Confirm Swab is required Complete assessment process as per PHO/PHA process Generate Pathlab form for Covid-19 testing ** Essential step**

Check if there are any other people in the house who also need swabbing and repeat process for them if needed

Western BOP WBOPPHO practices e-referral to WBOPPHO Nursing Services Western BOP NMO network (including Pirirakau Hauora) Email patient details to carliza@nmo.org.nz Eastern BOP (including WBOPPHO practices) Email patient details and pathlab form to practice.liaison@ebpha.org.nz

Results
Respiratory Symptoms consistent with Covid-19 or Close contact of case and requires swab on guidance from Medical Officer of Health House bound and unable to attend Swab centre Not Type 1 or 2 Diabetes and/or taking short term steroids • Refer to Practice Nurse for enrollment on programme

Print Pathlab form (from email or direct from pathlab in PMS) Contact patient and confirm details, address Confirm house visit is needed Check safety issues re visit – dogs. If unsafe do not visit

Full PPE to be worn Don before entry, Doff outside of house and place in yellow bag Confirm Patient identity and Swab on doorstep if possible Other household members to wear mask, observe 2m distancing Deliver swab to nearest Pathlab collection point
