The most frequently diagnosed cancers

C

ertain cancers are diagnosed far more frequently than others. But cancers that are diagnosed most often are not necessarily the most deadly cancers, nor are they always the most treatable.

They simply represent those cancers with the highest prevalence.

According to data from the American Cancer Society and the National Cancer Institute, excluding nonmelanoma skin cancers, the following cancers are those diagnosed with the greatest frequency.

Breast cancer: Topping the list in the

number of diagnoses is breast cancer.

An estimated 254,000 new cases occur each year. However, breast cancer that is caught early tends to be highly treatable.

Lung cancer: Lung cancer, including cancer of the bronchus, accounts for roughly 223,000 diagnoses each year.

The fatality rate for lung cancer is much higher than it is for breast cancer, with an estimated 156,000 deaths from lung cancer each year.

Prostate cancer: Prostate cancer only affects men, making the number

of cases even more striking. Prostate cancer diagnoses equal around 160,000 new cases annually. However, since prostate cancer grows slowly, treatment is often successful.

• Colorectal cancer: Estimates suggest new cases of colon cancer and rectal cancer will equal 135,000 in 2017, making cancers of the lower digestive system quite common.

• Melanoma: One of the deadliest forms of skin cancer, melanoma accounts for roughly 87,000 new cancer diagnoses each year. ■

‘Let the Sunshine In,” but carefully

love the sun. It warms my heart and soul especially after a long spell of rainy days. But, just like everything else in this world of ours, too much of a good thing can lead to unexpected consequences.

We all know how “healthy” a young sunworshiper looks towards the end of summer.

Their skin a deep bronze in comparison to a lily white-skinned indoor dweller who avoids the sun at all costs. What we forget is how the

sun-lover’s skin will look and feel in 20 to 30 years. We have all seen it, the dark, leather like skin that lacks luster and vibrancy because of overdoses of time in the sun.

The primary cause of skin cancer is exposure to ultraviolet (UV) radiation in sunlight. Frequent sun exposure and sunburn in childhood can cause irreversible damage that can lead to skin cancer later in life. A blistering sunburn in the younger years is

a risk factor for the development of malignant melanomas. More than 90 percent of non-melanoma skin cancers occur in fair-skinned people, who tend to sunburn. Dark-skinned people have a lower risk of skin cancer but they are still susceptible to the damaging effects of UV radiation, especially on the eye and immune system.

In the U.S. alone, each year over 3.5 million cases of skin cancer are diagnosed. How do we reduce the risk of getting skin cancer? Since its inception in 1979, The Skin Cancer Foundation has recommended using a sunscreen with an SPF 15 or higher.

Unfortunately, sunscreen alone is not enough. It is wise to keep the following information in mind in reference to protecting ourselves from UV radiation: seek the shade, especially between 10 a.m. and 4 p.m., do not burn, and avoid tanning and UV tanning booths. When using your daily sunscreen apply 1 ounce (2 tablespoons) to your entire body 30 minutes before going outside. Reapply every two hours. Cover up with clothing, including a broad-brimmed hat and UV-blocking sunglasses, and keep in mind that a cotton shirt will offer only an SPF 8. Other preventive measures include examining your skin from head-to-toe every month and seeing your physician every year for a skin examination.

When venturing out into the sun, do so with a defensive attitude. Wearing

the right kind of clothing will offer more protection than sunscreens.

Cloths made of tightly woven fabric are the best protectors of the skin.

If you hold your clothes up to a light and can see though the fabric, UV radiation can also pass through.

Darker-colored and brightly colored fabrics, i.e. oranges and reds, are more sun-protective than pastel or pale ones. Try using looser-fitting clothes as they allow less light to pass through than tight clothes that stretch a lot.

In today’s market, it is possible to buy UV-absorbing clothes, from swimsuits and shirts to hats and pants.

This clothing will usually have a high Ultraviolet Protection Factor (UPF) rating, indicating how much UVR it absorbs. A fabric with a 50 UPF allows only 1/50th of the sun’s UVR to pass through. You can also sun-proof your own clothing. Look for household laundry products containing special UV-absorbing agents that allow you to launder UV protection into garments.

They can raise the UPF of a white cotton T-shirt from approximately 5-8 to as high as 30.

Sunscreens are chemical agents that help prevent the sun’s ultraviolet (UV) radiation from reaching the skin.

