Carlisle Wide Plank Floors cares about the health, well-being and financial security of our associates and is proud to offer a comprehensive benefits program to all eligible employees. Your benefits are an important part of your total compensation; therefore, we invite you to familiarize yourself with the details of these plans.

Who is Eligible and When?

All regular full-time employees working 30+ hours a week are eligible to enroll in health insurance. Eligibility for newly hired employees is the 1st of the month following 60 days of employment. New employees will have 30 days to elect medical coverage. Current, eligible employees who have elected to waive coverage are able to enroll during Open Enrollment. Only if they experience a qualifying event (including losing coverage elsewhere, getting married, having a baby) are they able to enroll outside of the open enrollment period.

How Do I Enroll?

Newly hired, regular full-time employees will enroll for health insurance benefits during the new employee orientation period. Eligibility begins the 1st day of the month, following the 60th day of employment.

Waiving or Declining Health Insurance

If you are eligible for health insurance and will not be enrolling because of alternative coverage, or choose not to have insurance, a waiver form must be signed during the enrollment period. If you elect not to enroll, you may not join the plan until the next enrollment period, November 2026, unless there is a “qualifying event.”

All employees should be aware of possible Federal tax penalties for declining Carlisle’s health insurance plan enrollment, as well as alternatives for health insurance available through the Health Insurance Exchange. What happens if I leave Carlisle?

Under certain circumstances, you and your dependents may continue to participate in health coverage, dental insurance, vision insurance and the Medical Flexible Spending Account through the Consolidated Omnibus Budget Reconciliation Act. COBRA is a federal guarantee of the continuation of health insurance plan coverage after employment ends, which allows you to remain on medical and dental coverage, at the employee’s expense, for up to 18 or 36 months, depending on the circumstances. Former employees who enroll via COBRA will be responsible for the full cost of the monthly premiums.

Domestic Partnership: An employee’s domestic partner and domestic partner’s dependent children can enroll in Carlisle’s health, dental and vision insurance plans.

Cost to Use and Own

The major cost sharing components for Carlisle's plans are copays, deductibles, coinsurance, and Out-of-Pocket maximum amounts. All accumulators are tracked on a calendar year (January – December)

• A copay is an amount set under the medical plan that the member pays for covered servicessuch as an office visit - or prescription drugs.

• The EPO 1500 has copays for virtual visits, office visits, urgent care and prescriptions drugs.

• Deductible is the amount of deductible-eligible covered medical expenses (i.e. inpatient, outpatient, imaging, lab, ER) the member pays before HPI pays benefits.

• The EPO 1500 has a $1,500 deductible for single coverage. $3,000 for dual coverage and $4,500 for family coverage.

• Coinsurance is the amount the member pays for deductible-eligible services after meeting the deductible.

• The EPO 1500 has zero coinsurance for medical services and 20% coinsurance for durable medical equipment.

• Out-of-Pocket Maximum refers to the most you will pay for covered expenses under the plan. Copays, Deductibles and Coinsurance all accumulate toward the OOP Max.

• EPO 1500: $5,650/$11,300 (Individual/Dual & Family)

Where you receive your care can impact how much you pay:

• Urgent Care Centers, such as Convenient MD are generally less expensive than hospital emergency rooms

• Choosing standalone facilities for labs, X-rays, or outpatient surgery rather than a full-service hospital can significantly reduce your out-of-pocket costs.

Carlisle’s prescription drug benefits will be administered by Liviniti. All local pharmacies can be utilized to fill prescriptions.*

Preventive Care

Carlisle’s HPI plans provide 100% coverage for eligible preventive care.

The preventive care services listed in this guide are recommendations of the Affordable Care Act (ACA) and are subject to change. They may not be right for every person and this is not a comprehensive list. Ask your doctor what is right for you.

Flexible Spending Account

Who is eligible? Carlisle offers both health (FSA) and dependent care (DCA) flexible spending accounts. Employees may contribute pre-tax dollars into these accounts to help offset eligible medical expenses or dependent care expenses. The FSA & DCA will be administered through HPI.

