Clinical guidance for using the ‘Chart Review and contact’ claim type Claim description: Chart review and contact (via text, email, patient portal) for patients that are higher risk but do not require a full initial assessment. This new Chart review and contact service is a tool clinicians can use for higher risk patients where, following chart review where the GP/NP has identified some potential risk factors but doesn’t think that an immediate full clinical assessment is required. The text/email to the patient would suggest to the patient that if they’re feeling unwell and think they need assistance, they should get in touch with the practice to arrange a consultation. Through chart review and consideration of risk factors such as age, ethnicity, vaccination status and comorbidities, a clinician may choose to send a text/email/portal communication and a Halcyon claim for ‘Chart Review and Contact’ made. The purpose of this contact is to help the patient access information and encourage them to access health care if/when required. Suggested content that this text may contain: - If you have COVID-19 | Unite against COVID-19 (covid19.govt.nz) - Practice contact process/details for any health concerns, Healthline for afterhours. It is anticipated that this is a one-off claim at initial contact with the patient on notification of positive test. If the patient contacts the practice for health needs following this contact, then another claim can be utilised depending on the contact/ service/need of the patient, e.g., Initial clinical assessment, regular review. This is not a service to use if the Covid positive patient is undoubtedly low risk, as they will receive guidance from the text message sent via the national system. Chart review and contact does not cover bulk text messaging. If following the chart review the clinician decides to make phone contact with the patient to assess (rather than text email or contact via the patient portal) and finds the patient has no health or welfare needs, then an initial assessment can be claimed if undertaken.
