Connect and Navigate Health Projects
Background
Te Whatu Ora, Hauora a Toi Bay of Plenty (formerly the Bay of Plenty District Health Board) was provided funding in 2022 to support the community to build resilience due to the impacts of Covid-19. Between July and November, iwi, hapū, hauora and community services were approached and invited to put forward expressions of interest for projects which would meet the additional needs of their communities.
Who do the projects help?
Supporting whānau who don’t have a GP or health provider to enrol or select a health provider
Unenrolled Unengaged
Supporting whānau who have a GP or health provider but don’t come in when they need support with their health, to work towards their health goals
Whānau who have a GP or health provider, who have a health need but may not have received support in a while, due to COVID-19
What are the projects doing?
25 community projects have been funded across the rohe with a broad range of services provided. Many of the projects focus on supporting the community to navigate the health system to meet their health needs. This could be through navigator roles, wananga, whānau health days, kai resilience, improved housing, rongoa and more. Most of the projects are set outside of a general practice environment, utilising hauora and other community services but many involve partnerships with general practice.
Amongst the many aims of the projects is to support whānau to enrol with a general practice.
The projects are short term, set to run until June 2023.
