Introduction

Introduction This document overviews enhancements related to the Care in the Community (CitC) Release 22.05 across National Contact Tracing System (NCTS) and Covid Clinical Care Module (CCCM) (aka BCMS).

These enhancements will be released into production as part of the 22.05 Release on the evening of Wednesday 9 March 2022.

22.05 Release Notes

Change to Positive Text Notification to Cases Description What this means Wording to the positive text notification sent to Cases is being amended to encourage more people to engage with the COVID19 Contact Tracing Form and available supports.

Note: The change to Positive Text Notification to Cases went live, Tuesday 8 March 2022.

Updated wording for the positive text notification being sent is as follows: Kia ora {First Name},

You have COVID-19. You and your household need to isolate.

If your household needs support getting essentials such as food or medication do not get these yourself. Ask friends, whānau or neighbours to drop them off, or organise contactless delivery. If you do need extra welfare support, complete the short form or call 0800 512 337.

For most people the symptoms of COVID-19 will be mild.

If you need urgent medical care, call your local healthcare provider or Healthline on 0800 358 5453.

If you develop difficulty breathing, severe chest pain or feel faint call 111 immediately. Tell them you have COVID-19.

Self-serve Positive Case Contact Tracing Reminder Text Description

What this means

Text reminders will be sent to positive cases who have not completed their initial assessment either by selfserve contact tracing form or phone.

Note: The Self-serve Positive Case Contact Tracing Reminder Text went live, Tuesday 8 March 2022.

The text reminders will be sent at 12, 24 and 48 hours after their initial SMS to encourage the completion of the case self-serve contact tracing form.

The wording of this new reminder text is as follows:

Isolation Release Text Notification Description

What this means

An update has been made to enable an automatic Isolation Release Notification to be sent to positive Cases who are self-isolating once their required isolation period has been completed.

The wording of this new reminder text is as follows:

This notification will inform Cases that their isolation period has ended.

1.All Self Managed Cases + Actively Managed Cases already released in CCCM This Isolation Release text notification will be sent to Cases on Active Management where they have reached their predicted isolation end date and have been clinically released, and to Self Managed Cases.

Your COVID-19 isolation period has ended. If you feel well you can now return to all normal daily activities including work. If you continue to feel unwell please contact your local healthcare provider. For more information see https://covid19.health.nz/advice/positive 2. Actively Managed Cases not released in CCCM This Isolation Release text notification will be sent to Cases on Active Management where they have reached their predicted isolation end date and have not been clinically released.

Risk Score for Hub Call Prioritisation Description What this means The Risk Stratification tool uses information from the NHI data base to assign a number (Risk Score) to each Case record in the NCTS.

A population-based risk stratification tool (the Risk Score for Call Prioritisation, or, Risk Score for short) has been developed to help to identify who needs to be prioritised for contact for the first assessment which determines whether their COVID-19 care needs to be supported with active care management or the self-service pathway.

The Risk Score is also sent through and visible in the CCCM.

The risk score is a field in the back end of the NCTS that assigns a numeric score (0-1) to each case. A number closer to 1 is considered higher risk; a number closer to 0 is considered lower risk. ​

NCTS

This tool uses age, ethnicity and vaccination status data to inform risk of hospitalisation due to COVID-19. This tool will support Care Coordination Hubs by prioritising outreach from the hub to those wo have not responded to the initial text outreach to indicate that they are positive for COVID19. The Risk Score is separate to the Clinical Acuity Score and where an Acuity Score has been populated, this will override any use of the risk score.

In the NCTS the Risk Score can be used to prioritise patient call-backs if the Care Coordination Hubs have not had contact within 24 hours of their positive COVID-19 test result.

The Risk Score will display in CCCM as a Clinical Flag and be labeled Risk Score.

• From the full web version of CCCM, the Dashboard can be sorted by descending risk number, then Search & Filter the data to show your organisation and can be used to prioritise Initial Health Checks.​ • From CCCM via PMS, the Risk Score can be identified on the person banner or bubble list and can be used to prioritise Initial Health Checks.​ Please refer to our Risk Score for Call Prioritisation Training Materials HERE for further guidance.

Automated Case Closure (1/3) Description What this means Automatic NCTS case closure

To reduce the manual effort required to close NCTS Cases during a large outbreak, the following automations are being put into the system:

Automatically closing Case Follow-Up when the predicted isolation end date has been reached.

Automatically moving a COVID Contact Tracing Case through the pathway to the ‘Closed’ status.

For those in Self-Management this will automatically close the Border Health Record (BHR). For those in Active Management the Border Health Record will remain open until clinically released in CCCM, at which point they also automatically close.

This means Hub Teams with NCTS access are no longer be required to complete the Bulk Release process for Cases following Clinical Release from the CCCM.

Automated Case Closure (2/3) Description What this means As a result of the automated Case Closure in NCTS, new rules have been applied to the system that will enable CCCM Records on Active Management to be automatically removed from the CCCM where their predicted isolation end date is reached

1. A CCCM Record for a person on Self Management will automatically be removed from the CCCM once the predicted isolation end date has been reached. This means CCCM users are no longer required to manually release the Record from CCCM. 2. A CCCM Record for a person of Active Management will not be automatically removed from the CCCM once the predicted isolation end date has been reached.

This means CCCCM users should continue to manually release the Record via the Regular Health Check at the appropriate time.

Automated Case Closure (3/3) Description What this means As a result of the automated Case Closure in NCTS, new rules have been applied to the system that will enable CCCM Records on Active Management to be automatically removed from the CCCM where their predicted isolation end date is reached

In instances where a CCCM Record is required to be re-activated, the Border Health Record (BHR) will need to be re-activated in the NCTS for the record to appear in the CCCM again.

1. To re-active a BHR, a Hub NCTS user should change the Status from Closed to Under Management and select the Closed Status as None.

2. Scroll down and select Push to BCMS, this will ensure that the record is sent from NCTS to CCCM. To save your changes, select Save.

3. Refresh or open CCCM to view the re-activated CCCM Record.

Other Changes Description What this means Automatic DC Closure when DC becomes a Case

An update has been made to automatically close Disease Contact Records in the NCTS where the Disease Contact becomes a Case. Upon becoming a Case, Disease Contacts records will be automatically closed if the following conditions are met: • The Disease Contact is linked to a Person Profile that is then subsequently linked to a Case.

• The Case has been successfully sent an SMS confirming that they are a positive case.

• The Case is moved to any status other than ‘Registered’.

Cases in household bubbles

An update has been made to ensure a Case will be added to the same household bubble if they were already a Disease Contact of that household. This means that when a new Border Health Record is created, it will still be linked to the same household.