For women, cosmetics containing sunscreen are fine for incidental everyday exposure, but when the intention is to spend a long time on the beach, a separate, durable, waterresistant sunscreen is required. In addition to an SPF of 15 or higher

— which guarantees good UVB protection — look for ingredients such as avobenzone, oxybenzone, MexorylTM, zinc oxide and titanium dioxide. These compounds are ideal against UVA. Melanoma is the most dangerous of skin cancers, yet if caught early it is almost 100-percent curable. For many years, the early warning signs of melanoma have been identified by the acronym “ABCDE” (A stands for Asymmetry — or an uneven shape, B stands for Border — irregular is bad, C for Color — pearly, brown/black, or multicolored, D for Diameter — bigger than a pencil eraser, and E for Evolving or changing. Recently, a new method of sight detection for skin lesions which could be melanoma has been developed. It is based on the concept that developing melanomas look different — i.e., “the ugly duckling” — compared to surrounding moles. Thus, during skin self-examination, we should be looking for lesions that manifest the ABCDE’s and for lesions that look different compared to surrounding moles.

We are all free to “Let the Sunshine In,” but let’s be smart about it so that we may enjoy many coming years with family and friends as we take to the outdoors in our work or leisure.

Navigating diagnosis Vidant Cancer Care Navigators offer stability in difficult times

or anyone, a cancer diagnosis can come as a devastating, life-altering event. With such a diagnosis come many questions, concerns and fears. From worries about affordability, prescriptions and insurance to transportation needs and organizing appointments, the process of dealing with a diagnosis can be nothing short of overwhelming.

As a way of making the process easier to manage, every patient at Vidant Cancer Care has access to a cancer care navigator, an individual who works as a liaison to help handle all of the details of one’s treatment that might fall outside the duties of a doctor or a nurse.

“Knowing that there is one person you can call, it keeps them from having to call five different numbers and be bounced around, like we’ve all experienced in certain situations,” Social Work Navigator Judy Humphries said. “When you’re dealing with cancer and you’re scared to death, the less complicated things can be, the better.”

According to Danielle White, a registered nurse at the Marion L. Shepard Cancer Center, having a nurse and a navigator working in tandem ultimately serves to ensure better overall care for the patient.

“Although Navigators can have different roles at different facilities they share common goals striving to provide education, psychosocial support, access to resources and are a liaison between providers and other members of the multi-disciplinary team,” White wrote in a description of the navigator program.

diagnosis to end of life. Navigators have been found to play a huge role in the transition to end of life care.”

Originally pioneered in 1990 at Harlem Hospital in New York, navigation has been utilized at Vidant Beaufort Hospital since 2013. While case management has always played a role in the care provided, the implementation of navigation has allowed the hospital system to take

patient care to a new level.

“We can help patients with issues that they might usually be talking to the doctors and nurses about,” Humphries said. “We can take that away from them and give them more time to focus on the clinical issues. We all have our piece.”

With navigators throughout the Vidant system, when a patient needs to go to a new facility, the communication

between navigators becomes key.

Sharing information between facilities allows patients to experience a better continuity of care, ensuring that they don’t have to start from scratch at a new facility.

“We’re able to provide care close to home, which is our goal, but to have the resources of the bigger system when we need that,” Vidant Marketing Director Pam Schadle said. ■

Lifestyle choices that can decrease cancer risk

T

he United Nations estimates that the world is home to 7.6 billion people. None of those people, regardless of their ethnicity, race or gender, are immune to cancer.

According to the World Health Organization, cancer is the secondleading cause of death across the globe.

Responsible for nearly nine million deaths in 2015, cancer is a formidable foe but one that can be defeated. For example, the SEER Cancer Statistics Review released in spring of 2018 indicated that cancer death rates among women decreased by 1.4 percent between 2006 and 2015. Even more encouraging is the decline in breast cancer death rates, which the American Cancer Society says decreased by 39 percent between 1989 and 2015.

Various factors have contributed to the decline in cancer death rates. While advancements in treatment protocols and education programs that have emphasized the importance of cancer screenings have had profound impacts, individuals avoiding unhealthy lifestyle choices also has made a difference in reversing cancer death rates. Research into cancer prevention is ongoing, but the following are some healthy lifestyle choices that may help people reduce

their cancer risk.

• Avoid tobacco. T he U.S. Department of Health and Human Services reports that cigarette smoking kills almost half a million people in America alone each year. Nearly 10 percent of those deaths occur due to exposure to secondhand smoke.

