Best Start Kowae – Gen 2040 Key purpose / benefit The Best Start Kōwae is part of the Gen2040 project aiming to increase health equity, starting before birth and continuing throughout the individuals (Mama and Pepi) lifespan. The suite of PMS integrated decision support assessment tools are used at significant points across the pregnancy and postnatal journey - Best Start pregnancy comprehensive early pregnancy assessment tool completed at confirmation of pregnancy - 16 week check – recall assessment for pertussis and flu vaccinations + referral support service & monitor engagement/utilisation with support services - 6 week Pēpī – well child check - 6 week Māmā – standardised wellbeing check for mum, opportunity to refer onto any support services needed
Service description Designed to support consistent, comprehensive and quality care by providing recommendations for early intervention and best practice; promoting enquiry, management and referral of māmā to LMC and support services. The assessment forms are intended to be nurse-led with GP support with functional inbuilt clinical decision support, extensive links to the appropriate Regional HealthPathways, as well as easy to use e-referrals.
