31 December 2021 Dear Primary Health Team Re: Waipuna Hospice Syringe Driver Trial You may recall that Waipuna Hospice identified opportunity for improved service delivery following COVID-19 lockdown, level 4 in 2020 that involved a syringe driver trial. The ‘syringe driver trial’ has now been completed and evaluated. It was an overwhelming success both in terms of medication safety (aseptic preparation, improved labelling of medications, and longer shelf life of pre-drawn medications) and in terms of nursing staff able to concentrate on patient-centred care as opposed to medication preparation tasks. Waipuna Hospice is now in the process of implementing this new standard of care. What next to further improved service delivery: A shared medication management electronic platform Waipuna Hospice would like to enhance the shared care model for palliative patients in the community by using a shared electronic platform. This allows all parties to have clear visibility, with access to up to date information therefore reducing risk and improving overall patient care. The electronic platform currently in use at Waipuna Hospice is Medi-Map and we are aware that many local GP’s are also registered users. Medi-Map is a web-based system that you can access from any computer or device that has internet access. From the browser go to www.medi-map.co.nz, enter your username and password. If you are not a Medi-Map user you will need to register as a prescriber, please follow the instructions following:
Go to the website to www.medimap.co.nz Click on ‘Register Now’ Fill in the details on the secure form and submit with Medi-Map You will receive an email in response with login details and a user guide
How does this impact you? The impact is minimal from a prescriber perspective. Should your hospice patient need predrawn anticipatory subcutaneous medications or a continuous subcutaneous infusion (CSCI/Syringe Driver) please follow these steps: 1. Login to Medi-Map (www.medi-map.co.nz) and add/locate the patient (follow the Medi-Map user guide if unsure on this process and/or the steps below). 2. Ensure that ‘Unichem Metro Pharmacy Bethlehem’ is the selected pharmacy 3. Add/edit ‘syringe driver’ and/or ‘PRN medications’ as appropriate 4. Generate eScripts using Medi-Map 5. Send the eScripts (or normal prescriptions) to Unichem Metro Pharmacy Bethlehem 6. Notify our community hospice team by phoning the area CNS at Waipuna Hospice.
