Diabetes Insulin Starts Key Purpose/Benefit This a practice led service providing the necessary structure to meet the more short-term intensive time demand required to successfully initiate insulin therapy for patients with Type 2 Diabetes. The insulin start package includes six contacts with the patient to deliver education and advice in order to manage the process safely.
The package can be provided by General Practitioners and/or registered nurses that have completed WBOP PHO approved Insulin Therapy education within the last 3 years See Section D, page 8.
The Diabetes Insulin Starts package is to include the following patient services:
Full assessment of patient dietary habits and lifestyle routine
Identification of the starting insulin type, dose and site selection
Subsequent insulin adjustments
Demonstration and supervision of insulin injection and blood glucose monitoring technique
Assessment of patient injection technique and blood glucose monitoring technique
Discussion regarding hypo glycaemia, signs, symptoms treatment and causes
Discussion regarding the importance and implications of site rotation
Education regarding insulin storage and correct constitution
Education regarding any legal implications of insulin therapy such as driving; sharps disposal etc.
Specific advice regarding the relevance of HbA1c tests, interpretation of this test and identification of individually agreed patient targets.