Lung cancer is not the only type of cancer that smoking has been linked to, as the Mayo Clinic notes smoking also has been connected to cancers of the mouth, throat, larynx, pancreas, bladder, cervix, and kidney. By avoiding or quitting tobacco, men and women can reduce their own cancer risk and reduce the likelihood that their nonsmoking loved ones will develop cancer related to secondhand smoke.

• Eat right. The WHO notes that many countries have implemented programs encouraging the consumption of five or more portions of fruits and

vegetables per day. Those programs are a result of studies showing the potential relationship between a healthy diet and a reduced risk of cancer. For example, the National Cancer Institute says studies conducted on animals have shown that antioxidants, which can be found in various fruits and vegetables, can prevent the type of cell damage associated with the development of cancer.

• Protect skin from the sun. In 2018, the American Cancer Society notes that more people are diagnosed with skin cancer in the United States each year than all other cancers combined. While overexposure to the sun is not the only way a person can develop skin cancer, protecting skin from the sun is a great way for people to significantly reduce their risk for the disease. The Mayo Clinic recommends people avoid the sun between 10 a.m. and 4 p.m, when

the sun’s rays are strongest. People also should apply and reapply generous amounts of sunscreen when spending time outdoors.

• Speak with a physician about immunizations. The Mayo Clinic notes that protecting oneself against certain viral infections also may help men and women reduce their cancer risk. For example, hepatitis B has been linked to liver cancer. Certain people, including those who are sexually active but not monogamous, intravenous drug users and health care or public safety workers who may be exposed to infected blood or body fluids, are at greater risk of hepatitis B than others, and such people should speak with their physicians about being vaccinated.

Cancer can affect both healthy and unhealthy people. But certain cancer prevention strategies may help men and women lower their cancer risk. ■

Determining breast cancer stage

W

hen receiving treatment for breast cancer, women will learn about cancer staging. According to the nonprofit organization Breastcancer.org, determining the stage of the cancer helps patients and their doctors figure out the prognosis, develop a treatment plan and even decide if clinical trials are a valid option.

Typically expressed as a number on a scale of 0 through IV, breast cancer stage is determined after careful consideration of a host of factors. The staging system, sometimes referred to as the TNM system, is overseen by the American Joint Committee on Cancer and ensures that all instances of

breast cancer are described in a uniform way. This helps to compare treatment results and gives doctors and patients a better understanding of breast cancer and the ways to treat it.

Breastcancer.org notes that the TNM system was updated in 2018, but before then was based on three clinical characteristics: • T: the size of the tumor and whether or not it has grown into nearby tissue • N: whether the cancer is present in the lymph nodes • M: whether the cancer has metastasized, or spread to others parts of the body beyond the breast While each of those factors is still considered when

determining breast cancer stage, starting in 2018, the AJCC added additional characteristics to its staging guidelines, which make staging more complex but also more accurate.

• Tumor grade: This is a measurement of how much the cancer cells look like normal cells.

• Estrogen- and progesterone-receptor status: This indicates if the cancer cells have receptors for the hormones estrogen and progesterone. If cancer cells are deemed estrogen-receptor-positive, then they may receive signals from estrogen that promote their growth. Similarly, those deemed progesterone-receptor-positive may receive signals from progesterone that could promote their growth. Testing for hormone receptors, which roughly two out of three breast cancers are positive for, helps doctors determine if the cancer will respond to hormonal therapy or other treatments. Hormone-receptor-positive cancers may be treatable with medications that reduce hormone production or block hormones from supporting the growth and function of cancer cells.

• HER2 status: This helps doctors determine if the cancer cells are making too much of the HER2 protein.

HER2 proteins are receptors on breast cells made by the HER2 gene. In about 25 percent of breast cancers, the HER2 gene makes too many copies of itself, and these extra genes ultimately make breast cells grow and divide in ways that are uncontrollable. HER2-positive breast cancers are more likely to spread and return than those that are HER2-negative.

• Oncotype DX score: The oncotype DX score helps doctors determine a woman’s risk of early-stage, estrogenreceptor positive breast cancer recurring and how likely she is to benefit from post-surgery chemotherapy. In addition, the score helps doctors figure out if a woman is at risk of ductal carcinoma in situ recurring and/or at risk for a new invasive cancer developing in the same breast.

The score also helps doctors figure out if such women will benefit from radiation therapy or DCIS surgery.

Determining breast cancer stage is a complex process, but one that can help doctors develop the most effective course of treatment. More information is available at www.breastcancer.org. ■