The Health Care FSA is available to all regular full-time employees, whether they participate in Carlisle’s health insurance plan.

Medical FSA Basics

Medical FSAs are used to set aside money for planned or reoccurring qualified medical expenses that are not covered by insurance, such as dental care, vision care, contact lenses, co-pays, deductibles, out-of-network expenses. • You have full access to your total election amount on the 1st day of that plan year. You do not need to wait until your FSA payroll deductions equal the amount of your qualified medical expense.

• You can open a medical FSA and contribute up to $3,400 each year to cover qualified out-ofpocket costs.

• Most transactions can be paid at the point of sale with a debit card pre-loaded with your total FSA amount for that plan year.

• After the 90-day runout period, you may rollover up to $680 into the next plan year. Any remaining funds exceeding $680, would be forfeited.

Dependent

Care FSA (DCA) Basics

A DCA allows employees to set aside pre-tax dollars to pay for qualified dependent care expenses. Funds can be used to pay for day care, preschool, or other care for qualified tax dependents. To qualify for a dependent care FSA, the IRS requires that the dependent care is necessary for you or your spouse to work, look for work or attend school full-time.

In 2026 the maximum amount you may contribute to the dependent care FSA is $7,500* (if single or married & filing jointly) or $3,750* (if married & filing separately). Please note that the DCA is 100% employee funded and is only available to use as monetary contributions are made.

Eligible Daycare Expenses:

• Childcare or Adult Care by a licensed childcare facility, for children under 13 years of age who qualify as tax dependents.

• Childcare or Adult care for children or adults of any age who are physically or mentally unable to care for themselves and qualify as dependents.

The Delta Dental PPO plus Premier network arrangement offers access to the nation’s largest network of dental providers while simultaneously offering access to PPO providers who have agreed to accept lower fees for Delta Dental patients.

Northeast Delta Dental Double-Up Max Carryover Benefit

Promoting Regular Care

Northeast Delta Dental offers a Double-Up Max carryover benefit feature on select dental programs. With this feature, enrollees may double their annual maximum by accumulating $250 a year in additional benefits for use in future coverage periods. When a dental procedure is needed that costs more than the annual maximum allows, accrued carryover benefit dollars can help make up the difference.

To Qualify

• Northeast Delta Dental must have paid a claim for either an oral exam or a cleaning during a Calendar Year. If enrollees don’t receive one of these services, they will not be eligible to accumulate dollars for future use. This puts an emphasis on prevention.

• An enrollee’s paid claims during the Calendar Year cannot exceed a threshold of $500.

Guidelines

• Your dental plan must have an annual maximum based on a Calendar Year (January through December).

• Accumulated amounts may only be used after the current annual maximum has been exhausted.

• Carryover benefit amounts cannot be used toward lifetime orthodontic benefits.

• The total accumulated carryover amount cannot exceed the amount of your plan’s regular annual maximum.

• Because your group is effective in the carryover program between January 1 and June 30, enrollees will be eligible to begin qualifying immediately for carryover benefit dollars to be used in the following year.

How it Works

The Double-Up Max feature allows enrollees to accumulate additional benefit amounts to use toward future dental expenses. Members who have a paid claim for an oral exam or cleaning during the Calendar year and do not exceed the $500 claims threshold are eligible to receive an additional $250 added to their annual maximum for the following year, up to twice the plan’s base, annual maximum.

Vision Insurance

Carlisle offers a voluntary vision plan through Humana. Employees working 30 or more hours per week are eligible on the first of the month following 60 days of employment. The plan covers frames, lenses and contact lenses. (An annual vision exam is offered through your medical plan with an office visit copay).

Life, AD&D, and Disability Insurance can play an important role in reducing financial stress when a family is faced with the pre-mature death or disability of a wage-earner. To help employees during critical times of need Carlisle provides employer paid Group Life Insurance and employee-paid, Voluntary Life Insurance. Please see plan certificates for detailed information.

