Update #2 18 August 2022 Nau mai haere mai to the Interim EBOP Palliative Care Response Team Update. This regular update is for healthcare workers supporting Eastern Bay palliative care patients and their whānau.
Recap The interim Palliative Care Response Team has been set up within Regional Community Services, to improve access to care for palliative patients with a life limiting illness and their whānau in the Eastern Bay of Plenty community. The Team is working in partnership with Hospice EBOP, meeting weekly to identify additional support to meet patient needs.
Referral Criteria for Specialist Palliative Care Services Patients needing Specialist Palliative Care services can be referred to Hospice EBOP (Primary Care, Outpatients, ED) or the Palliative Care Response Team (secondary care inpatient). Not all people with life limiting illnesses will need the services of Specialist Palliative Care from Hospice EBOP or the Palliative Care Response Team. However, patients who do not need specialist palliative care, may still need support at home for services such as home help or equipment. This can be accessed through a referral to the Bay of Plenty Community Care Coordination (CCC) service via your existing referral pathways (BPAC/WebPAS).
