System Level Measures (SLM) Key purpose/benefit The SLM framework has been developed nationally and reflected within the PHO Service Agreement. Those expectations are also mirrored in your Practice Provider Agreements. Local activities support the achievement of whole of system objectives, captured within the 10 year BOP Strategic Health Services Plan (SHSP). Those objectives, in part, are also reflected in the BOPDHB Annual Plan. General Practice activities contribute towards specific quality improvement measures to improve health outcomes of our population by working collaboratively with our health system partners, i.e. the DHB, hospital and community services providers. The methodology is based on the principles of: ▪

Decisions based on evidence/data; and
▪
Prevention and early intervention approaches to health
Impact of Covid-19 The current pandemic situation means we will need to remain vigilant. This means we plan to focus our efforts on fewer actions for this years’ SLM Plan, ensure that our actions are linked with strategic efforts, and that we do fewer things well.
The System Level Measures The following six measures are areas of New Zealand’s healthcare system that the MOH have targeted for improvement. Each measure has a local Bay of Plenty milestone for 2021 – 2022. Primary health providers have activities referred to as Contributory Measures, to contribute to achieving these national, health industry outcomes. The System Level Measures listed below are shown in the SLM Framework chart, three pages over. The activities to be undertaken by general practices and the WBOPPHO to contribute to the SLM measures are listed in the table, four pages over. Updated October 2021
