Diabetes Care Improvement Programme (DCIP) Key purpose/benefit The funding is for general practice usage to support additional patient consultations which must be free of charge to the patient (via phone and face to face).

Eligibility criteria 

Enrolled & funded in the WBOP PHO

Diabetes Annual Review (DAR) within the previous 12 months

Current HbA1c result greater than 75mmol/mol

Referral to the Diabetes Self-Management Group Education

1 claim only per patient will be funded across the WBOP PHO every 12 months

A follow up diabetes assessment is required in 12 months. HbA1c will be monitored over the 12-month period.

Claim Process All PMS’s Claim using the Halcyon form, instructions on “How to Access Halcyon” from all Practice Management Systems are under H in this PHO Guide.  To start a DCIP claim click “New Registration” under ‘Current Programmes’ or ‘Other Options’ tab.  The following tabs are mandatory to complete: Patient, Provider, and Registration Details
