S Smoking Cessation - General Practice Service Delivery Key purpose / benefit This programme supports the goal to achieve a SmokeFree Aotearoa by 2025 and improvement in population health. Funding is to enable a general practice clinician to invest in clinical resources to:
Start patients on the journey to stop smoking
Support setting a quit date and then ongoing support to quit (approximately 4 contacts)
Motivate the patient to try again if this quit attempt not successful **Setting a quit date is preferred but NOT mandatory as the programme supports the journey to becoming ready to quit as well as quit attempts**
Mandatory requirements
Initial extended smoking cessation consultation which MUST: o
Be face to face or phone
o
Include completing “Agreed Quit Plan” with the patient
o
Record of plan in PMS & give a copy to the patient
Agreed Quit Plan MUST include: o
Date for next 2-way contact point (text, phone or face to face) Objective to achieve 3 – 4 contacts in total

At least one goal
o
Non-Medtech Practices: Manually complete a form for Quit Plan
o
MedTech practices: Use Outbox document for Quit Plan, the plan template can also be accessed via the Halcyon Smoking Cessation claim form.
