ONE55 HEALTH & FITNESS 5 DAY TRIAL PASS TERMS AND CONDITIONS EFFECTIVE: October 2023
1. The 5 Day Trial Pass Agreement is limited to local residents 16 years and older within a 10km radius only (proof of residency required or NSW Drivers Licence mandatory);* 2. All 5 Day Trial Pass Holders must consent to completing a Pre-Exercise Questionnaire and provide an appropriate form of photo identification at the time of redemption. At the discretion of the Company staff, a doctor’s certificate may be required prior to commencement of the 5 Day Trial Pass; 3. The trial pass once activated is valid for 5 consecutive days commencing and including the date the pass is redeemed; 4. A 5 Day Trial Pass entitles the Holder to unlimited use of the One55 gym floor, group fitness classes & SGAC Aquatic Centre; 5. A 5 Day Trial pass is for first time users only, not current or ex- members or any trial pass holders within the previous 12 month period; 6. Pool lane availability is subject to demand and lane space is not guaranteed; 7. This 5 Day Trial Pass does not entitle the Holder to any additional services such as Personal Training sessions, Aqua Aerobic classes and/or the Company’s promotional products; 8. The 5 Day Trial Pass once expired at the end of its term, access to One55 and SGAC pool will cease unless a One55 membership agreement is entered into; 9. A 5 Day Trial Pass cannot be used in conjunction with any other offer; 10. A 5 Day Trial Pass cannot be redeemed for cash, sold or transferred to other people; 11. A 5 Day Trial Pass is valued at $100.00; 12. 5 Day Trial Pass Holders must adhere to the Company’s policies and procedures (including the Code of Conduct) and follow all reasonable directions of the Company’s employees. The Company may terminate this Agreement as a result of a breach by the Holder of any terms of this Agreement or any policy or procedure (including the Code of Conduct) of the Company; 13. This Agreement entitles the 5 Day Trial Pass Holder to use the equipment and facilities of the Company in accordance with the terms and conditions contained in this Agreement. Failure to use the equipment and facilities does not relieve the member from any other obligations in this Agreement; 14. The Company may cancel a 5 Day Trial Pass or refuse access to its facilities if the Holder displays inappropriate behaviour towards employees, other members or visitors. Inappropriate behaviour may include but is not limited to bullying, harassment, foul language and aggression; 15. A 5 Day Trial pass holder may be refused entry if suspected under the influence of drugs, alcohol or effect of any medications that make use of equipment dangerously 16. The Company reserves the right to refuse the issue of a 5 Day Trial Pass; 17. These terms and conditions (including operating hours) may be varied by the company, at its discretion, which will notify the members of any changes; 18. The Company may contact you (including by email, SMS, telephone or such other means) for marketing purposes. *NOTE: This Agreement must be signed by the parent or legal guardian (referred to as the 5 Day Trial Pass Holder unless stipulated otherwise). In doing so, the parent or guardian warrants that he/she is duly authorised to enter into this Agreement and is responsible for the behaviour of the minor and all obligations under this Agreement. If the Holder is under 18 years of age, the ‘Under 18 years Rules’ need to be signed by the Holder and his or her Guardian.
