow to contain the loss of tissue, especially bone, leading regenerative processes from a qualitative and quantitative point of view, to improve functionally and aesthetically the oral rehabilitations. The totality of these techniques can be defined as “biostimulation“.

The connective tissue This section has been introduced as the laser therapeutic action affects the connective tissue.

The laser beam stimulates the fundamental components of the extracellular matrix, such as glucosamine sulfate, chondroitin sulfate and polysaccharides with high moleculatr weight (glycosaminoglycans).

As regards the chondroitin sulfate, exprimental data demonstrate its chondroprotective activity (stimulation of proeoglycan synthesis, inhibition of collagenolytic activity, inhibition of IL-1 effects on the proteoglycans and collagen synthesis, reduction of chondrocyte apoptosis).

The stimulation of chondroitin sulfate makes possible to stop the progression of the structural damage.

Connective Tissue is a type of mesodermal origin tissue that provides structural and metabolic support to organs and other tissues within the organism.

Types of laser Solid-state Laser Solid-state lasers use a crystalline or glass rod, which is “doped“ with ions that provide the required energy states. For instance, the first working laser was a ruby laser, made from ruby (chromium-doped corundum). The population inversion is maintained by the dopant chromium or neodymium. In this case, “solid state“ refers to the crystal or glass that provides ions. Neodymium is the most common dopant among the various solid-state lasers, including yttrium orthovanadate (Nd:YV04), yttrium lithium fluoride (Nd:YLF) and yttrium aluminium garnet (Nd:YAG). All these lasers, may produce high powers in the infrared spectrum at 1064 nm. Solid-state lasers are “doped“ crystal rods with rare earth elements or transition elements, such as YAG (yttrium aluminium garnet) doped with neodynium, or sapphire (aluminium oxide) doped with chromium (ruby).

Gas Lasers The active material is at gaseous or vapour state, is made of neutral atoms, ions, molecules and other elements (in particular helium) to facilitate the electric discharge and enhance the energetic profit. Gas lasers are pumped with ultraviolet, electron beam, electric current or chemical reactions.

Among these lasers, there is a mixture of: • atomic gas lasers (He-Ne) • molecular lasers (CO2 vapori di H2O) • ion lases (Argon, Kripton, Xenon, vapori di metallo, Elio-Cadmio)

Laser Diode A laser diode is composed of doped semiconductor material that is placed on a very thin layer of the surface of a crystal rod. The crystal is doped to create a N type semiconductor region and another one of type P, one on top of each other, to achieve a P-N junction, which is a diode. As in other diode lasers, when the diode is directly polarized, the holes of the P region are pumped in the N one, where the electrons are the major carrier of charge. In a similar way, the electrons of N region are pumped in P one , where the holes are the major carriers.

When a hole and an electron are in the same region, they may combine through spontaneous emission, that means the electron may occupy the bandgap state again by emitting a photon with an energy equal to the difference of the electrons state and the hole one that are involved. Some important properties of laser diodes are determined by the geometry of the optical cavity.

The emission wavelength is a function of the energy gap between the semiconductor bandgaps and the ways of propagation of the optical cavity. In general, the maximum gain is achieved for the photons with an energy slightly higher than the gap one. Hence, the ways of propagation closest to the gain peak will predominantly emit.

If the diode is controlled with enough power, there will be a number of additional “side modes“.

Latest technological developments of the Diode In the last few years, the laser diode has had significant developments. The research for the application of these types of laser at industrial level, made possible the development of laser diode with powers higher than 3 KW. For the application of these devices in cutting metals and incise them, is very important, more than the power, the brilliance of the light beam and the perfect incision, because in these cases are necessary very tiny focal spots. These technological developments lead the laser diode to be at the same level of CO2 devices and fiber lasers, substituting them due to their smaller dimensions, lower costs management and more reliability. This technological development achieved with the research and applied in the industrial field, can now be transferred in medical field without dreading any comparison with the most classical laser systems not used yet.

