Disability Question added to online Contact Tracing (Case Self-Serve) Form Description

What this means

A question aimed at identifying if a Case has a disability and will require additional support

The new disability question will appear in the “Your details” section of the Self-Serve Webform, it will be a Yes or No option.

Note: This question has been written by the Disability Directorate.

If the person (Case) chooses Yes, this will be reflected in both the NCTS and CCCM systems: • NCTS - on the Case record in the Risk Assessment & Underlying Conditions section.

It can be found on the Underlying Conditions field as 'Identify as having a disability'

• CCCM – using the 'Underlying Conditions in NCTS' button in the Initial Assessment this can be pulled into the Notes Timeline

The Underlying Condition field can be used to sort/ filter in NCTS Reports to help identify and prioritise these patients to ensure they receive a full primary care assessment to understand their needs

Accommodation Question added to online Contact Tracing (Case Self-Serve) Form Description

What this means

The Case SelfServe Webform will have questions added to ask about each Case’s accommodation

COVID-19 Care Coordination Hubs and housing agencies will have better visibility of Cases who may not be able to safely self-isolate at home These questions have been approved by Care in the Community teams

The accommodation question will be asked in the 'Your details' section of the Self-Serve Webform There is a Yes or No option which will then inform the survey to display the follow up details questions.

If the Case selects one of these options, it will appear in the NCTS on the Case record in the Needs and Compliance section

These fields can be added to reports that are used by SIQ coordinators, who are linked to hubs, to access these profiles and call the positive case to assess their additional housing needs.

Accessing data from the new accommodation questions Once the new accommodation questions are live in the Case Self-Serve Webform (refer to previous page), the data will display in the Needs & Compliance section on the case record in NCTS.

If you need to view bulk accommodation data for groups of cases, you will soon be able to access a new dedicated report on the ‘CinC Hub Operations Dashboard’ (expected to be live 22/03/22 am).

To access the CinC Hub Operations Dashboard, follow the below steps; 1.

Click on the dropdown menu on the Task bar Scroll and select the Dashboards option Click the “All Dashboards” option on the left hand menu Search this folder for the CinC Hub Operations Dashboard Click on the CinC Hub Operations Dashboard

Once the new report is created, an accommodation component will display on the Dashboard. Those with permission will be able to click on the report to access and filter case data.

This is the CinC Hub Operations Dashboard page. The new report (once created) will appear on this page when ready.

Note: For those who are already creating their own reports in NCTS, you will be able to create or edit your report to include the 2 new accommodation data fields.

This is an alternative way to access and manage the new accommodation data.
