PATIENT TRANSPORTATION SERVICE William Osler Health System uses Canadian Medical Response (CMR) Patient Transportation Services to transport patients between hospitals and to other care facilities. Canadian Medical Response (CMR) Fees: Medically Essential Transportation The patient is required to pay a $45 co-payment when discharged from the hospital to their private residence or a non-licensed facility when the attending physician deems special transportation is medically required. Non-Medically Essential Transportation: The patient must pay the full payment when discharged from the hospital to their private residence or non-licensed facility for non-medical purposes as deemed by the attending physician. Rates for patient transportation services for non-medically essential transport vary depending on the service and may include additional costs for distance. For your protection we recommend you contact CMR Patient Transportation Services about all costs associated with your transportation. Payment is made directly to CMR You will be requested to have cash or a major credit card (Visa, MC, AMEX) available at the time of the transportation. For more information contact CMR Patient Transportation Services (416) 751-5442. PRIVATE VEHICLE/TAXI/ACCESSIBLE TAXI Please arrange your transportation home before you are discharged. The hospital cannot hold your bed to accommodate delayed travel arrangements. Please note: If you arrived at the hospital by ambulance you will be asked for payment at the time of registration. If you are unable to pay then a bill will be mailed to you, payable to William Osler Health System. It is the hospital’s responsibility to collect ambulance fees on behalf of the Ministry of Health and Long-term Care. Ambulance fees collected by the hospital are remitted to the Ministry monthly.
