D Diabetes Annual Review (DAR) Service Description A diabetes assessment will involve conducting a thorough assessment that establishes the health status of each person, risk of deterioration and of developing diabetes related complications, and their level of need. Service delivery will meet the Ministry of Health’s Quality Standards for Diabetes Care which includes (but is not limited to) the following:
Basic care, self-management and education, e.g. smoking cessation advice, nutrition and physical activity advice, and patient participation in the development of their careplan
Management of diabetes for cardiovascular risk
Management of diabetes complications, e.g. retinal, renal, feet
Timely referral/access to expert/specialist providers for those with serious or progressive complications
Eligibility Criteria Patients enrolled and funded with a WBOP PHO general practice who have been diagnosed with Type 1 or Type 2 diabetes, excluding gestational diabetes or Impaired Glucose Tolerance.
DAR claims will be accepted 11 months from the date of the last funded DAR.
1 claim only per patient will be funded across the WBOP PHO every 12 months

Patient, Provider, Review Details, Feet and Eyes.