Laser Beam emission mode Continuous, Pulsed and Duty Cycle A laser may be classified according to the operative mode, such as continuous or pulsed, depending on whether the output signal is continuous over time or if it is emitted as pulsed light.

Cw = continuous, CWF = pulsed, and pulsed in Duty Cycle

Continuous mode The laser beam is emitted continuously during the time up to the device is switched off.

This type of laser is defined as Continuous Wave (CW).

Pulsed mode The laser beam is emitted in pulsed mode (CWF - Continuous Wave Fluctuation). The Duty Cycle is part of the pulsed mode.

Duty Cycle The Duty Cycle is a form of energy emission, in particular, is an ultrapulsed mode. With the Duty Cycle is possible to provide high energy doses to the organism without overheating, and thus damaging, the tissues.

This is because the gap between one energy pulse and the other is such to allow the tissue to absorb the energy and dissipate the excess; the tissue is then more “ready“ to absorb the next energy pulse.

Consider a square wave. The Duty Cycle is the relationship that exists between the period in which there is the signal and the gap period.

It can be expressed with a percentage value or with a fractional value.

If the Duty Cycle is at 50% (or 1/2) means that the energy signal lasts as long as the gap period.

If the Duty Cycle is at 20% (20/100) means that the signal is at 20% whereas the gap period is equal to 80%.

The outermost layer of the skin, the epidermis, is characterized by wrinkles and furrows and their structure varies according to several factors, such as elasticity, skin appendages arrangement, and, on hands and feet, fingerprints, that distinguish each one of us.

The outer epidermis is mostly composed of keratinocytes, cells with particular resistance to mechanical stresses and with very selective permeability against exogenous substances. The epidermis has melanocytes, which contain melanin that preotects us from sunbeams.

Strictly connected with the epidermis, there is the dermis, which constitutes the substrate for mechanical support and metabolic exchange, supplied by blood and lymph vessels. The dermis is composed of fibriblasts, collagen fibers, elastic fibers and ground substance, the bricks that support the skin architecture: this layer is treated in order to enhance the support and to keep the skin young.

Why does the skin get old?

The skin gets old because the collagen and the elastin “bricks“ get smaller and the TEWL increases considerably.

The dermis is a very elastic layer, resistant to traction; it has to be protected, defended and nourished in order to remove the signs of aging.

The ground substance is an amorphous gel that especially contains water. The glycosaminoglycans dissolved, including hyaluronic acid, assemble collagen and elastic fibers; thus, an optimal hydration is essential.

The so-called skin appendages (hairs and glands) have the main function of thermo-regularization and allow the secretion of substances from the body outwards: without them, this step would be minimum because the skin is a waterproof organ.

The hypodermis is the tissue placed between the dermis and the muscular apparatus, formed by adipocytes, round cells that isolate the deepest layers and dampen external stresses.

Clinical case of tibialis anterior muscle strain therapy A.P., a 27 years old basketball player, complains about pain on the anterolateral face of the left leg after a quick change of direction during a regular championship league match in June 2012.

He went to the doctor: during the inspection are evident redness and swelling on the left tibialis anterior muscle. The doctor recommended him an ultrasound scanning, done on June, 4th 2012; after having checked it, the doctor decided to start six sessions of SIN treatment following the protocol three times a week.

During the acute phase, has been chosen the program P1 with 20% of power using Demusco Gel. The treatment has been done on two proximal spots and on two distal ones, 4 minutes each spot. The handpiece and neutral plate placement has been respected so that the lesion must not find between the two elements.

The injured area has been treated with 4 minutes of program P1, power 20% using the phytocompound EDTA 10% to facilitate the absorption of the hematoma.

After the third treatment, the operator changed the opeating mode, choosing the program P1 with power 60% and Demusco Gel, moving the handpiece on the whole left tibialis anterior muscle and, thus, on the plantar fascia and triceps surae up to the hamstrings muscles, 12 minutes each part.

At the end of the six treatments recommended, the doctor sent A.P. to do a post-surgery rehabilitation ultrasound scanning on July, 6th 2012.
