Please share this email with colleagues in your network who work with people with or at increased risk of developing diabetes. I’d like to invite you to join our BOP Diabetes Network. The idea is to connect all our local diabetes service providers and make sure that collectively our services are accessible, appropriate and timely .... (and effective!). In simple terms the network will consist of a wide-reaching network of providers across what is currently the BOP DHB region. There is no requirement in terms of commitments – this is all about communication. Above the network will sit a smaller group of key people (The Network Group) which will provide the conduit between those on the ground and the Ministry of Health/DHB via the Long Term Conditions Service Alliance Team (LTC SLAT). This group would meet regularly and will be a more formal entity. Key objectives of the Group will be to: 1. Oversee delivery of equitable, diabetes care and services across the district, across secondary services and primary health organisations (PHOS); 2. Establish base-line data and undertake a stocktake of performance against agreed standards of care for diabetes and report results to the Ministry of Health; 3. Identify, disseminate and foster delivery of evidence-based best practice for diabetes care and improvement. 4. Prioritise, monitor and co-ordinate any resulting quality improvement initiatives and projects; 5. Provide advice to DHB Planning and Funding to input to the development of funding and contracting approaches that enhance equitable, safe and effective clinical care and management of diabetes; 6. Provide support for the wider network of providers of clinical care and support for people diagnosed with Diabetes. Draft Terms of Reference for the Networking Group are available on request. Covid-restrictions allowing, we hope to launch the network at a Hui to be held 28th September at Huria Marae.
