Practice Capacity Survey - Covid Care in the Community Kia ora
This survey has been developed to gauge Practice Capacity as the discussions around management of Covid-19 cases in the community progress at a local and national level.
We appreciate your honest feedback and support in completing this form. It should take approx. 5 to 10 mins to complete.

Please note - the community care model has not been finalised, but from initial information shared it appears the final model will involve GP’s working alongside Public Health and social welfare providers and leading the clinical delivery of care part of the community isolation programme via telehealth, and that there will be funding available, details yet to be released.
Please note - The DHB has a supply of pulse oximeters to support this community care model.

This survey is being circulated to all practices across the region, and feedback can be submitted by a variety of staff - Practice Managers, Clinical Leads etc. We want to ensure primary care are represented in the conversations happening in this space.
Finally, we want to acknowledge that some of questions in the survey request information that the PHO already has. These questions are included because the survey is collecting data for the DHB and will ensure all key workforce information is compiled in one data set, thank you in anticipation for your responses.