Basic Life AD&D Insurance

(100% Employer Paid)

➢ Eligibility is the first of the month following 60 days of employment for employees who work 30 hours or more per week

➢ Eligible employees are automatically enrolled in Basic Life & AD&D Insurance

➢ Coverage is a flat $30,000

➢ Benefits are reduced by 65% at age 65 and 50% at age 70

Voluntary Life Insurance

(100% Employee Paid)

➢ Eligibility is the first of the month, following 60 days of employment for employees who work 30 hours or more per week.

➢ Benefit amount is in units of $10,000

➢ Guaranteed coverage amount of $150,000

➢ Maximum coverage amount of lesser of 5x salary or $500,000

Spouse:

➢ Benefit amount is in units of $5,000

➢ Guaranteed coverage amount of $30,000

➢ Maximum coverage amount of $100,000, not to exceed 50% of employee’s elected benefit amount

Unmarried, Dependent Children

➢ Benefit amount is in units of $1,000

➢ Maximum coverage amount is $10,000

➢ From birth to 6 months of age it is $250

Disability Insurance

Disability Insurance can play an important role in reducing financial stress when a family is faced with the disability of a wage-earner. To help employees during critical times of need Carlisle provides employer paid Short-Term and Long-Term Disability.

Short Term Disability Insurance (STD)

(100% Employer Paid)

➢ Eligibility is the first of the month following 60 days of employment for employees who work 30 hours or more per week

➢ Eligible employees are automatically enrolled in Short Term Disability Insurance

➢ Coverage is 66.67% of pre-disability weekly earnings to a maximum of $950 per week

➢ Benefits begin on the 1st day of a disabling injury/accident and on the 8th day of a disabling illness

➢ The maximum benefit period is up to 26 weeks

Long Term Disability Insurance (LTD)

(100% Employer Paid)

➢ Eligibility is the first of the month following 60 days of employment for employees who work 30 hours or more per week

➢ Eligible employees are automatically enrolled in Long Term Disability Insurance

➢ Coverage is 60% of pre-disability monthly earnings to a maximum of $7,500 per month

➢ Benefits begin following the 180th day of disability

➢ The maximum benefit period is the duration of the disability up to age 65 or SSNRA

Employee Assistance Program

Carlisle employees have access to an Employee Assistance Program (EAP) and Wellness Support through New York Life. The New York Life EAP is a free, confidential service, designed to help employees reduce stress, balance their work and family responsibilities and improve the quality of their lives. The program consists of resources and referral services, up to three face-to-face problem resolution sessions, telephonic counseling and support services, online information and interactive tools. All services are free, confidential, accessible 24 hours a day, 365 days a year, and available to all members of your household.

Employee Assistance Program

Are you feeling overwhelmed by the demands of balancing work and family life? Maybe you have questions about a legal or financial concern. You and your family members now have access to various counseling services including legal, financial, and work-life balance assistance. All counseling calls are answered by a Master’s or PhD-level counselor who will collect some general information and will discuss your needs. The Employee Assistance Program provides a maximum of three in-person or virtual sessions, per issue, per year.

Guidance Resources

When you need information quickly to help handle life’s challenges, you can visit guidanceresources.com for resources and tools on topics such as health and wellness, legal regulations, family and relationships, work and education, money and investments, and home and auto. You will also have access to articles, podcasts, videos, slideshows, on-demand trainings and “Ask the Expert” which provides personal responses to your questions.

Well-being Coaching

Sometimes you may need help with personal challenges and physical issues that can be overwhelming. To help you achieve your goals, you will have access to a certified coach who will work with you, one on one, to address health and well-being issues such as burnout, time management and coping with stress. You have access to five sessions per year. All sessions are conducted telephonically.

Family Source

Managing the everyday concerns of home, work and family can be difficult. To help resolve those concerns, you have access to family care service specialists that provide customized research, educational materials and prescreened referrals for childcare, adoption, elder care, education, and pet care.

