Dear Practice Managers, Please see the below and attached information on Changes to the Halcyon Covid-19 National Claim Form. To improve the functionality of the existing national COVID-19 form some changes have been made. These are expected to come into effect on Wednesday 1st September – there is no planned standdown period for this update, and there should be no impact on useability when the update is launched. If you experience any issues, please contact GPSAdmin@wboppho.org.nz or your GPL as soon as possible. The main changes are: •
Two new ‘Assessment Types’ have been added: -
Surveillance Assessment: To be used for ALL ASYMPTOMATIC swabs. This replaces the previous ‘workaround’ of asymptomatic swabs being claimed as ‘Virtual’ Any symptomatic ‘surveillance’ swabs must still be raised as Simple Assessment.
-
Drive-Through Assessment* *if a pt was seen as a ‘drive-through’ but required full assessment, ‘Full Assessment’ should still be selected. If there was no full assessment then select ‘drive-through’
•
New Dropdown boxes have been added for the following criteria: - Vaccination Status - Location of interest - Come out of MIQ within the last 14 days
•
Changed tick-box wording from: “Has travelled internationally in the last 14 days” to: “Has been in an area of known community transmission of COVID”
•
Changed tick-box wording from: “This patient meets HIS criteria and I have received approval for this complex claim through a conversation with the Medical Officer of Health […]” to: “This patient meets HIS Criteria and if required, I have notified the Medical Officer of Health. […]”. This change allows this box to be ticked in cases where MoOH Notification is NOT required, fixing the recent issue of auto-rejected claims.

The form will look different – the following fields have been moved to the bottom of the form: Fits MOH case definition - Fits Higher Index of Suspicion (HIS) - Swab Taken (and associated fields ie. Sample Date, Sample Time and Procedures)