Colonial Supplemental Benefits

Carlisle offers the following Voluntary Benefits through Colonial Life Insurance: Term Life, Whole Life, Voluntary STD, Cancer and Accident. Employees pay the full cost of these benefits, and the premiums are payroll deducted. Term Life, Whole Life and Voluntary STD are post-tax, while Cancer and Accident are pre-tax. Accident Insurance

Accidents come with costs, such as emergency room fees, doctor’s bills and lost income from missing work. Deductibles and co-pays add up. With Accident Insurance you can receive benefits to help with expenses of covered accidents. This insurance covers injuries that happen on or off of the job. A set amount is payable based on the injury you suffer and the treatment you receive. Coverage options are available for you, your spouse and your eligible dependent children.

Cancer Insurance

A cancer diagnosis could lead to expenses that medical insurance doesn’t cover. You may need to pay for out of-network treatment, childcare, home health care services and other indirect treatment and recovery costs. You are paid regardless of any insurance you may have with other companies. You can use benefits to help pay for travel to and from treatment centers, lodging, meals, deductibles – it is your choice. You may have the option of purchasing additional riders for more financial protection. Coverage options are available for you, your spouse and your eligible dependent children.

Short-Term Disability Insurance

If a covered accident or sickness prevents you from earning a paycheck, Short-Term Disability insurance can provide a monthly benefit to help cover ongoing expenses. You can choose the amount of your disability benefits subject to income. You are paid regardless of any insurance you have with other companies. Benefits are paid to you and you can use these benefits however you choose.

Term Life Insurance

Term Life Insurance provides protection for a specified period of time, typically offering the greatest amount of coverage for the lowest initial premium. This makes term life insurance a good choice for supplementing cash value coverage during life stages when obligations are higher, such as while children are young. A beneficiary can receive a benefit that is typically free from income tax. The policy’s accelerated death benefit can pay a percentage of the death benefit if the covered person is diagnosed with a terminal illness. The policy can be converted to a Colonial Life cash value insurance plan, with no proof of good health, to age 75.

Additional Benefits

Whole Life Insurance (Offered through Colonial)

Whole Life Insurance provides guaranteed cash value accumulation, level premium and a death benefit (minus any loans and loan interest). This coverage can help protect your family’s way of life. A beneficiary can receive a benefit that is typically free from income tax. You have three opportunities to purchase additional coverage with no proof of good health required if you are 55 or younger when you initially purchase coverage. The policy’s accelerated death benefit can pay a percentage of the death benefit if the covered person is diagnosed with a terminal illness. A $3,000 immediate claim payment can be paid to the designated beneficiary as an advance of the death benefit.

To qualify for holiday pay, you must work the scheduled workday immediately before and after the holiday. Only excused absences and/or planned absences will be considered exceptions to this policy. You are not eligible to receive holiday pay when you are on a leave of absence.

401K Retirement/Savings Plan & PTO

Employees are offered the opportunity to make voluntary, pre-tax contributions to this retirement plan. Employees are eligible for the retirement plan on the first of the month following 90 days of employment. Employees must be 21 years of age on their eligibility date. Carlisle will match every dollar for dollar up to 3% of gross annual income.

Should a new employee of Carlisle Wide Plank Floors join us with a 401(k)-retirement plan from a previous employer, they are immediately eligible to roll it over into this plan. Employees will not be able to continue contributions until meeting the above eligibility requirements, however the rollover can happen prior to this time. Paid Time Off

Time away from work is an important element of an employee’s benefit package. All regular fulltime employees, after completing the probationary period of 90 days of employment, are eligible to use paid time off (PTO). It is important to note that your PTO bank is to be used for vacation, personal and sick time as Carlisle does not have a separate sick time policy. The accumulation of PTO is based on years of service as illustrated by the chart below:

Each employee will accumulate hours of paid time off (PTO) each week they work. The PTO accumulation is put into an individual PTO bank. In addition to accumulated PTO, each employee will be awarded 8 hours for a “Floating Holiday” which will be added to the employee’s PTO bank at the beginning of each year. PTO can be used at any time during the year (in 4-hour increments), when the employee follows the formal approval process and works within the guidelines of his/her individual department. Only time accrued, and positive balances can be used for PTO, negative balances are not allowed. PTO cannot be cashed in for payment. An employee continues to accrue PTO up to the cap set in the above chart.
